Skull base osteomyelitis (SBO) also commonly known as malignant otitis externa was first described by Meltzer and Kelemen in 1959. Prior to the advent of the antibiotic era, this disease carried a poor prognosis with significant morbidity. It often proved fatal with mortality rates as high as 50%. Commonly seen in the immunocompromised patients, diabetes mellitus is an important associated comorbidity in the pathophysiologic development of this disease. Treatment is instituted by medical therapy with surgery having a limited role. Surgical intervention has a limited role, for example, in fungal SBO. Such cases may require local debridement and intraoperative tissue biopsies for histopathologic confirmation. This is to demonstrate fungal invasion into the skull base, as well as to exclude other sinister differential diagnoses like squamous cell carcinoma of temporal bone. In this case report, we present a rare case of candida SBO and the literature review.

Bolest ucha (otalgia) patří k častým symptomům, které vedou k nutnosti návštěvy lékaře. Nejčastější příčinou jsou zánětlivé procesy v oblasti zevního a středního ucha, na které je článek zaměřen, je ale nutno v rámci diferenciální diagnostiky pomýšlet i na možnost bolesti propagované do ucha při onemocnění v oblasti dutiny ústní, hltanu, čelistního kloubu i krční páteře. Etiopatogeneze, rozdělení a klinická manifestace zánětu zevního zvukovodu (otitis externa) je diskutována v první části článku. Popsány jsou jednotlivé formy zánětu, typické subjektivní potíže nemocných i možné rizikové faktory. V rámci terapeutické rozvahy jsou uvedeny možnosti léčby zahrnující lokálně působící i celkově podávané preparáty. Akutní zánět středního ucha (otitis media acuta) je onemocněním typickým pro dětský věk, nicméně se setkáváme s touto problematikou i u dospělých pacientů. Klinické symptomy kromě bolesti zahrnují i různě závažnou poruchu sluchu, příznaky celkové a pominout nelze i varovné příznaky možných komplikací při šíření zánětlivého procesu do oblasti vnitřního ucha nebo intrakraniálně. Diskutována je otázka úlohy praktického lékaře v diagnostice a léčbě zánětu středouší a situace, při nichž je nezbytné vyšetření a léčba ORL specialistou.
Ear pain (earache, otalgia) in one of the common symptoms reffering patients to the ENT specialist. This article deals with earache,as a frequent cause of a GP visit. Although inflammations of the external auditory canal and middle ear are the most commoncauses of otalgia, other etiology including dental, oral and pharyngeal problems must be excluded. Also cervical spine and temporomandibularjoint affections presenting as ear pain (otalgy) can lead to otalgia. The first part of the article focuses on etiology,subjective symptoms and possible forms of acute inflammation of the external auditory canal (otitis externa). Possible risk factorsare also discussed (diabetes, immunodeficiency). Therapeutic approach includes especially local treatment (local antibiotics andcorticosteroids) and systemic medication in the severe course of the disease. Midle ear inflammations (otitis media) is typical forpaediatric population but it can be diagnosed also in adults. It can be accompanied with hearing loss, fever and other systemicsymptoms. Warning signs including peripheral vestibular syndrome (dizziness, nystagmus), haedache and other neurologicalsymptomatology can indicate severe complications when inflammation spreads to the inner ear and intracranial structures. Therole of the GPs and ENT specialist in diagnosis and therapy of the ottic inflammations are discussed.

Nekrotizující (maligní) zánět zevního zvukovodu je život ohrožující pseudomonádová infekce zevního zvukovodu, která vede k osteomyelitidě baze lební a zánětlivému postižení měkkých tkání fossa infratemporalis. Autoři uvádějí dvě kazuistiky s postižením lícního nervu a dalších hlavových nervů. Současný přehled literatury diskutuje klinický obraz, patogenezi, diagnostiku a léčbu osteomyelitidy baze lební ve spojení s nekrotizujícím zánětem zevního zvukovodu. Necrotizing (malignant) external otitis is a life-threatening Pseudomonas aeruginosa infection of the external auditory canal leading to skull base osteomyelitis and soft tissue inflammation of the infratemporal fossa. We present two cases with facial nerve palsy and multiple cranial nerve palsies. The review of recent literature discusses the history, pathogenesis, diagnosis, and treatment of skull base osteomyelitis in the context of necrotizing otitis externa. A collection of 178 Aspergillus isolates, recovered from Czech patients, mostly from 2007-2011, was subjected to multilocus DNA sequence typing using the ITS region, β-tubulin, and calmodulin genes. An unusually wide spectrum of etiologic agents that included 36 species of Aspergillus is discussed in the context of recent taxonomic and clinical reports. Invasive aspergillosis (IA), onychomycosis, and otitis externa were the predominant clinical entities. Five cases due to species newly proven as etiologic agents of human mycoses, as well as cases with unique clinical manifestations caused by unusual agents are discussed in more detail. Three species (i.e., A. insulicola, A. westerdijkiae and A. tritici) were identified as the confirmed etiologic agents of non-dermatophytic onychomycosis. Emericella rugulosa was recovered from a premature newborn with a fatal necrotising disseminated infection and is reported for only the second time as the cause of IA. Furthermore, we document the first infection due to A. calidoustus in a patient with chronic granulomatous disease. The infection manifested as a latent brain aspergilloma with an unusual clinical-laboratory finding. In addition to the well-known agents of human mycosis, several rarely isolated or poorly documented species were identified. An undescribed cryptic species related to A. versicolor was found to be common among isolates linked to proven and probable onychomycosis. An isolate representing A. fresenii, or an unnamed sister species, were causal agents of otomycosis. Three well defined, and tentative new species belonging to section Cervini, Candidi and Aspergillus (Eurotium spp.), were associated with cases of probable onychomycosis.

Chaetomium species have been rarely described as aetiological agents of invasive and dermatomycotic infections in humans. The majority of cases have been reported within the last two decades. Treatment failed in most of these cases. In this paper we present two cases in which Chaetomium spp. can be clearly identified as an aetiological agent in pathological conditions. In the first report, we describe a new aetiological agent, Chaetomium brasiliense, which was implicated in a case of otitis externa in a patient with spinocellular carcinoma basis cranii. The patient had been repeatedly treated for relapsing otitis externa and had previously undergone surgery several times for otitis media. The fungal aetiology was confirmed by repeated positive culture and histologic studies. The second case involved onychomycosis with strikingly brown nail discoloration due to Chaetomium globosum in an otherwise healthy patient. The nail lesion was successfully cured by oral terbinafine. The determination of both species was supported by sequencing of rDNA regions. The morphological aspect of Chaetomium spp. identification is also discussed. In vitro antifungal susceptibility tests demonstrated that both isolates were susceptible to terbinafine and azole derivates except fluconazole. Amphotericin B was effective only against the C. brasiliense strain. We review the literature to summarize clinical presentations, histologic findings, and treatment strategies.

A woman, 39 years old, with psoriatic foci on the palms and ears was given for six years, on account of the pathological findings in the auditory meatus, a corticoid lotion. The scaling deteriorated, she developed oedema, itching and earache. From the auditory meatus repeatedly Candida albicans was cultivated; treatment by a combined preparation containing natamycin and hydrocortisone had no effect. In cultures from both auditory canals massive growth of Candida albicans was found. After treatment with natamycin only in suspension the mycological finding became negative, on otoscopy a whitish coating persisted. After further treatment with the lotion containing tinctura carbonis detergens (without corticoid) the finding in the ear was restored to normal. One a half years after treatment the patient has no signs of relapse. The case demonstrates the importance of predisposing factors in the etiology and treatment of mycotic otitis.
