Pokud v terapii arteriální hypertenze nedosáhneme nefarmakologickou léčbou normalizace krevního tlaku, pak je třeba zahájit léčbu farmakologickou. Nedosáhneme-li normalizace krevního tlaku monoterapií, je vhodnější kombinovat lék s antihypertenzívem jiné třídy než zvyšovat dávku monoterapie. Výhodné je užití fixních kombinací. U pacientů s hypertenzí je třeba sledovat i ovlivnění metabolických rizikových faktorů. Velmi výhodná je kombinace inhibitorů osy renin-angiotenzinaldosteron (RAA) s diuretiky thiazidové řady nebo s indapamidem. Další vhodnou kombinací je ACE-inhibitor/ AT1-blokátor s blokátorem kalciového kanálu. V klinické praxi máme k dispozici i fixní kombinaci amlodipinu a atorvastatinu.
If non-pharmacological treatment fails to achieve normalised blood pressure in therapy of arterial hypertension, it is necessary to commence pharmacological therapy. If we do not achieve normalised blood pressure using a monotherapy, it tends to be better to combine the preparation we are using with an anti-hypertensive of a different class, rather than to further up the monotherapy dose. It is beneficial to use fixed combinations. In patients with hypertension it is necessary to monitor the influence on metabolic risk factors as well. The combinations of renin-angiotensin-aldosterone axis inhibitors (RAA) with thiazide-based diuretics or indapamid is very beneficial. Another fine combination is ACE-inhibitor/ AT1-blocker with a calcium channel blocker. In clinical practice, the fixed combination of amlodipine and atorvastatin is available as well.
