Sarcomas in the head and neck area are rare diseases with an incidence of under 1% of all head and neck malignant tumours. Osteosarcomas or osteogenic sarcomas consist of neoplastic cells that produce osteoid bone or immature bone. Sarcomas develop more in the mandible than the maxilla. The exact diagnosis of different types of sarcomas is based on the immunohistochemical investigation. These rare tumours are of mesenchymal origin; osteosarcomas and chondrosarcomas are the most common types-Ewing's sarcomas. The use of proton beam radiotherapy in the treatment of osteosarcoma of the maxilla is rarely reported in the literature. We present a case of successfully treated (surgery and proton beam radiotherapy) poorly differentiated highly aggressive osteosarcoma in the ethmoids and maxillary sinus and morbidity after the treatment. We were presented with a case of a 65-year-old man with anaesthesia and palsy of the right face. The stomatology department performed the extraction of a tooth. One month later, the wound was still open. The histology showed an osteogenic sarcoma in the area of the wound. The oncologist and maxillofacial surgeons in a catchment hospital recommended a nonsurgical approach. Hence, we performed a radical maxillectomy and ethmoidectomy, after which we continued with proton bean radiotherapy. The patient is now five years after therapy without signs of sarcoma; however, he has blindness in the right eye and reduced vision in the left eye, as well as gliosis of the brain, vertigo, tinnitus, trismus, and ancylostomiases. Head and neck osteosarcomas treatment is considered a complex multidisciplinary task. It is currently argued that there is no clear therapeutic protocol for successful treatment. Innovations in treatment modalities such as proton beam radiotherapy appear to have potential, although their effects on long-term morbidity and survival outcomes are still undetermined. We present a rare case report of an osteosarcoma of the maxilla involving an innovative, successful treatment procedure combining surgical excision followed by proton beam radiotherapy. This treatment approach may enable maximum tumour control. This protocol has not been adequately described in the world literature for this diagnosis.

Nejčastěji se vyskytujícími primárními kostními nádory jsou osteosarkom, Ewingův sarkom, chordom a chondrosarkom. Jejich léčba vyžaduje multimodální přístup, resekabilita je významným prognostickým faktorem. Radioterapie má v lokální léčbě primárních kostních nádorů svoji definovanou roli. Je standardní součástí terapie u Ewingova sarkomu v případě neresekability, neradikálního odstranění tumoru nebo špatné histologické odpovědi na neoadjuvantní léčbu. U chordomů a chondrosarkomů je indikována kurativní radioterapie v případě inoperability primárního nádoru či při makroskopickém pooperačním reziduu. Biologická povaha těchto nádorů vyžaduje užití vysoké celkové dávky 60 až 78 Gy. U osteosarkomu, historicky pokládaného za radiorezistentní nádor, se dnes jeví ozáření ve vybraných klinických situacích rovněž jako racionální, především v případě neradikálních zákroků. Vždy je zvažován prospěch z léčebného ozáření proti rizikům jeho akutních a pozdních nežádoucích účinků.
The most common primary bone tumours are osteosarcoma, Ewing's sarcoma, chordoma and chondrosarcoma. Their treatment requires a multimodal approach, the resectability is an important prognostic factor. Radiotherapy is an active modality for assuring local control and has its defined role in the treatment of primary bone tumours. It is recommended as standard part of management in inoperable Ewing's sarcoma tumours, in cases of subtotal resections or poor histological responses to neoadjuvant chemotherapy. In chordoma and chondrosarcoma, radiotherapy is reserved for clinical situations where complete surgery cannot be achieved. However, biological nature of these tumours requires the use of a high total doses of 60-78 Gy. Radiotherapy of osteosarcoma, that supposed to be radioresistant, seems to be effective in particular cases, especially in case of non-radical surgery. The therapeutic benefit of irradiation is always considered against the risk of acute and late side effects.

Sarkomy měkkých tkání a primární nádory kostí tvoří velmi heterogenní skupinu. Zlatým léčebným standardem u velké většiny z nich je chirurgický výkon. V současné době se však stále více uplatňuje kombinace resekčního výkonu s radioterapií, která díky nových technologiím umožní i aplikaci relativně vysokých dávek záření nutných pro léčbu právě těchto nádorů. Cílem sdělení je podat ucelený přehled o nejnovějších možnostech léčby zářením ve formě neoadjuvantní a adjuvantní zevní radioterapie, intraoperačního ozáření, intersticiální brachyterapie a kombinace s chirurgickým výkonem. Pro dosažení optimálních výsledků u skupiny donedávna velmi prognosticky špatné skupiny nemocných je ovšem nutná multidisciplinární spolupráce a koncentrace pacientů do center, které mají technické možnosti a praktické zkušenosti s léčbou sarkomů měkkých tkání a kostí.
Soft tissue sarcomas and primary bone tumours constitute very heterogenic group. Gold treatment standard is surgery in most of them. Nowadays the combination surgery with radiotherapy is preferred, because thanks new radiotherapeutic technology is possible to apply very high dose of radiation which necessary for local control of these tumours. The goal of our article is to describe new possibilities of radiotherapy, including neoadjuvant and adjuvant setting, intraoperative radiotherapy, interstitial brachytherapy and the combination with surgery. But very important is multidisciplinary cooperation to until recently in prognostic bad group of patients, concentration these patients to centers with modern technique and clinical experience with treatment of soft tissue sarcomas and bone tumours.
Východiska: V práci hodnotíme léčebné výsledky u pacientů s osteosarkomem. Soubor pacientů a metody: V období let 1997– 2006 jsme s touto diagnózou přijali 44 pacientů (27 chlapců a 17 dívek). Průměrný věk byl 12,8 let (2,5–20,2 let). Lokalizované onemocnění mělo 41 pacientů a generalizované onemocnění 3 pacienti. Použili jsme léčebný protokol severo americké kooperativní skupiny dětské onkologie CCG 7921 POG 9351 INT 0133. Medián sledování byl 5,5 let (2– 11 let). Výsledky: U 40 pacientů jsme dosáhli kompletní remise. Relaps onemocnění jsme prokázali u 18 pacientů. Zemřelo 17 pacientů (15krát progrese onemocnění, 1krát toxické úmrtí, 1krát sekundární leukemie). Celkové pětileté přežití (Overall Survival – OS) bylo 58,4 % a pětileté přežití do selhání (Event Free Survival – EFS) 46,7 %. Pacienti s postižením končetin (n = 40) měli pětiletý EFS 51 %. Pacienti s příznivou histologickou odpovědí (n = 22) měli pětiletý EFS 63,6 %, naproti tomu nemocní s nepříznivou odpovědí (n = 18) měli pětiletý EFS 30,5 % (p = 0,009). Závěr: Výsledky léčby pacientů s osteosarkomy postihujícími končetiny a s příznivou histologickou odpovědí na předoperační léčbu jsou velmi dobré. Osud pacientů s primárně generalizovaným onemocněním a osteosarkomem v axiálních lokalizacích byl nepříznivý.

The human checkpoint kinase Chk1 has been suggested as a target for cancer treatment. Here, we show that a new inhibitor of Chk1 kinase, CEP-3891, efficiently abrogates both the ionizing radiation (IR)-induced S and G(2) checkpoints. When the checkpoints were abrogated by CEP-3891, the majority (64%) of cells showed fragmented nuclei at 24 hours after IR (6 Gy). The formation of nuclear fragmentation in IR-treated human cancer cells was directly visualized by time-lapse video microscopy of U2-OS cells expressing a green fluorescent protein-tagged histone H2B protein. Nuclear fragmentation occurred as a result of defective chromosome segregation when irradiated cells entered their first mitosis, either prematurely without S and G(2) checkpoint arrest in the presence of CEP-3891 or after a prolonged S and G(2) checkpoint arrest in the absence of CEP-3891. The nuclear fragmentation was clearly distinguishable from apoptosis because caspase activity and nuclear condensation were not induced. Finally, CEP-3891 not only accelerated IR-induced nuclear fragmentation, it also increased the overall cell killing after IR as measured in clonogenic survival assays. These results demonstrate that transient Chk1 inhibition by CEP-3891 allows premature mitotic entry of irradiated cells, thereby leading to accelerated onset of mitotic nuclear fragmentation and increased cell death.
