V dermatopatologické praxi existuje skupina atypických melanocytárních lézí s hraničními histologickými rysy mezi benigními lézemi, podobnými melanomu, a maligním melanomem (MM), při konfl iktních dia gnostických kritériích a při rozporech mezi jednotlivými vyšetřujícími. V těchto případech je dermatopatolog oprávněn konzultovat s odborníkem v oboru, ale ani nejzkušenější odborník si nemusí být jistý, zda byla stanovena správná dia gnóza a následné bio logické chování. Existuje obecná shoda mezi kvalifi kovanými dermatopatology, která může pomoci zařadit tyto nejasné případy do dvou dia gnostických kategorií: SAMPUS (superfi ciální atypická melanocytární proliferace nejasného významu) a MELTUMP (melanocytární nádory nejistého maligního potenciálu). Podle koncepce dvoufázové progrese MM, fáze radiálního růstu a fáze vertikálního růstu, je možné identifi kovat nový podtyp tenkého melanomu (THIM) s nejistým metastatickým potenciálem v důsledku přítomnosti rozsáhlé regrese (≥ 75 % objemu lézí), které zde poprvé v literatuře defi nujeme pomocí zkratky THIMUMP (tenký melanom nejistého metastatického potenciálu).
In the dermatopathological practice, there is a group of atypical melanocytic lesions with borderline histological features between benign simulants and malignant melanoma (MM), due to confl icting dia gnostic criteria and inter-observer disagreement. In these cases, the dermatopathologist is authorized to seek consult with an established expert in the fi eld, but even the most experienced specialist may not be sure about the correct dia gnosis and the subsequent bio logical behavior. There is general consensus among qualifi ed dermatopathologists that can be helpful to insert these ambiguous cases into two dia gnostic categories: SAMPUS (Superfi - cial Atypical Melanocytic Proliferations of Unknown Signifi cance) and MELTUMP (MELanocytic Tumors of Uncertain Malignant Potential). According to the conception of MM progression through two phases, the radial growth phase and the vertical growth phase, it is possible to identify a novel subtype of thin melanoma (THIM) with uncertain metastatic potential, due to the presence of extensive regression (≥ 75% of the lesion volume), which we here defi ne with the acronym THIMUMP (THIn Melanoma of Uncertain Metastatic Potential) for the fi rst time in literature.
Mezi klasickými typy melanocytárních névů a melanomů je šedá zóna atypických melanocytárních lézí. Patří sem některé névy napodobující melanom, melanomy napodobující névy a nekonečné variace cytologicky a architektonicky atypických lézí, u kterých nelze s jistotou rozhodnout o jejich biologické povaze. Atypické melanocytární léze jsou běžnou součástí každodenní praxe klinické a histopatologické, s různou mírou rizika melanomu. Článek si klade za cíl nastínit variabilitu melanocytárních lézí a navrhnout algoritmus klinického a histologického postupu u atypických z nich.
There is a grey zone of atypical melanocytic lesions in between typical melanocytic nevi and melanoma. It comprises some melanoma-like nevi, nevus-like melanoma and endless variations of atypical lesions with atypical cytology and architecture with uncertain biologic potential. Atypical melanocytic lesions with variable risk of melanoma belong to daily clinical and histopathological practice. The article aims to outline the variability of melanocytic lesions and to suggest an algorithm of clinical and histological proceedings in atypical lesions.
