Český národní registr dárců dřeně (ČNRDD) byl založen v roce 1992 s cílem nalézt včas optimálního „národního“ dárce pro české pacienty, případně zajistit dárce ze zahraničí. V současnosti (k 31. 10. 2018) jeho databáze obsahuje 83 000 dárců a medián věku všech aktivních dárců v registru je 32 let. Na základě preferencí transplantačních center, tj. větší míry požadavků na mladší dárce, jakož i z ekonomických důvodů, snížil ČNRDD v roce 2009 maximální věkovou hranici vstupu do registru na 35 let. Cílem naší práce bylo zhodnotit, jak dalece se tato restriktivní kritéria a celkovou strategii náboru zaměřenou na mladé muže dařilo dodržet, zhodnotit kvantitativní i kvalitativní parametry náboru v letech 2013–2017. V letech 2013–2017 se nábor nových dárců pohyboval mezi 5 000–10 000/rok. Celkem bylo v tomto období zařazeno 36 720 dárců. Zastoupení pohlaví mezi nově registrovanými dárci je v posledních letech téměř vyrovnané. Medián věku registrovaných dárců v celém sledovaném období byl 27 let. Tento masivní nábor mladých dárců vedl k průběžnému snížení mediánu věku dárců celé databáze ČNRDD. Prezentované údaje jednoznačně dokládají efektivitu aktuální náborové strategie ČNRDD, protože v uplynulých letech došlo k výraznému snížení mediánu věku dárců celé databáze ČNRDD a současně nárůstu podílu mužů. To je v souladu s preferencí transplantačních center, která na podkladě publikovaných dat upřednostňují mladé dárce, a to především muže. Má-li být současná náborová strategie ČNRDD nadále úspěšná, je nezbytná trvající dobrá spolupráce dárcovských a náběrových center registru i práce dobrovolných spolupracovníků.
The Czech National Marrow Donors Registry (CNMDR) was established in 1992 to find an optimal „national" donor for Czech patients or to provide a donor from abroad as soon as possible. The current state of the CNMDR database (to October 31, 2018) stands at 83.000 donors and the median age of all active donors in the registry is 32 years. Given the transplant centres' preference for younger donors and for economic reasons, the CNMDR reduced the maximum age limit for entry into the registry to 35 years in 2009. The aim of our work was to evaluate the success of this age restriction and general recruitment strategy focusing on young males and to evaluate the quantitative and qualitative recruitment parameters in 2013–2017. Between 2013 and 2017, the recruitment of new donors ranged from 5.000 to 10.000/year. In total, 36.720 donors were recruited during this period. Newly recruited donor gender distribution has been almost equally balanced in recent years. The average median age of recruited donors was 27 years. This massive recruitment of young donors led to a continuous reduction of the CNMDR database donor median age. The presented data clearly demonstrate the effectiveness of the current CNMDR recruitme strategy. In the past years, there has been a significant decrease in the donor age median as well as an increase in male donor contribution. This is in accordance with the preferences of transplant centres that prefer young donors especially males, based on published data. For this recruitment strategy to remain successful, the CNMDR will need to maintain its continued good cooperation with donor and recruitment centres as well as with volunteers.

PURPOSE OF THE STUDY Periprosthetic distal femoral fractures (PDFF) constitute an unpleasant complication in patients with a total knee replacement (TKR). The incidence reported in literature is 0.3-2.5 %. The number of periprosthetic knee fractures has been increasing due to the ageing of population, a growing number of implants, a longer life expectancy of patients, a more intensive physical activity of patients, and osteoporosis. Most of these fractures are treated surgically, non-surgical treatment is reserved solely for patients unable to undergo a surgery for general health conditions. MATERIAL AND METHODS Our retrospective study evaluated the group of patients with PDFF who were treated at out department in the period 2007- 2016 and 2,975 primary TKR were performed. The total number of patients with PDFF was 56. The mean age of patients with PDFF was 77 years (56-94 years) and at the time of fracture the mean age was 71 years in men and 78 years in women. The average time from the TKR to periprosthetic fracture was 8.2 years (0-20 years). The fractures were assessed using the Su classification modified by Krbec. RESULTS A primary TKR was performed in 46 cases for gonarthrosis, in 6 cases for rheumatoid arthritis and in 4 cases for secondary, post-traumatic gonarthrosis. The average incidence of periprosthetic distal femoral fractures was 5-6 cases per year. Women represented 86 %, men 14 %. Su Type I fracture was diagnosed in 25 % of cases, Su Type II fractures in 71 %, and Su Type III fractures 4 %. 52 patients with PDFF were treated surgically, in 4 cases conservative treatment was opted for. The average treatment time of PDFF to healing by callus formation was 6.6 months (3-12 months). Mortality during the first 3 months after osteosynthesis of PDFF was 9 %. A failure of osteosynthesis of PDFF was reported in 4 cases. DISCUSSION Multiple classification systems were developed to assess these fractures. The most appropriate we consider the classification of Su et al. classifying the PDFF into 3 groups, namely based on the height of the fracture line relative to the femoral component. Osteosynthesis by retrograde femoral nail is indicated for periprosthetic fractures, with sufficient bone mass in distal femur, which allows stable distal fixation. The new generation of anatomically shaped angular stable implants gives us yet another option for osteosynthesis of PDFF. Many studies point at the advantages of these implants in osteoporotic bone as against the conventional plates. CONCLUSIONS The number of PDFF has been increasing. The main methods of internal osteosynthesis continue to be the angular stable plates and the retrograde femoral nail. Preoperative planning is important to determine the type and dimensions of the existing femoral component and to distinguish whether or not it has come loose. The choice of the implant may depend on the bone mass available for distal fixation. The retrograde femoral nail is usually the most suitable method of treatment for proximal PDFF (Su Type I). The angular stable plates can be used for PDFF originating at the femoral component (Su Type II and Type III). Very distal fractures classified as Su Type III with a loose femoral component require a revision surgery with a TKR with stems. The surgeon should be prepared for a revision surgery if the intraoperative finding is more complicated than anticipated based on the preoperative radiograph. Registry nepříbuzných dárců celosvětově evidují k začátku roku 2014 přes 24 milionů dobrovolných dárců a počet odběrů krvetvorných buněk ročně přesahuje 16 000. K zajištění kvality vyhledávání nepříbuzného dárce definovala pracovní skupina při Světové asociaci dárců (WMDA) klíčové ukazatele kvality, které byly retrospektivně vyhodnoceny i v rámci Českého národního registru dárců dřeně (ČNRDD). Cílem práce bylo zhodnotit plnění jednotlivých ukazatelů kvality, provést další rozbor u těch, které neodpovídají stanoveným kritériím a porovnat výsledky s publikovanými daty. Z pěti analyzovaných indikátorů kvality byly za rok 2013 úspěšně splněny tři: podíl provedených rozšířených typizací do dvou týdnů od obdržení žádosti, podíl dárců dostupných při předodběrovém vyšetření a podíl vstupně chybných typizačních výsledků zjištěných při verifikační typizaci. Nebyly splněny dva indikátory kvality: podíl vzorků dostupných dárců při verifikační typizaci (79 % u ČNRDD oproti 80 % doporučených WMDA) a podíl vzorků zaslaných k verifikační typizaci do dvou týdnů od obdržení žádosti (64 % u ČNRDD oproti 80 % doporučených WMDA). Vzhledem k těmto nesplněným indikátorům byly analyzovány důvody nedostupnosti dárce či jeho prolongovaného vyhledávání, kdy hlavní příčinou byly nedostatečné kontaktní údaje dárce. Při podrobnější analýze jsme pak identifikovali rizikovou skupinu těch dárců, kteří při vstupu do registru udali pouze poštovní adresu a telefon. Naopak nejméně nedostupných dárců bylo ve skupině těch, kteří udali e-mailový kontakt. Na základě analýzy potenciálních příčin u nesplněných indikátorů kvality byla definována možná opatření k jejich budoucímu kompletnímu splnění. At the beginning of 2014 over 24 million of unrelated donors being listed in the registries worldwide and number of hematopoietic stem cell collections exceed 16 000 per year. In order to insure the quality of unrelated donor searches Quality Assurance Working Group at the World Marrow Donor Association defined global key performance indicators for bone marrow donor registries, which were retrospectively evaluated also within Czech National Marrow Donors Registry. The aim of the study was evaluate the performance of quality indicators, perform further analysis for those that do not meet the specified criteria and compare the observed result with published data. For the year 2013 three of the five analysed quality indicators were fulfilled: the rate of extended typing performed within two weeks of receipt of the request, the rate of donors available during work up examination and the rate of discrepant typing identified in the verification typing. Two quality indicators were not fulfilled: the rate of samples of the donors available during verification typing (79% in the CNMDR compared to 80% WMDA recommended) and the rate of samples sent for verification typing within two weeks from receipt of the request (64% in the CNMDR compared to 80% WMDA recommended). Due to this unfulfilled indicators reasons for the unavailability of the donors were analysed and the lack of contact was identified as the main reason. Looking at the detail we identified the risk group of donors with only contact address and phone number provided upon entry to the registry. On the contrary minimum of unavailable donors were in the group of donor with e-mail contact. Based on the findings of unfulfilled quality indicators further quality improvement objectives were defined for the future.
