BACKGROUND: The role of high-frequency oscillatory ventilation (HFOV) has long been debated. Numerous studies documented its benefits, whereas several more recent studies did not prove superiority of HFOV over protective conventional mechanical ventilation (CV). One of the accepted explanations is that CV and HFOV act differently, including gas exchange. METHODS: To investigate a different level of coupling or decoupling between oxygenation and carbon dioxide elimination during CV and HFOV, we conducted a prospective crossover animal study in 11 healthy pigs. In each animal, we found a normocapnic tidal volume (VT) after the lung recruitment maneuver. Then, VT was repeatedly changed over a wide range while keeping constant the levels of PEEP during CV and mean airway pressure during HFOV. Arterial partial pressures of oxygen (PaO2) and carbon dioxide (PaCO2) were recorded. The same procedure was repeated for CV and HFOV in random order. RESULTS: Changes in PaCO2 intentionally induced by adjustment of VT affected oxygenation more significantly during HFOV than during CV. Increasing VT above its normocapnic value during HFOV caused a significant improvement in oxygenation, whereas improvement in oxygenation during CV hyperventilation was limited. Any decrease in VT during HFOV caused a rapid worsening of oxygenation compared to CV. CONCLUSION: A change in PaCO2 induced by the manipulation of tidal volume inevitably brings with it a change in oxygenation, while this effect on oxygenation is significantly greater in HFOV compared to CV.

Outdoor breathing trials with simulated avalanche snow are fundamental for the research of the gas exchange under avalanche snow, which supports the development of the international resuscitation guidelines. However, these studies have to face numerous problems, including unstable weather and variable snow properties. This pilot study examines a mineral material perlite as a potential snow model for studies of ventilation and gas exchange parameters. Thirteen male subjects underwent three breathing phases-into snow, wet perlite and dry perlite. The resulting trends of gas exchange parameters in all tested materials were similar and when there was a significant difference observed, the trends in the parameters for high density snow used in the study lay in between the trends in dry and wet perlite. These findings, together with its stability and accessibility year-round, make perlite a potential avalanche snow model material. Perlite seems suitable especially for simulation and preparation of breathing trials assessing gas exchange under avalanche snow, and potentially for testing of new avalanche safety equipment before their validation in real snow.The study was registered in ClinicalTrials.gov on January 22, 2018; the registration number is NCT03413878.

BACKGROUND: The study analyzes changes in lung function, pulmonary pressure and diffusing capacity of the lung in patients with mitral valve regurgitation (MR) treated by MitraClip implantation. METHODS: A total of 43 patients (19 women and 24 men with an average age of 78.0 ± 6.6 years) who were able to perform pulmonary function testing including diffusing capacity of the lung for carbon monoxide (DLCO), vital capacity (VC), total lung capacity (TLC), residual volume (RV) and forced expiratory volume in 1 s (FEV1) before and 6 weeks after MitraClip implantation participated in this study. Furthermore, clinical and echocardiographic parameters including systolic pulmonary artery pressure (sPAP), left ventricular ejection fraction (LVEF) and left atrial diameter (LAD) measurements were recorded in all patients. RESULTS: The procedure was performed successfully in all 43 patients leading to a reduction of MR in 97.7% of cases. One patient died on day 4 after the intervention most likely due to pulmonary artery embolism. Six weeks after the implantation 79.1% of patients showed a MR of at most mild to moderate. Furthermore, we could demonstrate a significant reduction of systolic pulmonary artery pressure during follow-up (from 48.8 ± 11.4 mmHg to 42.9 ± 9.0 mmHg (t(41) = - 2.6, p = 0.01). However, no changes in LVEF were detected. Comparing pre and post implant lung function tests, no significant alterations were seen for VC, TLC, DLCO and FEV1. Though, in a subgroup of patients with moderate to severe preexisting deterioration of DLCO at the baseline (max. 50%) the MitraClip procedure resulted in a significant improvement in DLCO (37.8% ± 9.0 to 41.6% ± 10.0, p < 0.001). CONCLUSIONS: Treatment of MR with the MitraClip system successfully reduces MR severity in the vast majority of patients. Consecutively, a reduction in pulmonary pressure could be observed, however no changes in LVEF were obvious. Lung function tests remained unaltered during follow-up. However, in a subgroup of patients with severe preexisting deterioration of DLCO the MitraClip procedure resulted in a significant improvement in DLCO. Cystická fibróza (CF) je závažné postupně progredující a život limitující autosomálně recesivně dědičné multiorgánové onemocnění, charakterizované v dětském věku zejména neprospíváním a opakovanými respiračními infekty s dlouhodobým zahleněním. Vývoj onemocnění nelze predikovat ani při stále se zlepšujících diagnostických a terapeutických možnostech. Snaha o maximální kompenzaci základního onemocnění, prevenci a časnou léčbu komplikací, a zejména personalizovaná péče s umožněním inovativní terapie nemocným s dobrou compliance při splnění genetického a věkového hlediska, jsou podstatou aktuálního léčebného přístupu k nemocným s CF. S objevením a zavedením inovativní terapie modulátory transmembránového regulátoru vodivosti - proteinu CFTR (cystic fibrosis transmembrane conductance regulator) - do klinické praxe se od roku 2012 významně zlepšila kvalita života nemocných i prognóza onemocnění. V současné době jsou v České republice k dispozici čtyři typy modulátorů CFTR: potenciátor ivacaftor samostatně (Kalydeko) či v kombinaci s korektorem lumacaftor (Orkambi), s korektorem tezacaftor (Symkevi) nebo s dvěma korektory elexacaftor a tezacaftor (Kaftrio). Při nesplnění indikačních kritérií kauzální terapie zůstává snaha o maximální zintenzivnění symptomatické léčby se začleněním přístroje PhysioAssist neboli Simeox do každodenního režimu k usnadnění expektorace a zajištění efektivní airway clearance (hygieny dýchacích cest). Jak dokládá prezentovaná kazuistika 13leté dívky, klinický i funkční benefit z kauzální terapie lze očekávat i u nemocných s CF splňujících indikaci k léčbě modulátory podle registrace amerického Úřadu pro kontrolu potravin a léčiv.
Cystic fibrosis (CF) is a serious, age-progressive, life-limiting, autosomal recessively inherited multiorgan disease, characterized in childhood mainly by failure to thrive and recurrent respiratory infections with chronic obstruction. The development of the disease cannot be predicted even with ever-improving diagnostic and therapeutic options. Efforts to maximally compensate for the underlying disease, prevention and early treatment of complications, and especially personalized care with the possibility of innovative therapy in patients with good compliance while meeting genetic and age aspects, is the essence of the current therapeutic approach to patients with CF. With the discovery and introduction of innovative therapy with CFTR protein modulators into clinical practice, the quality of life of patients and the prognosis of the disease have significantly improved since 2012. There are currently four types of CFTR modulators available in the Czech Republic: the ivacaftor potentiator alone (Kalydeko) or in combination with the lumacaftor corrector (Orkambi), the tezacaftor corrector (Symkevi), or two elexacaftor and tezacaftor correctors (Kaftrio). If the indication criteria for causal therapy are not met, the effort remains to maximize the intensification of symptomatic treatment with the incorporation of PhysioAssist or Simeox into the daily regimen to facilitate expectoration and ensure effective airway clearance. As the presented case report of a 13-year-old girl demonstrates, clinical, laboratory and functional benefits from causal therapy can also be expected in CF patients who meet the indication for modulator treatment according to the U.S. Food and Drug Administration.

Our previous study demonstrated increased expression of Heat shock protein (Hsp) 90 in the skin of patients with systemic sclerosis (SSc). We aimed to evaluate plasma Hsp90 in SSc and characterize its association with SSc-related features. Ninety-two SSc patients and 92 age-/sex-matched healthy controls were recruited for the cross-sectional analysis. The longitudinal analysis comprised 30 patients with SSc associated interstitial lung disease (ILD) routinely treated with cyclophosphamide. Hsp90 was increased in SSc compared to healthy controls. Hsp90 correlated positively with C-reactive protein and negatively with pulmonary function tests: forced vital capacity and diffusing capacity for carbon monoxide (DLCO). In patients with diffuse cutaneous (dc) SSc, Hsp90 positively correlated with the modified Rodnan skin score. In SSc-ILD patients treated with cyclophosphamide, no differences in Hsp90 were found between baseline and after 1, 6, or 12 months of therapy. However, baseline Hsp90 predicts the 12-month change in DLCO. This study shows that Hsp90 plasma levels are increased in SSc patients compared to age-/sex-matched healthy controls. Elevated Hsp90 in SSc is associated with increased inflammatory activity, worse lung functions, and in dcSSc, with the extent of skin involvement. Baseline plasma Hsp90 predicts the 12-month change in DLCO in SSc-ILD patients treated with cyclophosphamide.

The heart and lungs are intimately linked. Hence, impaired function of one organ may lead to changes in the other. Accordingly, heart failure is associated with airway obstruction, loss of lung volume, impaired gas exchange, and abnormal ventilatory control. Cardiopulmonary exercise testing is an excellent tool for evaluation of gas exchange and ventilatory control. Indeed, many parameters routinely measured during cardiopulmonary exercise testing, including the level of minute ventilation per unit of carbon dioxide production and the presence of exercise oscillatory ventilation, have been found to be strongly associated with prognosis in patients with heart failure.
