Článek se zabývá možnými způsoby léčby chronické bolesti silnými opioidy, která může být při neodborném vedení provázena řadou komplikací. Bez léčby silnými opioidy si nelze léčbu silné chronické bolesti představit, je však nutné držet se několika základních doporučení ke správné indikaci a monitoraci celého průběhu léčby. V posledních letech se prosazuje nový koncept klasifikace opioidů na základě jejich receptorového mechanismu působení a tento koncept může být vodítkem k šetrnější léčbě silnými opioidy, zejména u nenádorové bolesti. Léčba chronické bolesti pouze neopiopidními analgetiky je z hlediska nežádoucích účinků a léčebných interakcí rovněž při neodborném vedení riziková. Buprenorfin je parciální μ-receptorový agonista a byl dosud dostupný v transdermálních náplastech pro léčbu středně silné až silné chronické bolesti nádorového i nenádorového původu, aktuálně přichází na trh v novém dávkování náplasti s obsahem buprenorfinu uvolňující dávky 5, 10, 15, 20, 30 a 40 μg/h. Úhradu ze zdravotního pojištění mají aktuálně síly 5, 20, 30 a 40 μg/h. Jsou indikovány k terapii středně silné až silné nenádorové bolesti u dospělých pacientů v případech, kdy je k dosažení dostatečné analgezie nutné podání opioidů. Výhodou terapie je jednoduchá manipulace s náplastí s prodlouženým intervalem výměny 7 dní a bezproblémová titrace dávky buprenorfinu, léčba je dostatečně analgeticky efektivní a dobře pacienty tolerovaná.
The article discusses the possible causes of chronic pain with strong opioids , which can be accompanied by a number of complications if not managed professionally. Treatment of severe chronic pain cannot be imagined without strong opioids, but it is necessary to follow some simple recommendations for proper indication and monitoring of the whole course of treatment. In recent years, a new concept of classifying opioids based on their receptor mechanism of action has been promoted, and this concept may guide more gentle treatment with strong opioids, especially for non-cancer pain. The treatment of chronic pain with non-opioid analgesics is also risky in terms of adverse effects and drug interactions if not managed professionally. Buprenorphine is a partial μ-receptor agonist and was previously available in transdermal patches for the treatment of moderate to severe chronic pain of both tumor and non-tumor origin, and currently buprenorphine patches containing 5, 10, 15, 20, 30 and 40 μg/h are coming to market in new dosages. Health insurance reimbursement is currently for the 5, 20, 30 and 40 μg/h strengths. They are indicated for the treatment of moderate non-cancer pain in adult patients in cases where opioid administration is required to achieve sufficient analgesia. The therapy has the advantage of easy patch manipulation with an extended replacement interval of 7 days and easy titration of the buprenorphine dose, the treatment is sufficiently analgesic effective and well tolerated by patients.
Endogenous opioid peptides serve as potent analgesics through the opioid receptor (OR) activation. However, they often suffer from poor metabolic stability, low lipophilicity, and low blood-brain barrier permeability. Researchers have developed many strategies to overcome the drawbacks of current pain medications and unwanted biological effects produced by the interaction with opioid receptors. Here, we tested multifunctional enkephalin analogs LYS739 (MOR/DOR agonist and KOR partial antagonist) and LYS744 (MOR/DOR agonist and KOR full antagonist) under in vivo conditions in comparison with MOR agonist, morphine. We applied 2D electrophoretic resolution to investigate differences in proteome profiles of crude membrane (CM) fractions isolated from the rat brain cortex and hippocampus exposed to the drugs (10 mg/kg, seven days). Our results have shown that treatment with analog LYS739 induced the most protein changes in cortical and hippocampal samples. The identified proteins were mainly associated with energy metabolism, cell shape and movement, apoptosis, protein folding, regulation of redox homeostasis, and signal transduction. Among these, the isoform of mitochondrial ATP synthase subunit beta (ATP5F1B) was the only protein upregulation in the hippocampus but not in the brain cortex. Contrarily, the administration of analog LYS744 caused a small number of protein alterations in both brain parts. Our results indicate that the KOR full antagonism, together with MOR/DOR agonism of multifunctional opioid ligands, can be beneficial in treating chronic pain states by reducing changes in protein expression levels but retaining analgesic efficacy.

Chronická bolest představuje závažný zdravotní problém, který zatěžuje celý systém zdravotnictví. Její léčba by měla být vedena racionálně a na podkladě současných vědeckých poznatků. Článek shrnuje současný pohled na tuto problematiku a poukazuje na nová analgetika a doporučení.
Chronic pain is a serious health problem that puts a strain on the entire health system. Her treatment should be conducted rationally and on the basis of current scientific knowledge. The article summarizes the current view of this issue and points to new painkillers and recommendations.

OBJECTIVES: The aim of this review is to describe the effects of analgesics on sleep. DATA SOURCES: Systematic search of the databases of PubMed and the Cochrane Library was performed between January and September 2021. REVIEW/ANALYSIS METHODS: The search included all articles on the topic published during the past 20 years (2000-2020). The search strategy was developed using a controlled vocabulary of known studies meeting the inclusion criteria and focused on the following terms: chronic pain, pain, sleep disturbance, insomnia, analgesic, analgesic medication, antidepressants, antiepileptic drugs, nonsteroidal drugs, opioids, and quality of life. Two reviewers independently considered the studies for inclusion in the review, assessed the risk of bias, and extracted data. DESIGN: Review and analysis. RESULTS: A total of 37 studies met the inclusion criteria: 15 analyzed the effects of opioids, 6 those of nonsteroidal anti-inflammatory drugs and acetaminophen, and 16 the effects of adjuvant analgesics. CONCLUSIONS: Sleep quality may be adversely affected by a variety of medications used in clinical practice, including those used in analgesic indications. The class of analgesics most affecting sleep quality are opioids.

Opioidy jsou základem léčby středně silné (4-7/10 na numerické škále) a silné bolesti (nad 7/10) spojené s onkologickým onemocněním. U většiny pacientů vedou ke znatelné a uspokojivé úlevě rychle, část pacientů však vyžaduje pečlivé hledání optimálního režimu. Na straně lékaře je nutná znalost této lékové skupiny, zkušenost, trpělivost a komunikační schopnosti. V přehledovém článku je na základě aktuální literatury popsáno vnímání opioidní léčby odbornou i laickou veřejností, shrnuta etická východiska. Spolu s přehledem dostupných opioidů a jejich antagonistů jsou zmíněny nejčastější klinické scénáře, shrnuty nežádoucí účinky a připojeny tipy z praxe.
Opioids are the mainstay of treatment for moderate (4-7/10 on a numerical scale) and severe (above 7/10) cancer-related pain. In most patients, they rapidly lead to noticeable and satisfactory relief; however, in a proportion of patients, a thorough search for the optimal regimen is required. As for the doctor, knowledge of this particular drug class, experience, patience, and communicative skills are all required. Based on the current literature, the review article describes the perception of opioid treatment by both professionals and lay people and summarizes ethical considerations. In addition to an overview of available opioids and their antagonists, the most common clinical scenarios are presented, adverse effects are summarized, and tips from practice are included.
BACKGROUND: There is a lack of studies on methamphetamine (MA) exposure and morbidity in children beyond the perinatal period. OBJECTIVES: We compared morbidity in children (0-3 years) with prenatal MA exposure to opioid-exposed and to non-exposed children. METHODS: We used data from a Czech nationwide, registry-based cohort study (2000-2014). Children, who reached 3 years of age, of mothers hospitalized with (i) MA use disorder during pregnancy (MA; n = 194), (ii) opioid use disorder during pregnancy (opioids; n = 166), and (iii) general population (GP; n = 1,294,349) with no recorded history of substance use disorder (SUD). Information on inpatient contacts, length of stay, and diagnoses (International Statistical Classification of Diseases and Related Health Problems 10th Revision [ICD-10]) were assessed. Crude and adjusted odds ratios (aOR), 95% confidence interval (CI) for the risk of hospitalization, and for getting diagnosis from the ICD-10 diagnosis chapters were calculated using binary logistic regression. A stratified analysis on hospitalizations with SUD of mothers was performed. RESULTS: No significant differences were found in the measures of hospitalization between the MA and opioid groups. Children prenatally exposed to MA and opioids had higher numbers of hospitalizations and diagnoses and longer stays in hospital than children in the GP. Increased risks of certain infectious and parasitic diseases were found in both MA (aOR = 1.6; CI: 1.1-2.3) and opioid (aOR = 1.9; 1.3-2.8) groups as compared to the GP group. The most pronounced difference in stratified analysis on maternal hospitalizations related to SUD after birth was observed for injury, poisoning, and certain other consequences of external causes in the strata of the MA group who had hospitalized mothers (aOR 6.3, 1.6-24.6) compared to the strata without maternal hospitalizations (aOR 1.4, 0.9-2.3). CONCLUSION: This study suggests that children born to mothers using MA during pregnancy have similar morbidity during the first 3 years of life but higher than the GP. The excess of risk was primarily due to infections and injuries in the MA group.

Nejvýznamnější mikrovaskulární neurologickou komplikací diabetu mellitu je diabetická neuropatie. Tento termín zahrnuje kromě nejčastější distální senzitivní či senzitivně‐motorické polyneuropatie také neuropatii autonomní a řadu fokálních forem (mononeuropatie, plexopatie). Diagnostika je založena na anamnéze a klinickém vyšetření, volba dalších diagnostických metod (elektromyografické vyšetření, testy autonomních nervových funkcí, hodnocení intraepidermální inervace apod.) záleží na typu postižených nervových vláknech. Základním opatřením v terapii i prevenci rozvoje a progrese diabetické neuropatie je dlouhodobá optimální metabolická kompenzace diabetu a dalších kardiovaskulárních rizikových faktorů a pohybová aktivita. U algických forem se dále uplatňují preparáty ovlivňující neuropatickou bolest, tedy antiepileptika ze skupiny modulátorů funkce α2δ podjednotky kalciových kanálů (gabapentin, pregabalin) a antidepresiva ze skupiny inhibitorů zpětného vychytávání serotoninu a noradrenalinu (duloxetin, venlafaxin) či tricyklických antidepresiv (amitriptylin). Analgetický efekt má i kyselina thioktová (alfa‐lipoová) a řada opioidů. Diabetes mellitus je také nezávislým cerebrovaskulárním rizikovým faktorem a podílí se tedy na vzniku ischemických cévních mozkových příhod. Signifikantně zvyšuje také riziko rozvoje kognitivních poruch
The most important microvascular neurological complication of diabetes mellitus is diabetic neuropathy. In addition to the most common distal sensory or sensory-motor polyneuropathy, this term also includes autonomic neuropathy and a number of focal forms (mononeuropathy, plexopathy). Diagnosis is based on history and clinical examination; the choice of other diagnostic methods (nerve conduction studies and needle electromyography, autonomic nerve function tests, assessment of intraepidermal innervation from skin biopsy, etc.) depends on the type of nerve fibers affected. The basic intervention in the therapy and prevention of the development and progression of diabetic neuropathy is long-term optimal control of diabetes and other cardiovascular risk factors, and physical activity. In painful forms, drugs affecting neuropathic pain are also used, i.e. antiepileptics from the group of modulators of the α2δ subunit of calcium channels (gabapentin, pregabalin) and antidepressants from the group of serotonin and noradrenaline reuptake inhibitors (duloxetine, venlafaxine) or tricyclic antidepressants (amitriptyline). Thioctic acid (alpha-lipoic acid) and a number of opioids also have an analgesic effect. Diabetes mellitus is also an independent cerebrovascular risk factor and thus contributes to the development of ischaemic stroke. It also significantly increases the risk of developing cognitive impairment.
