The sciatic nerve (SN) is the biggest nerve in the human body and innervates a large skin surface of the lower limb and several muscles of the thigh, leg, and foot. It originates from the ventral rami of spinal nerves L4 through S3 and contains fibers from both the posterior and anterior divisions of the lumbosacral plexus. After leaving the neural foramina, the nerve roots merge with each other forming a single peripheral nerve that travels within the pelvis and thigh. Non-discogenic pathologies of the SN are largely underdiagnosed entities due to nonspecific clinical tests and poorly described imaging findings. Likewise, to the best of our knowledge, a step-by-step ultrasound protocol to assess the SN is lacking in the pertinent literature. In this sense, the aim of the present manuscript is to describe the normal sono-anatomy of the SN from the greater sciatic foramen to the proximal thigh proposing a standardized and simple sonographic protocol. Then, based on the clinical experience of the authors, a few tips and tricks have been reported to avoid misinterpretation of confounding sonographic findings. Last but not least, we report some common pathological conditions encountered in daily practice with the main purpose of making physicians more confident regarding the sonographic "navigation" of a complex anatomical site and optimizing the diagnosis and management of non-discogenic neuropathies of the SN.

The sciatic nerve has varying anatomy with respect to the piriformis muscle. Understanding this variant anatomy is vital to avoiding iatrogenic nerve injuries. A comprehensive electronic database search was performed to identify articles reporting the prevalence of anatomical variations or morphometric data of the sciatic nerve. The data found was extracted and pooled into a meta-analysis. A total of 45 studies (n = 7068 lower limbs) were included in the meta-analysis on the sciatic nerve variations with respect to the piriformis muscle. The normal Type A variation, where the sciatic nerve exits the pelvis as a single entity below the piriformis muscle, was most common with a pooled prevalence of 85.2% (95%CI: 78.4-87.0). This was followed by Type B with a pooled prevalence of 9.8% (95%CI: 6.5-13.2), where the sciatic nerve bifurcated in the pelvis with the exiting common peroneal nerve piercing, and the tibial nerve coursing below the piriformis muscle. In morphometric analysis, we found that the pooled mean width of the sciatic nerve at the lower margin of the piriformis muscle was 15.55 mm. The pooled mean distance of sciatic nerve bifurcation from the popliteal fossa was 65.43 mm. The sciatic nerve deviates from its normal course of pelvic exit in almost 15% of cases. As such we recommend that a thorough assessment of sciatic nerve variants needs to be considered when performing procedures in the pelvic and gluteal regions in order to reduce the risk of iatrogenic injury. © 2016 Orthopaedic Research Society. PURPOSE OF THE STUDY Injury to the sciatic nerve is a rare event. Apart from war time surgery, it usually presents as a closed lesion caused by traction. The aim of the study was to evaluate a group of patients treated for sciatic nerve injury, with an analysis of the cause for and the outcome of surgery. MATERIAL AND METHODS In this prospective study, the results in ten patients treated surgically were evaluated. Five patients underwent exoneurolysis, two were treated by end-to-end suture of the nerve and three by suture and sural nerve grafting. One patient was lost to follow-up and nine were followed up for minimally 24 months after surgery. RESULTS In the patients treated by exoneurolysis, sciatic nerve function recovered in the peroneal division in 60%, and in the tibial portion in 100% of them. The result depended mainly on the interval between injury and surgery. Of four patients with direct suture of the nerve or with sural nerve grafting, function of the tibial portion recovered in three (75%) and that of the peroneal portion in one (25%). CONCLUSIONS Although the course of the sciatic nerve is very long, its surgical treatment is fully justified because it shows good results even in buttock-level and thigh-level nerve injuries.

Poranění periferních nervů jsou v době míru součástí dopravních, sportovních a průmyslových úrazů. Jejich diagnostika je častou součástí běžné ambulantní neurologické praxe i konziliární neurologické služby na traumatologických odděleních. Kromě klasických řezných, sečných, bodných a střelných poranění jsou významná iatrogenní poranění periferních nervů a akutní kompresivní mononeuropatie (např. u sportovců). Existuje plynulý přechod od klasických traumat periferních nervů k akutním kompresivním mononeuropatiím. V diagnostickém procesu hraje důležitou roli mechanizmus poranění, topická diagnostika nervové léze, elektromyografické vyšetření a zobrazovací metody. V léčbě je klíčové rozhodnutí, zda a kdy má být u poraněného nervu indikována konzervativní (rehabilitační) nebo chirurgická léčba (revize nervu).
