The first to use the term Scapula was Vesalius (1514-1564) and thus it has remained ever since. Probably the oldest injured scapula, from 250 million years ago, was described by Chinese authors of a skeletal examination of a fossilised remains of a dinosaur Yangchuanosaurus hepingensis. In humans, the oldest known scapular fractures date back to the prehistoric and early historic times. In ancient times, a fracture of acromion was described in the treatises of Hippocrates. Early modern history of the treatment of scapular fractures is closely interlinked with the history of the French surgery. The first to point out the existence of these fractures were Petit, Du Verney and Desault in the 18th century. The first study devoted solely to scapular fractures was published by Traugott Karl August Vogt in 1799. Thomas Callaway published in 1849 an extensive dissertation on injuries to the shoulder girdle, in which he discussed a number of cases known at that time. The first radiograph of a scapular fracture was published by Petty in 1907. Mayo Robson (1884), Lambotte (1913) and Lane (1914) were pioneers in the surgical treatment of these fractures, followed in 1923 by the French surgeons Lenormat, Dujarrier and Basset. The first internal fixation of the glenoid fossa, including a radiograph, was published by Fischer in 1939.

Reálná historie prvních pokusů o operační léčbu zlomenin je mnohem komplikovanější a zajímavější, než se doposud uvádí. Jako první případ, u kterého byla provedena vnitřní osteosyntéza, je často zmiňována zlomenina humeru ošetřená drátěnou cerkláží, a to v roce 1775. Detailní rozbor originálních pramenů ukázal, že tato operace nikdy neproběhla. První a detailní popis vnitřní osteosyntézy publikoval v roce 1827 americký chirurg John Kearny Rodgers. Jednalo se o rok trvající suprakondylický pakloub diafýzy humeru u 15letého hocha. V roce 1838 se zapsal do dějin osteosyntézy francouzský chirurg Achille-Cléophas Flaubert. U 21leté ženy s pakloubem humeru provedl resekci pseudoartrózy a osteosuturu blíže nespecifikovaným materiálem. O několik týdnů později Flaubert úspěšně operoval 55letého kočího, který utrpěl otevřenou zlomeninu humeru. Zřejmě první, komu se podařilo před zavedením antisepse úspěšně provést osteosyntézu čéšky, byl americký chirurg Thomas Maldrup Logan. V roce 1864 provedl osteosyntézu inveterované zlomeniny čéšky u 30letého muže. Za duchovního otce nitrodřeňové osteosyntézy lze považovat významného berlínského chirurga Johanna Friedricha Dieffenbacha, který použil v roce 1846 k léčbě pakloubů femuru, tibie a humeru nitrodřeňové čepy ze slonoviny. První osteosyntézu zlomeniny krčku femuru perkutánně zavedeným vrtákem provedl roku 1858 německý chirurg Bernhard Rudolf Konrad von Langenbeck v Berlíně.
The history of the first attempts at surgical treatment of fractures is much more complicated and interesting than reported until now. A fracture of the humerus treated by wire cerclage in 1775 is often mentioned as the first case of internal fixation. However, a detailed analysis of original sources has revealed that this surgery has not been performed. The first and detailed description of internal fixation was published by the American surgeon John Kearny Rodgers in 1827. It was used to treat a supracondylar non-union of the humeral shaft in a 15-year old boy, persisting for one year. French surgeon Achille-Cléophas Flaubert entered the history of internal fixation in 1838 when he treated a 21-year old woman with a non-union of the humerus by its resection and osteosuture with a non-specified material. Several weeks later Flaubert successfully treated a 55-year old coachman who sustained an open fracture of the humerus. Probably the first to perform a successful internal fixation of the patella in the pre-antiseptic era was the American surgeon Thomas Maldrup Logan. In 1864, he used this method in an inveterate fracture of the patella in a 30-year old man. The father of the intramedullary fixation may be considered Johann Friedrich Dieffenbach, a prominent German surgeon from Berlin, who used intramedullary ivory pegs to treat non-unions of the femur, tibia and humerus in 1846. The first internal fixation of a fracture of the femoral neck by a percutaneously inserted nail was performed by German surgeon Bernhard Rudolf Konrad von Langenbeck in 1858.

The presented overview shows that the first efforts to address patellar fractures by operative treatment, although sporadic, emerged in the pre-antiseptic era. Introduction of Lister's antiseptic method in Great Britain, Germany and the USA in 1877-1882 contributed to a new, progressive trend in the treatment of fractures of the patella as well as of other bones. The authors, using Lister's antiseptic method, evaluated three basic aspects in their operations, namely healing of the surgical wound without infective complications, achievement of bony union and a good functional outcome, i.e. restoring the range of motion in the knee, even if limited in most cases. The majority of them used a longitudinal incision and fixation by a silver wire.

The most frequently used implants for internal fixation of proximal femoral fractures are currently the dynamic hip screw and the intramedullary hip nail. However, little has been written about one of the pioneers in this field, a German genius, the designer Ernst Pohl (1876-1962). Without his involvement the concepts of intramedullary nailing coined by Gerhard Küntscher, Richard Maatz and other surgeons could hardly have been implemented. Through his achievements Pohl has rightly merited his pre-eminent position in the history of bone surgery. This article outlines the extraordinary contribution of Ernst Pohl to the development of skeletal surgery and radiology, as well as other medical disciplines.

Operační léčba dislokovaných intra- a extraartikulárních zlomenin lopatky nabývá na aktuálnosti. Nezbytností při její indikaci je přesné určení typu zlomeniny a její dislokace. K tomu jsou nutné nejen obě Neerovy projekce, ale i 3D CT rekonstrukce při pohledu ze zadní, laterální, přední a mediální strany se subtrakcí hlavice humeru a klíčku. Cíl operační léčby závisí na typu zlomeniny. U dislokovaných intraartikulárních zlomenin je to obnovení kongruence a stability kloubu. U dislokovaných extraartikulárních zlomenin výběžků, především korakoideu, akromia a spiny, je cílem dosáhnout zhojení v anatomickém postavení, neboť zhojení v dislokaci může komprominovat rotátorovou manžetu. U dislokovaných extraartikulárních zlomenin těla a krčku lopatky je cílem obnovení původního postavení glenoidu a biomechanického těla lopatky (gleno-polární úhel), tzn. rekonstrukce délky a integrity laterálního okraje. Pro normální pohyb lopatky je důležité obnovit i kongruenci mezi její přední plochou a hrudní stěnou, popř. odstranit fragmenty lopatky zaražené v hrudní stěně. V současné době jsou k operační léčbě indikovány extraartikulární zlomeniny těla a krčku lopatky splňující tato kritéria: 100% translace fragmentů laterálního okraje, 25° úhlová dislokace glenoidu k laterálnímu okraji lopatky, medializace glenoidu vzhledem k laterálnímu okraji větší než 1 cm, fragment lopatky zaražený do hrudní stěny. Vzhledem k tomu, že většina zlomenin lopatky je operována odloženě, je nutná identifikace všech základních úlomků a jejich dokonalé očištění od formujícího se svalku. U zlomenin těla a krčku lopatky je laterální pilíř klíčovou strukturou pro úspěšnou osteosyntézu. Je třeba obnovit jeho délku a osový vztah ke glenoidu. Pro fixaci laterálního okraje se osvědčila 3.5 rekonstrukční DCP dlaha s dvěma šrouby v každém z hlavních fragmentů (2+2). Stabilitu osteosyntézy zlomenin těla výrazně zvýší fixace zlomeniny mediálního okraje.
Surgical treatment of displaced intra- and extra-articular fractures of the scapula becomes a topical issue. Exact determination of the type of the fracture and its displacement represents necessary prerequisite for its indication. This requires the two Neer views as well as 3D CT reconstruction from the posterior, lateral, anterior and medial views with subtraction of the humeral head and the clavicle. The aim of surgical treatment depends on the type of fracture. In displaced intra-articular fractures it is the restoration of the congruency and stability of the joint. In displaced extra-articular fractures of processes, particularly of the coracoid, acromion and spine, the aim is to achieve healing in anatomical position, since healing in displacement may compromise the rotator cuff. In displaced extra-articular fractures of the scapular body and neck the objective of treatment is to restore the anatomical position of the glenoid to the lateral border of the scapular body (the gleno-polar angle), i.e. to reconstruct the length and integrity of the lateral border. To achieve the normal range of motion of the scapula the restoration of congruency between its anterior surface and the chest wall is also important or, where necessary, it also requires removal of fragments penetrated into the chest wall. Indicated currently for surgical treatment are extra-articular fractures of the scapular body and neck that meet the following criteria: 100% translation of fragments of the lateral border, 25-degree displacement of the glenoid to the lateral border of the scapula, medial displacement of the glenoid to the lateral border exceeding 1cm, and a fragment of the scapula penetrated into the chest wall. Surgical treatment of fractures of the scapular body and neck requires adequate mastering of the posterior Judet approach. Given the fact that most of the fractures are operated on with a certain delay it is necessary to identify all important fragments and remove completely the callus being formed there. In particular it applies to intra-articular fragments and also to fractures of the lateral border, including its interfragments. In fractures of the scapular body and neck the lateral border is a key structure for a successful internal fixation. Therefore it is necessary to restore its length and axial alignment to the glenoid. The 3.5 reconstruction DCP plate with two screws in each of the main fragments (2+2) was proven suitable for fixation of the lateral border. Stability of internal fixation of the scapular body is considerably increased by fixation of the fracture of the medial border.
V období let 1818–1931 byly položeny základy pro úspěšnou léčbu zlomenin proximálního femuru. Počáteční přínos britské školy první poloviny 19. století spočíval kromě základní klasifikace v provedení prvních experimentů, dokonalém propracování klinické diagnostiky a popisu morfologie jednotlivých typů zlomenin (zlomeniny krčku, zlomeniny trochanterické) pakloubů i posttraumatických deformit, a to především na základě pitevních (sekčních) nálezů. Němečtí chirurgové Volkmann, Trendelenburg a König se v 50. až 70. letech 19. století jako první odvážili provést osteosyntézu zlomeniny krčku femuru, ale doba ještě nebyla zralá pro tento způsob léčby. Bezprostředně poté se zformovala americká chirurgická škola, které dal svými experimenty základní předpoklady N. Senn. Američtí ortopedi a chirurgové byli na přelomu 19. a 20. století velmi aktivní. Přinesli řadu nových prvků (otevřený způsob osteosyntézy z různých operačních přístupů, použití hřebů, šroubů, čepů z kosti a ze slonoviny, návrh zevní fixace a úhlové dlahy). Jejich evropským současníkům a mimořádným osobnostem kostní chirurgie Lambottovi, Delbetovi, Hey-Grovesovi a Puttimu se přes nesporné úspěchy nepodařilo prosadit osteosyntézu zlomenin krčku femuru jako standardní metodu a vytvořit školu, která by prosadila jejich myšlenky do běžné praxe. Nicméně zůstali inspirací pro své následníky. Teprve technický pokrok (nerezavějící ocel, nové technologické možnosti), propracování anestezie a asepse, zdokonalení rtg přístrojů, zavedení extenčního stolu, to vše vytvořilo předpoklady pro úspěšnou gradaci americké školy v podobě Smith-Petersenovy metody během 30. let 20. století.
From 1818 to 1931, foundations of the successful treatment of fractures of the proximal femur were laid. The initial contribution of the British school during the first half of the nineteenth century consisted of the first classification, first experiments on animals, detailed description of clinical symptoms and the morphologic features of different types of fractures (femoral neck fractures, trochanteric fractures), nonunions, and posttraumatic deformities, mainly on the basis of autopsy. In the 1850s through the 1870s, German surgeons Volkmann, Trendelenburg, and König were the first to do internal fixation of fractures of the femoral neck, but the time was not ready yet for this method of treatment. In 1891, the Hungarian surgeon Dolinger was probably the first one to perform open reduction and internal fixation (wire cerclage) of the extracapsular fracture of the proximal femur. In 1897-1899, the Norwegian surgeon Nicolaysen operated on a bigger group of patients, using radiograph both for preoperative diagnosis and for postoperative results control. The American surgical school was formed at the end of the 19th century and was based on the experiments of N. Senn. The American orthopaedic and general surgeons were quite active and brought numerous new elements, e.g. the open method of internal fixation from different surgical approaches, new implants (nails, screws, bone and ivory pegs, external fixation, angled blade plate). Despite unquestionable success in using internal fixation of fractures of the proximal femur as a standard method, their European contemporaries and outstanding personalities in the sphere of bone surgery, Lambotte, Delbet, Hey-Groves and Putti, failed to introduce the surgical treatment into general practice. They, however, inspired their successors. Further technical progress (stainless steel, new technologies), development of anaesthesia and asepsis, improvement of x-ray apparatus, and introduction of a traction table only later on created prerequisites for a successful development of the American school in the form of the Smith-Petersen method during the 1920s.
