Vyšší incidence poranění u zraněných nad 65 let je spojena i s narůstající morbiditou a mortalitou, především u polytraumat. Významnou roli v rozvoji poúrazové odpovědi má výskyt přidružených onemocnění, které jsou rizikovými faktory pro vznik poúrazových komplikací a také mortality zraněných. Narůstající počet polytraumat u zraněných starších 65 let přijatých do Traumatologického centra FNKV byl u této skupiny zraněných důvodem k vyhodnocení některých epidemiologických dat a jejich porovnání s daty ke zraněným mladšími 65 let. V rámci retrospektivní studie bylo vyhodnoceno celkem 239 zraněných s polytraumatem, přijatých na urgentní příjem KAR a TC FNKV. Z celkového počtu bylo 47 zraněných starších 65 let, 29 z nich bylo (61 %) mužů. Průměrný věk ve skupině zraněných starších 65 let byl 75 let. Do traumatologického centra bylo primárně přijato 42 (85 %) zraněných starších 65 let. Mezi příčinami úrazu byly nejčastěji dopravní úrazy a pády. Nejčastějším poraněním u starších nemocných bylo trauma hlavy 36 (76 %) a hrudníku 38 (80 %). Zlomeniny skeletu končetin postihly 21 (44 %) zraněných. Z porovnání skóre APACHE II u obou skupin zraněných (19 vs. 27,2) lze stanovit vyšší výskyt přidružených onemocnění u pacientů starších 65 let. Průměrné skóre ISS hodnotící závažnost poranění byl bez statisticky významného rozdílu (22 vs. 21). Zemřelo 42 (22 %) zraněných ve skupině do 65 let a 22 (47 %) pacientů starších 65 let. Zranění nad 65 let jsou vystaveni vyšší mortalitě ve srovnání se zraněnými do 65 let. Na výsledku léčby se vedle věku podílí i přidružená onemocnění. Mechanismus poranění a z toho rezultující typ poranění u starší populace vyžaduje nové strategie v poúrazové péči. K tomu směřuje i zajištění primárního přijetí staršího zraněného s polytraumatem do traumatologického centra.
Higher incidence of injury in trauma patients older than 65 years is connected with growing morbidity and mortality, first of all in polytrauma. Incidence of preexisting diseases play a great role in development of postinjury reaction. There are risk factors for genesis of postinjury complications and also mortality of victims. Growing number of polytrauma patients older than 65 years admitted to Trauma Centre Faculty Hospital Královské Vinohrady was reason for evaluation of epidemiological data and their comparison with data regarding to victims younger than 65 years. In retrospective study was evaluated 239 injured patients with polytrauma, admitted in Emergency unit Department of anesthesiology and resuscitation. From overall number was 47 victims older than 65 years (29/61 %). 75 years was an average age in victims older than 65 years. Primary admission to Trauma Centre was in case of 42 (85 %) victims older than 65 years. Between causes of injury were most frequent traffic accidents and falls. Most frequent injury in geriatric patients was craniocerebral injury in 36 patients (76 %) and thoracic trauma in 38 patients (80 %). Skeletal fractures of extremities were found in 21 (44 %) victims. From comparison of APACHE II score in both group of patients (19 in group of younger patients vs. 27,2 in group of older victims) is possible to establish higher incidence of preexisting diseases in patients older than 65 years. Average ISS score evaluated serious of trauma was without statistic significant difference (22 vs. 21). Number of deaths was 42 (22 %) in group of victims younger than 65 years and 22 (47 %) in group of victims older than 65 years. Older victims (over 65 years old) were exposed higher mortality in comparison with younger patients. On the result of treatment participate preexisting diseases besides age. Mechanism of injury and from this resulting type of trauma in older population need new strategy in postinjury care. Performance of primary admission of older victims with polytrauma to Trauma Centre is tend to this strategy

PURPOSE OF THE STUDY Based on the experience with using the Judet plates in stabilization of rib fractures an innovated Judet plate was constructed in cooperation with the Development Department of Medin company. During the preclinical part of the project, following the construction of the new Judet plate, a surgical technique was elaborated. Subsequently, the clinical application of rib osteosynthesis with innovated plates was commenced. MATERIAL AND METHODS In the course of the last three years the innovated Judet rib plate including the instruments were constructed by the Development Department of Medin company in cooperation with the Trauma Centre and the Department of Surgery of the University Hospital Královské Vinohrady. The goal of the innovation was the changes in technical parameters of rib plates which are compared with the new plates of other companies: 1. Adequate plate stability along the rib axis. 2. Creation of fixation clips for a stable, but not traumatized fixation of the plate around the rib. 3. Decreased robustness of the plate with sufficient stiffness. 4. Working out of a new technique of plate fixation with the use of new instruments. 5. Ensuring plate fixation with cortical locking screws. In the preclinical part of the project osteosynthesis of the broken rib was performed with a plate on a chest model for the basic types of rib fractures. Subsequently, the plate was used for rib osteosynthesis in a cadaver. The goal of the new instruments was besides temporarily maintaining the rib fracture reduction also the subsequent temporary application of plates with the assistance of fixation tongs before the final fixation of plates. In 2017, the clinical part of the project on the stabilisation of flail chest with the innovative Judet plates was launched. This type of innovative Judet plates has so far been used in 3 patients. RESULTS As to the surgical technique of ribs osteosynthesis: The profile of the plate has been adapted to the rib profile and can be further adjusted to rib curvature, also the fixation shoulders of the plates have been reshaped. The innovated plate has been complemented with a new configuration of fixation clips and the possibility to fix the plate with locking screws. The used cortical locking screws enable appropriate stabilisation of plates. They were tested on a laboratory model of ribs and on a cadaver using the new set of instruments. The plates can be easily shaped with tongs. The anchorage of fixation clips is adequately provided for also by means of tongs. The plates can be fixed to the rib without any significant compression of intercostal nerves. The essential change of the innovated Judet plate is its weight, which meets the contemporary trends in construction of rib plates designed for anatomical fixation. The first experience with the innovated plates in flail chest injury confirmed the safety of the procedure for stable osteosynthesis of rib fractures. Adequate stability of the chest wall facilitated an early withdrawal of ventilatory support. Osteosynthesis of rib fractures with innovated plates performed in the first three patient was without complications, including in the postoperative period. DISCUSSION A series of prospective studies prove the correctness of the indications of early rib osteosynthesis in flail chest. Further indications for rib osteosynthesis are postinjury chest deformity, dislocation of rib fracture fragments with a lung injury, and malunion of rib fracture. According to these studies the surgical stabilisation of the chest is a safe and efficient method resulting in pain reduction, decrease of ventilatory support time, and also in reduced morbidity. For rib osteosynthesis anatomical plates, Judet plates and intramedullary plates are used. Our innovation of the Judet plate aimed to improve the technical parameters of the plate. The innovative Judet plate means a significant improvement in technical parameters and stands comparison with the plates of others companies, which are used in the Czech Republic. This is also evidenced by preliminary clinical results. CONCLUSIONS Surgical stabilisation of the flail chest segment is considered to be he method of choice in treating selected patients, leading to the improvement of respiratory function and shortening of the ventilatory support time. The new technical parameters of the plate, including its weight, new fixation clips, locking screws and instruments are the priorities of the innovated Judet plates. The innovation of Judet plates represents an important step towards the extension of indications for surgical stabilisation of the chest. Úvod: Metodou volby v léčbě blokové zlomeniny žeber a dalších nestabilních poranění hrudní stěny je prováděná stabilizace osteosyntézou žeber. Indikací k osteosyntéze žeber je vedle blokové zlomeniny žeber i deformita nebo defekt hrudní stěny, dislokace zlomeniny žebra s poraněním plíce, otevřená zlomenina žeber, otevřený defekt hrudní stěny, porucha hojení zlomeniny žebra a osteosyntéza dislokovaných žeber u akutní torakotomie. Na základě zkušenosti s použitím Judetovy dlahy při stabilizaci zlomenin žeber byla ve spolupráci Traumatologického centra a Chirurgické kliniky FNKV a 3. LF UK s vývojovým oddělením firmy Medin a.s., Česká republika, provedena inovace žeberních dlah Judetova typu, které byly použity v rámci klinické části studie u první skupiny zraněných s nestabilitou hrudní stěny. Materiál, metoda: V průběhu posledních třech let byla konstruována inovovaná žeberní dlaha včetně instrumentária. Cílem inovace nových dlah bylo: 1.Nové technické parametry dlahy - možnosti ohnutí dlahy ve všech směrech, konstrukce nových fixačních klipů. 2.Použití uzamykatelných kortikálních šroubů k fixaci dlahy - adekvátní a bezpečná fixace dlahy při zavedení šroubu skrze obě kortikální části žebra. 3.Nové instrumentárium - kleště k uchopení dlahy, k úpravě ohnutí a fixační kleště. 4.Fixace dlahy k žebru pomocí ohnutí fixačních klipů - s minimální kompresí interkostálních cév a nervů. V roce 2017 byla zahájena již klinická část projektu stabilizace blokové zlomeniny žeber osteosyntézou inovovanými Judetovými dlahami, které byly použity u prvních šesti zraněných. Výsledky: Operační technika s použitím nových dlah byla provedena se suficientními výsledky u šesti nemocných s blokovou zlomeninou žeber. Fixace dlah v kombinaci s ukotvením fixačních klipů klipů a použitím kortikálních uzamykatelných šroubů skrz obě kortikální části žebra je adekvátní a bezpečný postup pro osteosyntézu. U zraněných byla provedena revize s torakotomie pro retinovaný hemotorax nebo již hematom v pleurální dutině. U všech zraněných byla prokázána kontuze plicní. Osteosyntéza žeber byla provedena v období od třetího do šestého dne po úraze. Pooperační mechanická ventilace byla v trvání od 24 hodin do šestého dne. Pooperační průběh byl bez komplikací s hojením per primam. Závěr: Inovovaná technika osteosyntézy žeber u blokové zlomeniny žeber představuje jednu z podmínek k rozšíření této metody stabilizace závažných poranění hrudní stěny. Tato metoda osteosyntézy žeber je jednoduchá, bezpečná s krátkým časovým intervalem pro fixaci dlahy s dostatečnou stabilizací. První klinické zkušenosti prokázaly bezpečné provedení fixace žebra se současnou možností revize dutiny hrudní s minimálním rizikem vzniku komplikací v pooperačním období.
Introduction: The method of choice in the treatment of flail chest and other unstable injuries of the chest wall is a rib osteosynthesis. Other indications for surgical stabilization of injured chest wall include deformities or defects of the chest wall, dislocated rib fractures with a lung injury, open rib fractures, open defects of the chest wall, impaired healing of rib fractures, and osteosynthesis of dislocated ribs in patients with acute thoracotomy. On the basis of extensive experiences with use of Judet plates for stabilisation of rib fractures, innovated version of the rib plates was designed, in cooperation between Traumatology Centre and Department of Surgery of the University Hospital Královské Vinohrady and 3rd Faculty of Medicine, Charles University, and the Department of Development of the Medin a.s., Czech Republic Company. These innovated plates were subsequently used in a clinical trial for treatment of injured patients with unstable chest wall. Materials, methods: An innovated rib plate, including appropriate instruments, has been developed and constructed in the course of the last three years. The aims of innovation of the new plates included the following: 1.New technical parameters of the plates - possibility to bend the plates in all directions, construction of new fixation clips. 2.Use of locking screws for plate fixation - adequate and safe fixation in cases when the screws penetrate through both cortical layers of the rib. 3.New Instruments - tongs for holding the plate, for bending and fixation tongs. 4.Fixation of plates to the rib by bending the fixation clips - with minimal compression of intercostal vessels and nerves. The clinical part of the project of stabilizing flail chest fractures with osteosynthesis using the innovated Judet rib plates began in 2017; the innovated plates were used in six patients. Results: The surgical technique using the innovated plates was performed with satisfactory outcomes in six patients with flail chest injuries. Fixation of the plates in combination with anchoring of the fixation clips and the use of cortical locking screws through both cortical parts of the rib is an adequate and safe procedure for osteosynthesis. All patients underwent revision thoracotomy due to retained haemothorax or haematoma in the pleural cavity. Pulmonary contusion was confirmed in all patients. Rib osteosynthesis was performed between the third and sixth day after injury. Postoperative mechanical ventilation lasted between 24 hours and six days. The postoperative course was without complications, with primary healing. Conclusion: The innovated technique of rib osteosynthesis in flail chest injuries is one of the conditions for extending this technique of stabilization of serious injuries of the chest wall. This technique of rib osteosynthesis is simple, safe, requiring a short time interval for plate fixation and providing sufficient stabilization. The first clinical experience showed safe performance of rib fixation, with a concomitant possibility to perform revision of the thoracic cavity, with a minimal risk of complications in the postoperative period.

Úvod: Včasná indikace operace v poúrazovém období je jedním z limitujících faktorů výsledku léčby závažných traumat s triage pozitivitou. Zastavení krvácení, dekomprese tělní dutiny a zabránění pokračování kontaminace jsou prioritní cíle urgentních operací u tupých i penetrujících poranění. Signifikantní snížení letality u triage pozitivních poranění je spojeno i se zařazením damagecontrol operací, a to u poranění hrudníku, břicha, pánve a končetin. Materiál, metodika, výsledky: V retrospektivní studii bylo zhodnoceno 1735 zraněných s triage pozitivitou, u kterých byla provedena akutní nebo odložená operace v období 2008-2014, přijatých do Traumatologického centra. Z celkového počtu bylo 1334 mužů a 401 žen. 1447 (83 %) zraněných mělo při přijetí ISS<16. Většina příjmů bylo primárních (1439 nemocných - 82,9 %). Úrazy byly nejčastěji následkem dopravních nehod u 51 % zraněných, u 36 % byl příčinou úrazu pád. Z celkového počtu byla triage pozitivita u 45 % s polytraumatem, u 33 % s izolovaným kraniocerebrálním poraněním, u 15 % se sdruženým poraněním a u 7 % s monotraumatem. Nejčastěji byla u zraněných poraněna hlava (79 %), dále hrudník (49 %) a končetiny (41 %). Operace byla provedena u 1320 zraněných, z toho u 72 % jako akutní operace. Odložený výkon byl indikován u 24 %, a reoperace u čtyřech procent pacientů. Letalita v souboru byla 17,9 %, z toho více než 65 % u zraněných s ISS 40 a více. Závěr: Včasná indikace a provedení urgentní nebo akutní operace u poranění s triage pozitivitou s cílem kontroly zdroje krvácení, kontaminace a dekomprese tělní dutiny je zásadním léčebným opatřením. Timing operací je stanoven na základě klinického vyšetření a výsledku zobrazovacích metod. Podle lokalizace poranění jsou nejčastějšími výkony v časné poúrazové fázi u triage pozitivních zraněných osteosyntézy zlomenin končetin, pánve, trepanace a laparotomie. Fázové operační řešení (damage control surgery, orthopedics) jsou metodou volby u zraněných s protrahovanou hypotenzí, metabolickou acidózou, hypotermií a koagulopatií.
Introduction: Timely indication of surgery in the early post-injury period is one of the factors influencing treatment outcomes of the seriously polytraumatized with triage positivity. Stopping the bleeding, decompression of the body cavities and prevention of continuous contamination are the primary aims of emergency surgical procedures in patients with blunt and penetrating injuries. Significant decrease of the lethality in patients with triage-positive injuries is associated with the use of damage-control surgeries, especially in patients with chest, abdominal and pelvic trauma, and patients with extremity injuries. Material, methods, results: We analysed a total of 1735 patients with triage-positive injuries admitted to our Traumatology Centre who underwent an emergency or delayed surgical procedure between 2008 and 2014. In this group there were 1334 males and 401 females. A total of 1447 (83 %) patients had the ISS<16 upon admission. In most cases, they were patients with primary injuries – 82.9 %. The injuries were most frequently caused by traffic collisions (51 %), followed by falls from height (36 %). From the total number of patients, triage positivity was observed in 45 % of patients with polytrauma, in 33 % of patients with an isolated craniocerebral injury, in 15 % of patients with associated injuries, and in 7 % of patients with a monotrauma. Head was the most frequently injured body region (79 %), followed with the chest (49 %) and extremities (41 %). A total of 1320 patients underwent surgery, 72 % of them as an emergency procedure. Delayed surgery was indicated in 24 % of patients, reoperation was performed in 4 % of patients. Mortality in our study group was 17.9 %, where more than 65 % of these patients had an ISS score 40 or higher. Conclusion: Timely indication and performance of emergent or acute surgical procedure in patients with triage-positive injuries, with the aim to control bleeding, reduce contamination and to achieve decompression of the body cavities is the principal aim of treatment. Timing of the surgical procedures is based on clinical examination of the patient and the results of imaging. Depending on the site of injury, the most frequent procedures performed in the early post-injury period in triage-positive patients include osteosynthesis of fractures of the extremities, the pelvis, craniotomy and laparotomy. Staged surgical management (damage-control surgery and orthopaedics) is a treatment method of choice in patients with prolonged hypotension, metabolic acidosis, hypothermia and coagulopathy.

PURPOSE OF THE STUDY: Multiple rib fractures with segmental chest wall instability are caused by high-energy chest trauma and are associated with significant morbidity and mortality. Flail chest injuries are mostly combined with lung injury (contusion, rupture, laceration) and subsequent pneumothorax or haemothorax. Early mechanical ventilation with internal pneumatic splinting is a conservative treatment for flail chest in patients with respiratory insufficiency. The surgical stabilisation of a flail chest is an effective method of treatment and is beneficial for selected patients. It shortens the duration of mechanical ventilation and thus reduces morbidity associated with prolonged ventilatory support. In addition, it decreases long-term pain and the inability of a flail chest to heal due to malunion, non-union or progressive collapse of the flail segment. Surgical stabilisation of a flail chest is indicated when the clinical examination shows progressive respiratory dysfunction confirmed by the results of multiple detector computer tomography (MDCT) of the thorax. MATERIAL AND METHODS: Thirty-three consecutive patients who underwent surgical stabilisation of a flail chest at the Trauma Centre between 2010 and 2014 were retrospectively evaluated. This included patient demographics, chest injury extent, results of pre-operative chest imaging (MDCT), surgical stabilisation technique and post-operative outcome. In addition to providing a radiographic finding of respiratory failure, the result of MDCT chest examination was considered an important criterion for surgical intervention. Surgical stabilisation of the chest wall was performed at an interval ranging from 2 hours to 11 days after injury. Intra-thoracic procedures were indicated in patients with lung injury (pulmonary laceration). The surgical procedure was completed by chest tube placement. RESULTS: Surgical stabilisation was carried out using 3 to 8 plates for flail segment fixation involving 3 to 4 ribs. The duration of post- operative mechanical ventilation was 5 days on the average. It was longer in patients with associated injuries such as craniocerebral trauma or severe pulmonary contusion. Tracheostomy was performed in seven patients requiring prolonged mechanical ventilation. Two patients had superficial surgical site infection. No death was recorded in the follow-up period. CONCLUSIONS: Surgical stabilisation of the flail chest segment is considered an effective procedure in selected patients, leading to improvement of respiratory function. By allowing for a shorter period of time on mechanical ventilation, it reduces the occurrence of complications due to ventilatory support. The result of MDCT chest examination in patients with fail chest is an important indication criterion for surgical fixation.

Úvod: Tupé poranění hrudníku je spojeno s vysokým rizikem morbidity a letality. Komplikace u poranění hrudníku se vyvinou druhotně u zlomenin žeber jako následek v podobě signifikantní bolesti a ventilační poruchy. V léčbě blokové zlomeniny žeber je doporučen multidisciplinární postup zahrnující konzervativní léčbu (fyzioterapii hrudníku, analgezii, mechanickou ventilaci) a chirurgickou intervenci (stabilizaci hrudní stěny použitím osteosyntézy žeber). Časná mechanická ventilace s vnitřní pneumatickou dlahou je základem konzervativní léčby u blokové zlomeniny žeber nemocných s respirační insuficiencí. Chirurgická stabilizace blokové zlomeniny žeber je recentně přijata jako validní metoda léčby. Zkracuje dobu mechanické ventilace, snižuje dlouhodobou bolest, a tak snižuje výskyt morbidity spojenou s prodloužením ventilační podpory. Kromě toho se snižuje neschopnost blokové zlomeniny žeber se zahojit pro poruchu hojení zlomeniny, vytvořeného pakloubu nebo progresivního kolapsu uvolněného segmentu hrudní stěny. Zlomenina žeber je stabilizována použitím zevních dlah (Judetovy dlahy, anatomické dlahy) nebo dlahami zavedenými do dřeňové dutiny. Materiál a metody: Dvacet devět po sobě jdoucích zraněných nemocných, kteří postoupili chirurgickou stabilizaci u blokové zlomeniny žeber v Traumatologickém centru FNKV v letech 2010-2014 byli retrospektivně posouzeni. Zahrnovalo to dermografická data nemocných, rozsah poranění hrudníku, techniku chirurgické stabilizace pomocí Judetových dlah a pooperační období. Chirurgická stabilizace hrudní stěny byla provedena v intervalu od jedné hodiny do devíti dnů po úraze. Chirurgický výkon byl doplněn o drenáž hrudníku. Na základě zkušeností s osteosyntézou žeber Judetovými dlahami byla provedena inovace žeberních dlah ve spolupráci s vývojovým oddělením firmy Medin a.s. Cílem inovace byly změny v technických parametrech , které jsou srovnatelné s novými dlahami dalších výrobců. Výsledky: Chirurgická stabilizace byla provedena po- užitím tří až osmi dlahami pro uvolněný segment hrudní stěny fixací tří až čtyř žeber. Doba pooperační mechanické ventilace byla v průměru pět dní. Delší doba byla (až 16 dní ) u sdružených poranění, jako bylo kraniocerebrální poranění, rozsáhlá kontuze plíce, komplikovaná zlomenina pánve a oboustranná bloková zlomenina žeber. Tracheostomie byla provedena u šesti pacientů vyžadujících prolongovanou mechanickou ventilaci. U dvou nemocných vznikla povrchní infekce chirurgického místa. Nebylo zaznamenáno úmrtí po dobu sledovaného období. Inovované dlahy byly vyzkoušeny na laboratorním modelu žeber a u kadáveru s použitím nového instrumentária. Nové technické parametry u dlahy zahrnující hmotnost dlahy, nové fixační svorky, uzamykatelné šrouby a instrumentarium jsou prioritami inovovaných dlah. Závěry: Chirurgická stabilizace segmentu hrudní stěny u blokové zlomeniny žeber je považována za účinný výkon u vybraných nemocných, vedoucí k zlepšení respirační funkce a zkrácení doby ventilační podpory. Inovovaná Judetova dlaha je důležitým krokem k rozšíření chirurgické léčby u blokové zlomeniny žeber.
Introduction: Blunt chest trauma is associated with high risk of morbidity and mortality. Complications of chest trauma develop secondary to rib fractures as a consequence of significant pain and inadequate ventilation. In the therapy of flail chest is recommended a multi-disciplinary approach including conservative treatment (chest physiotherapy, analgesia, mechanical ventilation) and surgical intervention (stabilisation of the chest wall to use rib osteosynthesis). Early mechanical ventilation with internal pneumatic splinting is the basis of conservative treatment for flail chest in patients with respiratory insufficiency. The surgical stabilisation of a flail chest is recently accepted as valid method of treatment. It shortens the duration of mechanical ventilation, decreses long – term pain and thus reduces morbidity associated with prolonged ventilatory support. In addition, it decreases inability of a flail chest to heal due to malunion, non-union or progressive collapse of the flail segment. Rib fractures have been stabilized with external plates (Judet plates, anatomical plates) or intramedullary rib splints. Materials and methods: Twenty nine consecutive injuried patients who underwent surgical stabilisation of flail chest at the Trauma Centre Faculty Hospital Královské Vinohrady between 2010-2014 were retrospectively evaluated. This included patient dermographics, chest injury extent, surgical stabilisation technics with Judet plates, and postoperative outcome. Surgical stabilisation of the chest wall was performed a interval from 1 hour to 9 days after injury. The surgical procedure was completed by chest tube placement. On the basis of the experience with rib osteosynthesis with Judet plates was performed innovation of rib plates in cooperation of developing department in Medin company. The goal of innovation were changes of technical parameters, which are compared with new plates of others companies. Results: Surgical stabilisation was carried out using 3 to 8 plates for flail segment fixation ivolving 3 to 4 ribs. The duration of post-operative mechanical ventilation was 5 days on the average. It was longer (till 16 days) with associated injuries such as craniocereberal trauma, large pulmonary contusion, complicated pelvis fractures and bilateral flail chest. Tracheostomy was performed in 6 patients requiring prolonged mechanical ventilation. Two patients had superficial surgical site infection. No death was recorded in the follow-up period. Innovating plates were testing in laboratory model of the ribs and in cadaver with using of new instruments. New technics parameters including mass of plate, new fixating clips, locked screws and instruments are priorities of inno-vating plates. Conclusions: Surgical stabilisation of the flail chest segment is considered an effective procedure in selected patients, leadi ng to improvement of respiratory function and shortens duration of ventilatory support. Innovating Judet plates are important step in extending of surgical treatment of the flail chest.

Východisko. Standardizace v postupu ošetření polytraumat, sdružených poranění nebo i komplikovaných monotraumat vedla ke zlepšení péče s poklesem poúrazové morbidity a letality. Zavedení algoritmu „definitivní chirurgická péče o trauma“ včetně koncepce „damage control“ chirurgie má zásadní přínos u polytraumatizovaných zraněných. Toho lze dosáhnou pouze v úrazových centrech s implementovaným systémem péče o závažná poranění. V retrospektivní studii byla provedena analýza zraněných se závažným traumatem ošetřených v traumatologickém centru Fakultní nemocnice Královské Vinohrady (FNKV). Metody a výsledky. Byl vyhodnocen soubor 515 zraněných přijatých na urgentní příjem KAR FNKV v letech 2009–2010. Z celkového počtu bylo 385 mužů a 130 žen. Průměrný věk byl 45 let. Z místa nehody bylo primárně přivezeno do TC 428 (83 %) zraněných. S polytraumatem bylo přijato 248 (48 %) zraněných, 158 (31 %) s kraniocerebrálním poraněním, 76 (15 %) se sdruženým poraněním a 33 (6 %) s monotraumatem. U 309 zraněných (60 %) bylo ISS > 16, průměr ISS byl 26,5. Péče o zraněné je na příjmové místě zajištěna základním týmem odborníků (chirurg- viscerální traumatolog, intenzivista, ortopéd-končetinový traumatolog, anesteziolog, radiodiagnostik). U zraněných bylo provedeno celkem 412 výkonů včetně radiointervenčních přístupů. Urgentních operací bylo 313 (76 %), odložených 99 (24 %). Z neodkladných operací byly nejčastěji výkony pro poranění pohybového ústrojí (36 %) a neurotraumatologické intervence (29 %). Operační revize poranění hrudní a břišní dutiny byla provedena u 24 % nemocných. Z odložených operací převažovaly výkony pro poranění pohybového ústrojí (66 %) a operace pro maxilofaciální poranění (15 %). Zemřelo 95 (18 %) zraněných s nejvyšší letalitou ve skupině s ISS > 40 (65 %). Závěry. Ustanovení trauma systému péče o závažná poranění znamená zvýšení primárních příjmů do traumatologického centra. Nejčastějším ze závažných poranění ošetřených na urgentním příjmu je polytrauma a kraniocerebrální poranění. Zavedení standardního postupu interdisciplinární spolupráce při fázovém ošetření traumat vede k optimalizaci v jejich péči. Včasná indikace a provedení urgentní operace s kontrolou zdroje krvácení a zamezení pokračující kontaminace u zraněných s polytraumatem je zásadním léčebným opatřením. Definitivní ošetření včetně reoperace při fázovém postupu je indikováno s časovým odstupem po stabilizaci zraněného. ISS < 40 je spojeno s rizikem letality u 65 % zraněných.
Background. The implementation of standardized protocols for management of polytrauma, multiple injuries and complicated monotrauma has led to improvement in trauma care with decrease in posttraumatic morbidity and mortality. The introduction of algorithm „Definitive Surgical Trauma Care” (DSTC) with concept of „damage control„ surgery assumed the principal role in care of polytrauma patients. It can be accomplished only in the trauma- centre with implementation of care system for severely injured patients. Analysis of severely injured patients treated in Trauma Centre Faculty Hospital Královské Vinohrady was performed in a retrospective study. Methods and results. 515 injured patients admitted to emergency unit of the Department of anaesthesiology and resuscitation in years 2009–2010 were evaluated. There were 385 men and 130 women. Average age of patients was 45 years. Primary trauma admissions were 482 (83%) victims. There were 248 (48%) polytrauma patients, 158 (31%) with craniocerebral injuries, 76 (15%) with associated injuries and 33 (6 %) injured with monotrauma. ISS > 16 were in 309 injured patients, average rate ISS was 26.5. Trauma care in emergency unit was provided with the team of specialists (general surgeon for visceral traumatology, orthopaedic surgeon for extremities trauma, specialist for intensive care, anaesthesiology and diagnostic radiology). 412 surgical or radiointervention procedures were performed. There were 313 (76%) urgent surgeries and 99 delayed (24%). The most frequent urgent surgery was for fractures of extremities, (36%) and neurotraumas (29%). Acute operations for thoracic and abdominal trauma were performed in 24% of patients. Delayed surgeries were mostly operations of extremities fractures (66%) and maxillofacial injuries (15%). Mortality was 18% (95 patients); being highest in group of patients with Injury Severity Score (ISS) > 40 (65%). Conclusions. The implementation of trauma care system in severely injured patients brings increased numbers of primary trauma admissions to trauma centres. Polytraumas and craniocerebral injuries were the most frequently admitted patients to emergency unit. Implementation of standardized protocols with interdisciplinary cooperation in trauma management represents improvements of trauma care. Early indication and performance of urgent operations with control of bleeding and prevention of subsequent contamination belongs to principal steps in the management of severely injured patients. Definitive management with reoperation in polytrauma patients can be indicated after the stabilisation. ISS > 40 was connected with high mortality rate (65%).
