Autoantibodies against complement C1q (anti-C1q) have been well described in patients with systemic lupus erythematosus, where they correlate with the occurrence of severe lupus nephritis. However, data on anti-C1q in organ-specific autoimmune diseases are scarce. In order to determine the prevalence of anti-C1q in patients with autoimmune thyroid disorders (AITD) and a possible association with thyroid function, we measured prospectively anti-C1q in 23 patients with Graves' disease (GD) and 52 patients with Hashimoto's thyroiditis (HT). Anti-C1q levels were correlated with parameters of thyroid function and autoantibodies against thyroperoxidase, thyroglobulin and thyroid stimulating hormone (TSH) receptor. Twenty-one patients with multi-nodular goitre and 72 normal blood donors served as controls. We found elevated concentrations of anti-C1q more frequently in patients with AITD than in controls: seven of 23 (30%) patients with GD and 11 of 52 (21%) patients with HT, compared with one of 21 (5%) patients with multi-nodular goitre and six of 72 (8%) normal controls. Anti-C1q levels did not correlate with thyroid autoantibodies. However, in GD absolute levels of anti-C1q correlated negatively with TSH and positively with free thyroxine (FT4) and triiodothyronine (FT3). In contrast, in HT, anti-C1q correlated positively with TSH levels. No correlation between TSH and thyroid autoantibodies was found. In conclusion, we found an increased prevalence of anti-C1q in patients with AITD and their levels correlated with the thyroid function in both GD and HT. This correlation seems to be independent of thyroid autoantibodies. Therefore, anti-C1q might point to a pathogenic mechanism involved in the development of AITD that is independent of classical thyroid autoantibodies.

Předmětem projektu je studium vybraných složek komplementu v etiopatogenezi autoimunitních tyreopatií (AITD), t.j. Gravesovy-Basedowovy tyreotoxikózy a chronické lymfocytární tyreoiditidy, a jejich vztahu ke klinickému průběhu onemocnění. Cílem první části projektu je: 1) zjistit prevalenci deficience mannan-vázajího lektinu (MBL) u nemocných s AITD; 2) ozřejmit vliv tyreoidálních hormonů na sérové koncentrace MBL; 3) definovat vztah MBL deficience a klinického průběhu AITD. Cílem druhé části projektu je sledovat sérové koncentrace protilátek proti C1q složce komplementu u nemocných s AITD v čase a popsat jejich vztah k průběhu onemocnění. Metodika bude zahrnovat genetickou analýzu genu pro MBL a opakovaná klinická a laboratorní vyšetření nemocných s AITD, zahrnující m.j. stanovení sérových koncentrací MBL a anti-C1q protilátek.; The goal of the project is to study the role of selected components of the complement system in the pathogenesis of autoimmune thyroid disorders (AITD), i.e. Graves´ disease and Hashimoto´s thyroiditis, and their relationship to the course of disease. The aims of the first part of the project include: i) to determine the prevalence of mannan-binding lectin (MBL) deficiency in patients with AITD; ii) to assess the impact of thyroid hormones on MBL serum concentrations; ii) to evaluate the relationship between MBL deficiency/serum levels of MBL and the clinical course of disease. The aim of the second part of the project is to follow the serum levels of antibodies against complement component C1q in time and to determine their relationship to the clinical course of AITD. Methods include genetic analysis of the MBL-2 gene and repeated clinical and laboratory examinations of patients with AITD, including measurements of serum MBL and anti-C1q levels.

Cílem projektu je přispět k objasnění role vybraných humorálních a buněčných regulačních mechanismů v etiopatogenezi orgánově specifických autoimunit: autoimunitních endokrinopatií a celiakie. Třikrát v odstupu 9 měsíců projektu bude vyšetřeno 140 pacientů s různými autoimunitními endokrinopatiemi a celiakií a kontrolní skupina 20 osob. U každé osoby budou vyšetřeny vybrané parametry imunitního systému (anti-C1q protilátky, nový komplementový inhibitor CRIT, regulační T lymfocyty, exprese kostimulačníchmolekul, intracelulární cytokiny) v krvi a v punktátech štítné žlázy. Předmětem výzkumu bude zjistit, liší-li se tyto parametry u jednotlivých typů autoimunit a kontrolní skupiny, liší-li se v periferní krvi a lokálně v punktátech získaných FNAB a mají-li vztah k průběhu onemocnění.; The aim of the project is to gain better unterstanding of the immunological pathogenesis of organ-specific autoimmune disorders, especially autoimmune endocrinopathies and celiac disease, and thus achieve a better therapeutical approach. In the 1., 2. and 3. year 140 outpatients with autoimmune endocrinopathies and celiac disease and 20 control persons will be repeatedly examined. Parameters of the immune system (anti-C1q antibodies, CRIT on thyrocytes, regulatory T-lymphocytes, co-stimulatory and adhesive molecules, intracellular cytokines) will be examined in blood and in case of thyroid disease, also in thyroid aspirates. We want do determine the differences between each autoimmune disorder (thyroiditis, diabetes type 1, polyglandular disorders, celiac disease), compare the data in peripheral blood and thyroid aspirates, and study their relation to the course of disease.
