Uvedená kazuistika dokumentuje předpokládaný vliv kombinace PAD na kompenzaci pacienta s recentně diagnostikovaným diabetes mellitus (DM) 2. typu. Díky správně zvolené kombinaci medikamentů ovlivňujících jak inzulínovou rezistenci, tak defekt inzulínové sekrece bylo dosaženo doporučených parametrů kompenzace diabetu. Nicméně až díky monitoraci glykemie formou FGM (flash glucose monitoring) bylo možné odhalit nečekaně intenzívní efekt kombinace metforminu s vildagliptinem (vlivem na snížení inzulinorezistence) na rozvoj popůlnočních hypoglykemií. Tyto vymizely po vysazení večerní dávky uvedené fixní kombinace PAD – tím se potvrdila tato hypotéza, současně byl vyloučen vliv repaglinidu na rozvoj hypoglykemických stavů.

The presented case report documents the expected effect of PAD combination therapy on diabetes compensation in a patient recently diagnosed with type 2 diabetes mellitus. Due to the correctly chosen combination of drugs affecting both insulin resistance and the defect of insulin secretion, the recommended parameters of diabetes compensation were achieved. However, it was not until flash glucose monitoring (FGM) that the unexpectedly intensive effect which the combination of metformin and vildagliptin has (due to its influence on reducing insulin resistance) on the development of midnight hypoglycaemia was revealed. These disappeared after discontinuation of the fixed combination of PAD, which confirmed this hypothesis, and, at the same time, the effect of repaglinide on the development of hypoglycaemic conditions was ruled out.
