Účelem článku je seznámit odbornou veřejnost s autorským výkladem nejdůležitějších změn nařízení vlády č. 272/2011 Sb., o ochraně zdraví před nepříznivými účinky hluku a vibrací, které nabývají účinnosti dnem 1. července 2023. Cílem novely je racionalizace a zjednodušení přístupu ochrany veřejného zdraví před hlukem v tom smyslu, aby byl zachován rozvoj veřejné infrastruktury se zásadou dodržování ochrany veřejného zdraví před hlukem. Změny se týkají zejména oblasti hluku z dopravy, resp. hluku z pozemních komunikací a drah. Současná právní úprava byla již překonána a způsobovala v některých případech i paradoxní situace. Z dočasných institutů se staly rigidní mechanismy, které již prakticky nezajišťovaly ochranu veřejného zdraví před hlukem z dopravních zdrojů a způsobovaly komplikace v dalším rozvoji zastavěných území sídel i dopravní infrastruktury.
The aim of this article is the author's presentation of the most important adjustmens of Government Regulation No. 272/2011 Coll. for the professional public. This Government Regulation is going to come into force on 01/07/2023. The main purpose is to rationalize and simplify the relationship between two policies - environmental noise regulation and sustainable urban and transport infrastructure development. The changes concern in particular the area of traffic noise, i.e. noise from roads and railways. The current national framework is obsolete and it is causing absurd legal interpretations. The temporary legal exceptations have become rigid obligations which are only formal and nowadays are not bringing the meaningful purpose - public health protection against exposure to traffic noise on the one hand and sustainable urban and transport infrastructure development on the onther hand.

Cílem příspěvku je hodnocení expozice obyvatelstva radiofrekvenčnímu záření emitovanému televizním vysílačem DVB-T2 a dalšími zdroji umístěnými na vrcholu Ještědu z pohledu ochrany veřejného zdraví. Zároveň článek zahrnuje historii, popis uvedených zdrojů a postup zástupců státní správy při výkonu zdravotního dozoru v této oblasti.
The paper aims to evaluate the population's exposure to radiofrequency radiation emitted by the DVB-T2 television transmitter and other sources located on the top of Jested from the perspective of public health protection. At the same time, the article includes history, description of these sources, and the procedure of the state administration representatives in exercising health surveillance in this area.

Během 21. století se objevilo mnoho globálních zdravotních hrozeb souvisejících se zoonózami, novými či znovu se objevujícími infekčními onemocněními, změnou klimatu a/nebo udržitelností životního prostředí. Dosažení harmonizovaných přístupů k detekci a prevenci těchto zdravotních rizik je mnohdy obtížné, protože tradiční přístupy jsou často určovány hranicemi finančně i legislativně oddělených vědních oborů, jako jsou například víceméně samostatně fungující humánní a veterinární medicína, zemědělství, klimatologie, sociologie, ekonomika aj. Hranice mezi těmito jednotlivými obory by však neměly být pevné a spolupráce by měla být nastavena na základě potřeby neboli účelu. K lepší spolupráci vznikají četné spolky, skupiny a sdružení, ať už zájmové nebo odborné. Meziresortní spolupráce se však často potýká s legislativními, kompetenčními a komunikačními uzly. Koncept Jedno zdraví se snaží tyto bariéry prolomit a na základě společné potřeby, která je definována jako zajištění podpory blahobytu a možnosti společného řešení hrozeb pro zdraví a ekosystémy, nastavit nové hranice. Je založen na myšlence dosáhnout lepších výsledků v oblasti veřejného zdraví na celém světě pomocí interdisciplinární spolupráce.
During the 21st century, many global health threats related to zoonoses, new or re-emerging infectious diseases, climate change and/or environmental sustainability have emerged. Achieving harmonized approaches to the detection and prevention of these health risks is often difficult because traditional approaches are usually determined by the boundaries of financially and legislatively separated scientific fields, such as more or less independently functioning human and veterinary medicine, agriculture, climatology, sociology, economics, etc. The boundaries between these individual fields however, should not be solid, and cooperation should be based on common need or purpose. For better cooperation, numerous associations and groups, professional or based on interest, are being formed. Interdepartmental cooperation often struggles with legislative, competence and communication nodes. The "One Health" concept tries to break down these barriers and set new boundaries based on a common need, which is defined as ensuring the support of well-being and the possibility of jointly solving threats to health and ecosystems. It is based on the idea of achieving better public health outcomes worldwide through interdisciplinary collaboration.

INTRODUCTION: The aim of this study is to analyse the impact of vaccination of infants with pneumococcal conjugate vaccine (PCV) on the incidence of invasive pneumococcal disease (IPD) in children under 5 years of age in the Czech Republic. MATERIAL AND METHODS: The present study includes all IPD cases reported in children aged 0-4 years within the surveillance program in 2007-2017. The impact of PCV is analysed for five categories of IPD: cases caused by all serotypes, cases caused by PCV7 serotypes (4, 6B, 9V, 14, 18C, 19F, and 23F), cases caused by three additional PCV10 serotypes (1, 5, and 7F), cases caused by three additional PCV13 serotypes (3, 6A, and 19A), and cases caused by non-PCV serotypes. To assess the impact of PCV, the study period was divided into the pre-vaccination period 2007-2008 and post-vaccination period 2009-2017, which was divided into three three-year parts: 2009-2011, 2012-2014, and 2015-2017. Analysis of differences between periods was based on the Poisson regression model where the population numbers were handled as an offset. RESULTS: The annual incidence of IPD in children under 5 years of age caused by all serotypes has had a downward trend since 2007: it dropped from 8.52/100 000 in 2007 to 2.67/100 000 in 2017, with slight increases in 2010 and 2013. All three post-vaccination periods show significantly lower (p<0.001) incidences in comparison to the pre-vaccination period, but they do not statistically significantly differ from each other. CONCLUSIONS: IPD surveillance data in the Czech Republic show that after the introduction of PCV vaccination of infants, there has been a significant decrease in the IPD incidence of children under 5 years of age. Continued IPD surveillance is essential to monitor for possible post-vaccination serotype replacement.

OBJECTIVES: Seasonal influenza causes high morbidity worldwide and high mortality in developing countries. As a result, the development of systems for seasonal influenza surveillance has been of great interest. The aim of this study is to explore the potential role of an Emergency Medical System (EMS) call centre to complement traditional surveillance systems of seasonal influenza. METHODS: Retrospective observational study in which data on influenza from the system of Notifiable Diseases List (Spanish acronym EDO) and Sentinel Physicians Network (Spanish acronym RMC) were compared with information on calls made to the Principality of Asturias EMS call centre that covers all the region population (1,027,659 inhabitants) based on a set of specific criteria to determine differences and explore this emergency call system as a complementary epidemiological surveillance system. Cases registered by different systems have been compared to the same 68 weeks period, from week 45 of 2011 to week 8 of 2013. RESULTS: RMC reported a total of 2,354 cases of influenza, EDO 43,071 cases and EMS call centre 4,360 "case calls" out of 180,720 total emergency calls. Case series of EDO and EMS call centre have shown a positive correlation (R = 0.42, p = 0.003). Case series from EMS call centre and RMC were correlated (R = 0.38, p = 0.007). Case series from EDO and RMC have shown a strong positive correlation (R = 0.91, p < 0.001). Correlation analysis of the cases reported by the three systems have shown a significant positive correlation between them (p < 0.001). The spike of EMS calls related to the studied influenza syndrome occurs one week in advance compared to traditional epidemiological surveillance systems. CONCLUSIONS: EMS call centre data on influenza could be potentially used as a complementary surveillance system to the traditional epidemiological surveillance systems for influenza.
BACKGROUND: The increasing incidence of Clostridium difficile infections (CDI) in healthcare settings in Europe since 2003 has affected both patients and healthcare systems. The implementation of effective CDI surveillance is key to enable monitoring of the occurrence and spread of C. difficile in healthcare and the timely detection of outbreaks. AIMS: The aim of this review is to provide a summary of key components of effective CDI surveillance and to provide some practical recommendations. We also summarize the recent and current national CDI surveillance activities, to illustrate strengths and weaknesses of CDI surveillance in Europe. SOURCES: For the definition of key components of CDI surveillance, we consulted the current European Society of Clinical Microbiology and Infectious Diseases (ESCMID) CDI-related guidance documents and the European Centre for Disease Prevention and Control (ECDC) protocol for CDI surveillance in acute care hospitals. To summarize the recent and current national CDI surveillance activities, we discussed international multicentre CDI surveillance studies performed in 2005-13. In 2017, we also performed a new survey of existing CDI surveillance systems in 33 European countries. CONTENT: Key components for CDI surveillance are appropriate case definitions of CDI, standardized CDI diagnostics, agreement on CDI case origin definition, and the presentation of CDI rates with well-defined numerators and denominators. Incorporation of microbiological data is required to provide information on prevailing PCR ribotypes and antimicrobial susceptibility to first-line CDI treatment drugs. In 2017, 20 European countries had a national CDI surveillance system and 21 countries participated in ECDC-coordinated CDI surveillance. Since 2014, the number of centres with capacity for C. difficile typing has increased to 35 reference or central laboratories in 26 European countries. IMPLICATIONS: Incidence rates of CDI, obtained from a standardized CDI surveillance system, can be used as an important quality indicator of healthcare at hospital as well as country level.

NRL pro enteroviry v rámci environmentální surveillance vyšetřuje odpadní vody z čističek devíti měst a pěti uprchlických táborů v České republice. V roce 2016 bylo vyšetřeno 191 vzorků odpadních vod, v 8 vzorcích byly nalezeny non-polio enteroviry (NPEV).
Within environmental surveillance, the National Reference Laboratory for Enteroviruses screens wastewater from sewage treatment plants in nine cities and five refugee camps in the Czech Republic. In 2016, 191 sewage samples were analyzed, and in eight of them, non-polio-enteroviruses (NPEV) were detected.
