Mezi vzácné příčiny dušnosti patří obrovský bulózní emfyzém, jinými slovy syndrom mizející plíce. Jedná se o bulu nebo buly vyplňující alespoň jednu třetinu hrudníku a utlačující okolní plíci s typickým radiologickým nálezem často zaměňovaným za pneumotorax. Předkládaná kazuistika ilustruje diagnostické a terapeutické rozpaky i omyly týkající se tohoto syndromu, včetně jeho komplikací, a zamýšlí se nad prováděním uvážlivých vyšetření a zákroků v medicíně 21. století.
Rare causes of dyspnoea include giant bullous emphysema, otherwise called vanishing lung syndrome. It is a bulla or bullae filling at least one third of the chest and oppressing the surrounding lung with a typical radiological finding often mistaken for pneumothorax. The presented case report illustrates diagnostic and therapeutic doubts and mistakes regarding this syndrome, including its complications, and contemplates performing judicious examinations and procedures in the 21st century medicine.
Kazuistika prezentuje případ seniora s manifestací těžkého refrakterního převážně neatopického eosinofilního astmatu ve vysokém věku téměř současně s progresí chronické rinosinusitidy s nosní polypózou. Během dvou let se nemocný dostal na 5. stupeň léčebného schématu dle doporučení Personalizovaného managementu astmatu (GINA) s nutností opakované terapie systémovými kortikoidy pro těžké exacerbace. Po nasazení biologické léčby.
This case report describes the case of a senior with severe refractory mainly non-atopic eosinophilic adult-onset asthma (in senior age), which comes almost simultaneously with the progression of chronic rhinosinusitis with nasal polyposis. Within two years, the asthma became refractory for treatment and the patient reached level 5 of the treatment scheme according to Asthma management guidelines (GINA) with repeated need for systemic corticosteroids due to severe exacerbations. After the use of biological treatment – dupilumab – the asthma was completely controlled with an improvement in the quality of life.
Kazuistika pacientky v šestém decenniu života, u které byl zachycen významný sekundární buněčný imunodeficit, významné snížení T-helperů a cytotoxických T-lymfocytů. Buněčný imunodeficit pacientky lze příčinně vztahovat k chemoterapii onkologického onemocnění (karcinom endometria) dva roky před imunologickým vyšetřením. Pacientčin sekundární buněčný imunodeficit je klinicky bezpříznakový. Prognóza úpravy tohoto sekundárního imunodeficitu směrem k fyziologickým hodnotám je nejistá. Časový horizont eventuelního posunu počtu T-lymfocytů směrem k fyziologickým hodnotám je nejistý. Pacientčin imunitní systém tak pracuje s nízkým počtem T-lymfocytů, imunologické sledování spolu s dalšími odbornostmi je nutné.
We present a case report of a 57-year-old woman with a severe secondary helper-T-lymphocyte and cytotoxic-T-lymphocyte deficiency. The patient does not suffer from clinical signs of immunodeficiency. The reason for this secondary T-lymphopaenia is most likely the chemotherapeutic treatment of her oncological diagnosis (endometrial carcinoma). The chemotherapy was finished two years before the immunological blood tests of the patient. The prognosis of the improvement of this secondary T-lymphopaenia is very uncertain and regular follow-up is necessary.
Následující kazuistické sdělení popisuje dva případy pacientek ukrajinské národnosti, které byly došetřovány pro respirační potíže. V prvním kazuistickém případě byla diagnostikována pneumocystová pneumonie (PCP) v rámci pokročilé HIV infekce ve stadiu AIDS. U druhé nemocné byla diagnostikována koincidence TBC plic, kvasinkové laryngotracheitidy, virové hepatitidy C a HIV infekce. V případě pacientů pocházejících ze zemí s vyšší incidencí HIV a tuberkulózy (TBC) je nutno v rámci diferenciální diagnostiky pomýšlet na tyto možné příčiny potíží. Tyto přenosné infekce jsou rizikem nejenom pro samotného pacienta, ale i pro okolní společnost.
The presented case report describes two cases of Ukrainian patients who underwent a follow-up examination for respiratory difficulties. In the first reported case, the patient was diagnosed with pneumocystic pneumonia (PCP) as a part of advanced HIV infection in the AIDS stage. The second patient was diagnosed with coincidence of pulmonary tuberculosis, yeast laryngotracheitis, viral hepatitis C and HIV infection. In the case of patients from countries with a higher incidence of HIV and tuberculosis (TBC), it is necessary to consider these possible causes of problems as part of the differential diagnosis. These transmissible infections are a risk not only for the patients themselves, but also for the surrounding society.
