OBJECTIVES: Prognosis after pericardiectomy for constrictive pericarditis (CP) is affected by the aetiology of the constriction as well as by concomitant cardiac and non-cardiac disease, including liver dysfunction. However, few data exist on the risk stratification that accounts for liver function in patients with CP. We evaluated the effectiveness of the model for end-stage liver disease (MELD) score, an established measure of liver dysfunction, in predicting long-term survival and identifying other risk factors for death. METHODS: A total of 79 patients who underwent pericardiectomy for CP at a single centre between 2009 and 2016 were analysed. The prognostic utility of the MELD score was evaluated in our cohort. Logistic regression and Cox proportional hazard regression analyses were performed to assess the association of various clinical variables with 1-year and overall mortality rates. RESULTS: With multivariable analysis, only the MELD score was an independent predictor of 1-year mortality (P < 0.001); apart from the MELD (P = 0.003) score, post-surgical CP (P = 0.016), total bilirubin level (P = 0.042) and the European System for Cardiac Operative Risk Evaluation score II (P = 0.002) were independent predictors of overall mortality after pericardiectomy. Overall survival decreased as the MELD score increased. Scores ≤ 7.5, 7.51-15.50 and >15.5 were associated with overall survival rates of 92.9%, 69.8% and 8.3%, respectively. CONCLUSIONS: In addition to the underlying aetiology, we demonstrated that assessment of liver dysfunction using the MELD score provides additional information about risk because it is associated with postoperative death in patients undergoing pericardiectomy for CP.

Játra jsou klíčovým orgánem hemokoagulačních dějů. Játra ovlivňují počet a kvalitu trombocytů. V játrech se syntetizuje většina koagulačních a fi brinolytických faktorů, ale i jejich inhibitorů. V játrech dochází k odbourávání aktivovaných koagulačních a fi brinolytických faktorů. Z jater může být uvolňován endogenní heparin a tzv. heparin like substance. Pacienti mnohdy nekrvácí i při výrazném defektu prokoagulačních faktorů, protože zároveň klesají i inhibitory koagulace a jindy krvácí při relativně dobrém stavu koagulačních faktorů z lokálních příčin (jícnové varixy) nebo se uplatňuje trombocytopenie a trombocytopatie. Správné posouzení hemokoagulačních poměrů zkušeným hematologem umožní racionální léčebný postup.
The liver is the key organ for blood coagulation processes. The liver has an effect on platelet count and their quality. Additionally, it is the organ where most coagulation and fi brinolytic factors as well as their inhibitors are synthesized. Degradation of activated coagulation and fi brinolytic factors also occurs in the liver. Likewise, endogenous heparin and what is called heparin-like substance may be released from the liver. Patients often do not bleed even in the presence of a substantial defi ciency of procoagulation factors as this condition is associated with decreased levels of coagulation inhibitors while, in other cases, they do bleed for local causes (esophageal varices) even at relatively adequate levels of coagulation factors, or thrombocytopenia and thrombocypathy are at play. Appropriate assessment of blood coagulation status by an experienced hematologist will allow choosing a rational therapeutic option.

Cieľ práce: Zistiť častosť, trvanie, priebeh a klinický význam poškodenia pečene u pacientov s rubeolovou infekciou. Metodika: Retrospektívne sme analyzovali klinické a laboratórne údaje 155 pacientov hospitalizovaných s diagnózou rubeoly na Klinike pre infekčné choroby v Košiciach v rokoch 1995-1997. Trvanie prejavov poškodenia pečene sme hodnotili metódou analýzy času do zlyhania. Výsledky: Z celkového počtu 155 hospitalizovaných sme zistili laboratórne známky poškodenia pečene v 88 prípadoch (56,8 %). U väčšiny z nich (76,1 %) došlo k hepatálnemu poškodeniu súčasne s kožným výsevom, najvyššie hodnoty aminotransferáz sme však zaznamenali pri prijatí len u 46,6 % prípadov, pričom u niektorých to bolo až na tretí týždeň. Priemerné trvanie poškodenia pečene bolo 3,6 týždňa. Išlo väčšinou o ľahké poškodenie, zaznamenali sme však aj vyše 10-násobný vzostup aktivity aminotransferáz. Záver: Hepatálne poškodenie pri rubeole je pomerne časté, vzniká najčastejšie súčasne s objavením sa prvých príznakov, dosahuje však maximum často až po odoznení ostatných prejavov ochorenia a pomerne dlho pretrváva. Vznik chronického poškodenia pečene sme pri rubeole nepozorovali.
Objective: To determine the occurrence, duration, course and the clinical importance of liver damage in patients with rubella infection. Methods: We retrospectively analyzed clinical and laboratory data of 155 patients with rubella hospitalized at the Clinic of Infectious Diseases, Košice during the period 1995 - 1997. We evaluated the duration of the damage using the method of failure data analysis. Results: We observed laboraU)ry signs of liver damage in 88 cases (56,8%). In most cases (76,1%) the damage was accompanied by skin rash. We observed peak activity of serum aminotransferase at the time of admission in 46,6% of cases only. In some cases this occured during the third week. The mean duration of liver damage was 3,6 weeks. Intensity of the damage was mild in most cases, but we observed more than a tenfold increase of aminotransferase activity as well. Conclusion: Liver damage in rubella is rather frequent, mostly appearingalong with the onset of the first symptoms but often peaks only after the other symptoms disappear and is of lengthy duration. We did not observe development of chronic liver injury in patients with rubella infection.
