Our and other studies suggest that myocardial hypertrophy in response to hypertension and hyperthyroidism increases propensity of the heart to malignant arrhythmias, while these are rare in conditions of hypothyroidism or type-1 diabetes mellitus associated with myocardial atrophy. One of the crucial factors impacting the susceptibility of the heart to life-threatening arrhythmias is gap junction channel protein connexin-43 (Cx43), which ensure cell-to-cell coupling for electrical signal propagation. Therefore, we aimed to explore Cx43 protein abundance and its topology in hypertrophic and hypotrophic cardiac phenotype. Analysis were performed in left ventricular tissue of adult male spontaneously hypertensive rat (SHR), Wistar Kyoto rats treated for 8-weeks with L-thyroxine, methimazol or strepotozotocin to induce hyperthyroid, hypothyroid and type-1 diabetic status as well as non-treated animals. Results showed that comparing to healthy rats there was a decrease of total myocardial Cx43 and its variant phosphorylated at serine368 in SHR and hyperthyroid rats. Besides, enhanced localization of Cx43 was demonstrated on lateral sides of hypertrophied cardiomyocytes. In contrast, total Cx43 protein and its serine368 variant were increased in atrophied left ventricle of hypothyroid and type-1 diabetic rats. It was associated with less pronounced alterations in Cx43 topology. In parallel, the abundance of PKCepsilon, which phosphorylates Cx43 at serine368 that stabilize Cx43 function and distribution was reduced in hypertrophied heart while enhanced in atrophied once. Findings suggest that differences in the abundance of cardiac Cx43, its variant phosphorylated at serine368 and Cx43 topology may explain, in part, distinct propensity of hypertrophied and atrophied heart to malignant arrhythmias.

The goal of this case report is to describe the young childhood cancer survivor who was treated for non-Hodgkin lymphoma with chemotherapy containing anthracycline doxorubicin and who developed symptoms of serious cardiovascular damage 27 years after diagnosis of cancer. The patient is in long-term complete remission of the lymphoma. He started guideline medical therapies for chronic heart failure and had a cardioverter defibrillator implanted for primary prevention of sudden cardiac death. He is currently a candidate for heart transplantation.
Cieľom kazuistiky je popísať prípad mladého pacienta, v detstve úspešne liečeného pre non-Hodgkinov lymfóm chemoterapiou s obsahom anthracyklínu doxorubicínu s neskorším rozvojom príznakov závažného kardiovaskulárneho poškodenia 27 rokov od diagnostiky lymfómu. Pacient je dlhodobo v kompletnej remisii onkologického ochorenia. U pacienta bola započatá liečba chronického srdcového zlyhávania v súlade s najnovšími odporúčaniami vrátane implantácie kardioverter-defibrilátora, ako primárnej prevencie náhlej kardiálnej smrti. Momentálne je pacient čakateľom na transplantáciu srdca.

Acute excessive ethyl alcohol (ethanol) consumption alters cardiac electrophysiology and can evoke cardiac arrhythmias, e.g., in 'holiday heart syndrome'. Ethanol acutely modulates numerous targets in cardiomyocytes, including ion channels, Ca2+-handling proteins and gap junctions. However, the mechanisms underlying ethanol-induced arrhythmogenesis remain incompletely understood and difficult to study experimentally due to the multiple electrophysiological targets involved and their potential interactions with preexisting electrophysiological or structural substrates. Here, we employed cellular- and tissue-level in-silico analyses to characterize the acute effects of ethanol on cardiac electrophysiology and arrhythmogenesis. Acute electrophysiological effects of ethanol were incorporated into human atrial and ventricular cardiomyocyte computer models: reduced INa, ICa,L, Ito, IKr and IKur, dual effects on IK1 and IK,ACh (inhibition at low and augmentation at high concentrations), and increased INCX and SR Ca2+ leak. Multiscale simulations in the absence or presence of preexistent atrial fibrillation or heart-failure-related remodeling demonstrated that low ethanol concentrations prolonged atrial action-potential duration (APD) without effects on ventricular APD. Conversely, high ethanol concentrations abbreviated atrial APD and prolonged ventricular APD. High ethanol concentrations promoted reentry in tissue simulations, but the extent of reentry promotion was dependent on the presence of altered intercellular coupling, and the degree, type, and pattern of fibrosis. Taken together, these data provide novel mechanistic insight into the potential proarrhythmic interactions between a preexisting substrate and acute changes in cardiac electrophysiology. In particular, acute ethanol exposure has concentration-dependent electrophysiological effects that differ between atria and ventricles, and between healthy and diseased hearts. Low concentrations of ethanol can have anti-fibrillatory effects in atria, whereas high concentrations promote the inducibility and maintenance of reentrant atrial and ventricular arrhythmias, supporting a role for limiting alcohol intake as part of cardiac arrhythmia management.

The aim of this study was to assess the effects of radiofrequency electromagnetic field (RF EMF) on heart rate variability (HRV) in rabbits with intensity slightly exceeding the limits for occupations. Totally 21 New Zealand white rabbits divided into two groups were used in this double-blind study. The first group of animals without general anesthesia was subjected to HRV examination under exposure to a device generated RF EMF source (frequency 1788 MHz, intensity 160 V/m, lasting 150 min.). The second group (premedications + alpha chloralose mg/kg) underwent the same protocol under the exposure to the real RF EMF signal from the base stations of mobile providers (frequency range 1805 - 1870 MHz - corresponding to the downlink signal of Slovak mobile providers, 160 V/m, 150 min., respectively). Individual 5 min records were used to analyze the HRV parameters: heart rate and root Mean Square of the Successive Differences (rMSSD) for time domain analysis and spectral powers in the low (LF-VFS) and high frequency (HF-VFS) bands for frequency domain analysis. Our study revealed the increased in HRV parameters (HF-HRV, rMSSD) associated with lower heart rate indicating increased cardiac vagal control under the exposure to RF EMF in experimental methods.

Primární dědičné arytmické syndromy patří do skupiny onemocnění, které způsobují poruchy elektrické aktivace srdečních buněk a tím predisponují ke vzniku život ohrožujících arytmií. Mezi dědičné arytmické syndromy patří syndrom dlouhého QT intervalu, krátkého QT intervalu, katecholaminergní polymorfní komorová tachykardie, či onemocnění sodíkového kanálu (gen SCN5A), která zahrnují např. syndrom bratří Brugadových, progresivní převodní poruchy, familiární sick sinus syndrom apod. Volně je k nim řazena i arytmogenní kardiomyopatie, která je však způsobena poruchou desmosomálních proteinů.Abnormality elektrické aktivace srdečních buněk jsou dány ve většině případů geneticky podmíněnou poruchou funkce iontových kanálů kardiomyocytů, proto se někdy označují i jako kanálopatie a jejich dědičnost je nejčastěji autosomálně dominantní. Prvním příznakem těchto syndromů můžou být palpitace, synkopy, křeče, ale i náhlá srdečná smrt. Jejich příčinou je shodně u všech uvedených onemocnění maligní komorová tachyarytmie vedoucí k mozkové hypoxii. Diagnostika těchto syndromů je založena na symptomech, typickém EKG obrazu a genetické stratifikaci, která přispívá k volbě terapie včetně specifických doporučení úpravy životního stylu. Nutná je úzká vzájemná mezioborová spolupráce pediatra, dětského kardiologa, dospělého kardiologa, klinického genetika, sportovního lékaře a v neposlední řadě i soudního lékaře a psychologa.
IInherited arrhythmic syndromes, also known as cardiac channelopathies, are group of cardiac electrophysiological disorders characterized by a specific pattern on electrocardiogram and a disease specific risk for malignant ventricular arrhythmias. This group includes long QT syndrome (LQT), short QT syndrome (SQT), catecholaminerg polymorphic ventricular tachycardia (CPVT) and a sodium channelopathies (SCN5A) like Brugada syndrome, progressive cardiac conduction disease (PCCD) etc. Channelopathies manifest mostly in form of syncopes, epileptic seizures or sudden cardiac death. Most types of cardiac channelopathies are inherited in autosomal dominant pattern, autosomal recessive variants are rare and associated with a severe course of the disease. In some cases arrhythmic syndromes overlap arrhythmic cardiomyopathies, which are mainly caused by disorders in desmosomal proteins. The genetic stratification allows in many cases installation of individualized therapy and presymptomatic treatment of mutation carriers, who are at risk of malignant arrhythmias. The proper clinical diagnosis, genetic analysis and long-term treatment demand multidisciplinary approach and close cooperation among pediatric and adult cardiologists, geneticist, molecular geneticist, psychologists, sport and forensic medicine professionals.
Reliable P wave detection is necessary for accurate and automatic electrocardiogram (ECG) analysis. Currently, methods for P wave detection in physiological conditions are well-described and efficient. However, methods for P wave detection during pathology are not generally found in the literature, or their performance is insufficient. This work introduces a novel method, based on a phasor transform, as well as innovative rules that improve P wave detection during pathology. These rules are based on the extraction of a heartbeats' morphological features and knowledge of heart manifestation during both physiological and pathological conditions. To properly evaluate the performance of the proposed algorithm in pathological conditions, a standard database with a sufficient number of reference P wave positions is needed. However, such a database did not exist. Thus, ECG experts annotated 12 chosen pathological records from the MIT-BIH Arrhythmia Database. These annotations are publicly available via Physionet. The algorithm performance was also validated using physiological records from the MIT-BIH Arrhythmia and QT databases. The results for physiological signals were Se = 98.42% and PP = 99.98%, which is comparable to other methods. For pathological signals, the proposed method reached Se = 96.40% and PP = 85.84%, which greatly outperforms other methods. This improvement represents a huge step towards fully automated analysis systems being respected by ECG experts. These systems are necessary, particularly in the area of long-term monitoring.
