BACKGROUND: Hypertension challenges arise in part from poor adherence due to inadequate patient education. VR offers immersive learning to improve hypertension knowledge. OBJECTIVE: To compare VR education with traditional verbal education to improve hypertension knowledge. METHODS: In this randomised trial, 182 patients with hypertension were assigned to receive either traditional physician-led education (n = 88) or VR education (n = 94) with equivalent content. The VR group experienced a 3D video using Oculus Quest 2 headsets. Knowledge was assessed post-intervention using a 29-item questionnaire. The primary outcome was the objective score. Subjective satisfaction and responder characteristics were secondary outcomes. RESULTS: Median objective scores were significantly higher for VR (14, IQR 3) versus traditional education (10, IQR 5), p < 0.001, indicating superior hypertension knowledge acquisition with VR. Subjective satisfaction was high in both groups. Participants were categorized into low (first quartile) and medium-high (second to fourth quartiles) responders based on their scores. Low responders had a significantly higher prevalence of older women than medium-high responders (57% vs. 40% female, p = 0.024; 68 vs. 65 years), p = 0.036). CONCLUSIONS: VR outperforms traditional education. Tailoring to groups such as older women can optimise learning.

Blood pressure (BP) dynamics during graded exercise testing provide important insights into cardiovascular health, particularly in athletes. These measurements, taken during intense physical exertion, complement and often enhance our understanding beyond traditional resting BP measurements. Historically, the challenge has been to distinguish 'normal' from 'exaggerated' BP responses in the athletic environment. While basic guidelines have served their purpose, they may not fully account for the complex nature of BP responses in today's athletes, as illuminated by contemporary research. This review critically evaluates existing guidelines in the context of athletic performance and cardiovascular health. Through a rigorous analysis of the current literature, we highlight the multifaceted nature of exercise-induced BP fluctuations in athletes, emphasising the myriad determinants that influence these responses, from specific training regimens to inherent physiological nuances. Our aim is to advocate a tailored, athlete-centred approach to BP assessment during exercise. Such a paradigm shift is intended to set the stage for evidence-based guidelines to improve athletic training, performance and overall cardiovascular well-being.

Sudden cardiac death (SCD) is a leading cause of death among athletes, and those with a positive family history (FH) of SCD and/or cardiovascular disease (CVD) may be at increased risk. The primary objective of this study was to assess the prevalence and predictors of positive FH of SCD and CVD in athletes using four widely used preparticipation screening (PPS) systems. The secondary objective was to compare the functionality of the screening systems. In a cohort of 13,876 athletes, 1.28% had a positive FH in at least one PPS system. Multivariate logistic regression analysis identified the maximum heart rate as significantly associated with positive FH (OR = 1.042, 95% CI = 1.027-1.056, p < 0.001). The highest prevalence of positive FH was found using the PPE-4 system (1.20%), followed by FIFA, AHA, and IOC systems (1.11%, 0.89%, and 0.71%, respectively). In conclusion, the prevalence of positive FH for SCD and CVD in Czech athletes was found to be 1.28%. Furthermore, positive FH was associated with a higher maximum heart rate at the peak of the exercise test. The findings of this study revealed significant differences in detection rates between PPS protocols, so further research is needed to determine the optimal method of FH collection.

Tento článek pojednává o preparticipačním skríninku, se zaměřením na kardiovaskulární rodinnou anamnézu. Probrány jsou čtyři nejvýznamnější protokoly PPS. Je představeno srovnání protokolů s ohledem na věkovou hranici pro pozitivní odpověď. Dále je diskutována otázka při jaké pozitivní odpovědi odeslat sportovce ke kardiologickému vyšetření. Dále je také zvažováno do jakého pokolení je nutno sledovat rodinnou anamnézu.
This article discusses preparticipation screening, with a focus on cardiovascular family history. The four most relevant PPS protocols are discussed. A comparison of the protocols concerning the age threshold for a positive response is presented. The question of at what positive response to refer an athlete for cardiology evaluation is also discussed. The family history is also considered to which generation should be traced.
Hodnocení EKG u sportovců se v rámci preparticipačního screeningu stává již zavedenou rutinou. Spektrum potenciálně přítomných patologií je široké, a proto je abnormální EKG křivka u sportovce indikací k vyšetření v centru sportovní kardiologie. Nález ST depresí na klidové EKG křivce patří mezi časté příčiny reference sportovce do takového centra. Tento článek formou kazuistického sdělení předkládá současný pohled na tuto problematiku.
The spectrum of possible pathologies is broad and thus an abnormal ECG in athlete is always an indication for evaluation in the Center of sports cardiology. The finding of ST depression on a resting ECG belongs to the most common triggers for referral to such a center. This article, while using a case report, informs its reader about a current understanding of this topic.
Svalový můstek je vrozená koronární anomálie, charakterizována zanořením koronární tepny do myokardu. V naší kazuistice prezentujeme případ 45letého sportovce bez předchozí kardiální anamnezy, přijatého pro atypické bolesti na hrudi do spádové nemocnice, následně referovaného k došetření na našem pracovišti. Provedená selektivní koronarografie prokazuje přítomnost svalového můstku v průběhu levé koronární tepny.
We present a case of 45-years old male, professional athlete, with atypical chest pain admitted to the emergency department at the local hospital, without any history of cardiovascular disease or risk factors. The patient was referred to our center for coronary angiography due to suspected unstable angina. Although coronary atherosclerotic disease wasn't apparent, a myocardial bridge in the course of LAD was detected.
Postupný rozvoj poznání v oblasti lékařské péče o sportovce přináší novou evidenci o komorových extrasystolách u sportovců. Dochází k potvrzení dlouho tušené dichotomie, kdy četnost komorových extrasystol u sportovců není vyšší oproti běžné populaci, ale souběžně mohou být komorové extrasystoly u sportovce první známkou život ohrožujícího onemocnění. První známkou hrozící náhlé srdeční smrti. První známkou takového onemocnění, u kterého pokračování v pohybové aktivitě přispívá k prohlubování stupně postižení myokardu s další akcentací rizika maligní arytmie. Vývoj publikované evidence dává nové pracovní nástroje, jak identifikovat atributy komorových extrasystol u sportovců spojené s nepříznivou prognózou. Za jejich pomoci pak s chirurgickou přesností určit ty jedince, kteří by ve sportovní činnosti neměli pokračovat. A naopak, umožnit nositelům benigních forem komorových extrasystol čerpat z rozsáhlých benefitů pohybové aktivity. Navzdory vršícím se publikovaným datům zůstává několik nezodpovězených otázek přesněji charakterizujících některé z atributů komorových extrasystol. Např. ucelenější popis benigních morfologií, bližší určení míry akceptovatelného výskytu komorových extrasystol při zátěži, nákladovou efektivitu kaskádového zobrazovacího vyšetřování, indikační nejistotu genetického vyšetření či jeho výsledků a v neposlední řadě správnou periodicitu opakovaného specializovaného kardiologického vyšetřování sportovců s komorovými extrasystolami při požadavku tělovýchovného lékaře před udělením lékařské způsobilosti ke sportu.
The gradual development of knowledge in medical care for athletes brings new evidence about premature ventricular beats in athletes. A long-suspected dichotomy is confirmed when the frequency of ventricular extrasystoles in athletes is not higher than in the general population, but at the same time, the presence of ventricular extrasystoles in athletes may be the first sign of a life-threatening disease. The first sign of impending sudden cardiac death. The first sign of situation in which a continuation of physical activity contributes to the worsening of myocardial pathology with an increased risk of malignant arrhythmia. Using these tools enables us to identify those individuals who should not continue in sports with surgical precision, whilst leaving carriers of benign forms of premature ventricular beats to enjoy vast benefits of sporting activity. Despite the accumulating published data, several unanswered questions remain that more accurately characterize some of the attributes of ventricular extrasystoles. E.g., a more comprehensive description of benign morphologies, closer determination of the acceptable occurrence of ventricular extrasystoles during exercise, cost-effectiveness of cascade imaging, indication uncertainty of genetic testing or its results, and finally, the correct periodicity of repeated specialized cardiological examination in athletes with premature ventricular beats.
Autoři shrnují problematiku koarktace aorty ve vztahu ke sportu a námahovou hypertenzi, která je s touto diagnózou asociovaná v dospělosti i v případě úspěšně řešené primární vady v dětském věku. Problematiku doplňují kazuistikou mladé sportovkyně s klidovou normotenzí, ale námahovou hypertenzí. Kazuistika je doplněna rozvahou ohledně možností současné farmakoterapie.

V době pandemie COVID-19 docházelo k omezování některých vyšetřovacích metod, mezi nimi i zátěžových testů. Zátěžová vyšetření jsou však v diagnostickém algoritmu často nezastupitelná. Na druhou stranu jejich provedení může představovat zvýšené riziko přenosu infekčních onemocnění. Následující doporučení má za cíl navrhnout možná řešení, jak co nejbezpečněji pro pacienty i pro zdravotnický personál zátěžové vyšetření provádět i v době zvýšeného rizika přenosných infekcí. Epidemická situace se může dynamicky měnit a je potřeba se řídit doporučeními hygienické služby.
During the COVID-19 pandemic, some investigations were restricted, including exercise stress tests. However, exercise stress tests are often irreplaceable in the diagnostic algorithm. On the other hand, their implementation may pose an increased risk of transmitting infectious diseases. The following recommendations aim to suggest possible solutions to perform exercise stress tests as safely as possible for patients and healthcare professionals, even at times of increased risk of communicable infections. The epidemic situation can change dynamically, and it is necessary to follow the recommendations of the epidemiologists.
