PURPOSE OF THE STUDY The method of stabilising the osteoporosis-related spinal fractures using an expandable stent (vertebral body stent - VBS) and bone cement is generally accepted for its minimal invasiveness, a low risk of complications and confirmed analgesic effect. The efficiency of reduction of a compressed vertebra is, however, still discussed in the literature. Our hypothesis was that the stent expansion in the vertebral body can achieve a statistically significant reduction in the anterior, middle and posterior part of the vertebra. MATERIAL AND METHODS The patients in whom the VBS technique was used to treat an osteoporosis-related fracture of Th/L spine in the period 2010-2014 were included in the study, namely 29 patients with 31treated fractures. The following radiographic parameters were monitored - anterior, middle and posterior vertebral body height, kyphotic angle between the upper and lower endplates of the vertebral body. Also, the radiation burden, painfulness according to VAS score and occurrence of complications were monitored. The minimum follow-up was 12 months. RESULTS The mean height of the anterior part of the vertebral body before the surgery, on the 1st postoperative day and at 6 weeks, 3 months, 6 months and 12 months was 23.5/25.4/23.9/23.6/23.6 mm respectively. The mean height of the middle part of the vertebral body at the same intervals was 17.9/24.0/23.4/22.9/22.9/22.9 mm. The mean height of the posterior part of the vertebral body was 29.6/29.8/29.4/29.3/29.2/29.2 mm. The kyphotic angle between the endplates of the vertebral body was 8.6/5.3/7.4/7.9/8.0/8.0°. The mean VAS values were 8.2/2.4/2.0/1.9/1.8/1.7. The mean duration of surgery was 54.3 minutes. The mean time of fluoroscopy was 33.4 s. The mean radiation dose was 443.1 cGycm2. The observed complications comprised 2 cases of dilatation balloon rupture, one case of a failure to expand the stents in a healed fracture due to incorrect indication. In total, four cases of cement leakage outside the vertebral body were reported, always with no clinical response. DISCUSSION All the authors agree that the method brings immediate analgesic effect, comparable to kyphoplasty or vertebroplasty. In our cohort, good reduction ability of the implant in the middle part of the vertebral body was confirmed. This has been confirmed also by other authors. Nonetheless, at a longer time interval the loss of correction was observed, which was reported only by Hartmann (5). The other authors mostly did not take into account the longer-term outcomes. CONCLUSIONS The hypothesis that the stent is capable of a statistically significant reduction in the anterior, middle and posterior portion of the vertebral body was only partially confirmed. A statistically significant reduction was seen only in the middle portion of the vertebral body. In the anterior portion of the vertebral body the reduction was demonstrable only immediately after the surgery, while the later follow-up checks revealed the loss of correction. The statistically significant improvement of the kyphotic angle between the endplates of the injured vertebra was also seen only on the first postoperative radiographs and at later follow-up checks the improvement was no more significant. The change in the height of the posterior portion of the vertebral body was not statistically significant at any of the follow-up intervals. PURPOSE OF THE STUDY In the management of dens axis fractures in patients older than 65 years of age the posterior approach is preferred due to osteoporosis and the risk of a failure of anterior osteosynthesis. The posterior approach, however, is associated with a higher incidence of complications. A combination of anterior transarticular fixation of C1/2 (ATS) with compression osteosynthesis of dens axis significantly increases the stability of osteosynthesis. MATERIAL AND METHODS In the period from 2009 to 2015 our hospital admitted 13 patients older than 65 years of age with a diagnosed type III dens axis fracture based on AO classification. 8 patients sustained a dens axis fracture combined with a stable atlas fracture. The cohort consisted of 13 women aged 67 to 90 years, with the mean age of 82.3 years. None of the female patients were affected neurologically. The dens axis fracture was treated by anterior approach. Once the screw was inserted in the dens axis, two more screws were added, the entrance points of those screws were medial and lateral to the odontoid screw and direction was divergently via C1/2 joints in order to reinforce stability. The patients were monitored at 6-week, 3-, 6- and 12-month follow-ups. Bone healing was confirmed by CT scan. RESULTS No complications were recorded during the surgery in any of the 13 patients. In one female patient the stabilization failed in the early postoperative period. A reoperation from anterior approach with the use of the same method was necessary. In eleven patients bone healing occurred after 6 to 12 months. In two patients pseudoarthrosis was formed with no clinical symptoms. No neurological deterioration or a patient s death was reported in the monitored period within 12 months after the treatment. DISCUSSION Where dens axis fractures in elderly patients are managed operatively, the posterior approach and transarticular fixation of C1/2 with sublaminar loop are preferred. This procedure is considered more reliable, owing to the fears of a failure of osteosynthesis by anterior approach. The most frequent cause of a failure is the pulling out of the screw through the anterior wall of dens axis base in patients with osteoporosis. Concurrently, the anterior approach is regarded as more considerate. The ATS fixation increases stability compared to isolated compression osteosynthesis, which makes it possible to manage the dens axis fractures by a more considerate anterior approach. CONCLUSIONS The ATS is a method that increases stability of compression osteosynthesis in managing dens axis in elderly patients. Cíl práce: Retrospektivní hodnocení výsledků operačního léčení, komplikací a funkčních výsledků u pacientů nad šedesát let věku se zlomeninami distálního humeru hodnocených podle AO klasifikace. Materiál a metodika: Od ledna 2003 do prosince 2012 bylo v Úrazové nemocnici v Brně léčeno 145 pacientů starších šedesáti let se zlomeninami distálního humeru otevřenou repozicí a vnitřní fixací. 116 pacientů bylo klinicky a radiologicky sledováno po dobu nejméně 18 měsíců. Všichni sledovaní pacienti byli v době úrazu starší šedesáti let. Průměrný věk byl sedmdesátdva let (61-89 let) . Mužů bylo 34 a žen 82. Zlomenin typu C bylo 74, typu B 38 a extraartikulárních zlomenin typu A bylo 14. Všichni pacienti zařazení do souboru byli léčeni otevřenou repozicí a vnitřní fixací. Dorzální přístup byl volen u většiny zlomenin typu A a u všech typu C. Osteotomie olekranu byla provedena u zlomenin typu C u 66 pacientů. Funkční výsledky byly hodnoceny podle modifikace Mayo Elbow Score. Výsledky: Při hodnocení funkčních výsledků mělo 23 pacientů výborné výsledky. U 44 pacientů dobré a u 34 uspokojivé. Průměrně bylo skóre 82 bodů (od 43 do 100 bodů). U 15 pacientů byly hodnoceny výsledky jako špatné. Průměrný rozsah pohybů lokte byl 100° (od 60 do 135°) ve smyslu flexe a extenze a 110° pro pronosupinaci (60–170°). Nejčastější pooperační komplikací bylo selhání implantátu (34 pacientů). Jednalo se o uvolnění šroubů, které si vyžádalo v 18 případech revizní operaci. Ankylózu lokte jsme v souboru nezaznamenali. V souboru jsme 9krát sledovali pakloub z toho 4krát po osteotomii olekranu. V šesti případech byla provedena rekonstrukční operace pakloubu. Závěr: Léčení zlomenin distálního humeru u starších pacientů je složité a nejednotné. Obtížnost léčení těchto zlomenin podtrhuje metafyzální kominuce, osteoporotická kost a špatná tolerance imobilizace lokte. Až 30% výskyt komplikací u zlomenin distálního humeru po šestém deceniu věku pacienta je toho důkazem. Za standard se dnes považuje otevřená repozice a fixace obou pilířů distálního humeru dvěma úhlově stabilními dlahami.
Objective: A retrospective clinical evaluation of surgical treatment and complications after distal humeral fractures in patients older than age 60 treated by open reduction a internal fixation by two plates. Material end methods: Between January 2003 and December 2012 in the Traumatological hospital Brno we treated 145 patients by open reduction and internal fixation. In 116 patients, a clinical and radiological follow up was obtained after a minimum follow up of 18 months. The age distribution was from 61 to 89 with an average of 72 years Thirty-four patients were male, eighty-two female. Fracture types were classified according to the AO classification. Type C fractures documented in 74 cases, type B in 38 and extraarticular fractures type A were seen in 14 cases. For all type A and C was used dorsal approach. For type B was used dorsal or lateral approach. Extraarticular olecranon osteo-tomy was routinely performed in type C (66 patients). The functional results were evaluated by Mayo Elbow Score. Results: According Mayo Elbow Score we found 23 patients with excellent results, 44 patients with good, 34 with fair and 15 patients with poor results. An average score was 82 points (range, 43 to 100 points). The median arc of motion was 100° (range, 60 to 135°) of flexion and extension and 110° pronation - supination (range, 60–170°). In none of the patients ankylosis of the elbow was found. Postoperative complication rate was high, predominantly seen as implant failure or screw loosening (34 patients). In this study, there were nine non-unions, including four non-unions after olecranon osteotomy, requiring revision. Conclusion: Management of distal humerus fractures in elderly patients is still controversy. Severe comminution, bone loss and osteopenia predispose to unsatisfactory results. Over 30 % of such fractures develop significant complications during treatment. Standard surgical techniques are used for fixation of both columns, using a locking compression plates.

ÚVOD: Zlomeniny proximálního konce stehenní kosti jsou již dlouhodobě vnímány jako celospolečenský a ekonomický problém stárnoucí populace ve vyspělých zemích. Metodou volby při jejich léčení je neodkladná operace - osteosyntéza nebo náhrada kyčelního kloubu. MATERIÁL A METODA: Náš soubor tvořilo 145 pacientů (54 mužů) starších 50 let s izolovaným poranění proximálního femuru, hospitalizovaných v průběhu roku 2008 v Úrazové nemocnici v Brně. Sledovali jsme vztah mezi volbou operačního či konzervativního řešení vzhledem k danému typu zlomeniny a zastoupení časných komplikací u jednotlivých způsobů léčby. VÝSLEDKY: Ze 138 operovaných pacientů byla osteosyntéza volena u 82 (57 %) a náhrada kyčelního kloubu u 56 (38 %). Konzervativně bylo léčeno 7 (5 %) pacientů. Operační výkon v den přijetí do nemocnice byl proveden u 68 (83 %) pacientů s osteosyntézou a u 23 (41 %) pacientů s endoprotézou. Převážná většina subkapitálních zlomenin byla ošetřena náhradou kyčelního kloubu. Bazicervikální zlomeniny byly nejčastěji řešeny pomocí DHS, u per- a intertrochanterických zlomenin převažovalo použití PFN. Během hospitalizace jsme zaznamenali celkem 2 úmrtí, přičemž nejčastější pooperační komplikace byly kardiopulmonální. ZÁVĚR: Časná intervence je rozhodující u intrakapsulárních zlomenin v případě, že volíme záchovnou osteosyntézu. V ostatních případech má zásadnější význam pro eliminaci pooperačních komplikací důsledná předoperační příprava pacienta.
INTRODUCTION: Hip fractures are persistent social and economic problem in the aging population in developed countries. Immediate operative treatment - osteosynthesis or total hip arthroplasty are the methods of choice. METHODS: Our patient population consisted of 145 subjects (54 men) over 50 years of age with the isolated proximal femur fracture who were admitted Trauma Hospital in Brno during the year 2008. Our aim was to investigate the relationship between the fracture type and the choice of the treatment, we also studied the complications and mortality rate. RESULTS: Among 138 patients who underwent the surgery, in 82 (57 %) osteosynthesis and in 56 (38 %) total hip arthroplasty was performed. Conservative treatment was chosen in 7 (5 %) patients. Same day surgery was performed in 68 (83 %) patients with osteosynthesis and 23 (41 %) patients with arthroplasty. Most of the subcapital fractures were treated with the hip arthroplasty. Basicervical fractures were mainly treated with the DHS (dynamic hip screw), while in the intertrochanteric fractures the use of PFN (proximal femoral nail) was dominant. During the hospitalisation two deaths was reported, the most common postoperative complications were cardiopulmonal. CONCLUSION: Early intervention is crucial in those intracapsular fractures that are treated with osteosynthesis. In other cases, strict preoperative preparation of the patient is much more important and contributes to the elimination of the postoperative complications.
