Cíl: Hlavním cílem studie je analyzovat trendy v obezitě u českých adolescentů ve vztahu k socioekonomickém statusu (dále SES) jejich rodin mezi lety 2002 a 2018 a zhodnotit, které z korelátů se vztahují k riziku výskytu obezity. Metodika: Soubor 29879 adolescentů (49,6 % chlapců) ve věku 10,5–16,5 let byl vybrán z reprezentativních vzorků českých adolescentů z opakovaného průřezového, dotazníkového výzkumu „Health Behaviour in School-aged Children“ realizovaného v letech 2002, 2006, 2010, 2014 a 2018. Pomocí škály rodinného blahobytu byl klasifikován SES rodin účastníků jako nízký, střední a vysoký. Obezitu reprezentuje interval > 97 % v percentilovém grafu body mass indexu (BMI) Světové zdravotnické organizace diferencovaný dle pohlaví a věku. χ2-testem byly testovány rozdíly v prevalenci obezity u chlapců a dívek ve všech kategoriích SES mezi lety 2002 a 2018. Pro zkoumání korelátů obezity u vzorku adolescentů z roku 2018 byla použita logistická regresní analýza. Výsledky: Mezi lety 2002 a 2018 byl u adolescentů ve všech kategoriích SES zaznamenán jasný nárůst prevalence obezity, přičemž nejvýrazněji se projevil (p < 0,001) u chlapců (+7,5 procentních bodů (dále p.b.): 4,5 %2002 → 12,0 %2018) a dívek (+2,4 p.b.: 2,7 %2002 → 5,1 %2018) s nízkým SES. Ve srovnání všech cyklů výzkumu byla nejvyšší prevalence obezity patrná u adolescentů s nízkým SES v roce 2018 u dívek i chlapců. Nižší pravděpodobnost výskytu obezity byla signifikantně (p < 0,05) asociována s pravidelnou intenzivní pohybovou aktivitou, účastí na organizovaném sportu a denní konzumací sladkostí bez ohledu na SES adolescentů. Avšak nejnižší podíl účastníků organizovaného sportu a adolescentů, kteří se pravidelně věnují intenzivní pohybové aktivitě, byl zaznamenán v kategorii nízkého SES. Závěry: Nárůst prevalence obezity u adolescentů s nízkým SES naznačuje, že národní programy podpory zdraví a sportu mají na adolescenty z rodin s nízkým SES nejmenší dopad. Závažná zjištění jsou adresována tvůrcům a garantům programů prevence obezity a zdůrazňují potřebu přesnějšího zaměření intervencí na děti a dospívající z rodin s nízkou úrovní SES.
Objective: The main objective of the study is to analyze time trends in obesity of the Czech adolescents between 2002 and 2018 with regard to socioeconomic status (SES) of adolescent families and to find SES-mediated correlates of adolescents' obesity. Methods: A nationally representative sample of 29,879 adolescents (49.6% boys) aged 10.5–16.5 years was drawn from the Health Behaviour in Schoolaged Children cross-sectional, self-reported questionnaire surveys conducted in 2002, 2006, 2010, 2014, and 2018 in the Czech Republic. Using the Family Affluence Scale, the SES of respondents' families was calculated as low, medium and high. Obesity was represented by > 97% on the age-differentiated World Health Organization Percentile Body Mass Index charts. The chi-square tests were performed to assess differences in prevalence of obesity in each gender and SES categories of adolescents between 2002 and 2018. Multiple logistic regression analysis was used for examining the correlates of adolescents' obesity in 2018 data collection. Results: Across the quadrennial surveys 2002–2018, we observed a clear increase in prevalence of obesity in all SES categories of adolescents, which was most striking (p < 0.05) in boys (+7.5 percent points (p.p.): 4.5%2002 → 12.0%2018) and girls (+2.4 p.p.: 2.7%2002 → 5.1%2018) of low SES. Comparing all the survey cycles, the highest prevalence of obesity was evident in the low SES adolescents in 2018 both in girls and boys. The lower odds of obesity were significantly (p < 0.05) associated with regular vigorous physical activity, participation in organized sport, and daily consumption of sweets irrespective of SES of adolescent families. However, the lowest proportion of participants in organized sports and adolescents engaged in vigorous physical activity regularly is in the low SES category. Conclusions: An unreasonable increase in prevalence of obesity in adolescents with low SES suggests that national health- -related and sports programs have the least impact on adolescents from low SES families. The findings are highly relevant to policy makers involved in obesity prevention programs and highlight the need for more accurate targeting of interventions on children and adolescents with low SES background.

Východiska: Úroveň pohybové aktivity (PA) prokazatelně ovlivňuje zdraví dětí. Každodenní využívání aktivního transportu (AT) je považováno za slibný způsob, který může výrazně pomoci dosáhnout doporučené úrovně 60 minut MVPA denně. Strategie pro podporu AT by měly zohledňovat různé aspekty života školáků, které výsledně podpoří větší počet dětí a dospívajících, k využívání aktivních forem dopravy při cestě do a ze školy. Cíle: Cílem této studie bylo charakterizovat vztah mezi AT českých dětí do školy a ze školy v kontextu školních opatření podporující AT a zdravý životní styl, a to s ohledem na ekonomický status rodiny (SES), ze které dítě pochází. Metodika: Do studie byli zahrnuti žáci 5., 7., a 9. tříd (N = 4404; 32,4% jedenáctiletých; 33% třináctiletých; 34,6% patnáctiletých; 51,5% děvčat) českých základních škol. Formu dopravy dětí ověřoval standardizovaný dotazník studie Health Behaviour of School-aged Children (HBSC) v roce 2010. Ke zjišťování opatření na školách byl použit dotazník pro ředitele škol, tzv. School Level Questionnaire (SLQ). Pro charakterizování vztahů mezi AT a školními opatřeními byla aplikována logistická regrese (metoda Enter). Výsledky: Počet žáků využívajících AT pro obě cesty do/ze školy je 58% (2140). AT žáků je signifikantně vyšší u škol, které mají písemně (formálně) zpracovány cíle podpory zdraví (OR/ CI: 2,88/1,15-7,20), než u škol bez těchto cílů, nebo jen s ústně formulovanými cíli podpory zdraví. Vyšší šance na AT byla u žáků jejichž škola byla v místě jejich bydliště (OR/CI: 50,49/23,09-110-40) než u žáků, kteří měli školu jinde než v místě bydliště. Vliv SES na AT českých školáků se neprokázal. Závěry: Výsledky naznačují, že školní opatření mohou být účinným nástrojem pro podporu PA a AT. V případě včasného zavedení opatření zvyšují míru využívání AT školáků na cestách do školy a ze školy.
Background: The level of physical activity (PA) significantly affects children's health. Daily use of active transport (AT) is considered a promising way that can greatly help to reach the recommended level of 60 minutes of MVPA per day. AT strategies should consider the different aspects of school life that will ultimately encourage more children and adolescents to use active forms of transport on their way to and from school. Objective: The aim of this study was to explore the relationship between Czech school children and school commuting to and from school in the context of a school's health promotion and PA-related policies with respect to the family status (SES) from which the child comes. Methods: Students of the 5th, 7th and 9th grades (N = 4404, 32.4%, eleven, 33% thirteen, 34.6% fifteen-year-old, 51.5% girls) of the Czech elementary schools were enrolled. The active transport of adolescents was self-reported within the Health Behavior in School-aged Children survey in the year 2010. Data about school polices were obtained from the school-level questionnaire which was responded to by school principals. Associations between active transport and school health or PA policies were calculated by logistic Regression (Enter method). Results: The number of children using AT for commuting to and from school is 58% (2,140). Children actively commuted more often (OR/CI: 2.88/1.15–7.20) in schools which have formally processed health promotion objectives than in school without these goals. In addition, children who had a school in the same village or city had a higher chance for AT (OR/ CI 50.49/23.09-110.40) than children who had a school elsewhere than in the place of residence. The effect of SES on AT has not been proven. Conclusions: The results suggest that systematic school policies and programs can be an effective tool for supporting PA and AT. Policies and programs increase AT in the case of timely implementation.

BACKGROUND: This study examined a) trends in overweight/obesity, moderate-to-vigorous physical activity (MVPA), and screen time (ST) among Czech adolescents over a 12-year study period (2002-2014) in relation to family affluence (FA) and b) correlates of adolescent overweight/obesity from different FA categories. METHODS: A nationally representative sample of 18,250 adolescents (51.4% girls) aged 10.5-16.5 years was drawn from the Czech Health Behaviour in School-aged Children questionnaire-based surveys in 2002, 2006, 2010, and 2014. Using the FA scale, the socioeconomic status (SES) of the respondents' families was assessed. SES-stratified trends in the prevalence of overweight/obesity meeting the MVPA (≥60 min/day), and ST (≤2 h/day) recommendations were assessed using logistic regression. RESULTS: A trend-related significant increase (p < 0.05) in the prevalence of overweight/obesity was observed in low-/medium-FA boys and medium-/high-FA girls. Unlike in high-FA adolescents, a significant decrease was revealed in the rates of meeting the MVPA recommendation in low-FA boys (28.9%2002 → 23.3%2014, OR = 0.75, 95% CI = 0.59-0.95, p < 0.05) and girls (22.3%2002 → 17.3%2014, OR = 0.73, 95% CI = 0.57-0.92, p < 0.01). A significant (p < 0.001) trend-related increase in excessive ST was evident in adolescents regardless of gender and FA category. Generally, girls and older adolescents had lower odds of overweight/obesity than boys and 11-year-old adolescents. While in the high-FA category of adolescents, achieving 60 min of MVPA daily and the absence of excessive ST on weekdays significantly (p < 0.01) reduced their odds of being overweight/obese, in low-FA adolescents this was not the case. CONCLUSIONS: High rates of overweight/obesity and a poor level of daily MVPA among low-FA children provide disturbing evidence highlighting the necessity of public health efforts to implement obesity reduction interventions for this disadvantaged population.

OBJECTIVE: Due to the importance of surveillance of injuries and accidents in the Czech Republic, the purpose of this study was to report the temporal trends of injuries of Czech adolescents between 2002 and 2014. METHODS: Adolescents (N=20,038) from the Czech Republic, that took part in the 2002, 2006, 2010 and 2014 HBSC study, reported the frequency of medically attended injuries in the past 12 months. Repeated binary logistic regressions with different years as reference categories were performed. RESULTS: Less than half (44.7%) of all Czech adolescents reported they had experienced at least one medically attended injuries in the past 12 months, with boys experiencing more injuries than girls (p<0.001), and the injuries were more common in older adolescents. Fewer boys reported injury in 2014 when compared to 2002 (OR, CI: 0.81, 0.72-0.90), there was also a significant decrease in injuries among girls between 2014 and 2006 (OR, CI: 0.77, 0.69-0.86). CONCLUSIONS: The trend was not linear amongst boys. Along with improved safety promotion education, the rate of injuries decreased among adolescents between the years 2002 and 2014.

OBJECTIVE: Brushing one's teeth twice a day is the main self-care method to prevent the most prevalent non-communicable diseases. The aim of the study is to describe the trends in brushing teeth in 11-, 13-, and 15-year-old Czech children between 1994 and 2014. METHODS: One question about oral health from the HBSC study protocol was used in the six surveys (1994, 1998, 2002, 2006, 2010, and 2014). Altogether, 21,170 answers from boys and girls were analyzed using statistical description and binominal logistic regression. RESULTS: The findings showed that the prevalence of brushing their teeth more than once a day increased throughout the twenty years, more so in boys, but a preventive programme is still needed to educate the 21-38% of those who brush their teeth less than twice a day. CONCLUSION: Although the recommended frequency of brushing their teeth has increased among Czech school-aged children, it still lags far behind the recommended twice-a-day regularity, especially among boys.

OBJECTIVE: Overweight and obesity in adolescents is associated with many health risks and considerable direct and indirect healthcare costs. The purpose of this study is to examine trends in the prevalence of overweight and obesity in 11-, 13- and 15-year-old adolescents in the Czech Republic from 1998 to 2014. METHODS: Data from five self-reported survey rounds (1998, 2002, 2006, 2010, and 2014) of the Health Behaviour in School-aged Children Study (HBSC) were used to assess trends in overweight and obesity among Czech adolescents. The total sample consisted of 19,103 adolescents (51.2% girls). A logistic regression analysis was used to assess trends in different age and gender categories. RESULTS: From 1998 to 2014 a significant increase in the prevalence of overweight and obesity was observed among boys in all age categories (11 years old 22.2%1998- 28.3%2014; 13 years old 17.9%1998- 26.7%2014; 15 years old 9.8%1998- 20.8%2014) and among 15-year-old girls (6.0%1998- 10.9%2014). None of the age and gender categories showed an overall decrease over the 16-year period. In boys, the prevalence of overweight was significantly higher with steeper negative trends compared with girls. However, stabilization in overweight rates was observed between 2010 and 2014 in all age and gender groups. CONCLUSION: Nationally representative self-reported data show a significant increase in overweight (including obesity) prevalence among children from 1998 to 2014 in the Czech Republic. The results also suggest stabilization in overweight prevalence between 2010 and 2014. Continuing research is needed to determine future trends while interventions aimed at reducing overweight and obesity in children should be implemented on different levels of public policy.

OBJECTIVE: The aim of the study was to examine trends in the prevalence of lifetime cannabis use among the Czech 15-year old students. METHODS: Data from the nationally representative Health Behaviour in School-aged Children Survey, conducted in the Czech Republic in 2002, 2006, 2010 and 2014, were used. Trends in cannabis use among both boys and girls were modelled through binary logistic regression with period as a predictor of the lifetime cannabis use. RESULTS: The prevalence of lifetime cannabis use has significantly decreased among young Czechs, particularly among boys. Gender differences in cannabis use have been also gradually decreasing since 2002, with no significant differences between genders in recent period. CONCLUSIONS: Although there are positive changes in the prevalence of adolescent cannabis use, from the European perspective, Czech students still belong to those with significantly higher rates in this respect. Thus, alongside with the use of other substances, adolescent cannabis consumption remains an important challenge for the national public health policy.

OBJECTIVE: Active commuting (AC) is an important factor in increasing the total daily physical activity (PA) in children, which is significant for their health and positive physical behaviour in adulthood. The objective of the study was to describe trends in active commuting to school among Czech girls and boys aged 11-15 years from 2006 to 2014, using data from the Health Behaviour in School-aged Children (HBSC) study. METHODS: To investigate trends in AC among Czech children of school age data obtained from the HBSC surveys carried out in 2006, 2010, and 2014 was used. The study sample comprised 12,273 respondents, out of whom 5,992 (48.8%) were boys and 6,281 (51.2%) girls. The overall trends in AC were evaluated using logistic regression analysis. RESULTS: Compared with 2006, there was an overall decrease in AC among the 13-year-old boys and 13-year-old girls (boys - 1.8% in 2006 to 50.1% in 2014, girls - 69.3% in 2006 to 46.3% in 2014). The proportion of children AC (walking and cycling) to school significantly decreased from 74.3% in 2006 to 53.4% in 2014. CONCLUSION: This study demonstrated that AC to school decreased sharply among Czech children of school age from 2006 to 2014. However, walking was the most frequently used mode of travel. The boys were significantly more likely to cycle to school compared to the girls. PA interventions for youth should encourage participation in AC to school in the Czech Republic.

AIM: The prevalence of sedentary behaviour has increased, and physical activity (PA) levels have declined with increases in urbanisation and modernisation. Researchers need to find solutions to address this lack of PA. The main aim of this study was to identify the PA level and sedentary behaviour in a randomised sample of the Czech adult population in 2002 and 2011. METHODS: To investigate PA in 20-64 year old adults, the International Physical Activity Questionnaire (IPAQ) (in 2002) and the Global Physical Activity Questionnaire (GPAQ) (in 2011) were applied. The randomised samples comprised 1,782 participants in 2002, and 1,359 participants in 2011. RESULTS: The results showed a significant decrease (F=83.84; p<0.001; ω²=0.03) in the time spent sitting between the years 2002 and 2011. The proportion of individuals reporting low PA has increased (11.4% in 2002 vs. 27.9% in 2011). Individuals with college/university education were more likely to have low PA levels (OR=1.51, p<0.001, CI=1.21-1.68) compared with those with elementary or secondary education. Along with educational level, low PA levels are also associated with older age, spending more than four hours a day sitting, and the survey year. CONCLUSION: Sedentary behaviour, PA, and factors influencing this behaviour may differ across countries; therefore, more trend-related research should be performed on these issues, especially in Central and Eastern Europe, where these studies are scarce.

VÝCHODISKA: Školní prostředí je považováno za vhodné pro podporu pohybové aktivity (PA) dětí, které zásadně ovlivňuje nejen jejich vztah k realizaci PA, ale i k sobě samým, spolužákům a učitelům. Vedle rodiny je školní prostředí významným jevištěm utváření životních hodnot, ke kterým patří pozitivní vztah dětí k pravidelné realizaci PA. CÍLE: Cílem této studie je pomocí školního dotazníku (School-level questionnaire) analyzovat podmínky pro PA na školách v České republice (ČR). METODIKA: Ke zjištění podmínek pro PA na školách v ČR bylo využito výběrového souboru 92 vzdělávacích zařízení (84 základních škol a 8 víceletých gymnázií). Výzkumný sběr dat proběhl v první polovině roku 2012. Pro statistické zpracování výsledků byla použita deskriptivní statistika a grafické znázornění četností zastoupení kladných odpovědí ve sledovaných oblastech dotazníku. VÝSLEDKY: Mimo vyučovací jednotky tělesné výchovy (TV) škola nejčastěji organizuje další formy PA po vyučování; každodenní realizaci PA po vyučování uvádí 29,4 % škol. Každodenní pohybově aktivní přestávky zařazuje 26,8 % škol. Více než 80 % škol disponuje dostatečným množstvím zařízení pro realizaci PA a každodenně je využívá. Pouze v 17 % škol mají žáci umožněn přístup do těchto zařízení také během přestávek a volných hodin. ZÁVĚRY: Většina škol v ČR disponuje dostatečným materiálním a prostorovým zázemím pro realizaci PA dětí. Nejčastěji realizovanou formou PA v průběhu vyučování je školní TV, která je vyučována v rozsahu 2 vyučovacích jednotek týdně. Jako významná se z pohledu celkové úrovně PA dětí jeví podpora PA v průběhu přestávek a volných hodin.
BACKGROUND: The school environment is considered suitable for pro- moting physical activity in children, which infl uen ces their attitude not only to physical activity but also to themselves, classmate s, and teachers. Besides the family, the school environment is a signifi cant scen e for the development of life values, which includes a positive children's attitude to regular physical activity. AIMS: The objective of the study is to use a school-leve l questionnaire to analyse spatial and organizational conditions for p hysical activity and school- based physical education in primary and secondary s chools in the Czech Re- public (CR). METHODS: To investigate the conditions for physical activity and physical education in schools in the CR, a selection sample of 92 educational institutions (84 primary schools and 8 multi-year grammar school s) were used. Research data were collected in the fi rst half of 2012. Statistic al data processing was performed by means of descriptive statistics and graphic illu stration from the questionnaire. RESULTS: Apart from physical education lessons, schools pro vide additional forms of after-school physical activity. As much as 29.4% of schools provide after- school physical activity, while physically active b reaks are promoted by 26.8% of schools. More than 80% of schools have suffi cient equipment for the delivery of physical activity and use it on a daily basis. O nly in 17% of schools students have free access to this equipment during breaks and free lessons. CONCLUSIONS: Most schools in the CR have suffi cient space for t he de- livery of children's physical activity.
