OBJECTIVES: Positron emission tomography using 2-[fluorine-18]-fluoro-2-deoxy-D-glucose ((18) F-FDG) is considered to be the most beneficial imaging method for staging patients with non-Hodgkin's lymphoma (NHL). The intensity of (18) F-FDG accumulation may be determined by calculating the so-called standardised uptake value (SUV). The study aimed at assessing the benefit of SUV(max) determination in staging (18) F-FDG PET/CT in untreated patients with NHL. METHODS: One hundred and forty-nine initial staging (18) F-FDG PET/CT scans performed in patients with NHL between January 2007 and August 2009 were assessed, and the SUV(max) was determined. RESULTS: The highest mean and median values of SUV(max) were observed in patients with diffuse large B-cell lymphoma (DLBCL), the lowest mean and median values were found in small lymphocytic lymphoma. The overlap in SUV(max) < 10 between DLBCL and the other subgroups of NHL was very significant. Statistically, no correlation was found between the lactate dehydrogenase and SUV(max) values. On the other hand, a correlation of the Ki-67 proliferative index of tumour cells and SUV(max) was revealed (r = 0.409, P < 0.001). The geometric mean of SUV(max) in patients with Ki-67 ≤ 60 and those with Ki-67 > 60 was 8.8 and 14.3, respectively (P < 0.001). CONCLUSIONS: The results confirm that SUV(max) is not beneficial for making a more precise diagnosis in most patients with NHL. Correlation of SUV(max) with the Ki-67 values suggests that SUV(max) might have a prognostic values in NHL.

Positron emission tomography (PET) using 2-[fluorine-18]-fluoro-2-deoxy-D-glucose (18F-FDG) combined with computed tomography (CT) represents a three-dimensional imaging method suitable for staging in patients with non-Hodgkin's lymphomas (NHLs). The aim of our prospective multicenter study was to assess the value of initial PET/CT as compared with CT and PET alone for determining the stage and extent of the disease. A total of 122 patients with newly diagnosed NHL were examined using PET/CT. Four patients with resected lymphoma lesion and negative PET/CT were therefore excluded from the study. Of the remaining 118 cases, a total of 117 (99%) were described as 18F-FDG-avid. When compared with PET/CT, CT and PET showed very good sensitivity of lymph node imaging (97% and 100%, respectively); the specificity, however, was significantly lower (66.7% and 94.4%, respectively; p=0.0001). When detecting organ lesions, the sensitivity of CT and PET was lower than that of PET/CT (92.5% and 96.3%, respectively; p=0.0001); specificity was significantly decreased in CT and a little lower in PET (59.5% and 91.9%; p=0.0001). When compared with CT alone, PET/CT changed staging of the disease in 11 patients (9%) and was able to detect a total of 82 discrepancies in 67 of the 117 patients (57%). In conclusion, PET/CT is a new standard in imaging the involvement of lymph nodes and extranodal organs in NHL patients regardless of their histopathological types. Both sensitivity and specificity of the examination are higher than those of CT as well as PET alone.

Pozitronová emisní tomografie (PET) s použitím 2-[18F] fluoro-2-deoxy-D-glukózy (18F-FDG) představuje neinvazivní trojrozměrnou vyšetřovací metodu vhodnou ke vstupní diagnostice a stážování ne-hodgkinských lymfomů (NHL), stejně tak jako k hodnocení léčebné odpovědi či k ověření relapsu choroby. 18F-FDG však není látka specificky metabolizovaná pouze buňkami lymfomu, ale její utilizace je zvýšena při různých nenádorových procesech nebo u řady neoplázií s vystupňovanou glykolýzou. Integrované 18F-FDG PET/CT systémy dnes sice umožňují precizní lokalizaci léze akumulující 18F-FDG a zvyšují tak senzitivitu a specificitu vyšetření, ale potenciální omyly nebo obtížnosti v interpretaci nálezů vyžadují zvýšenou pozornost a těsnou spolupráci mezi radiologem, lékařem nukleární medicíny a hematologem. V naší práci popisujeme soubor 20 pacientů s léčeným NHL, kteří měli pozitivní nález při 18F-FDG PET/CT vyšetření v průběhu terapie, po jejím skončení nebo při podezření na relaps choroby a u nichž byla detailní analýzou, delším pozorováním či biopticky prokázána neočekávaná diagnóza. 14 pacientů mělo provedenu tkáňovou biopsii s následným histopatologickým vyšetřením, u 6 nemocných byla diagnóza stanovena kombinací anamnézy, PET/CT snímků, paraklinických vyšetření a popřípadě dalšího sledování nemocného. U 13 pacientů se zvýšenou akumulací 18F-FDG byl nalezen zánět infekčního či neinfekčního původu, u 5 nemocných bylo prokázáno jiné nádorové onemocnění (1 maligní a 4 benigní nádory) a u zbylých 3 nemocných byl popsán jiný nález (hyperplazie thymu u 2 pacientů a koloidní eufunkční struma u 1 nemocné). Pomocí prezentace uvedených případů chceme upozornit na to, že pozitivní nálezy při 18F-FDG PET/CT vyšetření u pacientů v průběhu terapie nebo po jejím skončení musí být velmi pozorně společně interpretovány radiologem, lékařem nukleární medicíny a hematologem a u případů s podezřením na výskyt jiného onemocnění než lymfomu by měla být včas indikována biopsie takovéto tkáně. Tento individuální přístup sníží riziko potencionálních diagnostických a léčebných pochybení.
2-[18F] fluoro-2-deoxy-D-glucose (18F-FDG) positron emission tomography (PET) is a noninvasive, 3-dimensional imaging modality sufficiently reliable for the diagnosis and initial staging, for the evaluation of therapeutic response and for the detection of recurrence of various types of non-Hodgkin's lymphoma (NHL). 18F-FDG is not a tracer absolutely specific for lymphoma and its uptake is increased in various benign conditions or malignancies with enhanced glycolysis. Integrated 18F-FDG PET/CT systems provide precise localization of the 18F-FDG-avid lesions and increase sensitivity and specificity of the examination but potential pitfalls and interpretation difficulties still require awareness and close cooperation of radiologist, nuclear medicine physician and hematologist. We report 20 patients with previously treated NHL who presented positive 18F-FDG PET/CT scans during therapy, at the end of therapy or in suspected recurrence of lymphoma, where unsuspected diagnosis was finally confirmed. 14 patients underwent tissue biopsy and subsequent histopathological evaluation. The final diagnosis was based on analysis of PET/CT scans and follow-up only in 6 patients. Causes of increased 18F-FDG accumulation: inflammation or infection in 13 patients, tumors in 5 patients (4 benign, 1 malignant) and other diagnoses in 3 patients (thymus hyperplasia in 2 patients, colloid thyreopathy without functional abnormalities in one patient). We suggest that positive findings on 18F-FDG PET/CT scans during or after therapy in NHL patients must be carefully interpreted by radiologist, nuclear medicine physician and hematologist and in cases of inconclusive results tissue biopsy and histological confirmation should be carried out. Such individual approach may diminish potential diagnostic and therapeutic mistakes.
