BACKGROUND: We aimed to assess the clinical, oncological, and pathological impact of en bloc resection of bladder tumors (ERBT) compared with conventional transurethral resection of bladder tumors (cTURBT) for pT1 high-grade (HG) bladder cancer. PATIENTS AND METHODS: We retrospectively analyzed the record of 326 patients (cTURBT: n = 216, ERBT: n = 110) diagnosed with pT1 HG bladder cancer at multiple institutions. The cohorts were matched by one-to-one propensity scores based on patient and tumor demographics. Recurrence-free survival (RFS), progression-free survival (PFS), cancer-specific survival (CSS), and perioperative and pathologic outcomes were compared. The prognosticators of RFS and PFS were analyzed using the Cox proportional hazard model. RESULTS: After matching, 202 patients (cTURBT: n = 101, ERBT: n = 101) were retained. There were no differences in perioperative outcomes between the two procedures. The 3-year RFS, PFS, and CSS were not different between the two procedures (p = 0.7, 1, and 0.7, respectively). Among patients who underwent repeat transurethral resection (reTUR), the rate of any residue on reTUR was significantly lower in the ERBT group (cTURBT: 36% versus ERBT: 15%, p = 0.029). Adequate sampling of muscularis propria (83% versus 93%, p = 0.029) and diagnostic rates of pT1a/b substaging (90% versus 100%, p < 0.001) were significantly better in ERBT specimen compared with cTURBT specimen. On multivariable analyses, pT1a/b substaging was a prognosticator of disease progression. CONCLUSIONS: In patients with pT1HG bladder cancer, ERBT had similar perioperative and mid-term oncologic outcomes compared with cTURBT. However, ERBT improves the quality of resection and specimen, yielding less residue on reTUR and yielding superior histopathologic information such as substaging.

Úvod: Močovod sa v operačnom poli inguinálnej hernie vyskytuje v 0,5−4 % prípadov. Vyskytuje sa najmä u obéznych pacientov, mužov, pacientov po transplantácii obličky. Typicky výskyt je pravostranný spolu s indirektnou inguinálnou herniou. Klinický obraz je často asymptomatický, avšak medzi možné prejavy patrí zvýšená frekvencia močenia s urgenciou, noktúria, rekurentné pyelonefritidy až urosepsa, pocit neúplného vyprázdnenia močového mechúra, známky obštrukcie GIT. Medzi diagnostické metódy patrí retrográdna pyelografia či CT urografia. V prípadoch herniácie močovodu je vždy indikovaná chirurgická liečba. Repozícia močovodu retroperitoneálne a štandardná plastika inguinálneho kanálu je metódou voľby. Metódy: Pri príprave článku bolo vyhľadaných 33 prípadov herniácie močovodu a z prípadov bol spracovaný štruktúrovaný prehľad. Kazuistika popisuje 68ročného pacienta s hyperpláziou prostaty a dysurickými ťažkosťami. Predoperačné CT vyšetrenie s intravenóznym kontrastom popisuje herniáciu pravostranného močovodu do inguinálnej oblasti s hydronefrózou II. stupňa. Predoperačne bol zavedený mono-J stent do pravého močovodu a následne bola vykonaná repozícia močovodu retroperitoneálne s plastikou hernie s využitím aloplastického materiálu. Pooperačný priebeh bol bez komplikácií. Výsledky a záver: Chirurg by v rizikových prípadoch mal predpokladať možnú prítomnosť močovodu v inguinálnej oblasti. Opatrná preparácia v inguinálnej oblasti znižuje riziko iatrogénneho poškodenia močovodu.
Introduction: The ureter is present in surgical field during inguinal hernia repair in 0.5–4% of cases. It typically occurs in obese patients, in men and patients after kidney transplants. Right-sided and indirect location of ureteral herniation prevails. The clinical picture is mostly asymptomatic, but possible manifestations include increased frequency of urination with urgency, nocturia, recurrent pyelonephritis, urosepsis, feeling of incomplete emptying of the bladder, signs of GIT obstruction. Diagnostic methods include retrograde pyelography or CT urography. Surgical treatment is indicated in every case of ureteral herniation. Reposition of the ureter retroperitoneally and standard plasty of the inguinal canal is the method of choice. Methods: 33 cases of ureteral hernia were reviewed in order to write a systematized review of the topic. The case report describes a 68-year-old patient with prostatic hyperplasia and dysuria treated at our institution. A preoperative CT examination with intravenous contrast showed herniation of the right ureter into the inguinal area with hydronephrosis of 2nd degree. Preoperative insertion of a mono-J stent into the right ureter and reposition of the ureter retroperitoneally followed by hernia repair using alloplastic material was performed. There were no postoperative complications. Results and conclusion: In risky cases, the surgeon should assume the possible presence of a ureter in the inguinal region. Careful dissection in the inguinal area reduces the risk of iatrogenic damage to the ureter.

PURPOSE OF REVIEW: Transurethral resection of bladder tumour (TURBT) followed by pathology investigation of the obtained specimens is the initial step in the management of urinary bladder cancer (UBC). By following the basic principles of oncological surgery, en-bloc resection of bladder tumour (ERBT) aims to overcome the limitations associated with conventional transurethral resection, and to improve the quality of pathological specimens for a better decision making. The current bulk of evidence provides controversial results regarding the superiority of one technique over the other. The aim of this article is to summarize the recent data and provide evidence on this unanswered question. RECENT FINDINGS: Despite heterogeneous and controversial data, ERBT seems to have a better safety profile and deliver higher quality pathologic specimens. However, the recent evidence failed to support the hypothesized oncological potential benefits of ERBT in the initial surgical treatment of patients with UBC. SUMMARY: ERBT has gained increasing interest globally in the past decade. It continues to represent a promising strategy with a variety of features intended to solve the inherent limitations of TURBT. However, the current quality of evidence does not allow solid conclusions to be drawn about its presumed superiority compared with the conventional technique.

Podkovovitá ledvina je nejčastější tvarová anomálie ledviny, která spočívá ve spojení obou ledvin pomocí parenchymového můstku. Incidence se pohybuje mezi 0,15-0,25 %. Podkovovité ledviny jsou predisponovány v důsledku anatomických poměrů ke vzniku nefrolitiázy a mají vyšší riziko rozvoje některých tumorů. Operační řešení tumorů podkovovité ledviny je specifické, vhodné je dbát na předoperační plánování s použitím pokročilých zobrazovacích metod a pomocných perioperačních metod k minimalizaci komplikací.
Horseshoe kidney is the most common shape anomaly of the kidney, which consists in the connection of both kidneys by means of a parenchymal bridge. The incidence is between 0,15-0,25 %. Horseshoe kidneys are predisposed to nephrolithiasis due to anatomical conditions and have a higher risk of developing some type of tumors. Surgical treatment of horseshoe kidney tumors is specific with a help of preoperative planning using advanced imaging methods and auxiliary perioperative methods to minimize complications.
Při léčbě pacientů s nádory močového měchýře je důležité dělení na tumory svalovinu infiltrující a neinfiltrující, protože prognóza i terapie v těchto dvou skupinách se podstatně liší. Neinfiltrující nádory měchýře jsou onemocněním s velmi heterogenní prognózou, jejíž odhad je zásadní pro správný typ zvolené adjuvantní terapie. Základem léčby neinfiltrujících tumorů je transuretrální resekce, po které je pro úspěšnou léčbu důležité stanovení prognostických faktorů. Hodnocení těchto jednotlivých faktorů umožňuje zařadit pacienty do prognostických skupin dle odhadovaného rizika progrese. Pro každou rizikovou skupinu je doporučena rozdílná následná léčba a způsob sledování. Adjuvantní terapie spočívá v intravezikální chemoterapii nebo imunoterapii. Je nutné zdůraznit, že instilační léčba je indikovaná u každého pacienta po transuretrální resekci, výběr terapeutického agens a režim aplikace je závislý na individuální prognóze. Intravezikální chemoterapie má prokázaný benefit u pacientů v nízce a středně rizikové skupině, u vysoce rizikových pacientů je účinnější intravezikální imunoterapie.
When treating patients with bladder cancer, it is important to classify tumours into muscle-infiltrating and non-infiltrating tumours since both the prognosis and treatment differ substantially in these two groups. Non-infiltrating bladder tumours are associated with a very heterogeneous prognosis, the estimation of which is essential for choosing the right type of adjuvant therapy. The mainstay of treatment for non-infiltrating tumours is transurethral resection, following which it is important to determine prognostic factors in order for the treatment to be successful. Evaluation of these individual factors allows the inclusion of patients in prognostic groups according to the estimated risk of progression. A different method of subsequent treatment and follow-up is recommended for each risk group. Adjuvant therapy consists in intravesical chemotherapy or immunotherapy. It must be emphasized that instillation therapy is indicated in every patient after transurethral resection, with the choice of the therapeutic agent and administration regimen being dependent on the individual prognosis. The benefit of intravesical chemotherapy has been shown in low- and medium-risk groups of patients; in high-risk patients, intravesical immunotherapy is more effective.

V kazuistice prezentujeme případ 57leté pacientky s anamnézou pádu na kole, přicházející pro bolesti v podbříšku a polakisurie. CT vyšetření s retrográdní cystografií prokázalo drobnou intraperitoneální rupturu močového měchýře a volnou tekutinu v dutině bříšní. Provedli jsme laparoskopickou revizi a suturu močového měchýře. Poranění dalších orgánů v dutině bříšní bylo při revizi vyloučeno.
In our case report we present a 57 years old woman with a history of felling off the bicycle who was coming with pain in lower abdomen and frequency. According to CT cystography was described an intraperitoneal rupture of the bladder and free contrast medium in the peritoneal cavity. We performed a laparoskopic examination of the abdomen and suture of the bladder. No other abdominal organ injury was detected.
