STUDY DESIGN: Cross-sectional population-based observational study. OBJECTIVE: To estimate the prevalence of nonmyelopathic spondylotic cervical cord compression (NMSCCC) and cervical spondylotic myelopathy (CSM) in a population older than 40 years and to evaluate the magnetic resonance imaging (MRI) characteristics of these conditions. SUMMARY OF BACKGROUND DATA: The prevalence of neither NMSCCC nor CSM is known and there exists no commonly accepted quantitative MRI definition of cervical cord compression. METHODS: A group of 183 randomly recruited volunteers, 93 women, median age 66 years, range 40-80 years, underwent MRI examination of the cervical spine and spinal cord on a 1.5 T device using conventional sequences from disc levels C2/C3 to C6/C7. The imaging criterion for cervical cord compression was defined as a change in spinal cord contour at the level of an intervertebral disc on axial or sagittal MRI scan. RESULTS: MRI signs of cervical cord compression were found in 108 individuals (59.0%; 95% CI: 51.5%-66.2%); their numbers increased with age from 31.6% in the fifth decade to 66.8% in the eighth. Clinical signs of symptomatic CSM were found in two cases (1.1%), and 75 cases (41.0%) were without compression. An anteroposterior cervical canal diameter at the level of intervertebral disc (CDdisc) of less than 9.9 mm was associated with the highest probability of NMSCCC-odds ratio (OR) = 32.5, followed by a compression ratio of ≤0.5: OR = 11.1. CONCLUSION: The prevalence of NMSCCC in a population older than 40 years is higher than previously reported and increases with age. CDdisc and compression ratio had the highest capacity to discriminate between subjects with and without asymptomatic compression, and their cut-off values could be used to objectify criteria for cervical cord compression. STUDY DESIGN: A prospective, nonrandomized, multicenter study. OBJECTIVES: The purpose of this study was to evaluate the amount of motion present at instrumented but unfused segments and at motion segments adjacent to the instrumentation following application of a new posterior apical short-segment correction technique for correcting adolescent idiopathic scoliosis (AIS). SUMMARY OF BACKGROUND DATA: High-density pedicle screw instrumentation and posterior arthrodesis of all instrumented levels is the most common surgical treatment for AIS stabilization. The consequence of long fusion is an abnormal load on adjacent levels with an increased risk of future adjacent segment degeneration. METHODS: This new system applied translational and derotational forces over a short apical segment. The short apical region was prepared for fusion while maintaining motion of unfused vertebral segments. Radiographic data were collected pre-operatively, at surgery, and at 3, 6, and 12 months after surgery. RESULTS: Twenty-one female patients, mean age of 14.2 years (10.6-16.9 years) with Lenke 1A/1B curves, were enrolled. The range of motion in the unfused instrumented segment was significantly higher than the apical fused segment (11 vs. 0.9, P < 0.001). The range of motion of unfused vertebral levels distal to the construct at one year did not differ significantly from their respective pre-op values. When the analysis was extended to understand the impact of lower instrumented vertebra (LIV) on motion of unfused segments distal to the construct, it appeared that (1) the change in motion from pre-op to 12 months post-op as a function of LIV is not statistically significant; and (2) The motion of the unfused distal vertebral segments at 12 months does not statistically increase with a lower LIV. CONCLUSION: Through one year, this novel technique achieved and maintained similar AIS correction to current posterior fusion techniques while maintaining the mobility of unfused motion segments with less implant density. STUDY DESIGN: Consecutive case-series. OBJECTIVE: The main purpose of this study was to analyze the relationship between the preoperative neurological status and subsequent survival of patients undergoing surgical treatment for symptomatic spinal metastases. SUMMARY OF BACKGROUND DATA: The survival of cancer patients has increased over recent years with improvements in oncologic therapy. As many as 70% of cancer patients develop spinal metastases and ∼10% of all cancer patients are treated for metastatic spinal cord compression. METHODS: We retrospectively analyzed 166 patients who underwent surgery for symptomatic spinal metastases in our department between 2005 and 2012. The evaluated factors were age, primary tumor aggressiveness (slow, moderate, rapid growing), spinal location (cervical, thoracic, lumbar, and sacral), operation type (posterior decompression, anterior or posterior instrumented procedure, and radical combined instrumented surgery), preoperative evaluation using the revised Tokuhashi scoring system (<9, 9-11, and 12-15), pre- and postoperative neurological status according to the Frankel score (A-C and D-E), and the site of the main spinal cord compression (anterior, posterior, or combined). Postoperative complication and recurrence rate were also monitored. RESULTS: The mean age of the patients was 62 ± 12 years. The median survival time after surgery was 16.0 months. Preoperative neurological status influenced survival time significantly; the median survival was 5.1 months in Frankel A-C and 28.2 months in Frankel D-E (P < 0.001). Improvement on the Frankel scale did not influence the survival time (P = 0.131). When the patients' age was <65 years this related to a significantly longer survival time (P = 0.046). The Tokuhashi score predicted patient's survival independently (P < 0.001). The other factors had no statistical significance. CONCLUSION: The most important factors influencing postoperative survival time of these patients with symptomatic spinal metastases was the preoperative neurological condition and the Tokuhashi scoring system, which together represents a useful tool for planning the extent of surgical treatment.

STUDY DESIGN: A prospective study. OBJECTIVE: The aim of this study was to compare clinical and radiological efficacy of anterior cervical microdiscectomy and fusion done by the newly designed low-profile interbody spacer in cases of symptomatic cervical spine spondylosis. SUMMARY OF BACKGROUND DATA: There are basically 2 ways to provide interbody fusion in the degenerative cervical spine; the first is by way of an unanchored "stand-alone" bone graft or cage, and the second is with bone graft or a cage anchored with a plate. Both concepts have their own benefits as well as potential drawbacks. Low-profile angle-stable spacer Zero-P is an implant that can potentially limit the drawbacks of both these procedures. METHODS.: Prospective study collecting clinical and radiological data of 77 patients undergoing anterior cervical interbody fusion of 1 or 2 motion segments from C3-C7 was performed. Zero-P spacer was used in 44 patients (55 segments) and in 33 cases (41 segments), stabilization was done using interbody spacer and dynamic anterior cervical plate. Patients were followed a minimum of 2 years after surgery. RESULTS: There was no significant difference in neck disability index values, presence of dysphagia (P = 0.308), and Cobb C values during follow-up (P = 0.051) between both groups. A significant difference in the first 2 values of Cobb S was found (P < 0.001), but the next course of Cobb S changes showed no difference in either group. No difference was found in the radiological stability during follow-up, and no revision surgery was done. CONCLUSION: The results of this study confirm biomechanical assumptions associated with the Zero-P spacer. Implantation of this new cage results in setting required biomechanical conditions in the treated segment that are comparable with those when the segment is treated with a dynamic plate. However, the potential of the mentioned implant to reduce the incidence of postoperative dysphagia was not proven on this sample of patients.

STUDY DESIGN: A prospective study evaluating a cohort of patients with spondylotic cervical spine compression. OBJECTIVE: To analyze the potential of diffusion tensor imaging (DTI) of the cervical spinal cord in the detection of changes associated with spondylotic myelopathy, with particular reference to clinical and electrophysiological findings. SUMMARY OF BACKGROUND DATA: Conventional magnetic resonance imaging (MRI) may provide confusing findings because of a frequent disproportion between the degree of the spinal cord compression and clinical symptoms. The DTI is known to be more sensitive to subtle pathological changes of the spinal cord compared with conventional MRI. METHODS: The DTI of the cervical spinal cord was performed within a group of 52 patients with spondylotic spinal cord compression and 13 healthy volunteers on a 1.5-T MRI scanner. All patients underwent clinical examination that differentiated between asymptomatic and symptomatic myelopathy subgroups, and 45 patients underwent electrophysiological examination. We measured the apparent diffusion coefficient and fractional anisotropy of the spinal cord at C2/C3 level without compression and at the maximal compression level (MCL). Sagittal spinal canal diameter, cross-sectional spinal cord area, and presence of T2 hyperintensity at the MCL were also recorded. Nonparametric statistical testing was used for comparison of controls with subgroups of patients. RESULTS: Significant differences in both the DTI parameters measured at the MCL, between patients with compression and control group, were found, while no difference was observed at the noncompression level. Moreover, fractional anisotropy values were lower and apparent diffusion coefficient values were higher at the MCL in the symptomatic patients than in the asymptomatic patients. The DTI showed higher potential to discriminate between clinical subgroups in comparison with standard MRI parameters and electrophysiological findings. CONCLUSION: The DTI appears to be a promising imaging modality in patients with spondylotic spinal cord compression. It reflects the presence of symptomatic myelopathy and shows considerable potential for discriminating between symptomatic and asymptomatic patients.

STUDY DESIGN: A prospective study. OBJECTIVE: The aim of this study was to compare the 3 different methods of interbody fusion of the cervical spine-autograft in stand-alone technique, autograft with anterior plate, and polyetheretherketone cage supported by anterior plate. The clinical and radiological data obtained were analyzed and discussed. SUMMARY OF BACKGROUND DATA: Although degenerative cervical spine disease has been treated by an anterior approach for more than 50 years, there is not one generally accepted operative approach. There is a very low-quality evidence of little or no difference in pain relief between each of the techniques. Iliac crest autograft still seems to be the "gold standard" for interbody fusion. METHODS: Prospective study collecting clinical and radiological data of 81 patients undergoing anterior cervical interbody fusion, in which the interbody fusion of 1 or 2 motion segments from C3 to C7 was done by any of the 3 techniques--stand-alone insertion of autograft (group 1: 28 patients), autograft and anterior plate (group 2: 18 patients), and polyetheretherketone cage filled with beta-tricalcium phosphate and plate (group 3: 29 patients). Patients were followed for 2 years after surgery. RESULTS: Significant interaction of relative height in the segment and time was found (P < 0.001). The values of the relative height of stand-alone autograft dropped below 95% of initial height and the values of the other 2 groups remained above 105%. Significant interaction of time and group was found for Cobb S angles (P < 0.001). Values of group 1 decreased substantially and remained significantly lower than values of other 2 groups. Fusion rate was 100% in all groups. Neck Disability Index group and time interaction was found (P = 0.023). During postoperative follow-up, group 1 scored in all controls higher than the other 2 groups, but differences were not significant. Visual analogue scale showed effect of time (P < 0.001). This was due to a smaller improvement of patients in group 1 during the whole follow-up in comparison with the other 2 groups. Highest proportion of unsatisfied patients was in group 1 compared with the other 2 groups after 2 years (P = 0.034). CONCLUSION: Significantly worse radiological and clinical results after 2 years of follow-up were achieved using stand-alone autograft technique in comparison with autograft supported by anterior plating similarly as in comparison with cage implant and anterior plating. Using artificial fusion substrate together with plate and cage can offer the same clinical and radiological results such as iliac autograft and plating. Anterior plating seems to be an important factor influencing the postoperative cervical spine alignment and also the clinical outcome.

STUDY DESIGN: Clinical, neuroradiologic, and neurophysiologic description of 21 patients with compressive cervical spondylotic myelopathy (CCSM). OBJECTIVE: To describe the utility of cutaneous silent periods (CSPs) for functional evaluation of mild CCSM. SUMMARY OF BACKGROUND DATA: Electroneurography, electromyography, and somatosensory and motor evoked potentials (SEPs, MEPs) are routinely used for comprehensive functional neurophysiological evaluation of CCSM. CSPs have been reported in various intramedullary spinal cord lesions, however, they have not been systematically studied in mild CCSM. METHODS: We investigated 21 patients with multilevel CCSM as documented by magnetic resonance imaging. We recorded CSPs in thenar muscles after noxious digit II stimulation and compared them with median and tibial nerve SEPs and MEPs obtained from abductor digiti minimi and tibialis anterior muscles. Electroneurography and electromyography were obtained in affected myotomes. RESULTS: CSP onset and end latencies were delayed, and CSP duration was shortened, in CCSM patients. CSP abnormalities were present in 17 patients of whom all, but 1 presented with intramedullary magnetic resonance imaging hyperintensity. All 11 limbs affected by hypalgesia and thermhypesthesia had abnormal CSPs, whereas no spinothalamic deficit was noted in any limb with normal CSPs. CSP onset latency was inversely correlated with JOA score and N13 amplitude, and was positively correlated with central motor conduction time to abductor digiti minimi. CSP duration was inversely correlated with central motor conduction time to tibialis anterior. Electromyographic abnormalities were found in 7 patients. CONCLUSION: We confirm the value of neurophysiological evaluation of CCSM. MEPs were more frequently abnormal than SEPs. CSP abnormalities were almost equally sensitive as upper limb MEPs, and were highly associated with spinothalamic dysfunction. The high correlation of CSP abnormalities with corticospinal tract dysfunction suggests supraspinal influence on CSPs. Our findings corroborate the utility of CSP testing in the comprehensive assessment of intramedullary spinal cord dysfunction in CCSM.

STUDY DESIGN: Clinical, radiologic, and neurophysiologic description of 2 cases. OBJECTIVE: To describe 2 cases with spontaneous deep unilateral abdominal pain as the first symptom of thoracic disc herniation at a low thoracic vertebral level, further manifested by unilateral partial paresis of the obliquus abdominis muscle. SUMMARY OF BACKGROUND DATA: Clinical manifestation of lateral thoracic disc herniation with electrophysiologic results and conservative therapy as treatment of choice when spinal cord functions are preserved. METHODS: Magnetic resonance imaging revealed bilateral paramedian disc protrusions at T12-L1 in Patient 1 and foraminal herniation at T10-T11 and paramedian herniation at T11-T12 in Patient 2. Electromyography (EMG) and evoked potentials were investigated in the acute stage and after 6 months. RESULTS: Spontaneous activity on needle EMG confirmed axonal root impairment. Somatosensory and motor-evoked potentials were within normal limits and excluded spinal cord involvement. Nonsteroidal anti-inflammatory drugs and periradicular injection of steroids and local anesthetics rendered both patients pain-free. Normalization of muscle strength within 3 to 6 months was accompanied by EMG findings of reinnervation. CONCLUSION: Lateral disc herniation causing compression of a thoracic root associated with unilateral segmental paresis of the abdominal wall is a rare condition. Despite EMG documentation of axonal root lesion, however, a purely conservative therapeutic approach may be considered treatment of choice in cases without spinal cord involvement.

STUDY DESIGN: Retrospective study of 175 patients with hereditary motor and sensory neuropathy (HMSN), i.e., Charcot-Marie-Tooth (CMT) disease. OBJECTIVE: To investigate the frequency, age of onset, character, familial, and genotypical incidence of spinal deformities among HMSN patients. SUMMARY OF BACKGROUND DATA: Prior studies addressing HMSN discuss the associated spinal deformities. However, these data vary significantly while inconsistently including genotypes within the classification framework. METHODS: Plain-film radiographic spine studies of 175 HMSN patients were performed to determine the incidence, character, and severity of spinal deformity. The degree of the spinal deformity was evaluated measuring Cobb's angle of the main curve. The results of the entire cohort were initially assessed before being classified by genotype. RESULTS: The incidence of spinal deformity for the entire group was 26%. Of these, 58% demonstrated scoliosis, 31% had kyphoscoliosis, and 11% had thoracic hyperkyphosis; 73% of patients with spinal deformity were classified as HMSN Type I with confirmed duplication of the PMP 22 (peripheral myelin protein) gene on chromosome 17. The incidence of spinal deformity by genotype was: duplication of the PMP 22 gene: 29% (25 of 87); deletion of the PMP 22 gene: 0% (0 of 15); Cx32 (connexin 32) gene mutation: 24% (8 of 34); and MPZ (myelin protein zero) gene mutation: 100% (6 of 6). Familial incidence of spinal deformity was found in "MPZ gene mutation" and "duplication of PMP 22 gene" subgroups. CONCLUSION: This study demonstrates a 26% incidence of spinal deformity among HMSN patients. Spinal deformity was most frequently observed in patients with the MPZ gene mutation, where the most common familial incidence was also found.

STUDY DESIGN: Prospective observational cohort study. OBJECTIVE: To ascertain the threshold of critical spondylotic cervical cord compression and its relation to MRI-increased signal intensities. SUMMARY OF BACKGROUND DATA: The critical degree of spinal cord compression required to induce significant clinical signs remains unknown. METHODS: The study group consisted of 243 patients (mean age, 53.9 +/- 9.8 years), with spondylotic cervical spine compression. The transverse cross-sectional area of the spinal cord at the level of maximum compression was measured, while MRI hyperintensities were recorded and related to clinical status and quantified by modified JOA score (mJOA). RESULTS: A statistically significant difference in mJOA was shown between patients with a spinal cord sectional area of under 50 mm2 and a group of patients with a spinal cord sectional area of over 60 mm2. This difference was highly significant (P = 0.001) in a subgroup with MRI hyperintensities (187 patients, P = 0.001), whereas within the group of patients without hyperintensities this difference was not observed (P = 0.63). CONCLUSION: The critical degree of spinal cord compression needed to induce clinically significant signs was found between 50 and 60 mm2 of cross-sectional transverse area at the level of maximal compression in association with MRI hyperintensities.
