Úvod a cíl: V posledních letech přibývá v ordinacích zubních lékařů pacientů s vývojovým postižením skloviny u molárů a řezáků. Jedná se o onemocnění systémového původu postihující jeden až čtyři první stálé moláry, které je často spojené s poškozením stálých řezáků. Toto onemocnění nemá zcela objasněnou etiologii. V literatuře jsou zmíněny vlivy vnějšího prostředí, v úvahu je třeba brát i vlivy genetické. U těchto pacientů nahrazujeme ztracené tvrdé zubní tkáně molárů, ale také zlepšujeme estetiku řezáků, na kterých se mohou nacházet opacity pokrývající téměř celou vestibulární plošku. Při volbě terapie se rozhodujeme na základě stavu chrupu, spolupráce a věku pacienta. Ke každému pacientovi musíme přistupovat individuálně a zvolit pro něj vhodnou terapii. Cílem tohoto článku je shrnout dostupné informace o etiopatogenezi, klasifikaci, histologických změnách, klinickém obrazu, prevenci a terapii molárové a řezákové hypomineralizace. Metodika: Pro tento přehledový článek byly využity odborné publikace z databází PubMed a Web of Science. Publikace byly vyhledávány v anglickém jazyce za pomoci klíčových slov: MIH, molár, řezák, molárová a řezáková hypomineralizace, DDE, hypomineralizace, opacita. Jako zdroje pro tento přehled byly využity články publikované mezi lety 2006 a 2021. Závěr: U molárové a řezákové hypomineralizace je důležitá včasná diagnostika a zavedení preventivních postupů tak, abychom zamezili možným komplikacím souvisejícím s tímto onemocněním. Navíc prevalence tohoto onemocnění v populaci narůstá a s pacienty postiženými molárovou a řezákovou hypomineralizací se setkáváme stále častěji.
Introduction, aim: The number of patients with affected enamel in molar incisor hypomineralization is increasing in the dental offices in recent years. It is a hypomineralization of systemic origin affecting at least one first permanent molar, often associated with affected permanent incisors. The etiology of this disease is not fully understood. The literature mentions influences of external environment and contemplates genetic factors. We reconstruct the hard dental tissues loss of permanent molars and improve the aesthetics of incisors in these cases. The therapy is based on the condition of the teeth, cooperation and age of the patient. We must approach each patient individually and choose the appropriate therapy for them. The aim of this article is to summarize the available information on etiopathogenesis, classification, histological changes and clinical picture, prevention and therapy of molar incisor hypomineralization. Methods: The PubMed and Web of Science databases were used to gather articles for this review. The publications were searched in English language using the key words: MIH, molar, incisor, molar incisor hypomineralization, DDE, hypomineralization, opacity. Articles published between 2006 and 2021 were used as sources for this review. Conclusions: Early diagnosis and implementation of preventive procedures are important to avoid possible complications related to molar incisor hypomineralization. Moreover, the prevalence of this desease in the population is increasing and patients affected by molar incisor hypomineralization are more frequently encountered in the dental office.
ABSTRACT: The aim of the study is to identify possible differences in the prevalence of biological stress markers (cribra orbitalia, linear enammel hypoplasia), dental caries, activity markers (Schmorl's nodes) and markers of violence (traumatic lesions) in populations from a typical graveyard around the first church (9(th)-10(th) century AD) in the stronghold and from settlement burials in the southern bailey (second half of the 9(th) - beginning of the 10(th) century AD) of Slavs from Pohansko (Czech Republic). Cribra orbitalia and enamel hypoplasia did not markedly differ in either population. On the other hand, co-occurrence of both markers was statistically different and distinction in higher age categories could indicate diverse living conditions. Lower cariousness in the population from the southern bailey was identified. The nature of postcranial traumatic lesions in both populations indicates rather their accidental origin. However, a higher incidence of cranial injuries points to more violent activities in the population from Pohansko compared to Mikulčice. A high number of healed face injuries in males from the southern bailey could indicate higher intragroup violence in this population.

Enamel hypoplasia and disruption of dentinogenesis are the most common abnormalities of development and mineralization of human teeth. Several reports are available in the literature on the influence of dietary calcium on the formation of human and rodent tooth; however, the information about the influence of dietary phosphorus on the tooth formation is scarce. The aim of the present investigation was to examine the chronic effect of high phosphorus diet and improper dietary calcium to phosphorus ratio on the mandibular incisor microstructure in a hystricomorph rodent--Octodon degu--using macroscopic observation, histopathological examination, transmission and scanning electron microscopy. The present study shows that enamel and dentin development is disturbed under high phosphorus diet and improper calcium to phosphorus ratio. Disturbed mineral metabolism resulted in enamel depigmentation, enamel hypoplasia, enamel pitting and altered dentin morphology. The results suggest that more attention should be focused on dietary phosphorus content when facing altered tooth structure in young patients with deciduous or permanent dentition. Furthermore, we showed that degus can be used as an experimental animal model for the study of the developmental teeth disturbances.

A macroscopic, microradiographic and scanning electron microscope study was performed on the structure of fluorosed dental enamel in red deer from a fluoride polluted region (North Bohemia, Czech Republic). As was revealed by analysis of mandibular bone fluoride content, the rate of skeletal fluoride accumulation in the fluorotic deer was about 6 times that in controls taken from a region not exposed to excessive fluoride deposition. In all fluorosed mandibles, the 1st molar was consistently less fluorotic than the other permanent teeth. This was related to the fact that crown formation in the M1 takes place prenatally and during the lactation period. Fluorosed teeth exhibited opaque and posteruptively stained enamel, reduction or loss of enamel ridges, moderately to grossly increased wear and, in more severe cases, also enamel surface lesions of partly posteruptive, partly developmental origin. Microradiographically, fluorosed enamel was characterised by subsurface hypomineralisation, interpreted as a result of fluoride interference with the process of enamel maturation. In addition, an accentuation of the incremental pattern due to the occurrence of alternating bands with highly varying mineral content was observed in severely fluorosed teeth, denoting fluoride disturbance during the secretory stage of amelogenesis. A corresponding enhancement of the incremental pattern was also seen in the dentine. The enamel along the more pronounced hypoplasias consisted of stacked, thin layers of crystals arranged in parallel, indicating that the ameloblasts in these locations had lost the distal (prism-forming) portions of their Tomes processes. The findings of the present study indicate that red deer are highly sensitive bioindicators of environmental pollution by fluorides.
