Primárna pľúcna hemosideróza je raritné intersticálne ochorenie s udávanou incidenciou 6,3 detí na 1 000 000 detských pacientov v Slovenskej republike. Ochorenie má chronický charakter s obdobiami relapsov a remisie. Etiológia nie je jasná, predpoklad dysfunkcie imunitného systému naznačuje autoimunitný pôvod. Určenie diagnózy je založené na kombinácii recidivujúcej hemoptýzy v klinickom obraze, anémie v laboratórnych odberoch a pľúcnych infiltrátov na RTG vyšetrení pľúc. Vzhľadom na neskorú diagnostiku a progresívny priebeh, sa prognóza nepovažuje za priaznivú. Od začiatku vzniku našej nemocnice sa u nás objavili dva prípady idiopatickej pľúcnej hemosiderózy. Prvý v roku 2004, v liečbe bola použitá aplikácia rekombinantného ľúdskeho erytropoetínu. Druhý sa vyskytol o 13 rokov neskôr – v roku 2017. Autori v nasledujúcom článku približujú obidve kazuistiky. Došlo k zmene v pohľade na hemosiderózu za uplynulé obdobie?
Idiopathic pulmonary hemosiderosis is rare inetersticial lung disease, the incidence is 6.3 children on million of childhood patients in Slovak Republic. It is chronic disease with period of relapse and remission. The ethiology is not clear, probably the immune system is dysfunctional, indicating the autoimmune origin of the disease. Diagnosis is based on combination of reccurent hemoptysis , anemia in laboratory samples, pulmonary infiltrates on chest X-ray. Due to late diagnosis and progression, the prognosis is not favorable. Since the establishment of our hospital, there have been two cases of idiopathic pulmonary hemosiderosis. The first one in 2004, in treatment was used human recombinant erytropoetin. The second one ocuured 13 years later – in 2017. In the next article approach the authors the two case studies. Has ist changed something in the view of hemosiderosis over the past period?

Respiračné infekcie sú jednou z najčastejších príčin návštevy u lekára prvého kontaktu pre detskú aj dospelú populáciu. Navyše sú jednou z hlavných príčin hospitalizácie detí, seniorov a imunokompromitovaných pacientov. Časť z nich je preventabilná a vakcíny a antibiotiká výrazne znížili chorobnosť a úmrtnosť napr. na pneumóniu, epiglotitídu a invazívne baktériové a vírusové ochorenia. Najčastejším príznakom respiračných infekcií je kašeľ, pričom na jeho liečbu sa okrem medikamentóznej odporúča aj alternatívna terapia. Účinná je tiež fytoterapia a komisia pri EMA (European Medicines Agency) v Londýne, ktorá sa zaoberá rastlinnými liečivými produktmi (HMCP – Committee on Herbal Medicinal Products) uviedla vo svojej ostatnej monografii rastliny, ktoré sa používajú ako zdroj liečivých extraktov pre liečbu nachladnutia a kašľa pre detskú populáciu.
Respiratory infections are one of the most frequent causes to see a general practitioner for children and also for adults. What is more they are one of the main reasons for children, seniors and immunocompromised patients to be hospitalized. Part of them is preventable and vaccines and antibiotics significantly lowered morbidity and mortality e.g. for pneumonia, epiglottitis and invasive bacterial and viral diseases. The most frequent symptom of respiratory infections is cough while for its treatment besides medicamentous also alternative therapy is recommended. Phytotherapy is also effective and the committee at EMA (European Medicines Agency) in London, which deals with herbal healing products (HMPC – Committee on Herbal Medicinal Products), released in its latest monograph list of herbs which are used as a source of healing extracts for treatment of cold and cough in children population.

Many severe diseases of the respiratory tract lead to hospitalisation. These diseases are often caused by viral infections and may cause increased mortality. The most common viral pathogens involved in these cases, which are also associated with significant morbidity and mortality during the influenza seasons are influenza viruses. Rapid differential diagnosis of influenza viruses is therefore of great importance. Classical diagnosis of these viruses involves virus cultures. Of the rapid diagnostic methodologies which have been developed are RT-PCR, multiplex PCR, real-time PCR. In the present study we have monitored clinical samples from patients of different age groups from selected regions in Slovakia and compared the effectiveness of the classical and molecular biological diagnostic methods. The molecular biological methods proved to be rapid, accurate and effective. Application of these techniques in diagnosis of the respiratory illnesses should help in the prevention, therapy and disease control.
