Cíl studie: Přehled týkající se idiopatického polyhydramnia a s ním spojených komplikací. Typ studie: Přehledový článek. Název a sídlo pracoviště: Gynekologicko-porodnická klinika 1. LF UK a Nemocnice Na Bulovce, Praha a Gynekologicko-porodnická klinika UK 2. LF a FN Motol, Praha. Metodika: Literární rešerše s využitím databází Web of Science, Ovid, Cochrane, Medline a analýza článků publikovaných v recenzovaných časopisech s impakt faktorem v letech 1958–2020. Výsledky: Polyhydramnion se vyskytuje u 0,2-2 % těhotenství. Gravidity s polyhydramniem jsou častěji komplikovány abnormální polohou plodu nebo poruchou naléhání a mohou být spojeny s nepříznivými těhotenskými výsledky, zejména s vyšším výskytem intrauterinních úmrtí plodu, abrupcí placenty, indukovaných porodů, císařských řezů, prodlouženou první porodní dobou a dystokií ramének. Závěr: Středně těžké a těžké formy idiopatického polyhydramnia jsou spojeny s vyšším rizikem nepříznivých těhotenských výsledků, přičemž hlavním preventivním opatřením je intenzivní sledování těhotné ženy a plodu v těhotenství a v průběhu samotného porodu.
Objective: A comprehensive overview on idiopathic polyhydramnios and associated complications. Design: Review article. Setting: Department of Obstetrics and Gynaecology, First Faculty of Medicine, Charles University and Hospital Na Bulovce, Prague, Czech Republic; Department of Obstetrics and Gynaecology, Second Faculty of Medicine, Charles University and Motol University Hospital, Prague, Czech Republic. Methods: Literature search using the databases Web of Science, Ovid, Cochrane, Medline databases and an analysis of articles published in peer-reviewed journals with impact factor in the years 1958–2020. Results: Polyhydramnios occurs in 0.2-2% of pregnancies. Pregnancies with polyhydramnios are more often complicated by abnormal foetal lie or presentation and, moreover, should be associated with adverse pregnancy outcomes, particularly with the higher incidence of intrauterine foetal death, placental abruption, labour induction, caesarean sections, prolonged first stage of labor and shoulder dystocia. Conclusion: The moderate and severe forms of idiopathic polyhydramnios are associated with the higher risk of adverse pregnancy outcomes that implies the need for intensive monitoring of a pregnant woman and the foetus both in pregnancy and during the delivery itself.

Cíl studie: Analýza předpokladů a dosavadních výsledků robotického přístupu k minimalizaci chirurgické morbidity a zachování pooperační kvality života žijících dárkyň dělohy podstupujících odběr štěpu k transplantaci. Metodika: Literární rešerše s využitím databází Web of Science, Google Scholar a PubMed podle klíčových slov: absolute uterine factor infertility, living donor, nerve-sparing surgery, robotic-assisted hysterectomy, uterus transplantation. Analýza článků publikovaných v recenzovaných časopisech s impakt faktorem. Výsledky: Laparotomické, laparoskopické a robotické nervy šetřící radikální hysterektomie jsou postavené na zachování kvality života operovaných žen následkem neporušení hypogastrických a pánevních splanchnických nervů, včetně vezikálních větví pánevního plexu. Precizní robotická disekce močovodů a uterinních arterií může zabránit poranění těchto nervových struktur i při odběru dělohy k transplantaci, což by mělo přispět k redukci nežádoucích konsekvencí charakteru pooperačních poruch evakuace močového měchýře a stolice a rovněž i sexuálních dysfunkcí u žijících dárkyň dělohy. Preparace uterinních vén představuje nejobtížnější, časově nejnáročnější a zároveň nejhůře predikovatelnou část chirurgického odběru dělohy. Preparace ovariálních vén je technicky jednodušší a pro zajištění odtoku krve z dělohy po transplantaci také efektivní. Závěr: Simplifikace techniky odběru děložního štěpu od žijící dárkyně je jedním ze základních kroků, které by mohly v budoucnu vést k implementaci transplantace dělohy do léčby neplodnosti u žen s absencí dělohy. Robotický odběr dělohy i použití venózního odtoku z děložního štěpu výhradně cestou ovariálních žil mohou přispět ke zvýšení dostupnosti transplantace dělohy, a to zejména kvůli lepší reprodukovatelnosti odběrových operací u žijících dárkyň.
Objective: An analysis of assumptions and current results of a robotic-assisted approach to minimize the surgical morbidity and maintain the postoperative quality of life in living donors of uterus undergoing graft retrieval for transplantation. Design: Review article. Setting: Department of Obstetrics and Gynaecology, First Faculty of Medicine, Charles University and Hospital Bulovka, Prague and Department of Obstetrics and Gynaecology, Second Faculty of Medicine, Charles University and Motol University Hospital, Prague. Methods: Literature search using the Web of Science, Google Scholar and Pubmed databases with keywords: absolute uterine factor infertility, donor surgery, living donor, robotic-assisted hysterectomy and, uterus transplantation, and an analysis of the articles published in peer-reviewed journals with impact factor. Results: The experience with open, laparoscopic and robotic-assisted nerve-sparing radical hysterectomies to maintain the quality of life in women undergoing surgery confirms the need to preserve hypogastric and pelvic splanchnic nerves, including the vesical branches of the pelvic plexus. Precise robotic dissection of the ureters and uterine arteries should prevent injury to these nerve structures even in the procurement of the uterus for transplantation. It should help to the reduction of postoperative side effects as the bladder and rectum evacuation disorders as well as sexual dysfunction in the living donors of uterus. The preparation of uterine veins is the most difficult, time-consuming and worst predictable part of the uterine retrieval. In comparison, the preparation of the ovarian veins is technically simpler as well as effective in venous outflow from the uterine graft. Conclusion: The simplification of the technique of uterine graft retrieval in the living donor should be one of the basic steps to introduce uterine transplantation as a causal treatment of infertility in women without uterus. Robotic-assisted procurement of the uterus and, moreover, the use of venous outflow from the graft exclusively via ovarian veins could contribute to the higher availability of the uterine transplantation, particularly due to a better reproducibility of the procurement surgeries in the living donors.

Východiska: Přehled změněného stagingu karcinomu vulvy, vysvětlení změn a jejich vlivu na prognózu onemocnění. Cíl: Spolehlivý stagingový systém by měl umožnit odhad prognózy onemocnění, plánovat léčbu a informovat o jejím výsledku. Dobrý stagingový systém by měl mít tři základní charakteristiky – odůvodněnost na základě poznatků medicíny založené na důkazech, důvěryhodnost a praktičnost. Na základě nových poznatků byly výborem Mezinárodní federace gynekologie a porodnictví – FIGO (FIGO Committee on Gynecologic Oncology) v září 2008 navrženy, odsouhlaseny a o rok později i publikovány změny ve stagingu karcinomu vulvy. Stadium 0 bylo zrušeno, protože odpovídá prekanceróze. Stadium IA zůstalo beze změny a stadia I a II byla sloučena. Do stagingu byl zahrnut počet a morfologie postižených lymfatických uzlin, nikoliv však jejich bilaterální postižení. Závěr: Účelem dobrého stagingového systému je poskytnout klasifikaci šíření zhoubného nádoru vycházející z klinické zkušenosti, aby bylo možno srovnat účinnost různých modalit léčby. Výsledkem lékařského výzkumu na poli onkologie bylo zastarání některých klasifikačních systémů v onkogynekologii, které přestaly shrnovat skupiny onemocnění se stejnou prognózou. Přepracovaný staging karcinomu vulvy rozděluje pacienty do skupin s podobnou prognózou. Tím umožňuje relevantní výměnu informací mezi onkologickými centry a přispívá k poznávání a výzkumu malignit v rozlišných částech světa.
Background: Review of revised staging system for vulva, explaining the changes of staging and their impact on the prognosis of disease is presented. Aim: The main objectives of a reliable staging system include an assessment of prognosis, planning treatment, and the evaluation of their outcomes. A good staging system must meet three basic characteristics: validity, reliability and practicality. Since medical research and practice in the field of oncology have shown explosive growth, the staging of vulvar cancer and some other cancers did not give a good spread of prognostic groupings. Changes based on new findings were proposed in 2008 by the FIGO Committee on Gynecologic Oncology, approved, and published a year later the changes in the staging system for carcinoma of the vulva. Stage 0 was deleted, since it represents pre‑invasive lesion. Stage IA remained unchanged and stage I and II were combined. The number and morphology of the involved nodes were taken into account, and the bilaterality of positive nodes has been discounted. Conclusion: The purpose of a good staging system is to offer a classification of the extent of gynecological cancer, in order to provide a method of conveying one‘s clinical experience to others for the comparison of different treatment methods. As a result of the explosion of medical research in the field of oncology, the staging of some of the gynecological cancers became outdated and did not give a good spread of prognostic groupings. According to the revised staging for carcimona of the vulva, patients are divided to groups with similar prognosis. Therefore, exchange of relevant information between oncological centers is facilitated, thus disseminating knowledge and stimulating research in other parts of the world.
