Úvod a cíl: Přímá perorální antikoagulancia (DOAC) jsou podávána ve fixních denních dávkách bez nutnosti rutinního monitorování. Přestože u jednotlivých pacientů existuje významná variabilita v systémové expozici, jsou údaje o populacích, u nichž lze očekávat abnormální koncentrace, nedostatečné. Cílem přehledového článku bylo analyzovat vybrané faktory, které významně ovlivňují koncentrace DOAC v klinické praxi s důrazem na situace, ve kterých by laboratorní monitorování DOAC mohlo najít své uplatnění. Metodika: Do přehledu byly zařazeny publikace z databází Pubmed a Scopus posuzující pomocí anti‐Xa aktivity či dilutovaného trombinového času vliv pokročilého věku, změněných renálních funkcí, sarkopenie, obezity, adherence k léčbě DOAC a lékových interakcí na plazmatické koncentrace DOAC. Výsledky: Zařazovací kritéria splnilo 34 studií. Z námi hodnocených faktorů ovlivňovaly statisticky významně plazmatické koncentrace DOAC rostoucí věk, renální funkce, klesající hmotnost, křehkost pacienta, adherence k léčbě DOAC a lékové interakce. Naopak vliv obezity nebyl prokázán. Závěr: Laboratorní monitorování DOAC by u pacientů s výše zmíněnými faktory mohlo přispět k optimalizaci jejich léčby.
Introduction and Aim: Direct oral anticoagulants (DOACs) are administered in fixed daily doses without the need for routine monitoring. Despite significant inter-patient variability in systemic exposure, data regarding populations predisposed to abnormal concentrations are limited. The objective of this literature review was to analyse selected factors that may significantly influence DOAC concentrations within clinical settings, emphasising situations in which laboratory monitoring of DOACs might be applicable. Methods: Publications from Pubmed and Scopus databases assessing the effect of advanced age, altered renal function, sarcopenia, obesity, medication adherence, and drug interactions on DOAC concentrations, as determined by anti-Xa activity or diluted thrombin time were included. Results: In total, 34 studies fulfilled the inclusion criteria. Among the assessed factors, increasing age, renal function, decreasing weight, frailty, medication adherence, and drug interactions demonstrated a statistically significant impact on therapeutic DOAC concentrations. Conversely, the influence of obesity was inconclusive. Conclusion: Therapeutic monitoring of DOAC concentrations in patients with the above-mentioned factors could optimize treatment outcomes.

OBJECTIVES: Degradation of coagulation proteins in frozen plasma may influence assay results. The aims of this study were to explore the changes in coagulation parameters in patient plasma and internal quality control (IQC) after different freezing and storage conditions during the short-term and long-term periods. METHODS: Platelet poor plasma was prepared from citrated peripheral blood collected from a group of healthy donors. The plasma was pooled, frozen and stored in a variety of freezing and storage conditions. The changes were monitored using routine coagulation assays, as well as factor VIII (FVIII) and protein S (PS) assays. RESULTS: Plasma stored in liquid nitrogen (LN 2 ) or in -80°C showed long-term stable values for routine tests for a period of over 12 months, and 6 months for FVIII. Interestingly, the activated partial thromboplastin time (aPTT) showed a temporary significant prolongation over the first two weeks. Plasma frozen and stored in -40°C is not viable for aPTT and FVIII testing, otherwise it can be used for other parameters for up to 4 months. PS showed a significant increase in all frozen samples. Freezing rate has a significant impact on plasma quality and the final storage temperature influences the long-term stability. CONCLUSION: The optimal storage conditions are ultra-low temperatures (LN 2 or -80°C) and the highest freezing rate possible. However, frozen plasma is not viable for IQC of aPTT during a period of two weeks after freezing. This study is unique in its conception as a practical guide for the handling of frozen plasma samples in modern laboratory settings.

Very high-frequency oscillations (VHFOs, > 500 Hz) are more specific in localizing the epileptogenic zone (EZ) than high-frequency oscillations (HFOs, < 500 Hz). Unfortunately, VHFOs are not visible in standard clinical stereo-EEG (SEEG) recordings with sampling rates of 1 kHz or lower. Here we show that "shadows" of VHFOs can be found in frequencies below 500 Hz and can help us to identify SEEG channels with a higher probability of increased VHFO rates. Subsequent analysis of Logistic regression models on 141 SEEG channels from thirteen patients shows that VHFO "shadows" provide additional information to gold standard HFO analysis and can potentially help in precise EZ delineation in standard clinical recordings.
