Terapie levothyroxinem je součástí léčby diferencovaného karcinomu štítné žlázy již po desetiletí. Levothyroxin je podáván pacientům po totální tyreoidektomii (s nebo bez následné aplikace radiojodu) nejen k navození eutyreózy, ale také k potlačení produkce thyreotropního hormonu (TSH), protože TSH je považován za růstový faktor pro folikulární buňky štítné žlázy. Hlavní nevýhodou této léčby jsou známá rizika spojená s iatrogenně navozenou subklinickou či až klinickou hypertyreózou. Proto je nezbytný individuální přístup s porovnáním rizika rekurence onemocnění a rizika nežádoucích účinků supresní terapie v korelaci s komorbiditami a celkovým stavem pacienta. Časté a důsledné sledování pacientů s adekvátní úpravou dávky levothyroxinu k udržení TSH v cílovém rozmezí dle doporučení Americké tyroidální asociace (ATA) je zcela klíčové.
Levothyroxine therapy in management of diferentiated thyroid carcinoma (DTC) has been common practice for decades. Levothyroxine is being administered to patiens with DTC after total thyreoidectomy (with or without postopreative radioiodine treatment) not only to restore euthyroidism but to suppress the production of thyroid-stimulating hormone (TSH) as well because TSH is considered as a growth factor for thyroid follicular cells. However there has been a downside to this threatment recently. The main concerns are the known risks related to iatrogenic subclinical or even mild but clinicaly overt iatrogenic hyperthyroidism. Therefore individualized treatment approach aiming to balance between the risk of tumor recurence and the risks related to hypertyhroidism in view of pateints age, risk factors and comorbidities is essential. Close folow-up is therefore necessary with frequent dose adjustments according to target TSH values published in American Thyroid Association guidelines.

Standardní léčbou karcinomu močového měchýře infiltrujícího svalovinu je radikální cystektomie s pánevní lymfadenektomií. Téměř polovina pacientů však po operaci progreduje do metastazujícího onemocnění, tedy u značné části pacientů není samotná chirurgická léčba dostačující. Neoadjuvantní chemoterapie s cílem zlepšit tyto výsledky je využívána již od 80. let 20. století a v dnešní době máme dostatek důkazů pro její využití v rutinní klinické praxi. Článek poskytuje přehlednou informaci o významu, předpokladech, indikaci, režimu, tolerabilitě a účinnosti neoadjuvantní chemoterapie u karcinomu močového měchýře včetně aktuálních doporučení.
Radical cystectomy with pelvic lymphadenectomy represents the standard treatment of muscle-invasive bladder cancer. However, almost half of the patients progress to metastatic disease so surgical treatment alone is not sufficient in a significant proportion of patients. Neoadjuvant chemotherapy to improve these results has been used since the 1980s, and we now have enough evidence for its use in clinical practice. The article provides an overview of importance, purposes, indications, regimen, tolerability and effectivity of neoadjuvant chemotherapy in bladder cancer, including current recommendations.

INTRODUCTION: Standards for chemotherapy against choroid plexus tumors (CPT) have not yet been established. METHODS: CPT-SIOP-2000 (NCT00500890) was an international registry for all CPT nesting a chemotherapy randomization for high-risk CPT with Carboplatin/Etoposide/Vincristine (CarbEV) versus Cyclophosphamide/Etoposide/Vincristine (CycEV). Patients older than three years were recommended to receive irradiation: focal fields for non-metastatic CPC, incompletely resected atypical choroid plexus papilloma (APP) or metastatic choroid plexus papilloma (CPP); craniospinal fields for metastatic CPC/APP and non-responsive CPC. High risk was defined as choroid plexus carcinoma (CPC), incompletely resected APP, and all metastatic CPT. From 2000 until 2010, 158 CPT patients from 23 countries were enrolled. RESULTS: For randomized CPC, the 5/10 year progression free survival (PFS) of patients on CarbEV (n = 20) were 62%/47%, respectively, compared to 27%/18%, on CycEV (n = 15), (intention-to-treat, HR 2.6, p = 0.032). Within the registry, histological grading was the most influential prognostic factor: for CPP (n = 55) the 5/10 year overall survival (OS) and the event free survival (EFS) probabilities were 100%/97% and 92%/92%, respectively; for APP (n = 49) 96%/96% and 76%/76%, respectively; and for CPC (n = 54) 65%/51% and 41%/39%, respectively. Without irradiation, 12 out of 33 patients with CPC younger than three years were alive for a median of 8.52 years. Extent of surgery and metastases were not independent prognosticators. CONCLUSIONS: Chemotherapy for Choroid Plexus Carcinoma is feasible and effective. CarbEV is superior to CycEV. A subset of CPC can be cured without irradiation.

Metastatický karcinom prsu je stále nevyléčitelným onemocněním. Doba přežití se liší u jednotlivých fenotypů nádoru, stejně tak i typ terapie a jednotlivé sekvence. V případě HR pozitivního/HER2 negativního metastatického karcinomu prsu lze využít v systémové terapii hormonální léčbu, cílenou léčbu a chemoterapii. V posledních letech dominuje úvodním liniím systémové léčby hormonoterapie, aktuálně v kombinaci s inhibitory CDK4/6 – palbociklibem, ribociklibem a abemaciklibem. Abemaciklib lze kombinovat s inhibitorem aromatáz nebo s fulvestrantem – to závisí na předchozím předléčení a okamžiku, kdy dojde k progresi nemoci. Kombinace abemaciklibu s hormonoterapií výrazně prodlužuje dobu do progrese onemocnění, v kombinaci s fulvestrantem již prokázala i prospěch v přežití. Tolerance terapie je dobrá, nejčastěji se vyskytující nežádoucí účinek, jímž je průjem, lze ovlivnit podáním loperamidu nebo redukcí dávky, která nesnižuje efektivitu léčebné kombinace.
Metastatic breast cancer is still an untreatable disease. Overall survival is different in the individual phenotypes of the carcinoma as well as with the type of therapy and individual sequence. In case of HR-positive/HER2-negative metastatic breast cancer, it is possible to use systemic therapy, hormonal treatment, targeted therapy and chemotherapy. In recent years, the initial lines of systemic therapy are dominated by hormonal treatment, at the moment combined with CDK4/6 inhibitors – palbociclib, ribociclib and abemaciclib. Abemaciclib can be combined with aromatase inhibitor or fulvestrant – that depends on the previous pre-treatment and the time of disease progression. The combination of abemaciclib with hormonal therapy significantly prolongs time to disease progression, confirming benefit for survival in combination with fulvestrant. The therapy is well-tolerated, the most common side effect is diarrhoea that can be managed by loperamid or dose reduction without lowering the efficacy of treatment combination.

Karcinom prsu představuje heterogenní onemocnění. I přes rozšíření nových terapeutických možností pro jednotlivé podtypy karcinomu chemoterapie stále zaujímá důležité místo v adjuvantní léčbě i v léčbě metastatického onemocnění. Cílem léčby metastatického onemocnění je prodloužení života a udržení jeho kvality. Pořadí léčebných modalit a přípravků se stanovuje pro každou pacientku individuálně. Antracykliny mají významné postavení z hlediska účinnosti léčby. V managementu metastatického onemocnění je problémem jejich kumulativní dávka, při jejímž překročení se výrazně zvyšuje kardiotoxicita. Snížení dávky při současném zachování účinnosti lze dosáhnout využitím lipozomálních forem léků.
Breast cancer is a heterogeneous disease. Despite the expansion of new therapeutic options for individual subtypes, chemotherapy still plays an important role in adjuvant treatment as well as in the treatment of metastatic disease. The goal of treating metastatic disease is to prolong life and maintain its quality. The order of treatment modalities and preparations is individual for each patient. Anthracyclines have an important place for their efficiency. A problem in the management of metastatic disease is their cumulative dose, above which cardiotoxicity increases significantly. Its reduction and at the same time maintaining efficiency can be achieved by using liposomal forms.

INTRODUCTION: Platinum-based combination chemotherapy is the standard treatment for patients with chemotherapy-eligible metastatic urothelial carcinoma (mUC). Immune-checkpoint inhibitors (ICIs) are currently assessed in this setting. This review aimed to assess the role of ICIs alone or in combination as first-line treatment in chemotherapy-eligible patients with mUC. METHODS: Multiple databases were searched for articles published until November 2020. Studies were deemed eligible if they compared overall survival (OS), progression-free survival (PFS), objective response rates (ORRs), complete response rates (CRRs), durations of response (DORs) and adverse events (AEs) in chemotherapy-eligible patients with mUC. RESULTS: Three studies met our eligibility criteria. ICI combination therapy was associated with significantly better OS and PFS, higher CRR and longer DOR than chemotherapy alone (hazard ratio [HR]: 0.85, 95% confidence interval [CI]: 0.76-0.94, P = 0.002; HR: 0.80, 95% CI: 0.71-0.90, P = 0.0002; odds ratio [OR]: 1.48, 95% CI: 1.12-1.96, P = 0.006; and mean difference: 1.39, 95% CI: 0.31-2.46, P = 0.01, respectively). ICI-chemotherapy combination therapy was also associated with significantly better OS and PFS, higher ORR and CRR and longer DOR than chemotherapy alone. Although OS and PFS benefits of ICI combination therapy were larger in patients with high expression of programmed death-ligand 1 (PD-L1), PD-L1 low expression patients also had a benefit; HR for OS (high PD-L1: HR 0.79 versus low PD-L1: HR 0.89) and PFS (high PD-L1: HR 0.74 versus low PD-L1: HR 0.82). ICI monotherapy was not associated with better oncological outcomes but was associated with better safety outcomes than chemotherapy alone. CONCLUSIONS: Our analysis indicates a superior oncologic benefit to first-line ICI combination therapies in patients with chemotherapy-eligible mUC over standard chemotherapy. In contrast, ICI monotherapy was associated with favorable safety outcomes compared with chemotherapy but failed to show its superiority over chemotherapy in oncological benefits. PD-L1 status alone cannot help guide treatment decision-making. However, caution should be exercised in interpreting the conclusions drawn from this study, given that there is the heterogeneity of the population of interest, risk of bias and the nature of the studies evaluated whose data remain immature or unpublished.
