Východiska: Nazofaryngeální tumory jsou v našich zeměpisných šířkách vzácným onemocněním. Kvůli anatomickým poměrům a špatné operabilitě je jako primární léčebná modalita využívána radikální radioterapie s chemoterapií nebo bez ní. Vzhledem k očekávanému dlouhodobému přežití jsou hledány cesty ke zvýšení lokální kontroly za únosné toxicity léčby. Soubor pacientů a metody: Na Klinice onkologické Fakultní nemocnice Ostrava bylo v letech 2010–2014 léčeno 14 pacientů s pokročilým nemetastatickým spinocelulárním karcinomem nazofaryngu stadia II–IVa. Jedenáct (79 %) pacientů bylo léčeno dávkou 70 Gy na primární tumor a postižené lymfatické uzliny normofrakcionovaně (à 2 Gy) a 56 Gy (à 1,6 Gy) na elektivní lymfatické uzliny technikou radioterapie s modulovanou intenzitou s integrovaným konkomitantním boostem. Tři (21 %) pacienti z důvodů komplikací nebo celkového stavu ukončili radioterapii dříve. Devět (64 %) pacientů bylo ozářeno s konkomitantní chemoterapií a 6 (43 %) pacientů dále podstoupilo adjuvantní chemoterapii. Čtrnáct (100 %) pacientů do měsíce od radikální radioterapie absolvovalo cyberboost na oblast primárního tumoru a metastatických lymfatických uzlin, 5 (36 %) nemocných 5 Gy v 1 frakci a 9 (64 %) nemocných 10 Gy ve 2 frakcích. Výsledky: Z 14 pacientů dosáhlo lokální kompletní remise po léčbě 10 (71 %) pacientů. Dva (14 %) pacienti měli poléčebný lokální nález zhodnocen jako stacionární, u 1 byla konstatována lokální progrese s generalizací za 18 měsíců a u druhého pacienta za 13 měsíců. Jeden pacient s poradiační kompletní remisí zgeneralizoval za 43 měsíců při lokální kompletní remisi onemocnění. Průměr 24,6 měsíce do progrese. Pětiletého přežití dosáhlo 9 (64 %) pacientů za akceptovatelné akutní i pozdní toxicity. Závěr: Stereotaktický boost po absolvování komplexního radiačního programu, s přidáním konkomitantní a adjuvantní chemoterapie nebo bez ní, přináší dobrou lokální kontrolu a 5leté přežití za uspokojivé toxicity léčby.
Background: Nasopharyngeal tumors are a rare disease in our latitudes. Due to anatomical conditions and poor operability, radical radiotherapy with or without chemotherapy is used as a primary treatment modality. Due to the unsatisfactory treatment outcomes, ways to intensify the therapy and increase the curable potential under tolerable toxicity are sought. Patients and methods: Fourteen patients with advanced nasopharyngeal non-metastatic squamous cell carcinoma, stage II–IVa, were treated at the Department of Oncology of the University Hospital Ostrava in 2010–2014. Eleven (79%) patients were treated with 70 Gy per primary tumor normalized (2 Gy) and affected lymph nodes and with 56 Gy (à 1,6 Gy) for elective lymph nodes by intensity modulated radiotherapy with integrated concomitant boost. Three (21%) patients completed radiotherapy earlier due to complications or general condition. Nine (64%) patients were exposed to concomitant chemotherapy and 6 (43%) received adjuvant chemotherapy. Fourteen (100%) patients underwent cyberboost in the primary tumor and affected lymph nodes (if any) area within a month of radical radiotherapy, five (36%) patients received 5 Gy in one fraction and nine (64%) patients received 10 Gy in 2 fractions. Results: Out of the 14 patients, local complete remission was achieved in 10 (71%) of them after the treatment. Two (14%) patients had a post-treatment local finding as stationary, local progression with generalization occurred in one case at 18 months and in another patient at 13 months. In one patient with complete remission after the primary treatment, distant metastasis occurred in 43 months at the time of local complete remission of the disease. The average time to progression was 24.6 months. Five (64%) patients achieved 5-year survival with satisfactory toxicity of treatment. Conclusion: Stereotactic boost after a comprehensive radiation program, with or without addition of concomitant and adjuvant chemotherapy, provides good local control and 5-year survival with satisfying toxicity of the treatment.

BACKGROUND: To evaluate the safety, feasibility and outcomes of patients treated for colorectal liver metastases (CLM) with an innovative combined approach - hepatic resection and Stereotactic body radiotherapy (SBRT) using CyberKnife® system. METHODS: This was a retrospective cohort study conducted in a single institution. Patients with CLM and no evidence of extrahepatic disease were included during a 6-year study period. RESULTS: In total, 19 patients with 63 liver lesions underwent liver resection combined with SBRT of unresectable lesions. Major hepatectomy was performed in 42.1% patients; postoperative complications were noted in 31.6% patients. 27 unresectable lesions were treated by SBRT with a total dose of 50-60 Gy in five fractions. The median follow-up of study patients was 29.7 ± 20.58 months. Local control of CLM at 1 and 2 years was achieved in 89.5% of patients. Out-of-field hepatic recurrence was diagnosed in 63.1% patients. The 1-year disease-free survival (DFS) was 52.6%; 2-year DFS was 31.6%. The overall actuarial survival rates at 1 and 2 years were 88.2% and 50.4%. CONCLUSION: Liver resection combined with SBRT presents a promising therapeutic option for patients with CLM which traditionally are unresectable. The additional use of SBRT allows for the effective clearance of the disease for thoroughly selected patients.

Úvod: Chirurgická resekce je dosud metodou volby při léčbě jaterních malignit. Především u pacientů, kteří jsou z nejrůznějších důvodů nevhodní pro radikální chirurgickou léčbu, představuje další léčebnou možnost radioterapeutický systém Cyberknife®. Cílem této práce je porovnat krátkodobé i dlouhodobé výsledky těchto léčebných modalit. Metody: Byla provedena retrospektivní analýza prospektivně shromažďovaných dat u pacientů podstupujících léčbu jaterních malignit chirurgicky či systémem Cyberknife®. Zkoumané období zahrnuje roky 2013 až 2016. Do studie byli zařazeni pacienti podstupující léčbu pouze jednou z uvedených metod. Výsledky: Celkem bylo analyzováno 260 pacientů, přičemž u 142 byla zvolena chirurgická terapie, zbylých 118 pacientů podstoupilo radioterapii. Medián přežití byl 30,65 měsíce pro chirurgické pacienty a 22,93 měsíce pro Cyberknife®, celkový medián přežití byl 27,63 měsíce. Procento tříletého přežívání bylo 47,4 %, respektive 19,9 %. Kaplan-Meierova analýza ukázala statisticky nevýznamný rozdíl mezi přežíváním pacientů obou skupin (p=0,082, CI 95%). Při porovnání výsledků omezených pouze na metastázy kolorektálního karcinomu byl shledán statisticky významný rozdíl (p=0,031, CI 95%). Závěr: Výsledky této studie ukazují statisticky nerozdílné dlouhodobé přežívání pacientů léčených oběma léčebnými metodami. Významný rozdíl ve tříletém přežívání však stále ukazuje na dominantní postavení chirurgie v diagnosticko-terapeutickém managementu pacientů s malignitami jater. Radioterapie Cyberknife® může představovat alternativní léčebnou metodu především u pacientů neschopných chirurgického výkonu, avšak pro kvalitní srovnání dlouhodobých výsledků je třeba delšího sledování pacientů.
Introduction: Surgical resection is the method of choice in treating liver malignancies. In patients who are not suitable for radical surgical treatment, the radiotherapeutic system Cyberknife® is a viable treatment option. The aim of this study is to compare short- and long-term results of both treatment methods. Methods: A retrospective analysis of prospectively collected data was performed, focused on patients undergoing treatment of liver malignancies either by surgical resection or by the Cyberknife® system from 2013 to 2016. Only patients treated using a single treatment method were included in the study. Results: A total of 260 patients were analysed; 142 were treated by performing surgical resection and the remaining 118 using Cyberknife® radiotherapy. Median survival was 30.65 months for the surgical resection and 22.93 for the Cyberknife® therapy; median overall survival was 27.63 months. Three-year cumulative survival was 47.4% for the resection and 19.9% for radiotherapy. Kaplan-Meier analysis did not demonstrate a statistically significant difference in disease-specific survival between both groups (p=0.082, CI 95%). Results limited only to colorectal liver metastases showed a statistically significant difference in disease-specific survival (p=0.031, CI 95%). Conclusions: Results of this study show statistically indifferent overall disease-specific survival of both groups. However, the significant difference in 3-year survival still indicates a predominant position of surgery in the diagnostic and therapeutic management of patients with liver malignancies. Nevertheless, Cyberknife® radiotherapy may actually represent a viable treatment alternative, particularly in patients unable to undergo surgical resection, although a longer follow-up period is necessary to obtain more robust results.

Background: The management of patients with liver metastases presents a challenging problem in clinical oncology. Patients with limited involvement of the liver may be suitable for surgical resection or local ablative techniques. Stereotactic body radiotherapy (SBRT) presents an emerging new technology that has shown high efficacy in ablating tumors at various disease sites. Methods: A comprehensive literature search was performed to identify articles in regard to the SBRT in the treatment of patients with liver metastases. Results: SBRT allows for the delivery of high-dose radiation in few fractions to the tumor with extreme accuracy, while minimizing the damage to normal surrounding tissue. The CyberKnife® system is an image-guided robotic system that delivers SBRT, tracks tumors during respiration, and automatically adjusts treatment for any patient movement. The most frequently used indications for CyberKnife® therapy are ≤5 liver metastases with maximum tumor sizes of 6 cm, no extrahepatic disease, good performance status, and adequate hepatic functions. Local control rates range from 70%-100% at 1 year and from 60%-90% at 2 years. Severe toxicity related to SBRT is uncommon - grade three side effects occur in less than 5% of cases. Despite excellent local control rates, out-of-field metastatic progression (out-of-field hepatic metastases and extrahepatic metastases) develops in a substantial proportion of patients after SBRT. Therefore, it seems essential to improve the selection of patients with liver metastases for SBRT. Conclusion: The CyberKnife® system presents an effective minimally invasive treatment modality for patients with hepatic oligometastases who are not suitable candidates for radical liver resection. The available data suggest that liver metastases can be treated by CyberKnife therapy with very low toxicity and excellent local control rates.

Cíl: Cílem této práce je zhodnocení toxicity a prognostických faktorů celkového přežití (overall survival – OS) při použití hyperfrakcionované akcelerované radioterapie s modulovanou intenzitou u pacientů s pokročilými nádory hlavy a krku nevhodných ke konkomitantní radiochemoterapii. Metodika: Od května 2008 do dubna 2013 bylo léčeno 122 pacientů s lokálně pokročilým nemetastazujícím spinocelulárním karcinomem hrtanu (14 %), hypofaryngu (30 %), orofaryngu (30 %) a dutiny ústní (27 %). Střední věk, KPS (Karnofského performance status) a GTV (gross tumor volume) byly 63 let (46–87 let), 80 % (50–100 %) a 46 ml (5–250 ml). Střední dávka radioterapie byla 72,6 Gy (62–77) ? 1,4–1,5 Gy na oblast GTV (primární tumor a lymfadenopatie) s lemem 0,7 cm a 55 Gy ? 1,1 Gy na regionální lymfatické oblasti s lemem 0,3 cm. Pomocí lineárního urychlovače 6 MeV byla doručena léčebná dávka 2× denně s přestávkami 6–8 hod. OS bylo zhodnoceno Kaplan-Meierovou metodou. Prediktory OS byly analyzovány pomocí Coxova modelu proporcionálních rizik. Výsledky: Průměrná doba radioterapie trvala 37 dní (32–45 dní). Výskyt akutní toxicity 3. stupně byl 62 % na sliznicích (dutina ústní, hltan) a 0 % na kůži. Konfluentní zánět sliznice se zotavil ve všech případech do 21 dní. Stupeň toxicity 4 nebo 5 nebyl zaznamenán. U pacientů v kompletní remisi není žádná závislost na PEG (perkutánní endoskopické gastrostomii). Jednoleté a 2leté OS bylo 65, resp. 32 %. Multivariantní analýzou byly zjištěny negativní faktory OS – KPS horší než 80 % (RR 2,4, 95% CI 1,3–4,2; p = 0,004), jiný karcinom než orofaryngu nebo hrtanu (RR 2,0, 95% CI 1,1–3,5; p = 0,016) a velikost GTV (RR 1,006, 95% CI 1,001–1,011; p = 0,018). Dva roky po léčbě přežívá přibližně 30 % pacientů a výkonnostní stav před zahájením léčby je nejsilnějším faktorem pro OS. Další prediktory OS je potřeba hledat na molekulární úrovni.
Aim: The aim of this study was to evaluate overall survival (OS) and prognostic factors in patients ineligible for chemotherapy who were treated with a hyperfractionated accelerated schedule with simultaneous integrated boost. Material and Methods: From May, 2008, to April, 2013, 122 patients with locally advanced nonmetastatic squamous laryngeal (14%), hypopharyngeal (30%), oropharyngeal (30%), and oral cavity (27%) cancer were treated at our institution. The median age, Karnofsky Performance Status (KPS), and gross tumor volume (GTV) of the patients were 63 years (range, 46–87 years), 80% (range, 50–100%), and 46 ml (range, 5–250 ml), resp. The median total dose of radiotherapy was 72.6 Gy (range, 62–77 Gy) at 1.4–1.5 Gy per fraction, and 55 Gy at 1.1 Gy per fraction was delivered for GTV (primary and lymphadenopathy) with a margin of 0.7 cm and regional lymphatic areas with a margin of 0.3 cm. The dose was delivered 2× a day, with a 6–8 hour interval between doses, via a 6 MeV linear accelerator. OS was estimated using the Kaplan-Meier method, and predictors of OS were analyzed using Cox proportional hazards regression. Results: The median duration of the radiotherapy series was 37 days (range, 32–45 days). The incidence of grade 3 acute toxicity was 62% for mucosa (oral cavity and/or pharynx) and 0% for skin. Confluent mucositis cleared in all cases within 21 days. No grade 4 or 5 toxicities were recorded. PEG was introduced before treatment in 55 patients (45%). The 1-and 2-year OS was 65% and 32%, resp. KPS less than 80% (RR 2.4, 95% CI 1.3–4.2; p = 0.004), cancers other than oropharyngeal or laryngeal cancer (RR 2.0, 95% CI 1.1–3.5; p = 0.016), and capacity of high GTV (RR 1.006, 95% CI 1.001–1.011; p = 0.017) were found to be negative prognostic factors for OS. Conclusion: More than 30% of patients with poor prognosis survived for longer than 2 years. KPS before treatment was the strongest prognostic factor for better OS.
Východiska: Karcinom prostaty je nejčastějším nádorovým onemocněním mužské populace, jehož incidence nadále stoupá. Velká část onemocnění je diagnostikována v časném bezpříznakovém období, kdy má velice dobrou prognózu. Při rozhodnutí o léčbě pro konkrétního pacienta hraje roli účinnost jednotlivých terapeutických modalit a jejich toxicita. Jednou z metod léčby časných stadií karcinomu prostaty je stereotaktická radioterapie, jejíž první výsledky z našeho pracoviště článek prezentuje. Soubor pacientů a metody: Soubor zahrnuje 261 pacientů s diagnózou nízce nebo středně rizikového karcinomu prostaty léčených na našem pracovišti stereotaktickou radioterapií v období od srpna 2010 do července 2012. Pacienti absolvovali SBRT (stereotactic body radiotherapy) v dávce 36,25 Gy v pěti frakcích po 7,25 Gy obden. Toxicita léčby byla hodnocena dle škály RTOG. Pro hodnocení kvality života pacienti vyplňovali modifikovaný dotazník EPIC (Expanded Prostate Index Composite). Výsledky: Celkové přežití pacientů po léčbě dosahuje 93,1 %. Vysoká je i hodnota bRFS (biochemical relapse free survival) – 97,7 % pro celý soubor našich pacientů, očekávaně horší výsledky jsou ve skupině pacientů se středním rizikem rekurence. Akutní i chronická urologická a gastrointestinální toxicita léčby dle RTOG je nízká. Kvalita života pacientů po léčbě dle dotazníku EPIC byla bezprostředně po léčbě mírně snížena, s delším časovým odstupem se vrátila minimálně ke vstupním hodnotám. Závěr: Stereotaktická radioterapie je účinnou léčebnou modalitou časného karcinomu prostaty s akceptovatelnou časnou a minimální pozdní toxicitou.
Background: Prostate cancer is the most prevalent cancer in males and its incidence is steadily increasing. Most cases of prostate cancer are diagnosed during the early asymptomatic period, in which case the prognosis is very good. Therapies differ widely in their efficacies and toxicities, and this is an important consideration when it comes to deciding which treatment is optimal for a particular patient. One treatment method for early stage prostate cancer is stereotactic body radiotherapy (SBRT). We present the first results obtained using this modality at our institution. Patients and Methods: A total of 261 patients with low or intermediate risk prostate cancer were treated with SBRT between August 2010 and July 2012. Patients received a total dose of 36.25 Gy in five fractions of 7.25 Gy every other day. The toxicity of the treatment was evaluated according to RTOG criteria. For assessment of quality of life, patients filled out a modified EPIC questionnaire (Expanded Prostate Composite index). Results: Overall survival (OS) in this study was 93.1%. Biochemical relapse free survival (bRFS) was 97.7%. As expected, OS and bRFS were worse in the group of patients with an intermediate risk of recurrence. Acute and chronic urinary and gastrointestinal RTOG toxicity was very low. Quality of life after treatment, as determined using the EPIC questionnaire, was slightly reduced immediately after treatment but returned to baseline or even improved during long term follow-up. Conclusion: SBRT is an effective therapeutic modality for early prostate cancer and has acceptable rates of acute and low late toxicity.
Karcinom prostaty je nejčastějším nádorem urogenitálního systému. Díky včasné diagnostice se řadí mezi onemocnění s velmi vysokým procentem vyléčení, u nichž se klade důraz na kvalitu života pacienta. Mezi preferované možnosti léčby časného stadia onemocnění patří radikální prostatektomie, externí radioterapie, intersticiální nebo permanentní brachyterapie a zejména u starších pacientů je možné zvolit přístup aktivního sledování. Cílem práce bylo zjistit, zda stereotaktická radioterapie negativně neovlivňuje kvalitu života pacienta. K hodnocení kvality života byl použit standardizovaný dotazník EPIC (Expanded Prostate Index Composite).
Prostate cancer is the most common tumor of the urogenital system. With early diagnosis it is a disease with a very high percentage of successful cure rate with the emphasis on the quality of life of the patient. Preferred treatment options in early stages of this disease include radical prostatectomy, external radiotherapy, interstitial or permanent brachytherapy and especially in case of elderly patients, it is possible to choose an active surveillance approach. The aim of the thesis was to determine whether stereotactic radiotherapy negatively impact patient’s quality of life. A standardized questionnaire EPIC (Expanded Prostate Index Composite) was used to assess the quality of life.
