Ve dvou souborech kriticky nemocných byly hodnoceny parametry hemodynamiky a oxygenace získávané pravostrannou srdeční katetrizací pomocí Swan-Ganzova katétru. První soubor tvořilo 15 nemocných s multiorgánovou dysfunkcí (109 vyšetření), druhý 20 nemocných po kardiochirurgickém výkonu se syndromem nízkého srdečního výdeje (100 vyšetření). Byly hodnoceny a srovnány: srdeční výdej (CI), systémová a plicní cévní rezistence (SVRI a PVRI), paO2/FiO2, plicní zkraty (Qs/Qt), dodávka a spotřeba O2 (DO2, VO2), hladina hemoglobinu (Hb) a laktatemie. V obou souborech se pohybovaly hodnoty CI v referenčních mezích. Index paO2/FiO2 byl v 1. souboru snížen v 86,1 % do hodnot typických pro syndrom akutní dechové tísně, ve 2. souboru jen ve 14 %. Hladiny Hb byly v obou souborech sníženy pod dolní hranici referenčního rozmezí. DO2 byla snížena v 65,1 %, resp. v 57 %, VO2 byla snížena ve 41,3 %, resp. v 11 %. Laktatemie byly zvýšeny v 19,9 %, resp. v 27,6 %. V obou souborech byl zjištěn statisticky významný vztah (p<0,01) pro DO2/VO2; v 2. souboru navíc pro CI/DO2 a CI/VO2. V obou souborech nebyla zjištěna významnost vztahu laktatemie k CI, DO2 ani k VO2. Diskutuje se o názorech na význam laktatemie pro rozpoznání tkáňové hypoxie.
Hemodynamic and oxygen transport parameters in two group of critically ill patients were compared in the presented study. The data were obtained from right heart catheterization using Swan-Ganz catheter. First group consisted of 15 patients with multi-organ dysfunction (109 measurements), the other group consisted of 20 patients after cardiac surgery with low cardiac output syndrome (100 measurements). Following parameters were compared: cardiac index (CI), indexed systemic and pulmonary vascular resistance (SVRI and PVRI), paO2/FiO2, pulmonary shunt (Qs/Qt), O2 delivery and consumption (DO2, VO2), hemoglobin level a lactate level. In both groups CI was within reference limits. In the first group, paO2/FiO2 index was decreased in 86,1% to levels typical for acute respiratory distress syndrome; in the second group, this was observed only in 14%. Hemoglobin levels were below reference range in both groups. DO2 was decreased in 65,1% and 57%, VO2 was decreased in 41,3 and 11%, respectively. Lactate levels were increased in 19,9% and 27,6%, respectively. In both groups there was observed statistically significant relationship (p<0.01) in DO2/VO2; in the second group also CI/DO2 and CI/VO2. For both groups there was not observed statistically significant relationship of lactate levels to CI, DO2 nor VO2. We discuss over the importance of lactate level for detection of tissue hypoxia.
