Současné diagnostické metody umožňují zachycení malých nádorů ledvin a řada tumorů je zjištěna náhodně při vyšetření z jiné indikace. To zvyšuje počet nízkých stadií nádorů a otevírá prostor pro méně invazivní léčebné techniky, pokud není indikováno aktivní sledování. Mikrovlnná ablace (MWA) je jednou z metod termální ablace, která se v současnosti dostává do popředí zájmu, především pro svou miniinvazivitu, komfort pro pacienta i intervenčního radiologa a dobré výsledky. Cílem tohoto sdělení je shrnutí současných poznatků o MWA, která je jednou z termoablačních metod používaných v terapii malých renálních nádorů. Doporučení uvedená v tomto článku vycházejí z aktuálních guidelines mezinárodních urologických společností a našich zkušeností s touto metodou. Předkládán je souhrn indikačních kritérií a doporučení, která ložiska jsou vhodná a naopak nevhodná pro ošetření MWA a jakým způsobem sledovat pacienty po výkonu.
Current diagnostic methods are capable of detection small kidney tumors, and many of them are found incidentally during imaging for a different indication. This increase the number of low-stage tumors opens the door to less invasive treatment techniques in situations, when active surveillance is not indicated. Microwave ablation (MWA) is one of thermal ablation methods, which is coming to the fore recently. Its advantage is minimal invasiveness, comfort for the patient and the interventional radiologist, and good results. The aim of this article is to summarize current knowledge about MWA in the therapy of small renal tumors. The recommendations given in this article are based on the current guidelines of international urological societies and our own experience with this method. A summary of the indication criteria, recommendations which tumors are suitable and unsuitable for treatment with MWA and how to monitor patients after the procedure are presented.
V tomto článku autoři přibližují problematiku endometriózy močových cest jako diagnostické jednotky, s níž se urolog ve své praxi může setkat. Jedná se o formu hluboké infiltrující endometriózy a označuje přítomnost ektopické endome‐ triální tkáně invadující do močového měchýře, ureteru, ledvin a uretry. Symptomatologie je vyjádřena v 50–70 % případů a zahrnuje cyklickou hematurii, recidivující dysurie, polakisurii, cystalgie či nefralgie. Terapie je watchful waiting, hormonální či chirurgická. Zvolení nejvhodnějšího přístupu závisí na preferencích pacientky a jejím věku, symptomatologii, anatomických poměrech a možnostech operatéra. Navíc autoři uvádějí dvě nedávné kazuistiky pacientek s endometriózou močového měchýře a ureteru, které podstoupily vyšetření a operativní řešení na daném pracovišti.
In this article, the authors approach the issue of endometriosis of the urinary tract as a diagnostic unit that a urologist may encounter in his practice. It is a form of deep infiltrating endometriosis and refers to the presence of ectopic endometrial tissue invading the bladder, ureter, kidney and urethra. Symptomatology is expressed in 50-70% of cases and includes cyclic hematuria, recurrent dysuria, pollakisuria, cystalgia or nephrolgia. Therapy management is watchful waiting, hormonal or surgical. The choice of the appropriate approach depends on the patient's preferences and on her age, symptomatology, anatomical conditions and the capabilities of the surgeon. Furthermore, the authors present two recent case reports of patients with endometriosis of the bladder and ureter who underwent examination and operative treatment at the given department.
Po dobu téměř celého století se jako zlatý standard operací pro benigní hyperplazii prostaty (BHP) etablovaly metody omezené velikostí prostaty. Pro žlázy nad 80 g otevřená prostatektomie (OP), pro menší transuretrální prostatektomie (TURP). Přes nesporný efekt přinášejí vysokou míru morbidity. Technologický rozvoj 90. let 20. století umožnil nové operační postupy, z nichž se jako metoda nezávislá na velikosti prostaty ustavuje enukleace prostaty holmiovým laserem (HoLEP). Snažíme se podat přehled o vývoji metody, alternativních postupech a nových technologických možnostech. Zároveň přinášíme porovnání v různých parametrech s doposud standardními postupy. Stále více literárních pramenů považuje Ho - LEP za efektní, bezpečnou a dlouhodobě účinnou metodu operační léčby BHP. Na základě toho v doporučených postupech postupně nahrazuje TURP a OP jako nová metoda volby. HoLEP je nadále nutno zvažovat jako možnost číslo jedna při plánování operací na prostatě, bez ohledu na její velikost. Lze jej nabídnout i pacientům s vyšším rizikem krvácení, po biopsii i po předchozích výkonech na prostatě. Rozvoj technologií slibuje další zrychlení a zvýšení bezpečnosti výkonu, který se stává novým zlatým standardem.
For nearly a century, methods limited by the size of the prostate have been established as the gold standard of surgery for benign prostatic hyperplasia (BPH). For glands over 80g open prostatectomy (OP) and for smaller ones transurethral prostatectomy (TURP). Despite their indisputable effect, they bring a high rate of morbidity. The technological development of the 90s of the 20th century enabled new surgical procedures of which Holmium laser enucleation of the prostate (HoLEP) is established as a size-independent method. We try to provide an overview of the development of the method, alternative procedures and new technological possibilities. At the same time we bring a comparison of various parameters with standard procedures up to now. More and more literary sources consider HoLEP to be an effective, safe and long-term effective method of surgical treatment of BPH. Based on this, it is gradually replacing TURP and OP as the new method of choice in the guidelines. HoLEP should be considered as the number one option when planning prostate surgery regardless of prostate size. It can also be offered to patients with a higher risk of bleeding, after a biopsy or after previous procedures on the prostate. The development of technologies promises further speed-up and better safety profile of the procedure which is becoming the new gold standard in BHP surgery.
Prevalence a incidence malignit v České republice i celosvětově roste. Pokroky v diagnstice a léčebných modalitách aplikovaných v oblasti onkologie během poslední dekády vedly k prodlou- žení celkového přežití pacientů s mnoha různými typy nádorů. V důsledku toho je zachování kvality života další důležitý problém onkologických pacientů. Sexuální problémy u onkologických pacientů mohou pramenit z poškození libida, vzrušivosti, orgasmu a ejakulace, sexuální satisfakce a bolesti. Karcinom prostaty, močového měchýře a kolorektální karcinom představují nejčastější malignity, které jsou diagnostikovány u mužů ve středním věku, kteří na sexualitu po prodělané léčbě nerezignují. Součástí konzultace s pacientem před započetím komplexní léčby by proto mělo být seznámení s rizikem vzniku sexuálních dysfunkcí, modalitami léčby a s možností penilní rehabilitace.
The prevalence and incidence of malignancies in the Czech Republic is increasing. Advances in the diagnosis and treatment modalities applied in the field of oncology during the last decade have led to an increase in the overall survival of patients with many different types of tumors. As a result, maintaining quality of life is another important issue for oncology patients. Sexual problems in oncology patients can stem from impairment of libido, arousal, orgasm and ejaculation, sexual satisfaction and pain. Prostate, bladder and colorectal cancer are the most common malignancies that are diagnosed in middle-aged men who do not renounce their sexuality after undergoing treatment. The consultation with the patient before starting complex treatment should therefore include familiarization with the risk of sexual dysfunctions, treatment modalities and the possibility of penile rehabilitation.
Neoadjuvant chemotherapy (NAC) with radical cystectomy (RC) is the recommended treatment for muscle invasive bladder tumors (MIBC). However, more than half of patients are unable to undergo NAC due to contraindications. The aim of the study is to evaluate the long‐term oncological results from one tertiary center and thus demonstrate the effectiveness of neoadjuvant chemotherapy in muscle‐invasive bladder tumors. Methods: In the years 2010-2021, 100 patients with MIBC underwent neoadjuvant (for cN0) / induction (for cN+) chemotherapy - most often as a combination of cisplatin and gemcitabine. Patients with cT3-4 or cN+ were indicated for NAC; in the subgroup of cT2N0, the patients were selected according to risk factors. Out of a of 100 patients, 49 were without lymphadenopathy (cN0) and 51 with enlarged nodes (cN1-3). Progression occurred in 8 patients during NAC (2× in cN0, 6× in cN+), two patients refused surgical treatment after NAC. Subsequent radical cystectomy was performed in 47/49 cN0 and 43/51 cN1-3 patients. The study evaluated the intention‐to‐treat population, even the patients who did not undergo surgical treatment were included. We assessed five‐year cancer‐specific survival (CSS) using Kaplan‐Meier curves and compared survival within subgroups using the log‐rank test. Response to chemotherapy was defined as complete (pCR - ypT0N0), partial (pPR ≤ ypT1N0) or no response (non‐R; ≥ypT2N0-3). The influence of clinical and histopathological parameters on progression‐free survival (PFS) was evaluated using the Cox regression model. Median follow‐up was 43 months (IQR 16-87). Results: The average age of the patients was 64 years, men and women comprised 76% and 24% of the cohort, respectively. The extent of the primary tumor was cT2 in 39 and cT3-4 in 61 cases. The median number of lymph nodes removed during procedure was 24 (IQR 19;29). Five‐year CSS reached 84% in cT2-4N0 and 52% in cT2-4N1-3. Five‐year CSS was not different for cN1 versus cN2-3 (53% and 50%, respectively). We recorded a response to chemotherapy (cCR+pCR) in 34/49 (69%) patients with cN0 and in 24/51 (47%) cN1-3. Achieving response to chemotherapy improved 5-year CSS in both subgroups, in cN0 patients 93% versus 60% (p=0.001), in cN1-3 85% versus 27% (p<0.001). In the entire group, the 30-day and 90-day mortality reached 0% and 3%, respectively. In multivariate analysis, significant predictors of progression‐free survival were initial nodal involvement (HR 2.92; p=0.007) and failure to respond to NAC (HR 6.56; p≤0.001) Conclusion: In patients with invasive bladder cancer who are treated with neoadjuvant chemotherapy followed by radical cystectomy, excellent oncological results can be achieved in clinically localized disease. Even in the case of clinical lymphadenopathy, up to half of patients survive five years. Achieving a response to chemotherapy and the absence of lymphadenopathy at the time of diagnosis reduce the risk of recurrence.
