Dříve se dietní sestra zabývala převážně administrativní činností, zejména se ručně vyplňovaly tiskopisy, jako např. přehled stravy na oddělení, normování stravy pro pacienty či sestavování jídelních lístků. Jen málo času věnovala edukaci pacientů. V současné době je administrativa usnadněna prací na počítači, takže nutriční terapeut má více času na práci se samotnými pacienty.

UNLABELLED: BACKGROUND: The present study investigated plasma levels of gut-brain axis peptides ghrelin, obestatin, NPY and PYY after consumption of a high-carbohydrate (HC) and high-protein (HP) breakfast in patients with anorexia nervosa, bulimia nervosa and in healthy controls. These peptides play an important role in regulation of energy homeostasis and their secretion is disturbed under condition of eating disorders. As various types of consumed macronutrients may induce different plasma hormone responses, so we examined these responses in women patients with eating disorders and compared them with those of healthy controls. METHODS: We examined plasma hormone responses to HC and HP breakfast in patients with AN (n = 14; age: 24.6 ± 1.8 years, BMI: 15.3 ± 0.7), BN (n = 15; age: 23.2 ± 1.7 years, BMI: 20.5 ± 0.9) and healthy controls (n = 14; age: 24.9 ± 1.4 years, BMI: 21.1 ± 0.8). Blood samples were drawn from the cubital vein, the first blood drawn was collected before meal, and then 30, 60, 90, 120 and 150 min after breakfast consumption. Plasma hormone levels were determined by commercially available RIA kits. RESULTS: Fasting and postprandial plasma obestatin levels were significantly increased in both AN and BN patients, while plasma ghrelin levels were significantly increased in AN patients only. After breakfast consumption, plasma levels of ghrelin and obestatin decreased, although they were still above the range of values of healthy controls. Fasting NPY plasma levels were significantly increased in AN and BN patients and did not change postprandially. Fasting PYY levels were comparable in AN, BN and healthy controls, but postprandially significantly increased after HP breakfast in AN and BN patients. Different reactions to breakfast consumption was found for ghrelin and PYY among investigated groups, while for obestatin and NPY these reactions were similar in all groups. CONCLUSIONS: Significant increase of obestatin and NPY in AN and BN patients may indicate their important role as the markers of eating disorders. Different reactions of ghrelin and PYY to breakfast consumption among groups suggest that role of these hormones in regulation of energy homeostasis can be adjusted in dependence to acute status of eating disorder.

Peptides ghrelin, obestatin and neuropeptide Y (NPY) play an important role in regulation of energy homeostasis, the imbalance of which is associated with eating disorders anorexia (AN) and bulimia nervosa (BN). The changes in ghrelin, obestatin and NPY plasma levels were investigated in AN and BN patients after administration of a high-carbohydrate breakfast (1604 kJ). Eight AN women (aged 25.4+/-1.9, BMI: 15.8+/-0.5), thirteen BN women (aged 22.0+/-1.05, BMI: 20.1+/-0.41) and eleven healthy women (aged 25.1+/-1.16, BMI: 20.9+/-0.40) were recruited for the study. We demonstrated increased fasting ghrelin in AN, but not in BN patients, while fasting obestatin and NPY were increased in both AN and BN patients compared to the controls. Administration of high-carbohydrate breakfast induced a similar relative decrease in ghrelin and obestatin plasma levels in all groups, while NPY remained increased in postprandial period in both patient groups. Ghrelin/obestatin ratio was lower in AN and BN compared to the controls. In conclusions, increased plasma levels of fasting NPY and its unchanged levels after breakfast indicate that NPY is an important marker of eating disorders AN and BN. Different fasting ghrelin and obestatin levels in AN and BN could demonstrate their diverse functions in appetite and eating suppression.

Východisko. Nedávný objev nového peptidového hormonu obestatinu, odvozeného od stejného genu jako ghrelin, přidal další poznatky ke komplexnosti fyziologie ghrelinu. I tak však zůstává nezodpovězených mnoho otázek, včetně vlivu složení potravy na regulaci orexigenního ghrelinu a obestatinu považovaného spíše za anorexigen. V této studii byl sledován vliv složení kalorické a nekalorické potravy na plazmatické hladiny ghrelinu a obestatinu u zdravých žen (n = 6; věk 23,83 ± 1,1 let; BMI 20,85 ± 0,87 kg/m2) a u žen s bulimia nervosa (n = 6; věk 26,6 ± 5,2 let; BMI 19,2 ± 1,44 kg/m2) charakterizovaných abnormálním jídelním chováním a narušenou energetickou rovnováhou. Metody a výsledky. Plazmatické hladiny ghrelinu a obestatinu byly měřeny po celonočním hladovění před a po podání rozpustné vlákniny nebo vlákniny s glukózou. Hladiny hormonů byly stanoveny pomocí radioimunologických kitů. U obou skupin žen se hladiny ghrelinu a obestatinu po podání rozpustné vlákniny v průběhu křivky významně neměnily. Po podání rozpustné vlákniny s glukózou byl zjištěn významný pokles ghrelinu i obestatinu u pacientek s bulimia nervosa i u zdravých žen, a to postupně v prvních 30–90 minutách po požití. Poté se hladiny obou hormonů vracely k preprandiálním hodnotám. Závěry. Postprandiální plazmatické hladiny ghrelinu i obestatinu klesají v závislosti na kalorickém obsahu potravy u zdravých žen i u pacientek s bulimia nervosa.
Background. The recent identification of obestatin, a novel peptide hormone derived from the same gene as ghrelin, has added further complexity to ghrelin physiology. Despite the rapid progress, many questions remain unanswered, including the regulation of orexigen ghrelin and putative anorexigen obestatin secretion by food composition in humans. The present study was undertaken to investigate the influence of caloric and noncaloric food on plasma ghrelin and obestatin concentrations in healthy women (n = 6; age 23.83 ± 1.1 years; BMI 20.85 ± 0.87 kg/m2) and in bulimia nervosa patients (n = 6; age 26.6 ± 5.2 years; BMI 19.2 ± 1.44 kg/m2), characterized by abnormal eating behaviour and imbalance in energy homeostasis. Methods and Results. After overnight fasting, plasma ghrelin and obestatin were measured by commercial radioimmunoassay kits before and after consumption of soluble fiber alone or with glucose. In both groups plasma ghrelin and obestatin levels did not change after fiber alone, but decreased after fiber with glucose. During 30–90 min after ingestion we observed significant decrease (p < 0.05) of plasma ghrelin and obestatin levels after soluble fiber with glucose in healthy women and also in patients with bulimia nervosa, after then the levels of both hormones started to increase to preprandial levels. Conclusions. We conclude that postprandial ghrelin and obestatin plasma levels decrease in relation to caloric content of the meal in healthy women and in patients with bulimia nervosa.

Anorexia nervosa je charakterizována výraznými změnami v sekreci hormonů ovlivňujících příjem potravy, energetickou homeostázu a dlouhodobou regulaci tělesné hmotnosti. Cílem této studie bylo stanovit u pacientek s anorexia nervosa plazmatické hladiny neuropeptidu Y, ghrelinu a leptinu a změny těchto hormonů po 6týdenní částečné nutriční realimentaci, která probíhala během hospitalizace. Pacienti a metodika: Do studie bylo zařazeno 10 žen s anorexia nervosa restriktivního typu (body mass index 14,74 +/- 0,43; věk 23,3 +/- 1,0), diagnostikovaných podle DSM-IV a v době studie hospitalizovaných na jednotce specializované pro poruchy příjmu potravy, a 10 zdravých kontrolních žen (body mass index 21,45 +/- 0,72; věk 24,3 +/- 0,8). Bazální plazmatické hladiny neuropeptidu Y, ghrelinu a leptinu byly měřeny na počátku a konci léčby. Výsledky: U pacientek s anorexia nervosa byly zjištěny významně zvýšené bazální plazmatické hladiny neuropeptidu Y a ghrelinu ve srovnání s kontrolními ženami, hladiny leptinu měly tyto pacientky významně nižší. Po 6 týdnech léčby hladiny ghrelinu u pacientek s anorexia nervosa významně poklesly a hladiny leptinu se zvýšily. Plazmatické hladiny neuropeptidu Y se během léčby nezměnily, průměrný body mass index u pacientek s anorexia nervosa významně vzrostl. Závěr: Plazmatické hladiny ghrelinu a leptinu se u pacientek s anorexia nervosa měnily očekávaným směrem podle změny tělesné hmotnosti. Plazmatické hladiny neuropeptidu Y nevykázaly v tomto směru významnou změnu. Hladiny leptinu tak spolu s dlouhodobě zvýšenými hladinami neuropeptidu Y ukazují na přetrvávající dysregulaci mechanizmů řídících tělesnou hmotnost a apetit.
