INTRODUCTION: This study aimed to assess the impact of midline lumbar fusion with cortical bone trajectory screws (MIDLF/CBT) on the multifidus muscles, focusing on the evaluation of their postoperative atrophy. CLINICAL RATIONALE FOR THE STUDY: MIDLF/CBT is a relatively new technique increasingly used to treat spinal instability. Despite its reduced invasiveness compared to traditional posterior lumbar interbody fusion with traditional pedicle screws (PLIF/TP), concerns remain about potential damage to the multifidus muscles that are crucial for spinal stability. Understanding the extent of muscular atrophy post-MIDLF/CBT is vital for improving surgical outcomes, and potentially patient rehabilitation strategies. MATERIAL AND METHODS: This study retrospectively analysed preoperative and postoperative MRI scans of patients who underwent MIDLF/CBT for degenerative segmental spondylolisthesis. The bilateral width of the multifidus muscles at the operated segment and adjacent segments was measured using axial T2-weighted MRI scans. Statistical comparisons were made using a paired t test, with significance set at p < 0.05. RESULTS: The study included 16 patients with an average age of 57 ± 10 years, 10 of whom (62.5%) were women, and featured a mean follow-up period of 37 ± 25 months. Postoperative measurements showed a significant reduction in the width of the multifidus muscles at the operated segment (mean difference -3.3mm, p = 0.02) and the inferior adjacent segment (-7.4 mm, p < 0.01). A decrease in muscle width at the superior adjacent segment was also observed, although this was not statistically significant. CONCLUSIONS AND CLINICAL IMPLICATIONS: Our study concluded that MIDLF/CBT results in significant multifidus muscle atrophy at and below the operated segment, potentially impacting postoperative rehabilitation and recovery. These findings highlight the need for further research comparing MIDLF/CBT to other spinal stabilisation techniques. Additionally, incorporating functional electromyographic assessments of paraspinal muscles could provide deeper insights into the long-term consequences of spinal surgeries and helpdevelop new approaches and strategies to mitigate paravertebral muscles atrophy, thus enhancing patient outcomes.

BACKGROUND: The acromion projects laterally in a curving orientation. When a fracture of the acromion occurs, despite being rare, matching the best-fit fixation plate is challenging due to few options and limited availability of fixation plate types. Alternative fixation methods can carry risk of nonunion complications. PURPOSE: The objective of this study was to investigate the morphological curvature pattern of the acromion to assess the level of the fixation plates fitness and provide the suitable imaging modalities for evaluating the acromion curvature. BASIC PROCEDURES: The correlation between the acromion curvature and five fixation plates were calculated and their fitness level was evaluated statistically. The curvature of acromion and the five available fixation plates were photographed and assessed digitally by software (FIJI ImageJ and Microsoft Excel). The method entails plotting seven points along the curving surface and margins of the acromion, while the Excel Solver function calculates the regression, ultimately giving curvature values. First, the acromion parameters were studied on 180 paired healthy dry scapulae. Then, the acromion curvature values were compared to the fixation plates curvatures. Likewise, the acromion curvature was assessed as applicable on 153 (100 AP- views, 50 Y- views, and 3 superior-views) retrospective plain X-rays and 40 3D-CT scapula reconstructions of healthy acromia. MAIN FINDINGS: The mean length of the acromion was 48.70 ± 5.29 mm, mean thickness was 8.51 ± 1.67 mm, and mean width was 25.97 ± 5.97 mm. The calculated values of the mean curvatures were 0.050 ± 0.015 mm-1 for the mean acromion surface, 0.042 ± 0.027 mm-1 for the internal margin, and 0.055 ± 0.010 mm-1 for the external margin. The curved geometry of the acromion was plotted on a graph giving a spectrum of curvature patterns with distribution values revealing fixation plates fitness represented by area under the curve with frequencies of 4.32 % for the acromion-specific fixation plate, 14.28 % for the large clavicle fixation plate, 0.26 % for the small clavicle fixation plate, 53.38 % for the flexible universal fixation plate, and none for the rigid universal fixation plate. PRINCIPAL CONCLUSIONS: Approximately 27.76 % of the acromion surface curvatures distribution does not overlap with any of our measured fixation plates. Evaluating the acromion surface curvature was possible on plain X-rays in the Y-view only.

BACKGROUND: Lateral plating of calcaneal fractures using variable-angle locking plates is still the golden standard for severely comminuted cases. The aim of this study is to explore the possibilities of improving stability of osteosynthesis by changing screw directions. It provides an assessment and comparison of cadaveric biomechanical experiment with retrospective radiologic data analysis. METHODS: In the cadaveric study 8 intact calcaneus-talus specimens were obtained from 4 deceased donors. Fracture type 2b according to Sanders' classification was created in each specimen and fixed with variable-angle locking plate. The specimens were divided in 2 groups differing in orientation of anterior screws and fixed in PMMA base. A push-in test was performed by a two-column testing machine until gross failure. Retrospective cohort study was performed, reviewing data of 74 patients which underwent surgical treatment of calcaneal fractures with the same construct. Evaluation was performed at scheduled CT and X-Ray controls. Direction of inserted screws and implant failure were noted. FINDINGS: The cadaveric study proved that there is no significant difference in mean failure force between two abovementioned screw configurations in Sanders 2b fracture. A significant difference was observed in initial stiffness. The radiologic retrospective study showed that difference in screw position within all fracture types but type 2b is significant. INTERPRETATION: Screw configuration in the anterior part of variable-angle locking plate appears to affect primary stiffness and stability of the construct. Particularly in more comminuted fractures, screw inserted in the direction of sustentaculum improves the stability and lowers risk of implant failure.

PURPOSE OF THE STUDY: Intertrochanteric hip fractures in elderly patients are a significant cause of morbidity and mortality, with increasing incidence due to the aging population. Despite advancements in intramedullary nailing (IMN) technology, fixation failure remains a concern. This study aims to evaluate pre-and postoperative radiographic risk factors for varus collapse in geriatric intertrochanteric fractures treated with twin interlocking derotation and compression screw cephalomedullary nail (InterTAN, Smith & Nephew). MATERIAL AND METHODS: This retrospective study included patients over 60 years with AO 31A1 and 31A2 intertrochanteric femur fractures treated with InterTAN at a tertiary referral center from August 2012 to August 2017. Patients with high-energy fractures, AO 31A3 fractures, or those requiring open reduction were excluded. Data on demographics, fracture classification, implant sizes, imaging studies, and follow-up were collected. Radiographic assessments included Chang's medial cortical support concept, tip-apex distance (TAD), calcar-referenced TAD (CalTAD), neck-shaft angles (NSA), and screw placement according to Cleveland zones. Varus collapse was defined as a >5° change in NSA within three months postoperatively. Multivariate logistic regression analysis was used to identify risk factors for varus collapse. RESULTS: The study included 136 patients with a mean age of 79.8 years, of whom 38.2% were male. The early postoperative tipapex distance (TAD) averaged 21.9mm, with 30.1% of patients experiencing varus collapse greater than 5°. Positive medial cortical support (PMCS) or neutral position (NP) type reduction was highly protective against varus collapse (p < 0.001), as well as TAD less than 25mm (p < 0.001). Additionally, the placement of screws in the central-central or central-inferior zones provided a protective effect against varus collapse (p = 0.031). Conversely, having an OTA/AO type A2.2 or A2.3 fracture significantly increased the risk of varus collapse (p = 0.030). Other factors, such as CalTAD and the nail width to medullary canal ratio, did not significantly predict varus collapse (p = 0.831 and p = 0.952, respectively). DISCUSSION: Our findings align with previous studies highlighting TAD and screw placement as critical factors in preventing fixation failure. The protective effect of PMCS or NP reduction and the increased risk associated with OTA/AO type A2.2 and A2.3 fractures are noteworthy. Unlike previous studies, CalTAD was not significantly associated with varus collapse in our cohort. The study underscores the importance of surgical technique and radiographic parameters in optimizing outcomes for elderly patients with intertrochanteric fractures. CONCLUSIONS: In elderly patients treated with InterTAN nails, varus collapse is influenced by fracture type, TAD, reduction quality, and screw placement. Ensuring a TAD <25mm, achieving PMCS or NP reduction, and placing screws in central-central or central-inferior zones are crucial for minimizing varus collapse. These findings highlight the importance of meticulous surgical technique and radiographic assessment in managing intertrochanteric fractures in the elderly. Cíl: Retrospektivní observační analýza přesnosti zavedení pedikulárních šroubů, komplikací, chirurgického času a technických zákonitostí 100 pacientů operovaných pomocí robotického systému ExcelsiusGPS. Metodika: Sto dospělých pacientů podstupující stabilizační výkony lumbosakrální páteře v průběhu období 2021–2024. Pacienti podstoupili jednu z tří možných operací – otevřená meziobratlová fúze, minimálně invazivní meziobratlová fúze nebo kombinace otevřené meziobratlové fúze s neinstrumentovanou mikrodekompresí. Všechny operační výkony byly provedeny za asistence robotického systému ExcelsiusGPS s následnou intraoperační kontrolní CT. Pacienti byli sledováni v intervalech 6 týdnů, 3 měsíců a 6 měsíců. Výsledky: Celkem bylo zavedeno 418 pedikulárních šroubů, z nichž 413 (99 %) mělo optimální zavedení (Gerzbein- -Robbins A) a zbylých pět vykazovalo drobné asymptomatické narušení kortikalis stěny pediklu (Gerzbein-Robbins B). Jednou byla narušena horní hrana pediklu a ve čtyřech případech došlo k narušení spodní plochy pediklu. V sedmi případech bylo nutné opakovat navigační CT sken v důsledku chyb přenosu mezi CT a robotickou platformou, ve dvou případech bylo nutné přeplánovat trajektorie pedikulárních šroubů v důsledku pohybu dynamické reference v průběhu zavádění šroubů. Průměrný chirurgický čas byl 154 min pro jednoprostorový stabilizační výkon a 202 min pro vícepatrový stabilizační výkon. Naše práce neprokázala významný pokles chirurgického času v obrazu učební křivky. V časném pooperačním období byla nutná revize dvou pacientů pro přítomnost seromu operační rány. Závěr: Druhá generace robotických systémů vykazuje vysokou přesnost zavádění pedikulárních šroubů, čímž snižuje výskyt komplikací souvisejících s malpozicí. Hlavní nevýhodou je prodloužení chirurgického času v důsledku nastavení robotické a navigační platformy.
Aim: Retrospective observation analysis of pedicle screw accuracy, complications, surgical time, and technical nuances of 100 patients operated via the ExcelsiusGPS robotic system. Methods: One-hundred adult patients undergoing lumbosacral interbody fusion between 2021 and 2024. Patients underwent either open interbody fusion, minimally invasive interbody fusion, or open interbody fusion combined with a non-instrumented decompression. All surgeries were assisted by the ExcelsiusGPS robot, and an intraoperative control CT scan was performed in all cases. Patients were followed at 6-week, 3-month, and 6-month intervals. Results: A total of 418 pedicle screws were implanted and 413 (99%) had optimal placement (Gerzbein-Robbins A) with the remaining five having minor asymptomatic cortical breaches of the pedicle wall violation (Gerzbein-Robbins B). The superior pedicle wall was breached once, and the inferior pedicle wall was breached four times. In seven cases, the CT scan had to be repeated due to transferring errors between the CT and robotic platform, and in two cases, screw trajectories had to be replanned due to dynamic reference base shift during screw implantation. The mean surgical time was 154 min for single-level procedures and 202 min for multiple-level procedures; our study did not show a significant decrease in surgical time throughout the learning curve. During early follow-up, two patients underwent surgical revisions due to the presence of seromas. Conclusion: Second generation robotic platforms have a high percentage of optimally implanted pedicle screws decreasing malposition-related complications. The main drawback is an increase in surgical time due to set-up of the robotic and navigation platforms.

INTRODUCTION: German-speaking surgeons have left a deep imprint on the history of diagnostics and treatment of proximal femur fractures. Some of the studies published in German have fell into oblivion, others are cited until today, although sometimes quite inaccurately. The cause of such inaccuracies are citations from secondary sources due to unavailability of the original or inability to read it because of a language barrier. In the current literature, globally predominated by English articles, the "German history" of treatment of proximal femur fractures remains undervalued. The aim of the present article is to point out its contribution. MATERIALS AND METHODS: Literature search was performed in original publications and historical sources. RESULTS: The German-speaking surgeons have considerably contributed to the development of the treatment of proximal femur fractures. The analyzed period between 1847 and 1970 may be divided into three basic periods. In the first period (1847-1896), the German-speaking surgeons (Langenbeck, Trendelenburg, König, Heine, Dolinger) were the first worldwide to start treating the fractures of proximal femur operatively. In the second period (1897-1935), mainly in the 1930s, the initiative in Europe was taken over by surgeons publishing studies in France and in Belgium. Overseas, American surgeons were coming to the forefront. In Germany, only Pauwels developed the first biomechanical classification of femoral neck fractures. In the third period (1936-1970), mainly in the 1940s and 1950s, implants were designed (Pohl, Künstcher), based on close collaboration between German engineers and surgeons, that served as a model for a dynamic hip screw and a proximal femoral nail, which are currently the implants of choice in the treatment of trochanteric fractures. CONCLUSION: The historical contribution of German-speaking surgeons to understanding the issue of proximal femur fractures and their operative treatment is far more significant than presented in the historical studies published in English.

BACKGROUND: Surgical treatments of benign primary bone tumors of the femur face the challenge of limiting tissue damage and contamination while providing sufficient stabilization to avoid fracture. While no clear treatment guidelines exist, surgical treatment commonly consists of femoral fenestration and curettage with optional filling and plating of the defect. Mono- or bicortical plating of distal femoral defects aim to reduce fracture risk and have been shown to increase axial stability. However, it remains unclear whether plating increases torsional stability of the affected femur. QUESTIONS/PURPOSES: This biomechanical study aimed to determine how much additional stability can be achieved by mono- or bicortical plating of femoral defects after fenestration. The following hypotheses were investigated: 1. Preventive plating of distal femur bone defects enhances torsional stability when compared to femoral fenestration alone. 2. A condition close to the intact (nonpathological) bone can be achieved by bone plating. 3. Defect shape influences torsional stability. PATIENTS AND METHODS: Thiel embalmed human femora (n = 24) were left intact or subjected to the following surgical treatments (A) defect creation via fenestration, (B) defect with short monocortical plating, (C) defect with long bicortical plating. All femora were torsion tested in midstance position using pre-cycling and testing until failure. Quantitative computed tomography pre and post testing allowed bone mineral density calculation and crack path analysis. Finite element analysis provided insight into defect shape variations. RESULTS: Torsion experiments showed no relevant enhancement of torsional stability due to mono- or bicortical plating. There were no significant differences in maximum torque between unplated and plated femora with defect (defect: 35.38 ± 7.53 Nm, monocortical plating: 37.77 ± 9.82 Nm, bicortical plating: 50.27 ± 9.72 Nm, p > 0.05). Maximum torque for all treatment groups was significantly lower compared to intact femora (155-200 Nm, p < 0.001). Cracks originated predominantly from the proximal posterior corner of the defect and intersected with screw holes in plated femora. The influence of variations of the defect corner shapes had no significant influence on maximum torque and angle. CONCLUSION: This biomechanical study shows that mono- or bicortical plating is not an effective preventive treatment against torsional failure of femora with distal defects as the resulting maximum torque was drastically reduced compared to intact femora. Thus, the initial hypotheses have to be rejected. As habitual loading of the femur includes a combination of axial and torsional loading, the observed lack of prevention against torsional failure might help to explain the occurrence of fractures despite plating. Future research towards ameliorating clinical outcome should address the role of defect filling with bone cement or bone grafts regarding the improvement of torsional stability after primary bone tumor treatment in the femur.

PURPOSE OF THE STUDY: The purpose of this study was to assess the clinical outcomes and complications associated with intramedullary and transverse K-wire fixations of 5th metacarpal neck fractures. MATERIAL AND METHODS: Patients who were operated for 5th metacarpal neck fractures between 2019 and 2022 were evaluated retrospectively. Regarding the surgical treatment methods, patients were assessed by dividing them into two groups. The first group comprised patients who underwent treatment with an intramedullary K-wire. The second group comprises patients who underwent transverse K-wire fixation. RESULTS: The average quick DASH score of all patients was 5.6±4.7 in the intramedullary K-wire fixation group and 5.9±5.1 in the transverse K-wire fixation group. An average 5th finger metacarpophalangeal joint extension limitation was 6.2±5.7° in the intramedullary fixation group and 6.1±5.8° in the transverse K-wire group. The mean radiological union time was 4.9±0.7 weeks in the intramedullary fixation group and 5.1±0.7 weeks in the transverse K-wire group. No statistically significant difference was found between the quick DASH scores and degrees of the MCP joint extension limitation ( p=0.785). Intramedullary fixation and transverse K-wire fixations are effective surgical treatment methods for metacarpal neck fractures. DISCUSSION: It has been reported that the intramedullary fixation method in metacarpal bone fractures is more effective than the fixation methods with cross and transverse K-wire. But our results revealed no difference in clinical outcomes between the two surgical fixation methods. CONCLUSIONS: We observed no statistically significant difference between the two fixation techniques with regard to union, clinical outcomes, or complications. PURPOSE OF THE STUDY: To examine the clinical impact of screw internal fixation on the process of fracture healing and ankle alignment in individuals diagnosed with posterior malleolar (PM) fracture, specifically those with a fracture involving less than 25% of the articular surface (ASR) area. MATERIAL AND METHODS: A total of 120 patients diagnosed and treated for PM fracture, encompassing less than 25% of the distal tibial ASR area, were selected from our hospital's records spanning from September 2021 to June 2023. These people were subsequently divided into two groups, namely the control group (group A) and the observation group (group B), based on the distinct treatment methods employed, with each group consisting of 60 patients. The people in the group A were treated with posterior malleolus non internal fixation, while the patients in the group B were treated with posterior malleolus screw internal fixation. The visual analog scores, peak plantar pressure and AOFAS scores of the two groups were subjected to comparison. RESULTS: The visual analog scores in the observation group at 6 months and 12 months after operation were reduced than the group A. Three months after operation, the peak plantar pressure of the affected foot (full foot, hind foot) in the group B was reduced than that of the healthy foot; There was a lack of statistically significant variation observed in the peak plantar pressure (full foot, hind foot) between the affected foot and the healthy foot 12 months after operation in the group B, and the plantar pressure tended to be balanced. Three months after operation, the peak plantar pressure of the affected foot (full foot, hind foot) in the group A was reduced than that of the healthy foot; After a period of 12 months following the surgical procedure, no notable disparity in the maximum pressure exerted on the sole of the foot was observed between the foot that underwent the operation and the unaffected foot in the group A, but the peak plantar pressure of the whole foot was reduced than that of the healthy foot, and the plantar pressure did not tend to be balanced. At the intervals of 6 months and 12 months following the surgical procedure, AOFAS ankle hind foot score in the group B was increased than the group A. CONCLUSIONS: The utilization of screw internal fixation demonstrates favorable clinical outcomes in patients presenting with PM fracture encompassing less than 25% of the articular surface area, which is conducive to promoting fracture healing, maintaining good ankle alignment, and promoting patient rehabilitation. Úvod: Antibiotickým cementem potažené nitrodřeňové hřeby (ACCINs – antibiotic cement-coated intramedullary nails) jsou stále oblíbenější metodou v terapii FRI (fracture-related infections). FRI je velká výzva, která představuje pro ošetřujícího lékaře a zdravotní systém velký problém jak v nákladech na terapii, časové náročnosti, tak v komplikovanosti léčby s nejistým výsledkem. Cílem našeho sdělení je zhodnocení výsledků operační léčby pomocí ACCINs u pacientů s FRI, kteří byli léčeni během pětiletého období na našem pracovišti. Metody: Retrospektivní zhodnocení výsledků operační léčby ACCINs u pacientů s diagnózou FRI léčených na Klinice úrazové chirurgie LF MU a FN Brno v letech 2018–2022. Na základě exkluzních kritérií bylo ze studie z celkového počtu 18 pacientů vyloučeno 5 pacientů z následujících důvodů: doba sledování byla kratší než 6 měsíců, chronická imunosupresivní medikamentózní terapie, pacient s onkologickým nebo autoimunitním onemocněním. Mezi inkluzní kritéria pro léčbu ACCINs jsme zařadili kostní infekty v oblasti diafýzy femuru, tibie a infekce v oblasti hlezenního kloubu. Sledovaný soubor tvořilo 13 pacientů (3 ženy, 10 mužů), průměrný věk v souboru byl 48 let a medián pak 44 let. U 3 pacientů se jednalo o infekt po osteosyntézách v oblasti diafýzy femuru, u 7 pacientů o infekt v oblasti diafýzy tibie a u 3 pacientů v oblasti hlezenního kloubu. Výsledky: V našem souboru bylo pozorováno kostní zhojení bez klinických a laboratorních známek infekce nebo stabilní artrodéza u 7 (54 %) pacientů. Ve 3 případech (23 %) probíhá hojení bez známek infekce. U 3 pacientů (23 %) byla nutná další operační léčba, jednak z důvodu nezhojení zlomeniny (n=1) a také z důvodu kombinace nezhojení kosti s infekcí (n=2). Celková míra záchrany končetiny byla 100 %. Závěr: Použití ACCINs naplňuje po důkladném debridementu další tři základní principy terapie kostního infektu: lokální působení ATB (antibiotik), vyplnění mrtvého prostoru a stabilizace skeletu. ACCINs je účinnou metodou v terapii FRI diafýz dlouhých kostí a septických artritid hlezna.
Introduction: Antibiotic cement-coated intramedullary nails (ACCINs) are an increasingly popular method in the treatment of FRI (fracture-related infections). FRI is a major challenge that poses a major problem for the treating physician and the health care system, both in terms of cost of therapy, time required, and the complexity of treatment with an uncertain outcome. The aim of our report is to evaluate the results of surgical management using ACCINs in patients with FRI who were treated during a 5-year period at our institution. Methods: A retrospective evaluation of the results of surgical treatment using ACCINs in patients with FRI treated at the Department of Trauma Surgery, University Hospital Brno and Faculty of Medicine, Masaryk University in Brno in 2018–2022. Based on the exclusion criteria, 5 out of 18 patients were excluded from the study for the following reasons: a follow-up period less than 6 months, chronic immunosuppressive drug therapy, and any oncological or autoimmune disease. Bone infections in the femoral diaphysis, tibial diaphysis and infections in the ankle joint were among the inclusion criteria for the use of ACCINs. The study population consisted of 13 patients (3 women, 10 men); mean age was 48 years and median age was 44 years. Three patients had an infection after osteosynthesis in the femoral diaphysis, 7 patients had an infection in the tibial diaphysis and 3 had an infection in the ankle joint. Results: In our cohort, bone healing without any clinical and laboratory signs of infection or stable arthrodesis was observed in 7 (54%) patients. In 3 cases (23%), healing proceeded without any signs of infection. In 3 patients (23%), further surgical treatment was required because of fracture nonhealing (n=1) and because of the combination of bone nonhealing and infection (n=2). The overall limb salvage rate was 100%. Conclusion: The use of ACCINs fulfills the three basic principles of bone infection therapy after thorough debridement: local action of antibiotics, filling of the dead space, and stabilization of the skeleton. ACCINs provide an effective method in the treatment of FRI of the diaphysis of long bones and septic arthritis of the ankle.
