PURPOSE: Primary plasma cell leukemia (PCL) is the most aggressive monoclonal gammopathy. It was formerly characterized by ≥ 20% circulating plasma cells (CTCs) until 2021, when this threshold was decreased to ≥ 5%. We hypothesized that primary PCL is not a separate clinical entity, but rather that it represents ultra-high-risk multiple myeloma (MM) characterized by elevated CTC levels. METHODS: We assessed the levels of CTCs by multiparameter flow cytometry in 395 patients with newly diagnosed transplant-ineligible MM to establish a cutoff for CTCs that identifies the patients with ultra-high-risk PCL-like MM. We tested the cutoff on 185 transplant-eligible patients with MM and further validated on an independent cohort of 280 transplant-ineligible patients treated in the GEM-CLARIDEX trial. The largest published real-world cohort of patients with primary PCL was used for comparison of survival. Finally, we challenged the current 5% threshold for primary PCL diagnosis. RESULTS: Newly diagnosed transplant-ineligible patients with MM with 2%-20% CTCs had significantly shorter progression-free survival (3.1 v 15.6 months; P < .001) and overall survival (14.6 v 33.6 months; P = .023) than patients with < 2%. The 2% cutoff proved to be applicable also in transplant-eligible patients with MM and was successfully validated on an independent cohort of patients from the GEM-CLARIDEX trial. Most importantly, patients with 2%-20% CTCs had comparable dismal outcomes with primary PCL. Moreover, after revealing a low mean difference between flow cytometric and morphologic evaluation of CTCs, we showed that patients with 2%-5% CTCs have similar outcomes as those with 5%-20% CTCs. CONCLUSION: Our study uncovers that ≥ 2% CTCs is a biomarker of hidden primary PCL and supports the assessment of CTCs by flow cytometry during the diagnostic workup of MM.
Amyloidosis is a rare metabolic disorder primarily brought on by misfolding of an autologous protein, which causes its local or systemic deposition in an aberrant fibrillar form. It is quite rare for pulmonary tissue to be impacted by amyloidosis; of the three forms it can take when involving pulmonary tissue, nodular pulmonary amyloidosis is the most uncommon. Nodular pulmonary amyloidosis rarely induces clinical symptoms, and most often, it is discovered accidentally during an autopsy or via imaging techniques. Only one case of nodular pulmonary amyloidosis, which manifested as a spontaneous pneumothorax, was found in the literature. In terms of more precise subtyping, nodular amyloidosis is typically AL or mixed AL/AH type. No publications on AH-dominant type of nodular amyloidosis were found in the literature. We present a case of an 81 years-old male with nodular pulmonary AH-dominant type amyloidosis who presented with spontaneous pneumothorax. For a deeper understanding of the subject, this study also provides a review of the literature on cases with nodular pulmonary amyloidosis in relation to precise amyloid fibril subtyping. Since it is often a difficult process, accurate amyloid type identification is rarely accomplished. However, this information is very helpful for identifying the underlying disease process (if any) and outlining the subsequent diagnostic and treatment steps. Even so, it is crucial to be aware of this unit and make sure it is taken into consideration when making a differential diagnosis of pulmonary lesions.

V posledních letech se pro diagnostiku i pro celý průběh léčby dostává do popředí celotělová zobrazovací metoda pozitronová emisní tomografie (PET) kombinovaná s výpočetní tomografií (PET/CT). Její výhodou je zachycení metabolicky aktivních charakteristických ložisek a dobrá detekce extramedulárních i paramedulárních lézí. Následující text shrnuje společné stanovisko odborníků České společnosti nukleární medicíny ČLS JEP z. s. (ČSNM ČLS JEP) a České myelomové skupiny (CMG) jak k provedení vyšetření PET/CT, tak k jeho hodnocení a reportování. Cílem je sjednotit metodiku vyšetření PET/CT napříč pracovišti v ČR, která bude srozumitelná a jednoznačná.

In recent years, whole-body positron emission tomography (PET) imaging combined with computed tomography (PET/CT) has become a preferred imaging technique for diagnosis and treatment. It has the benefit of detecting metabolically active characteristic focuses and good detection of extramedullary and paramedullary lesions. The following text presents the collective opinion of the representatives of the Czech Society of Nuclear Medicine ČLS JEP z.s. (CSNM ČLS JEP) and the Czech Myeloma Group (CMG) on PET/CT as well as on its evaluation and reporting. The aim is to standardize the methodology of PET/CT examination across the Czech Republic, which will be clear and simple to understand.

Cíl: COVID-19 je vysoce nakažlivé virové onemocnění se signifikantní mortalitou. Jednu z nejohroženějších skupin představují pacienti s krevním nádorovým onemocněním. V našem článku hodnotíme průběh onemocnění, souvislost komorbidit a tíže průběhu a schopnost sérokonverze u skupiny pozitivních pacientů naší kliniky. Metody: Provedli jsme 1 600 diagnostických nazofaryngeálních PCR stěrů v období 8 měsíců. Zavedli jsme soubor preventivních opatření za účelem ochrany pacientů i personálu. Zaměřili jsme se na kohortu 50 pozitivních pacientů, u kterých jsme zkoumali průběh onemocnění, vliv přidružených rizikových faktorů, základní diagnózy a u 15 pacientů také schopnost sérokonverze. Výsledky: Ke snížení šíření nákazy se ukázala být zásadními striktní bariérová opatření, zejména u pacientů podstupujících autologní transplantaci krvetvorby. Nezaznamenali jsme žádné ohnisko nákazy na naší klinice a pouze jeden pozitivní případ v peritransplantačním období. Ve sledovaném souboru 50 nemocných zemřelo na virem SARS-CoV-2 způsobené komplikace 14 % (7/50) nemocných. Nejčastějšími komorbiditami byla arteriální hypertenze či jiné kardiovaskulární onemocnění, diabetes mellitus 2. typu a renální insuficience. Dvě třetiny pozitivních pacientů byly v 1. linii léčby. Neprokázali jsme hypogamaglobulinémii jako rizikový faktor těžkého průběhu choroby, stejně jako jsme nepozorovali její negativní vliv na dosažení sérokonverze. Závěr: Preventivní opatření mají význam k zabránění šíření nákazy virem SARS-CoV-2 zejména u pacientů s krevním nádorovým onemocněním. Striktní bariérová opatření a pravidelné testování nemocných i personálu byly nejúčinnějšími opatřeními v prvním roce pandemie, kdy nebyly dostupné vakcíny ani účinné léky. Nemocní s krevními nádory patří jednoznačně k vysoce rizikovým osobám z důvodu vážných, až fatálních komplikací souvisejících s infekcí virem SARS-CoV-2.
Aim: The novel coronavirus (SARS-CoV-2) is a highly contagious disease with a significant mortality rate. Haematological patients are among those most at risk. We evaluate here the disease course, association between comorbidities and COVID-19 severity and seroconversion potential in 50 positive patients at our clinic. Methods: We performed 1,600 diagnostic PCR nasopharyngeal swabs over a period of 8 months. We introduced a set of preventive measures so as to protect our patients and personnel. In 50 COVID-19 positive patients, we closely evaluated the course of the disease, the impact of underlying risk factors and the principal haematological diagnoses. We also evaluated the potential for seroconversion in 15 COVID-19 positive patients. Results: Strict barrier measures, especially in patients undergoing autologous stem cell transplantation, have been shown as being crucial for reducing the spread of disease. We did not record any disease outbreak and registered only one positive case during the peri-transplant period at our facility. The most common comorbidities were arterial hypertension or other cardiovascular disease, type 2 diabetes and renal impairment. Two-thirds of positive patients were on first line treatment. Hypogammaglobulinemia did not prove to be a risk factor for a severe COVID-19 course and we did not observe it to be an obstacle for seroconversion. Conclusion: Preventive measures are significant for reducing the spread of disease, especially in haematology centres. In our single centre experience, we report a mortality of 14%. Although we report a relatively small cohort and much remains yet to be clarified, our results can even now be implemented in daily practice.
