OBJECTIVE: Pain, fatigue, cognitive complaints and psychiatric comorbidities are common in patients with functional movement disorder and may significantly affect their quality of life. The aim of the study was to assess the impact of motor and non-motor symptoms on health-related quality of life in patients with functional movement disorder. METHODS: Sixty-one patients with clinically established functional movement disorder and 61 matched healthy controls completed standardized questionnaires for depression, anxiety, cognitive complaints, fatigue, pain, sleepiness, apathy and health-related quality of life. Motor disorder severity was assessed using The Simplified Functional Movement Disorders Rating Scale. Personality traits were assessed using the 44-Item Big Five Inventory. RESULTS: Compared to controls, patients reported significantly lower health-related quality of life and higher levels of all assessed non-motor symptoms except for apathy. No difference was found in personality traits. In both groups, health-related quality of life scores negatively correlated with depression, anxiety, pain, cognitive complaints, apathy, and neuroticism. No correlation was found between health-related quality of life and motor symptom severity in patients with functional movement disorder. Multiple regression analysis of the predictors of health-related quality of life showed significant impact of trait anxiety and cognitive complaints scores. CONCLUSIONS: Multiple non-motor symptoms but not motor symptom severity correlated with impaired health-related quality of life in patients with functional movement disorder. Impaired health-related quality of life was predicted by anxiety and cognitive complaints. Our results highlight the importance of assessing and treating both motor and non-motor symptoms in patients with functional movement disorder.

Poruchy chůze patří mezi nejčastější projevy psychogenních poruch hybnosti, nozologické oblasti na pomezí neurologie a psychiatrie, kde se často lékař i pacient ocitají v riziku diagnostických a terapeutických chyb. Cílem tohoto přehledu je ukázat na typické projevy, zdůraznit diagnostické zásady a naznačit terapeutické možnosti u psychogenních poruch hybnosti se zvláštním zaměřením na poruchy chůze. Mezi typické vzorce psychogenních poruch chůze patří 1. chůze s poruchou rovnováhy (o inadekvátně široké bázi, často s paradoxním zlepšením při zkoušce tandemové chůze, případně spontánní provazochodecká chůze nebo chůze rázu astázie-abázie); 2. pseudoparetická chůze nebo 3. extrémní zpomalení chůze. Zároveň bývají vyjádřeny obecné rysy psychogenních poruch hybnosti jako inkonzistence příznaků v čase, inkongruence se známými obrazy organicky podmíněných poruch hybnosti, vysoká energetická náročnost pohybu a charakteristické změny příznaků při změnách zaměření pozornosti. Diagnóza psychogenní poruchy hybnosti je klinická a je plně v kompetenci neurologa, který by neměl alibisticky opakovat negativní pomocná vyšetření k „vyloučení organicity“ ani očekávat, že diagnózu učiní a pacientovi sdělí jiný odborník. Úspěšnost léčebného postupu u psychogenních poruch hybnosti klesá s délkou trvání obtíží a s rozvojem jejich sociálních důsledků.
Gait disturbances are ranked among the most common manifestations of psychogenic movement disorders, a nosological area lying between neurology and psychiatry where both physicians and patients often find themselves at risk of diagnostic and therapeutic errors. The aim of this review is to show the typical manifestations, emphasize diagnostic principles and outline the therapeutic options for psychogenic movement disorders, with a special focus on disorders of gait. Typical patterns of psychogenic gait disorders include 1. balance disorders (with inadequately broad-based gait, often with paradoxical improvements in tandem walking test, spontaneous rope-walkers gait or astasia-abasia), 2. pseudoparetic gait or 3. extremely slow gait. At the same time, expressed are general features of psychogenic movement disorders such as inconsistency of symptoms over time, incongruence with typical organic movement disorders, high energy expenditure and characteristic changes in symptoms with distracted attention. The diagnosis of psychogenic movement disorders is clinical and on full responsibility of the neurologist, who should not repeat the negative auxiliary examinations to „exclude organicity“ or expect that another specialist will make the diagnosis and communicate it to the patient. Success of the treatment regimen for psychogenic movement disorders decreases with their duration and with development of their social consequences.

Psychogenní poruchy chůze jsou funkční psychické poruchy, které vedou k hybným obtížím bez nálezu strukturálního poškození a tvoří velkou skupinu pacientů nejen neurologických ambulancí. Vzhledem k heterogenitě klinických projevů, obtížnosti při stanovení diagnózy a nedostatečné terapeutické odezvě jsou zdrojem frustrace lékařů i pacientů. Článek shrnuje klinické projevy, specifika vyšetření a diagnostická kritéria, která musí být splněna pro stanovení diagnózy. Úspěšnost terapie závisí na včasné diagnóze, motivaci pacienta a dobré spolupráci neurologů, psychoterapeutů, psychiatrů a fyzioterapeutů.
Psychogenic gait disorders are functional mental disturbances causing difficulties of movement without any detection of structural lesion. Heterogeneity of clinical symptoms, diagnostic difficulties and insufficient responses to therapy lead to frustration of both physicians and patients. The article reports clinical symptoms, specifity of examination and diagnostic standards, which have to be fulfilled to make a diagnosis. Successful therapy depends on prompt diagnosis, patients´ motivation and good cooperation between the neurologist, psychotherapists, psychiatrist and physiotherapists.
