Rozdíl mezi očekáváním a realitou v léčbě schizofrenie lze nazvat nenaplněnými potřebami ("unmet needs"). V rámci farmakologické léčby se jedná o problematickou adherenci, nežádoucí účinky (snášenlivost), nízkou účinnost antipsychotik na konkrétní symptomy, zejména negativní, kognitivní a depresivní příznaky. Pokud je naším cílem v léčbě schizofrenie dosažení úzdravy v podobě sociálního fungování, pak jsou nedostatečné možnosti zlepšení negativních příznaků učebnicovým příkladem nenaplněné potřeby. Účinnost antipsychotik na negativní příznaky je obecně nízká. Nadějně se v této oblasti jeví kariprazin, nový parciální agonista D2/D3 receptorů s preferenční vazbou k D3 receptorům. V Evropě je registrován pro léčbu schizofrenie u dospělých. Účinnost kariprazinu byla prokázána na akutní příznaky schizofrenie i v prevenci relapsu při dlouhodobé léčbě. Navíc v půlroční studii zaměřené na léčbu převládajících (predominantních) negativních příznaků schizofrenie byl kariprazin významně účinnější než risperidon jak v redukci primárních negativních symptomů, tak ve zlepšení psychosociálního fungování pacientů. Krátkodobá i dlouhodobá snášenlivost kariprazinu byla velmi dobrá, většina nežádoucích účinků byla mírné nebo střední intenzity. Mezi nejčastější nežádoucí účinky patřily akatízie a EPS, které ve většině případů nevedly k vysazení medikace. Kariprazin má minimální vliv na změnu hmotnosti, je metabolicky neutrální, nezvyšuje hladinu prolaktinu a nemá sedativní efekt. Jedná se o účinné antipsychotikum s dobrou snášenlivostí, které lze navíc využít pro léčbu převládajících negativních příznaků schizofrenie. Vzhledem k jeho receptorovému působení lze navíc předpokládat i jeho využití u jiných nenaplněných potřeb v léčbě schizofrenie.
The gap between expectations and reality in treatment of schizophrenia can be termed "unmet needs". Examples of needs in pharmacotherapy are problematic adherence, side effects (tolerability), and poor efficacy of antipsychotics in specific symptom domains, especially negative, cognitive, and depressive symptoms. If our ultimate goal in schizophrenia treatment is recovery, as indexed by social functioning, then insufficient improvement of negative symptoms represents a copybook example of unmet need. Efficacy of antipsychotics in negative symptoms is generally low; promising potential was demonstrated by cariprazine, a new partial agonist of D2/D3 receptors with preferential affinity to D3 receptors. In Europe cariprazine has been registered for treatment of schizophrenia in adults. Cariprazine proved its efficacy in acute schizophrenia and in long-term therapy in relapse prevention. Additionally, in a six-month study with predominant negative symptoms of schizophrenia, cariprazine was significantly more effective than risperidone in reduction of primary negative symptoms and also in improvement of psychosocial patient functioning. Short-and long-term tolerability of cariprazine was very good; most side effects were of mild or moderate severity. The most frequent side effects reported were akathisia and EPS, however in most cases they did not lead to discontinuation of the treatment. Cariprazine has minimal effect on weight gain, is metabolicaly neutral, does not increase prolactin level and has no sedation effect. It proved to be and effective and well-tolerated antipsychotic that can be used in treatment of predominant negative symptoms of schizophrenia. Moreover, given its receptor profile, it can be presumably utilized for other unmet needs in schizophrenia treatment, as well.

Obsessive-compulsive disorder (OCD) is a chronic psychiatric illness and 1 of the most common anxiety disorders with the prevalence of 3%. Although its pathogenesis remains unclear, the traditional model focused on alternations in the serotonin system. Selective serotonin reuptake inhibitors provide the most effective treatment; however, as much as 40-60% of patients do not respond to antidepressants therapy. Thus, attention has shifted towards other neurotransmitter systems and related neuroanatomical structures. Recently, there is extensive evidence showing a key role of glutamate pathways abnormalities within the cortico-striatal-thalamo-cortical circuitry and temporal lobes in OCD pathogenesis. In this review, we link together the existent neuroanatomical, neurophysiological, and neuropsychological evidence to argue for potential benefits of adjuvant treatment with glutamatergic agents, especially memantine. By a targeted de-excitation effect on the glutamatergic system in the temporal lobes and connected brain regions, memantine might further alleviate OCD symptoms. This effect should be even more pronounced in certain subtypes of patients with specific cognitive deficits and maladaptive compensatory memory processes (e.g., checkers).

Some age-dependent epilepsies of infancy and childhood cannot be sufficiently treated with contemporaneous antiepileptics, therefore search for new mechanisms and new drugs active in pediatric patients is necessary. Possible anticonvulsant action of AMN082 (N,N'-dibenzhydryl-ethane-1,2-diamine dihydrochloride), a positive allosteric modulator of metabotropic glutamate receptor subtype 7, was examined in a model of epileptic afterdischarges elicited in three age groups of immature rats (12, 18 and 25days old) by electrical stimulation of sensorimotor cortex. AMN082 did not exhibit an anticonvulsant action against spike-and-wave type of afterdischarges generated in the thalamocortical system; on the contrary, moderate proconvulsant action was found in 12- and 18-day-old rats (more marked prolongation of afterdischarges with repeated stimulations). On the other hand, AMN082 significantly shortened the second type of afterdischarges reflecting spread of epileptic activity into limbic system frequently recorded in 25-day-old rats. These contradictory results mean that AMN082 will not be a broad-spectrum anticonvulsant but it might be useful as a seizure-specific antiepileptic drug.
