Článek si klade za cíl zjistit, jakým způsobem přistupovali k tišení bolesti během operace lékaři před objevem éterové anestezie. Z dostupných pramenů se zdá, že ačkoliv chirurgické postupy jsou staré nejméně 6 tisíc let, snaha nalézt metodu, jak potlačit bolest s nimi spojenou, se v evropské a islámské kultuře objevuje až ve středověku, a to spíše sporadicky. Podobně zmínky o používání anestezie některými lékaři starověké Indie a Číny jsou pravděpodobně spíše mýty než realita. Zdá se, že současná anesteziologická mantra o pradávné snaze lidstva potlačit bolest během chirurgického ošetření neplatí a spíše lze věřit tvrzení, že tehdejší lidé v porovnání s obdobím po nástupu humanismu a rozvoji věd byli k bolesti více tolerantní, obzvláště pokud se týkala někoho jiného. Ačkoliv již od starověku byly známé rostlinné výtažky použitelné k tišení bolesti, byly omezené na konzervativní léčbu různých onemocnění, nikoliv na anestezii během operace a základním postupem byla fixace pacienta. Ve článku jsou citované údaje o zachovaných lékařských textech s možnou souvislostí použití anestezie od starověkých kultur Mezopotámie, Egypta, Číny a Indie přes antické Řecko a Řím, středověké a islámské státy a Evropu až do 19. století.
The aim of this article is to investigate how pain management during surgery was approached by physicians before the advent of ether anaesthesia. From the available sources, it appears that although surgical procedures are at least 6,000 years old, efforts to find a method to suppress the pain associated with them only appeared in European and Islamic culture in the Middle Ages, and then only sporadically. Similarly, references to the use of anaesthesia by some doctors in ancient India and China are probably more myth than reality. The current anaesthetic mantra about mankind’s ancient efforts to suppress pain during surgical treatment does not seem to apply, and the claim that people of that time, compared with the period after the advent of humanism and the development of science, were more tolerant of pain, especially when it involved someone else, is more plausible. Although plant extracts had been known to be used for pain relief since antiquity, they were limited to conservative treatment of various ailments, not anaesthesia during surgery, and the basic procedure was to fix the patient. The article cites data on extant medical texts with possible links to the use of anaesthesia from the ancient cultures of Mesopotamia, Egypt, China and India through ancient Greece and Rome, medieval and Islamic states and Europe up to the 19th century.

Článek podává přehled využití kurare od prostředku k lovu až po použití v medicíně jako svalového relaxancia a přehled dalších látek, které ho následovaly. Jsou uvedeny i osoby, které se do této historie významně zapsaly. Dále je popsán i vývoj antidot svalových relaxancií, objev nikotinového acetylcholinového receptoru a využití a zneužití svalových relaxancií mimo operační sály.
The article gives an overview of the use of curare from a poison of hunting arrows to its use in medicine as a muscle relaxant and a review of other substances that followed. People who have made a significant contribution to this history are listed. The development of muscle relaxant antidotes, the discovery of the nicotinic acetylcholine receptor, and the use and abuse of muscle relaxants outside of operating rooms are described.

PURPOSE OF THE STUDY Many physicians believe that loco-regional anaesthesia and analgesia improve the postoperative course of patients indicated for total hip arthroplasty compared to general anaesthesia. However, there are many patients who refuse subarachnoid or epidural anaesthesia, or have contraindications or conditions making the use of such techniques impossible. An alternative option is the combination of general anaesthesia and a peripheral nerve blockade. The aim of this prospective randomized open-label clinical trial was to compare the efficacy and quality of postoperative analgesia between fascia iliaca block combined with general anaesthesia (GA) and subarachnoid anaesthesia with morphine and bupivacaine (SAB). MATERIAL AND METHODS After having obtained the ethics committee approval and the patients consent, a prospective, open-label, randomized trial was conducted in patients referred for total hip arthroplasty (THR). The GA group was administered ultrasound-guided fascia iliaca block with 40 ml of 0.25% bupivacaine solution after the induction of general anaesthesia. In the SAB group, subarachnoid blockade was performed with a mixture of 3 ml of 0.5% bupivacaine with 0.150 mg morphine prepared in the hospital pharmacy. Right after surgery the patients were taken to the ICU for 24 hours, after which they were transferred to a general ward. In addition to vital signs monitoring, pain intensity using a 0-10 numeric rating scale (NRS), first morphine administration at NRS >4, total morphine consumption and potential adverse effects were observed over the period of 72 hours. RESULTS There was no statistical difference between the GA (14 persons) and the SAB (14 persons) group in demographic parameters, time to first morphine administration (10 hrs vs. 19 hrs, p=0.10), number of persons with no need for morphine after surgery (5 vs. 7), tingling sensation (1 vs. 0) or numbness of the limb (0 vs. 1). There was no difference in cardiorespiratory parameters or side effects of therapy. In neither case was there respiratory depression or delayed rehabilitation. No patient developed delirium after surgery, and no patient reported dissatisfaction with pain management. DISCUSSION The fascia iliaca block and subarachnoid anaesthesia using local anaesthetic with opioid addition have been repeatedly published for patients after total hip arthroplasty, but this study is unique by comparing the two methods. The study added a new piece of knowledge to the findings of several recent meta-analyses on the comparable outcomes of general and subarachnoid anaesthesia for hip replacement in the perioperative period. CONCLUSIONS If subarachnoid anaesthesia cannot be used in hip arthroplasty, general anaesthesia with fascia iliaca block provides comparable analgesia and quality of postoperative course. Kazuistika popisuje retrospektivní případ bdělosti během anestezie, jehož následky se projevují ještě 35 let po této příhodě. Přestože se tato komplikace vyskytuje vzácně, může mít dlouhodobé účinky. Je podán přehled výskytu, rizikových faktorů, možností prevence a postupu, co dělat, pokud se bdělost během anestezie vyskytne.
This case report describes a retrospective case of awareness during anesthesia, the consequences of which are still manifested 35 years after the event. Although this complication is rare, it can have long-term effects. An overview of the incidence, risk factors, prevention options, and what to do if awareness during aaesthesia occurs is presented.
