Enterobacterales produkující karbapenemázu (CPE – Carbapenemase-Producing Enterobacterales) představují globální zdravotnický problém. V Evropě dochází zejména v jižních a východních státech ke zvyšování podílu enterobakterií rezistentních vůči karbapenemům. Od roku 2021 roste také v České republice výskyt kmenů s produkcí karbapenemázy, změnila se i struktura zastoupení jednotlivých enzymů, zejména ve prospěch metalobetalaktamázy typu NDM. Tato situace je zřejmě následkem epidemie covid-19 a zhoršenou epidemickou situací ve zdravotnických zařízeních.
Carbapenemase-producing Enterobacterales (CPE) are a global health problem. In Europe, the proportion of carbapenem resistant enterobacteria is increasing, particularly in the southern and eastern countries. Since 2021, there has also been a widespread increase in the occurrence of CPE in the Czech Republic, the structure of carbapenemases has also shifted towards higher proportion of the NDM type. This situation is apparently the result of the COVID-19 epidemic and the worsened epidemic situation in healthcare facilities.
V letošním roce uplyne 75 let od založení České národní sbírky typových kultur (CNCTC), která působí v rámci Centra epidemiologie a mikrobiologie ve Státním zdravotním ústavu v Praze. CNCTC byla oficiálně ustanovena v r. 1947 jako centrální sbírka lokálních souborů kultur spravovaných jednotlivými národními referenčními laboratořemi, navázala na tradici sbírky kultur, založené v SZÚ na konci 20. let minulého století. Prvním kurátorem byl doc. Juraj Strauss, za jehož působení byly do sbírky zařazeny typové kmeny získané ze zahraničních sbírek, zavedena jednotná dokumentace a evidence kultur a byly uplatněny moderní metody konzervace kmenů (lyofilizace). Dalším kurátorem sbírky se stal dr. Jiří Šourek, který vedl sbírku přes 40 let a významně ji rozšířil až na 5000 kmenů. Pod jeho vedením byla sbírka už s akronymem CNCTC registrována ve Světové federaci sbírek kultur (WFCC) a v Evropské organizaci sbírek kultur – European Culture Collections Organization (ECCO). Od roku 1999 vedla sbírku doc. MUDr. Helena Žemličková, Ph. D., sbírka získala statut Národní referenční laboratoře (NRL), byla provedena rozsáhlá revize sbírky (biochemické metody). V roce 2014 byla sbírka předána RNDr. R. Šafránkové, Ph.D., pokračuje revize sbírky pomocí moderních metod (MALDI-TOF MS, sekvenační analýza 16S rRNA, WGS...). Souhrn činností/aktivit CNCTC: dlouhodobé uchovávání kultur včetně distribuce (prodej), příprava okruhu EHK – Bakteriologická diagnostika, lyofilizace kultur na zakázku, revizní a publikační činnost.
This year marks 75 years since the foundation of the Czech National Collection of Type Cultures (CNCTC), which operates within the Centre for Epidemiology and Microbiology at the National Institute of Public Health (NIPH) in Prague. The CNCTC was officially established in 1947 as a central collection of local collections of cultures managed by individual national reference laboratories, continuing the tradition of the collection of cultures established at NIPH at the end of the 1920s of the last century. The first curator was doc. Juraj Strauss, during whose tenure type strains obtained from foreign collections were included in the collection, uniform documentation and records of cultures were introduced, and modern methods of strain preservation (lyophilization) were applied. A next curator of the collection was Jiří Šourek, who led the collection for over 40 years and significantly expanded it to 5,000 strains. Under his leadership, the collection, already with the acronym CNCTC, was registered in the World Federation of Culture Collections (WFCC) and in the European Culture Collections Organization (ECCO). From 1999, the collection was led by doc. MUDr. Helena Žemličková, Ph.D. It received the status of the National Reference Laboratory (NRL) and its extensive revision (biochemical methods) was performed. In 2014, the collection was handed over to RNDr. R. Šafránková, Ph.D.; its revision continues to be carried out using modern methods (MALDI-TOF MS, 16S rRNA sequencing analysis, WGS...). Summary of CNCTC activities: long-term preservation of cultures including distribution (sale), preparation of EQA (External Quality Assessment) – Bacteriological diagnostics, lyophilization of cultures to order, revision and publication activities.
Kazuistika suspektní septikemie s izolací raritního kmene Corynebacterium sp. Izolace tohoto kmene ani kmenů geneticky blízkých druhů z klinického materiálu nebyla zatím v dostupné literatuře popsána. V článku jsou uvedeny kromě stručného klinického popisu také morfologické a biochemické vlastnosti, metody identifikace a vyšetření k antibiotikům. Cílem práce bylo především upozornit na nový a vzácně se vyskytující kmen korynebakteria.
A case report is presented of a patient with suspected septicaemia from whose blood culture a new strain of Corynebacterium sp. was isolated. Until now, no report of this strain isolated from human clinical materials has been available in the literature. In addition to a brief clinical description of the case, the article also features morphological, biochemical properties as well as antibiogram of the bacterium. It describes also methods used for the identification of this isolate. The aim of the work was to highlight a novel and rare coryneform strain.
Enterobacterales produkující karbapenemázu (CPE – Carbapenemase-Producing Enterobacterales) představují globální zdravotnický problém. V Evropě dochází zejména v jižních státech ke zvyšování podílu enterobakterií rezistentních vůči karbapenemům. Od roku 2016 dochází také v České republice k plošnému nárůstu počtu případů, v roce 2019 bylo zaznamenáno 270 případů kolonizace nebo infekce CPE. Tento trend je zčásti způsoben několika epidemickými epizodami, odráží ale i zhoršující se endemickou situaci ve zdravotnických zařízeních. Přesto lze Českou republiku stále hodnotit jako stát s nízkým výskytem CPE.
Carbapenemase-producing Enterobacterales (CPE) are a global health problem. In Europe, the proportion of carbapenem-resistant enterobacteria is increasing, particularly in the southern states. Since 2016, there has also been a widespread increase in the number of cases in the Czech Republic, with 270 cases of colonization or CPE infection reported in 2019. This trend is partly due to several outbreaks, but it also reflects the deteriorating endemic situation in healthcare facilities. Nevertheless, the Czech Republic still remains a low CPE incidence country.
In 1986, Bouvet and Grimont delineated two related taxa of the genus Acinetobacter termed genospecies (GS) 8 and 9. They proposed the name Acinetobacter lwoffii for GS8, which included the supposed type strain (CIP 64.10). As the authenticity of CIP 64.10 was later questioned, this study aimed at reassessing the taxonomy of these genospecies. We investigated 52 strains of GS8 or GS9, including CIP 64.10 and the genuine type strain of A. lwoffii (NCTC 5866T). All strains were subjected to the genus-wide comparative analyses of MALDI-TOF whole-cell mass spectra, rpoB gene sequences and metabolic traits while whole-genome sequences were analysed for 16 strains. The strains were classified into two distinct groups corresponding to GS8 (n=15) and GS9 (n=37). CIP 64.10 fell within GS8 whereas NCTC 5866T belonged to GS9. Intraspecies ANIb values for the genomes of GS8 (n=6) and GS9 (n=10) were ≥96.1% and ≥95.4%, respectively, whereas the ANIb values between them were 86.8-88.6%. Based on core genome phylogeny, GS8 and GS9 formed a distinct clade within the genus, with two respective, strongly supported subclades. GS8 and GS9 were similar in physiological and catabolic properties but were separable by MALDI-TOF MS. We conclude that the name A. lwoffii pertains to GS9 and not to GS8 as originally assumed and that these groups represent two species. We propose the name Acinetobacter pseudolwoffii sp. nov. for GS8, with ANC 5044T (=CCM 8638T=CCUG 67963T=CIP 111642T) as the type strain, and provide the emended description of A. lwoffii.
Cíl: Účelem surveillance prováděné v období říjen až prosinec v letech 2010–2017 v České republice bylo sledování stavu a trendů citlivosti k betalaktamovým a makrolidovým antibiotikům u kmenů Streptococcus pneumoniae izolovaných od pacientů s infekcí dýchacích cest. Materiál a metody: Studie se každoročně zúčastnilo 42–55 laboratoří. Do studie byly konsekutivně zařazeny neduplicitní izoláty pneumokoků izolované z relevantních mikrobiologických vzorků od pacientů s komunitní bakteriální respirační infekcí dýchacích cest. Laboratoře zaznamenávaly do protokolu kvalitativní výsledky vyšetření citlivosti k penicilinu a erytromycinu, citlivost k antibiotikům byla stanovena diskovým difuzním testem. Izoláty necitlivé k penicilinu a/nebo rezistentní k erytromycinu byly zasílány do Národní referenční laboratoře pro antibiotika, kde byla pomocí mikrodiluční bujónové metody vyšetřena minimální inhibiční koncentrace antibiotik, v Národní referenční laboratoři pro streptokokové nákazy byl stanoven sérotyp. U 26 izolátů z roku 2017 byla provedena molekulární typizace pomocí multilokusové sekvenace. Výsledky: Celkem bylo vyšetřeno 7 491 kmenů pneumokoků, 53,7 % (4 023) pocházelo z materiálu z horních cest dýchacích, 47,7 % (3 573) tvořily vzorky od dětí mladších 15 let. Necitlivost k penicilinu klesla z 2,6 % v roce 2010 na 1,2 % v roce 2017, rezistence k erytromycinu ve stejném období stoupla ze 7,4 % na 9,7 %. Penicilin necitlivé izoláty patřily zejména do sérotypů 19A, 19F a 15A. Izoláty rezistentní k makrolidům, ale citlivé k penicilinu byly zastoupeny především sérotypem 19A a 3. Multilokusová sekvenace u penicilin necitlivých izolátů prokázala přítomnost klonu Taiwan19F-14, nejčastějším detekovaným sekvenčním typem (ST) izolátů rezistentních k makrolidům byl ST416, který patří do klonu Netherlands15B-37. Závěry: Respirační studie antibiotické rezistence u S. pneumoniae potvrdila klesající trend výskytu rezistence k penicilinu, ale odhalila narůstající rezistenci k makrolidovým antibiotikům v České republice. Výsledky naší studie potvrzují, že antibiotická rezistence je v éře vakcinace spojena především s nevakcinačními sérotypy a klonální expanze sérotypu 19A rezistentního k makrolidům byla patrně navíc podpořena narůstající preskripcí makrolidových antibiotik.
Aim: The purpose of the surveillance performed from October to December in 2010-2017 was to monitor the trends in the susceptibility to beta-lactam and macrolide antibiotics in Streptococcus pneumoniae isolates from respiratory tract infections in the Czech Republic. Material and methods: Between 42 and 55 laboratories participated in the study every year. Consecutive non-duplicate pneumococcal isolates from relevant microbiological specimens from patients with community-acquired bacterial respiratory tract infection were sequentially included in the study. Laboratories recorded qualitative results of penicillin and erythromycin susceptibility testing; susceptibility to antibiotics was determined by the disk diffusion method. Penicillin non-susceptible and/or erythromycin resistant isolates were referred to the National Reference Laboratory for Antibiotics, where the minimum inhibitory concentration of each antibiotic was tested using the broth microdilution method, and their serotyping was performed in the National Reference Laboratory for Streptococcal Infections. Twenty-six isolates from 2017 were analysed by the multilocus sequence typing method. Results: In total, 7 491 pneumococcal strains were examined, of which 53.7% (4 023) were from the upper respiratory tract and 47.7% (3 573) from children under 15 years of age. Non- -susceptibility to penicillin decreased from 2.6% in 2010 to 1.2% in 2017, while resistance to erythromycin increased from 7.4% to 9.7% over the same period. Penicillin non-susceptible isolates were mostly of serotypes 19A, 19F, and 15A. Macrolide resistant but penicillin susceptible isolates were predominantly represented by serotypes 19A and 3. The presence of the Taiwan19F-14 clone was confirmed in penicillin non-susceptible isolates by MLST, and the most frequently identified sequence type (ST) in macrolide resistant isolates was ST416 classified into the Netherlands15B-37 clone. Conclusions: The respiratory study of antibiotic resistance in S. pneumoniae confirmed the decreasing trend of resistance to penicillin but revealed a growing resistance to macrolide antibiotics in the Czech Republic. The results of our study confirm that antibiotic resistance in the vaccination era is associated primarily with the non-vaccine serotypes, and the clonal expansion of macrolide resistant serotype 19A was apparently supported by the growing prescription of macrolide antibiotics.

Enterobakterie produkující karbapenemázu (CPE – Carbapenemase-Producing Enterobacteriaceae) představují globální problém, který má přímý vliv na zvládání jimi způsobených závažných infekcí. V Evropě dochází v některých státech k nárůstu karbapenemové rezistence. Existují však státy (např. Izrael), kde se situaci podařilo stabilizovat a dokonce i zlepšit. V roce 2014 bylo v České republice zaznamenáno 19 případů kolonizace nebo infekce CPE. V roce 2015 tento počet stoupl na 30 případů. Přesto lze Českou republiku hodnotit, jako zemi s nízkým výskytem CPE. Dosud byly zaznamenány pouze ojedinělé outbreaky, které se díky metodickému pokynu vydanému Ministerstvem zdravotnictví daří zvládat.
Carbapenemase-Producing Enterobacteriaceae (CPE) represent a global problem which has a direct effect on the treatment of serious infections caused by such bacteria. In Europe, carbapenem resistance has been increasing, but in some countries (e.g., Israel), the prevalence of CPE stabilized or is even declining. In 2014, 19 cases of colonization or infection by CPE was detected in the Czech Republic. In 2015, the number of those cases increased to 30. Even so, the Czech Republic remains a low-prevalence country. Only few sporadic outbreaks have been identified. Those outbreaks, however, were quickly brought under control by following the Czech guidelines issued by the Ministry of Health.
Během necelých dvou let v období 6/2012 až 4/2014 došlo oproti rokům 2010 a 2011 k nárůstu výskytu enterobakterií produkujících karbapenemázu (CPE) v nemocnicích v České republice. Během sledovaného období bylo Národní referenční laboratoří pro antibiotika SZÚ v Praze a Ústavem mikrobiologie LF UK a FN v Plzni potvrzeno 24 případů. Jednalo se o metalo-β-laktamázy typu VIM (n=7) a NDM (n=8) převážně u Klebsiella pneumoniae, Enterobacter spp. a Serratia marcescens. Dále bylo zaregistrováno 6 případů produkce KPC (K. pneumoniae, C. freundii). Nově byly zachyceny tři případy K. pneumoniae produkující karbapenemázy typu OXA-48. Nejedná se již pouze o importované případy ze zemí s endemickým výskytem CPE. Důsledným dodržováním metodického pokynu Ministerstva zdravotnictví ČR lze zabránit šíření tohoto nebezpečného typu rezistence.
Over less than two years, between June 2012 and April 2014, carbapenemase (CPE) producing Enterobacteriaceae showed an upward trend in Czech hospitals in comparison to 2010 and 2011. During the period specified above, the National Reference Laboratory for Antibiotics of the National Institute of Public Health and the Department of Microbiology, Faculty of Medicine, Charles University and University Hospital, Plzeň confirmed 24 cases. VIM (n=7) and NDM (n=8) metallo-beta-lactamases with CPE activity were mostly detected in Klebsiella pneumoniae, Enterobacter spp., and Serratia marcescens. Klebsiella pneumoniae carbapenemase (KPC) production was identified in six cases (K. pneumoniae, C. freundii).Three K. pneumoniae strains producing OXA-48 carbapenemase were newly detected. CPE production was detected not only in strains imported from endemic countries. Careful adherence to the guidelines of the Ministry of Health of the Czech Republic can prevent the spread of this dangerous type of resistance.
