BACKGROUND: Identifying a donor for facial vascularized composite allotransplant recipients can be a lengthy, emotionally challenging process. Little is known about the relative distribution of key donor characteristics among potential donors. Data on actual wait times of patients are limited, making it difficult to estimate wait times for future recipients. METHODS: The authors retrospectively reviewed charts of nine facial vascularized composite allotransplant patients and provide data on transplant wait times and patient characteristics. In addition, they analyzed the United Network for Organ Sharing database of dead organ donors. After excluding donors with high-risk characteristics (e.g., active cancer or risk factors for blood-borne disease transmission), the authors calculated the distribution of relevant donor-recipient matching criteria (i.e., ethnicity, body mass index, age, ABO blood group, cytomegalovirus, Epstein-Barr virus, hepatitis C virus) among 65,201 potential donors. RESULTS: The median wait time for a transplant was 4 months (range, 1 day to 17 months). The large majority of United Network for Organ Sharing-recorded deaths from disease were white (63 percent) and male (58 percent). Female donors of black, Hispanic, or Asian descent are underrepresented, with 7, 5, and 1 percent of all recorded deaths from disease, respectively. Potential donors show cytomegalovirus and Epstein-Barr virus seropositivity of 65 and 95 percent, respectively. The number of annual hepatitis C-positive donors increased over time. CONCLUSIONS: Actual facial vascularized composite allotransplant wait times vary considerably. Although most patients experience acceptable wait times, some with underrepresented characteristics exceed acceptable levels. Cytomegalovirus-seropositive donors present a large portion of the donor pool, and exclusion for seronegative patients may increase wait time. Hepatitis C-seropositive donors may constitute a donor pool for underrepresented patient groups in the future.

BACKGROUND: Europe is currently the most active region in the field of pancreatic islet transplantation, and many of the leading groups are actually achieving similar good outcomes. Further collaborative advances in the field require the standardization of islet cell product isolation processes, and this work aimed to identify differences in the human pancreatic islet isolation processes within European countries. METHODS: A web-based questionnaire about critical steps, including donor selection, pancreas processing, pancreas perfusion and digestion, islet counting and culture, islet quality evaluation, microbiological evaluation, and release criteria of the product, was completed by isolation facilities participating at the Ninth International European Pancreas and Islet Transplant Association (EPITA) Workshop on Islet-Beta Cell Replacement in Milan. RESULTS: Eleven islet isolation facilities completed the questionnaire. The facilities reported 445 and 53 islet isolations per year over the last 3 years from deceased organ donors and pancreatectomized patients, respectively. This activity resulted in 120 and 40 infusions per year in allograft and autograft recipients, respectively. Differences among facilities emerged in donor selection (age, cold ischemia time, intensive care unit length, amylase concentration), pancreas procurement, isolation procedures (brand and concentration of collagenase, additive, maximum acceptable digestion time), quality evaluation, and release criteria for transplantation (glucose-stimulated insulin secretion tests, islet numbers, and purity). Moreover, even when a high concordance about the relevance of one parameter was evident, thresholds for the acceptance were different among facilities. CONCLUSIONS: The result highlighted the presence of a heterogeneity in the islet cell product process and product release criteria.

Český národní registr dárců dřeně (ČNRDD) byl založen v roce 1992 s cílem nalézt včas optimálního „národního“ dárce pro české pacienty, případně zajistit dárce ze zahraničí. V současnosti (k 31. 10. 2018) jeho databáze obsahuje 83 000 dárců a medián věku všech aktivních dárců v registru je 32 let. Na základě preferencí transplantačních center, tj. větší míry požadavků na mladší dárce, jakož i z ekonomických důvodů, snížil ČNRDD v roce 2009 maximální věkovou hranici vstupu do registru na 35 let. Cílem naší práce bylo zhodnotit, jak dalece se tato restriktivní kritéria a celkovou strategii náboru zaměřenou na mladé muže dařilo dodržet, zhodnotit kvantitativní i kvalitativní parametry náboru v letech 2013–2017. V letech 2013–2017 se nábor nových dárců pohyboval mezi 5 000–10 000/rok. Celkem bylo v tomto období zařazeno 36 720 dárců. Zastoupení pohlaví mezi nově registrovanými dárci je v posledních letech téměř vyrovnané. Medián věku registrovaných dárců v celém sledovaném období byl 27 let. Tento masivní nábor mladých dárců vedl k průběžnému snížení mediánu věku dárců celé databáze ČNRDD. Prezentované údaje jednoznačně dokládají efektivitu aktuální náborové strategie ČNRDD, protože v uplynulých letech došlo k výraznému snížení mediánu věku dárců celé databáze ČNRDD a současně nárůstu podílu mužů. To je v souladu s preferencí transplantačních center, která na podkladě publikovaných dat upřednostňují mladé dárce, a to především muže. Má-li být současná náborová strategie ČNRDD nadále úspěšná, je nezbytná trvající dobrá spolupráce dárcovských a náběrových center registru i práce dobrovolných spolupracovníků.
The Czech National Marrow Donors Registry (CNMDR) was established in 1992 to find an optimal „national" donor for Czech patients or to provide a donor from abroad as soon as possible. The current state of the CNMDR database (to October 31, 2018) stands at 83.000 donors and the median age of all active donors in the registry is 32 years. Given the transplant centres' preference for younger donors and for economic reasons, the CNMDR reduced the maximum age limit for entry into the registry to 35 years in 2009. The aim of our work was to evaluate the success of this age restriction and general recruitment strategy focusing on young males and to evaluate the quantitative and qualitative recruitment parameters in 2013–2017. Between 2013 and 2017, the recruitment of new donors ranged from 5.000 to 10.000/year. In total, 36.720 donors were recruited during this period. Newly recruited donor gender distribution has been almost equally balanced in recent years. The average median age of recruited donors was 27 years. This massive recruitment of young donors led to a continuous reduction of the CNMDR database donor median age. The presented data clearly demonstrate the effectiveness of the current CNMDR recruitme strategy. In the past years, there has been a significant decrease in the donor age median as well as an increase in male donor contribution. This is in accordance with the preferences of transplant centres that prefer young donors especially males, based on published data. For this recruitment strategy to remain successful, the CNMDR will need to maintain its continued good cooperation with donor and recruitment centres as well as with volunteers.

Navzdory zvýšené poptávce po orgánech k transplantaci je počet kadaverózních dárců stabilní a doba čekání na transplantaci ledvin se pozvolna prodlužuje. Kromě využití možnosti darování ledvin od žijících dárců, dárců s nebijícím srdcem (NHBD, Non Heart Beating Donors) je další možností transplantace obou ledvin od dospělého kadaverózního marginálního dárce, který byl jinak označen jako nevhodný k darovaní jedné ledviny. Duální transplantace znamená použití obou ledvin od marginálního dárce pro jednoho příjemce, aniž by se příjemce musel vyrovnávat s nevýhodami omezeného počtu funkčních nefronů. Jedná se o ledviny, které jsou za normálních okolností vyloučeny z transplantačního programu a tak nevyužity. Předkládané sdělení podává informace o kritériích pro výběr dárce a příjemce a popisuje průběh první duální transplantace ledvin u dospělého příjemce v České republice.
Despite the increasing demand for organs for transplantation, the number of cadaveric donors remains stable and waiting time for transplantation is gradually getting longer. In addition to the options of using kidneys from living donors and those of non-heart-beating donors (NHBD), an alternative approach is transplantation of both kidneys from adult marginal donors who would otherwise be considered unsuitable for single-kidney donation. Dual kidney transplantation involves the use of both kidneys from a marginal donor for a single recipient without the recipient having to cope with the drawbacks of a limited number of functioning nephrons. Normally, these kidneys would be excluded from the transplantation program and remain unused. The submitted presentation provides information on donor and recipient selection criteria and describes the course of the first dual kidney transplantation in an adult recipient in the Czech Republic.
