Cíle. Rodičovské vyhoření je relativně nově zkoumaným fenoménem, projevuje se vyčerpáním v rodičovské roli, emocionálním odstupem od vlastních dětí a negativním hodnocením sebe jako rodiče. Parental Burnout Assessment (PBA) je v současnosti zlatým standardem v měření rodičovského vyhoření. Cílem této studie bylo prozkoumat psychometrické vlastnosti českého překladu Parental Burnout Assessment – otestovat jeho shodu s teoretickou čtyřfaktorovou strukturou, určit jeho vnitřní konzistenci a dále souvislosti mezi skórem rodičovského vyhoření a některými sociodemografickými charakteristikami. Metody. Vzorek sestával z 355 rodičů žijících v České republice (219 matek a 136 otců), struktura vzdělání výzkumného vzorku neodpovídala normální populaci České republiky (vyšší frekvence rodičů s vysokoškolským vzděláním). Studie byla provedena v rámci druhé vlny mezinárodního výzkumu rodičovského vyhoření (International Investigation of Parental Burnout; IIPB; Factors in Parental Satisfaction and Exhaustion). Hypotézy. Autoři očekávali dobrou reliabilitu (konzistenci) české verze škály, vyšší vyhoření u matek ve srovnání s otci a nízkou korelaci s dalšími sociodemografickými faktory. Statistická analýza. Faktorová struktura byla zkoumána konfirmační faktorovou analýzou. Pro testování vnitřní konzistence bylo použito McDonaldovo omega, pro výpočet vztahů mezi sociodemografickými proměnnými a rodičovským vyhořením neparametrické Kendallovy korelace. Výsledky. Konfirmační faktorová analýza podpořila čtyřfaktorový model i model s jedním latentním faktorem druhého řádu. Vnitřní konzistence měřená pomocí McDonaldovy omegy byla vysoká. Matky vykazovaly vyšší rodičovské vyhoření ve srovnání s otci a vyhoření matek se zvyšovalo s počtem dětí a s počtem dětí mladších 4 let. Vyhoření otců se zvyšovalo s počtem synů a počtem dětí ve věku 5 až 9 let. Limity výzkumu. Hlavním limitem studie byla nereprezentativita vzorku, a to zejména vysoká úroveň vzdělání respondentů. Dalším limitem je to, že data byla sbírána v období končící první vlny epidemie covidu-19, což mohlo ovlivnit míru vyhoření českých rodičů.
Objectives. Parental burnout is a relatively newly studied phenomenon characterized by exhaustion in one’s parental role, emotional distance from children and low parental self-esteem. Parental Burnout Assessment (PBA) is the current gold-standard instrument for the measurement of parental burnout (PB). The aim of this study is to examine the psychometric properties of the Czech translation of the PBA (PBA-Czech). We tested the theoretical four-factor structure of PBA-Czech and its internal consistency. We also calculated the correlations of PBA scores with some sociodemographic characteristics. Sample and settings. Data were collected from 355 highly-educated parents living in the Czech Republic, of whom 219 were mothers and 136 fathers. The study was conducted as part of the second wave of the International Investigation of Parental Burnout (IIPB), Factors in Parental Satisfaction and Exhaustion. Hypotheses. We expected good reliability of the Czech version of the scale and a similar frequency of the particular burnout stages as measured in the original Belgian population. We also expected higher burnout in mothers compared to fathers and low correlations with other socio-demographic factors. Statistic analysis. The factor structure was examined by confirmatory factor analyses. For testing the internal consistency we used Cronbach’s alpha. Nonparametric Kendall correlations, Chi Squared Automatic Interaction Detection and General Linear Model were used for calculating relations between sociodemographic variables and PB. Results. The confirmatory factor analyses supported both the four-factor model and the second-order factor model. The inner consistency measured by Cronbach’s alpha was high. Mothers showed higher PB compared to fathers and mothers’ burnout increased with a number of children as well as with a number of children under 4 years of age. Fathers’ burnout increased with a number of sons. In both mothers and fathers, their burnout was decreasing with age with a similar slope. Limitations. The main limitation of our study was a non-random sample selection strategy and the high level of education in our sample. Another limitation relates to the time of the data collection at the end of the COVID pandemic as it could have affected the level of PB.

Matrix metalloproteinases (MMPs) are associated with the alteration of extracellular matrix. The purpose of this study was to investigate how the levels of matrix metalloproteinases and their inhibitors - TIMPs are influenced by the presence of inguinal hernia as well as by its surgical treatment. The studied group consisted of 25 patients with inguinal hernia and 21 healthy controls for comparison. Two blood samples - before and after the treatment were collected from patients. Serum concentrations of MMPs and TIMPs were analysed by multiplex immunoassays. There was a difference in circulating levels of MMPs in patients before the surgery compared to healthy controls - the concentrations of MMP-2 and MMP-9 were significantly lower (p=0.026, p=0.018, respectively). After the surgery, the levels of MMPs, especially MMP-2 (p<0.0001), were significantly decreased in patients compared to the preoperative values, apart from MMP-9. On the contrary, MMP-9 showed significant increase after the surgery (p<0.0001). Circulation levels of TIMP-2 in patients were significantly decreased in comparison with controls (p=0.004), whereas levels of TIMP-1 were similar to controls. Both tested metalloproteinase inhibitors showed a significant decrease in detected levels (TIMP-1 p=0.0004; TIMP-2 p<0.0001) after the procedure compared to the preoperative values. The levels of MMPs, especially MMP-2 and MMP-9, and their inhibitors TIMP-1 and TIMP-2 are involved by the presence of inguinal hernia as well as are influenced by the surgery.

The crucial requirement of molecular genetic methods is high-quality input material. The key question is "how to preserve DNA during long-term storage." Biobanks are recommended to aliquot isolated DNA into provided volumes. The aim of this study was to analyse the effect of repeated freezing and thawing on the genomic DNA integrity, quality and concentration. The aliquoted DNA isolated from blood cells using the automatic MagNA system and manual salting out method underwent freeze/thaw cycles at different storage conditions (-20 °C, -80 °C and liquid nitrogen). The average initial concentrations were 270.6 ng/μl (salting out method) and 125.0 ng/μl (MagNA). All concentration deviations relative to the concentration after the first freeze/ thaw cycle were less than 5 % for -20 °C and -80 °C cycling with both isolation methods. The average percentage differences of liquid nitrogen samples were higher, and the MagNA isolation method showed significant differences. There were no significant changes in the DNA purity or quality. The repeating freeze/ thaw up to 100 cycles (through -20 °C and -80 °C, respectively) did not significantly influence the integrity, concentration, or purity of genomic DNA, suggesting that storage of samples in high-volume pools without multiple aliquoting is possible. Storage in a freezer seems to be the most suitable way of long-term DNA preservation, because liquid nitrogen storage leads to formation of DNA clumps.

Úvod: Pády pacientů patří mezi časté nežádoucí události. Je důležitou úlohou managementu nemocnic zavést do praxe efektivní program prevence pádů pacientů, který je komplexní, multifaktoriální a zároveň vysoce individualizovaný. Cílem výzkumu bylo vyhodnotit účinnost zavedeného intervenčního programu prevence pádů hospitalizovaných pacientů ve vybraných nemocnicích Jihočeského kraje. Metodika: Jednalo se o epidemiologickou studii observačního a intervenčního typu. V roce 2018 byl na 16 odděleních čtyř nemocnic Jihočeského kraje zaveden intervenční program, jehož cílem bylo minimalizovat rizikové faktory pádů pacientů. Intervenční program byl zaveden u všech pacientů hospitalizovaných ve sledovaném období na vybraných pracovištích. V roce 2017 šlo o 24 379 pacientů a v roce 2018 o 25 773 pacientů. Následně byla sledována účinnost zavedených intervencí. Byl vyhodnocen rozdíl ve výskytu pádů v období 2017 před intervenčním programem a v roce 2018 v době realizace programu. Index pádů byl stanoven jako počet pádů na 1 000 pacientů a počet zranění z pádu na 1 000 lůžkodnů. Výsledky: Po zavedení preventivního intervenčního programu byl pokles pádů zaznamenán na odděleních následné a rehabilitační péče (z 39,12 na 30,9 pádů/1 000 pacientů a z 0,79 na 0,58 pádů se zraněním/1 000 lůžkodnů), na chirurgických odděleních (z 5,88 na 5,78 pádů/1 000 pacientů a z 0,98 na 0,59 pádů se zraněním/1 000 lůžkodnů) a oddělení psychiatrie (z 14,27 na 7,48 pádů/1 000 pacientů a z 0,58 na 0,23 pádů se zraněním/1 000 lůžkodnů). Naopak vyšší index pádů byl potvrzen na interních odděleních (z 8,54 na 10,4 pádů/1 000 pacientů a z 1,22 na 1,63 pádů se zraněním/1 000 lůžkodnů). Závěr: Monitoring pádů a kořenová analýza jejich příčin umožňuje managementu zdravotnických zařízení nastavit účinná nápravná a preventivní opatření snižující výskyt pádů a minimalizující jejich důsledky.
Introduction: Patient falls are frequent unwanted events. Hospital managements have the important task of implementing an efficient programme of patient fall prevention - which is complex, multifactorial and highly individualized. The goal of this research was to assess the efficiency of the implemented fall prevention programme in selected South-Bohemian hospitals. Methods: This is an epidemiological, observational and interventional study. In 2018, 16 departments in 4 South-Bohemian hospitals implemented the intervention programme - the aim of which was to minimize risk factors of patient falls. All patients were included in the programme during the monitored period at selected workplaces. In 2017, 24,379 patients were included, and in 2018 there were 25,773 patients. We then monitored the efficiency of the implemented interventions. We assessed the differences between the incidence of falls in 2017 before the implementation of the intervention programme and the incidence of falls in 2018 during the realization of the programme. The fall index was set to the number of falls of 1,000 patients and the number of fall injuries during 1,000 days in hospital. Results: After the implementation of the prevention programme, the decrease in the number of falls was recorded at the departments of subsequent and rehabilitation care (from 39.12 to 30.9 falls/1,000 patients and from 0.79 to 0.58 falls with injuries/1,000 hospital days), departments of surgery (from 5.88 to 5.78 falls/1,000 patients and from 0.98 to 0.59 falls with injuries/1,000 hospital days) and the department of psychiatry (from 14.27 to 7.48 falls/1,000 patients and from 0.58 to 0.23 falls with injuries/1,000 hospital days). On the contrary, a higher fall index of falls was confirmed at internal departments (from 8.54 to 10.4 falls/1,000 patients and from 1.22 to 1.63 falls with injuries/1,000 hospital days). Conclusions: The fall monitoring and the root analysis of their causes enable the managements of medical institutions to establish efficient remedial and preventative measures that decrease the incidence of falls and minimize their consequences.

Sclerostin is a protein which is involved in bone metabolism and probably also in vessel wall function. This prospective observational cohort study evaluated the prognostic significance of sclerostin in hemodialysis (HD) patients. In total, 106 HD patients and 25 healthy controls participated in the study. HD patients were prospectively followed up for five years. Sclerostin was measured in serum using standard ELISA kits by Biomedica. Sclerostin concentrations in serum were higher in HD patients compared to the controls (89.2±40.3 pmol/l vs. 32.8±13.0 pmol/l, p<0.001). Sclerostin levels were significant for cardiovascular mortality but not for overall mortality and mortality due to infection. A higher cardiovascular risk was connected to sclerostin concentrations above the median (>84 pmol/l), HR (95 % CI): 2.577 (1.0002-10.207), p=0.04. When sclerostin was evaluated together with residual diuresis in Kaplan-Meier analysis the worst prognosis due to cardiovascular events was observed in the group with high sclerostin and zero residual diuresis compared to all other patients (p=0.007). In summary, serum sclerostin levels in HD patients were increased when compared to healthy subjects. High sclerostin levels were demonstrated as a risk factor for cardiovascular mortality. Further studies are required to clarify the pathophysiological mechanisms of sclerostin action in patients with renal failure before therapeutic measures can be established.
