Úvod: V našem souboru byly hodnoceny výsledky zadní transpedikulární fixace s použitím PMMA fenestrovaných šroubů u osteoporotických zlomenin obratlů hrudní a bederní páteře s nebo bez přítomného neurodeficitu. Metody: Předmětem sledování byl klinický efekt operace, od ledna 2014 do prosince 2016 jsme zhodnotili 31 pacientů s potvrzenou osteoporotickou zlomeninou, kteří podstoupili transpedikulární fixaci s použitím fenestrovaných PMMA augmentovaných šroubů. V souboru byl hodnocen klinický stav pomocí vizuální analogové škály (VAS) a Oswestry Disability Index (ODI). Délka sledování byla minimálně 2 roky. Výsledky: Hodnoty VAS (snížení z 8,1 na 3,6) a ODI (snížení ze 76 na 34) poukazují na významné snížení lokálních bolestí i invalidity. Asymptomatický únik cementu se vyskytl u 7 pacientů, další méně závažné komplikace byly též zhodnoceny a zaznamenány. Radiologické zhodnocení poukazuje na zlepšení lokální kyfózy, s raritním výskytem selhání instrumentace. Závěr: Augmentovaná transpedikulární fixace s kanylovanými šrouby je dobře proveditelná a relativně bezpečná chirurgická technika pro ošetření porotických zlomenin páteře. Důležitými faktory jsou selekce vhodných pacientů a správné technické provedení výkonu.
Introduction: We assessed our results of posterior transpedicular fixation using PMMA augmented fenestrated screws for osteoporotic vertebral fractures of thoracic or lumbar spine with or without neurological deficits. Methods: From January 2014 to December 2016, 31 patients with confirmed osteoporotic (T-score <-2.5) vertebral fractures underwent pedicular screw fixation using cement-injectable cannulated pedicle screws. Clinical outcomes were evaluated using a Visual Analog Scale and the Oswestry Disability Index. The patients had radiographic follow-up examinations after 1, 3, 6, and 12 months and once per year thereafter, with the minimum follow-up duration of 2 years.Results: The Visual Analog Scale (decreased from 8.1 to 3.6) and Oswestry Disability Index (decreased from 76 to 34) scores showed a significant reduction of local pain and disability (p <0.01). Asymptomatic cement leakage occurred in seven patients, without any neurological sequelae; other less serious complications were also observed. Radiological observation indicated a satisfactory improvement of local kyphosis, without significant loosening or pulling out of the screws.Conclusion: The polymethylmethacrylate augmentation technique using bone cement-injectable cannulated pedicle screws is a feasible and rela-tively safe method for operative management of osteoporotic vertebral fractures. Crucial factors for successful use of augmented screws include careful patient selection and proper technique implementation.

PURPOSE OF THE STUDY Periprosthetic distal femoral fractures (PDFF) constitute an unpleasant complication in patients with a total knee replacement (TKR). The incidence reported in literature is 0.3-2.5 %. The number of periprosthetic knee fractures has been increasing due to the ageing of population, a growing number of implants, a longer life expectancy of patients, a more intensive physical activity of patients, and osteoporosis. Most of these fractures are treated surgically, non-surgical treatment is reserved solely for patients unable to undergo a surgery for general health conditions. MATERIAL AND METHODS Our retrospective study evaluated the group of patients with PDFF who were treated at out department in the period 2007- 2016 and 2,975 primary TKR were performed. The total number of patients with PDFF was 56. The mean age of patients with PDFF was 77 years (56-94 years) and at the time of fracture the mean age was 71 years in men and 78 years in women. The average time from the TKR to periprosthetic fracture was 8.2 years (0-20 years). The fractures were assessed using the Su classification modified by Krbec. RESULTS A primary TKR was performed in 46 cases for gonarthrosis, in 6 cases for rheumatoid arthritis and in 4 cases for secondary, post-traumatic gonarthrosis. The average incidence of periprosthetic distal femoral fractures was 5-6 cases per year. Women represented 86 %, men 14 %. Su Type I fracture was diagnosed in 25 % of cases, Su Type II fractures in 71 %, and Su Type III fractures 4 %. 52 patients with PDFF were treated surgically, in 4 cases conservative treatment was opted for. The average treatment time of PDFF to healing by callus formation was 6.6 months (3-12 months). Mortality during the first 3 months after osteosynthesis of PDFF was 9 %. A failure of osteosynthesis of PDFF was reported in 4 cases. DISCUSSION Multiple classification systems were developed to assess these fractures. The most appropriate we consider the classification of Su et al. classifying the PDFF into 3 groups, namely based on the height of the fracture line relative to the femoral component. Osteosynthesis by retrograde femoral nail is indicated for periprosthetic fractures, with sufficient bone mass in distal femur, which allows stable distal fixation. The new generation of anatomically shaped angular stable implants gives us yet another option for osteosynthesis of PDFF. Many studies point at the advantages of these implants in osteoporotic bone as against the conventional plates. CONCLUSIONS The number of PDFF has been increasing. The main methods of internal osteosynthesis continue to be the angular stable plates and the retrograde femoral nail. Preoperative planning is important to determine the type and dimensions of the existing femoral component and to distinguish whether or not it has come loose. The choice of the implant may depend on the bone mass available for distal fixation. The retrograde femoral nail is usually the most suitable method of treatment for proximal PDFF (Su Type I). The angular stable plates can be used for PDFF originating at the femoral component (Su Type II and Type III). Very distal fractures classified as Su Type III with a loose femoral component require a revision surgery with a TKR with stems. The surgeon should be prepared for a revision surgery if the intraoperative finding is more complicated than anticipated based on the preoperative radiograph. PURPOSE OF THE STUDY With the ageing of population the vertebral insufficiency fractures are increasing in number and occur ever more frequently. Symptomatic relief is often difficult to achieve by non-operative treatment. The aim of this study was to determine the level of pain relief and functional outcomes in patients who failed initial non-operative treatment and, because of persisting or growing symptoms, underwent kyphoplasty. MATERIAL AND METHODS Total number of 303 patients who underwent balloon kyphoplasty between January 2011 and December 2016 were included in our study. These 303 patients had 357 surgeries. This is a retrospective review of a prospectively updated database maintained by our spinal surgery department. In total, 575 levels were augmented. The patients were mostly females (246). The age of the patients ranged from 30 years to 98 years, with the mean age being 72.6 years. The average follow-up for our series was 17.2 months (2-63 months). Pain relief was assessed using the Visual Analogue Scale (VAS) and functional outcome using the Oswestry Disability Index (ODI). RESULTS The average pre-operative VAS was 6.34 (p = 0.00003). At 6 weeks postoperatively the average VAS decreased to 3.80 (p = 0.00000). In our series, the VAS scores showed a progressive decline progressing from the mean value of 3.18 (p = 0.15890) at 1 year to the mean value of 2.85 (p = 0.00205) at 2 years. The average pre-operative ODI was 25.65 (p = 0.03604). At 6 weeks, the value improved to 17.69 (p = 0,00120) and further improvements were seen at 1 year (ODI 14.13) and at 2 years (ODI 12.08). In our series no clinically significant complications were reported. The social drift was observed in 17 patients in our study. DISCUSSION The pain relief and the improvement of functional outcomes were maintained even at a two-year follow-up. No clinically significant complications were reported that would require further surgical intervention and that would affect the good clinical results of our study. CONCLUSIONS Balloon kyphoplasty proved to be a safe surgical technique and should be considered in patients with an ongoing pain following vertebral insufficiency fractures that do not improve with the initial non-operative treatment. Cement augmentation significantly improves pain levels and as well functional status in elderly patients. V kazuistickém sdělení prezentujeme neobvyklou osteoporotickou zlomeninu křížové kosti s neurologickou symptomatologií. Lomná linie procházela obratlovým tělem S1 do disku L5-S1, stropu foramina S1 a do laterální masy. Kaudální dislokace fragmentu způsobila kompresi ipsilaterálního kořene S1. Pacientka byla úspěšné léčená krátkou iliolumbální stabilizací.
We present a case report of an unusual insufficiency fracture of the sacrum with neurological impairment. The fracture line passes through the S1 vertebral body to the L5-S1 disc, the upper part of the S1 foramen and the lateral mass. The downward dislocation of the fragment was the cause of the ipsilateral S1 nerve root compression. The patient was successfully treated with the short iliolumbar stabilisation.

V článku jsou představeny dvě minimálně invazivní léčebné metody v oblasti páteře - vertebroplastika a epiduroskopie. Vertebroplastika se hojně využívá v terapii převážně osteoporotických fraktur obratlů, lze ji rovněž využít při paliativní léčbě metastáz do obratlových těl a u symptomatických hemangiomů. Je prováděna pod skiaskopickou a často i CT kontrolou většinou v analgosedaci. U cca 90 % pacientů vede při správné indikaci k významnému analgetickému efektu i zpevnění obratlového těla. Epiduroskopie je novou technikou, která si teprve hledá své pevné místo. Jeví se jako slibná zvláště u pacientů s tzv. failed back surgery syndrome, tedy u nemocných s klinicky symptomatickými fibrózními srůsty po většinou opakovaných neurochirurgických operacích. Při správné indikaci mívá v současné době pozitivní efekt u cca třetiny až poloviny pacientů.
Two minimally-invasive techniques in the spinal region are introduced: vertebroplasty and epiduroscopy. Vertebroplasty is mostly used in the treatment of osteoporotic vertebral body fractures, as well as in the palliative treatment of spinal metastases or symptomatic hemangiomas. It is usually performed under fluoroscopic, and often also CT control under analgesic sedation. If correctly indicated an analgesic effect occurs in roughly 90 % of patients. Epiduroscopy is a new technique, which is promising especially in patients with failed back surgery syndrome; patients with clinically-symptomatic fibrous adhesions, generally after repeated surgery. If correctly indicated a positive effect occurs in roughly one third to one half of patients.

PURPOSE OF THE STUDY Fragility fractures of the pelvic ring in the elderly population are a serious problem in orthopaedics. The treatment options range from conservative treatment to diverse operative options. We present a balloon guided new technique of implanting cement augmented screws aiming at improved implant anchorage and reduced cement leakage. MATERIAL AND METHODS We describe a new technique of balloon- guided cement augmented iliosacral screws. After the balloon has been insufflated and contrast medium has been instillated for leakage detection, iliosacral screws can safely be placed with a relevant cement depot. RESULTS All patients (n = 8) treated in this way were allowed to weight bear on both legs and mobilisation was tolerated the next day after surgery. In the post-operative controls including X-ray and CT scan we noticed no cement leakage. Pain level as measured by the visual analog scale was significantly reduced to pre- surgery (mean 7.6 pre-operatively, mean 2.5 postoperatively). In addition pain medication could be reduced gradually soon after surgery. CONCLUSIONS The described procedure to address fragility fractures of the pelvic ring is a safe and easy to handle method. There are three advantages: First the osteoporotic S1 vertebral body can by stabilised by the amount of cement; second the cement leakage can be prevented by the balloon-compaction of the cancellous bone and the possibility to control the created cavity with contrast medium; third the bone - screw interface is much stronger compared to other procedures and is biomechanically superior against vertical shear stress. In contrast to sacroplasty, the cement application is guided by the previous use of a balloon. Key words:pelvic ring, fragility fracture, osteoporosis, elderly.
