OBJECTIVES: To confirm the changes in the results of EMG assessment of lower-limb peripheral nerves in patients treated with statins in the longer follow-up period of 3 years. BACKGROUND: Long-term treatment with statins may have adverse effects: affection of muscles or peripheral nervous system. The frequency of affection of the peripheral nervous system has not been thoroughly investigated; our previous study showed the signs of peripheral nerve damage in the results of EMG assessment. DESIGN/METHODS: Forty-two patients (23 males, 19 females, mean age 51.9 and 52.3 years) with a definitive diagnosis of combined hyperlipidemia were studied. Other metabolic disorders or chronic ethanol abuse were excluded. Initial examinations included laboratory and neurophysiological measures (peroneal and tibial nerves: MNCV, CMAP, F-wave mean latency; superficial peroneal and sural nerve: SNCV, SNAP). Subsequently, treatment with simvastatin 20 mg daily was initiated. Patients were followed for 36 months with repeated neurophysiological examinations on 24 and 36 months after statin treatment initiation. RESULTS: None of the patients reported subjective symptoms typical for peripheral neuropathy. Neurophysiological examination of lower-limb peripheral nerves demonstrated statistically significant prolongation of F-wave mean latency on peroneal and tibial nerves (p<0.0001, paired t-test). CONCLUSIONS: The study confirmed that long-term treatment with statins caused a clinically silent but still definite damage to peripheral nerves when the treatment lasts longer than 2 years.

STUDY DESIGN: Clinical, neuroradiologic, and neurophysiologic description of 21 patients with compressive cervical spondylotic myelopathy (CCSM). OBJECTIVE: To describe the utility of cutaneous silent periods (CSPs) for functional evaluation of mild CCSM. SUMMARY OF BACKGROUND DATA: Electroneurography, electromyography, and somatosensory and motor evoked potentials (SEPs, MEPs) are routinely used for comprehensive functional neurophysiological evaluation of CCSM. CSPs have been reported in various intramedullary spinal cord lesions, however, they have not been systematically studied in mild CCSM. METHODS: We investigated 21 patients with multilevel CCSM as documented by magnetic resonance imaging. We recorded CSPs in thenar muscles after noxious digit II stimulation and compared them with median and tibial nerve SEPs and MEPs obtained from abductor digiti minimi and tibialis anterior muscles. Electroneurography and electromyography were obtained in affected myotomes. RESULTS: CSP onset and end latencies were delayed, and CSP duration was shortened, in CCSM patients. CSP abnormalities were present in 17 patients of whom all, but 1 presented with intramedullary magnetic resonance imaging hyperintensity. All 11 limbs affected by hypalgesia and thermhypesthesia had abnormal CSPs, whereas no spinothalamic deficit was noted in any limb with normal CSPs. CSP onset latency was inversely correlated with JOA score and N13 amplitude, and was positively correlated with central motor conduction time to abductor digiti minimi. CSP duration was inversely correlated with central motor conduction time to tibialis anterior. Electromyographic abnormalities were found in 7 patients. CONCLUSION: We confirm the value of neurophysiological evaluation of CCSM. MEPs were more frequently abnormal than SEPs. CSP abnormalities were almost equally sensitive as upper limb MEPs, and were highly associated with spinothalamic dysfunction. The high correlation of CSP abnormalities with corticospinal tract dysfunction suggests supraspinal influence on CSPs. Our findings corroborate the utility of CSP testing in the comprehensive assessment of intramedullary spinal cord dysfunction in CCSM.

Poranění periferních nervů jsou v době míru součástí dopravních, sportovních a průmyslových úrazů. Jejich diagnostika je častou součástí běžné ambulantní neurologické praxe i konziliární neurologické služby na traumatologických odděleních. Kromě klasických řezných, sečných, bodných a střelných poranění jsou významná iatrogenní poranění periferních nervů a akutní kompresivní mononeuropatie (např. u sportovců). Existuje plynulý přechod od klasických traumat periferních nervů k akutním kompresivním mononeuropatiím. V diagnostickém procesu hraje důležitou roli mechanizmus poranění, topická diagnostika nervové léze, elektromyografické vyšetření a zobrazovací metody. V léčbě je klíčové rozhodnutí, zda a kdy má být u poraněného nervu indikována konzervativní (rehabilitační) nebo chirurgická léčba (revize nervu).
