Chronická lymfocytární leukemie (CLL) patří v mnoha západních zemích mezi nejčastěji se vyskytující maligní lymfoproliferativní onemocnění starších dospělých. Prognóza tohoto heterogenního onemocnění je založena na řadě klinických a biologických markerů. Významným prognostickým markerem CLL je také detekce poškození genu TP53. Práce přináší výsledky retrospektivní analýzy výskytu změn v genu TP53 u souboru 72 pacientů s CLL. Pomocí přímého sekvenování a analýzy křivek tání s vysokým rozlišením (hrMCA) byly změny genu TP53 detekovány u 46 (64 %) vyšetřených pacientů. Přímým sekvenováním exonů 2-10 kódujícící oblasti genu TP53 bylo u 45 (62 %) pacientů nalezeno 59 změn. Po vyloučení polymorfismů bylo mezi těmito změnami u 20 (28 %) pacientů identifikováno 23 mutací. Jednalo se o substituce (83 % mutací) a delece (17 % mutací). Funkčním důsledkem substitucí byly záměnové (90 %), nesmyslné (5 %) a tiché mutace (5 %). Mutace byly z 91 % lokalizovány v DNA-vazebné doméně (exony 5-8) genu TP53. 87 % mutací bylo detekováno u pacientů s rizikovým onemocněním. Byl prokázán statisticky významný vztah mezi výskytem mutací v genu TP53 a delecí 17p (P < 0,000001). Byla také pozorovaná korelace mezi přítomností mutací v genu TP53 a stadiem onemocnění podle Bineta (P = 0,027). Pomocí hrMCA analýzy DNA-vazebné oblasti genu TP53 (exony 5-8) bylo u 26 (36 %) pacientů detekováno 32 nespecifických PCR produktů. Ve srovnání s přímým sekvenováním je tato metodika citlivější a také méně časově a laboratorně náročná. Problematická standardizace hrMCA ovšem prozatím znemožňuje její rutinní využití při detekci změn genu TP53 u pacientů s CLL.
Chronic lymphocytic leukemia (CLL) is the most common form of adult leukemia in the Western countries. CLL is a heterogeneous disease characterized by a number of clinical and biological prognostic markers, one of the most important being mutations of the TP53 gene. To study the TP53 gene, we retrospectively investigated 72 patients with CLL using direct sequencing and high-resolution melting curve analysis (hrMCA). We found modifications of the TP53 gene in 46 (64%) patients. We detected 59 modifications of the TP53 gene (exons 2-10) in 45 (62%) patients by direct sequencing. Among these modifications, we identified 23 mutations in 20 (28%) patients. These were substitutions (83% of the mutations) and deletions (17% of the mutations). The substitutions comprised missense (90%), nonsense (5%) and silent mutations (5%). Ninety one percent of the mutations were located in the DNA-binding domain (exons 5-8) of the TP53 gene. We detected 87% of the mutations in patients with high-risk disease. We showed a statistical relationship between mutations in the TP53 gene and chromosome 17p deletion (P < 0.000001). We also observed correlation between mutations in the TP53 gene and Binet stage of CLL (P = 0.027). We found 32 aberrant PCR products of the TP53 gene (exons 5-8) in 26 (36%) patients by hrMCA. The method is more sensitive and faster than direct sequencing. However, our experience suggests that due to problems with standardization, hrMCA is not suitable for routine detection of modifications of the gene TP53 in patients with CLL.

Cryptococcus neoformans was grown in 96-well microtiter plates sealed by foil which is less than 0.01 % permeable to oxygen. On day 14 of the cultivation, we observed peculiar clusters of small droplike daughter cells arranged around ?4 % of mother cells. The fact that most of the other cells had died indicates that few cells had been able to survive hypoxic conditions and escape the cell-cycle arrest. However, their daughters were unable to separate from them and to continue their proliferation under such conditions.
Chronická B-lymfocytární leukemie (B-CLL) představuje maligní onemocnění s velmi variabilním klinickým průběhem. Pro správnou a včasnou léčbu především rizikových pacientů je nezbytné toto onemocnění co nejpřesněji prognosticky charakterizovat. Podle recentních literárních údajů se zdá, že na prognózu může mít vliv i míra exprese genu hTERT, který kóduje katalytickou podjednotku reverzní telomerázy. V naší studii byla vyšetřena exprese genu hTERT u 47 pacientů s B-CLL. RNA byla získána z mononukleární frakce periferní krve z doby diagnózy. Kvantita transkriptů hTERT byla vztažena k expresi genu ABL. Exprese hTERT byla srovnána s mutačním stavem variabilních částí těžkých řetězců imunoglobulinů (IgVH). Medián exprese hTERT u pacientů s nemutovanými řetězci IgVH byl čtyřnásobný ve srovnání s mediánem exprese hTERT ve skupině s mutovanými řetězci IgVH. Vysoká exprese hTERT statisticky signifikantně korelovala s nemutovaným stavem IgVH (p < 0,00001). Celková shoda mezi mutačním stavem IgVH a expresí hTERT byla 87 %. Naše výsledky naznačují, že exprese hTERT vykazuje vysokou konkordanci s mutačním stavem řetězců IgVH. Pochopení role hTERT v patogenezi a progresi B-CLL by mohlo přispět k lepší prognostické stratifikaci nemocných s B-CLL.
B-cell chronic lymphocytic leukemia (B-CLL) is clinically heterogeneous disease. Therefore it is very important to identify all reliable molecular and cellular markers to predict the tendency for disease progression. One of the promising prognostic markers seems to be expression of hTERT gene, which encodes catalytic subunit of human telomerase. We performed real-time reverse transcription PCR to quantitate the amount of hTERT transcripts in mononuclear blood cells from 47 B-CLL patients. We used ABL gene as a housekeeping gene.We compared the expression of hTERT to the IgVH mutational status. Median of hTERT expression was fourfold higher in the IgVH unmutated group than in the IgVH mutated group (p<0.00001). The level of hTERT expression discriminated the IgVH unmutated from IgVH mutated B-CLL in 87% cases. Our results suggests that hTERT expression strongly correlates with IgVH mutational status. Full understanding of the hTERT role in pathogenesis and progression of B-CLL could contribute to better prognostic stratification of patients with B-CLL.
