Entrapment syndrom arteria poplitea je vzácnou příčinou ischemie dolních končetin. Je způsoben abnormálním vztahem podkolenní tepny a okolních svalově-šlachových struktur (anatomický entrapment). Funkční entrapment je charakterizován normálními anatomickými poměry podkolenní jamky. Jeho přesná incidence není známa, zpravidla postihuje mladé jedince, častěji muže, a klinicky se obvykle manifestuje lýtkovými klaudikacemi, křečemi, chladem končetiny, paresteziemi. CT angiografie a MR angiografie jsou zobrazovacími metodami první volby. Základním operačním výkonem je dekomprese podkolenní tepny, která je v případě jejího významného poškození doplněna náhradou autologním žilním štěpem, který zajišťuje nejlepší dlouhodobé výsledky. Autoři na základě dvou kazuistik popisují klinický průběh, vyšetřovací postup a chirurgickou léčbu pokročilého anatomického entrapment syndromu podkolenní tepny.
Popliteal artery entrapment syndrome is a rare cause of lower limb ischaemia. It is caused by an abnormal relationship between popliteal artery and its surrounding musculotendineous structures (anatomic entrapment). Functional popliteal entrapment is characterized by normal anatomic relationships within popliteal fossa. The true incidence is unknown, it usually affects young patients, typically men, and is clinically presented with symptoms of calf claudication, calf cramping, coldness and paresthesia. CT angiography and MR angiography have become an imaging technique of choice. The mainstay surgical procedure is popliteal artery release. In cases of popliteal artery damage autologous saphenous vein bypass grafting offers the best long-term results. Authors on the basis of two case reports describe the clinical course, diagnosis and surgical technique used in the treatment of patients with advanced popliteal artery entrapmentsyndrome.

Úvod: Záchrana kriticky ischemické končetiny pomocí implantace pedálního, event. distálního krurálního bypassu je metodou s velmi dobrými dlouhodobými výsledky. U pacientů s nevhodným nebo chybějícím autologním žilním štěpem je možné použít allogenní žilní náhradu. Kazuistika: 68letý pacient s ICHDKK a gangrénou levé nohy je sledován na našem pracovišti od srpna 2014. Provedená perkutánní transluminální angioplastika bércových tepen nevedla ke zlepšení prokrvení periferie končetiny. Pacient byl proto indikován k pedální či distální krurální cévní rekonstrukci. Vzhledem k absenci adekvátního autologního žilního štěpu bylo jedinou možností na záchranu končetiny našití distálního bypassu s použitím dárcovského štěpu. Byla nutná amputace prstů levé nohy pro gangrénu. Pacientovi byl implantován femoro-pedální bypass na arteria plantaris communis vlevo pomocí allogenní vena saphena magna. Pooperační průběh byl bez komplikací. Pedální bypass je funkční, defekt po amputaci prstů se příznivě hojí. Závěr: Předpokladem cévní rekonstrukce s použitím allogenní náhrady je dostupnost vhodného alloštěpu a také spolupráce pacienta při následné léčbě. U prezentovaného klinického případu znamenala implantace pedálního bypassu pomocí allograftu zachování končetiny pacienta.
Introduction: Pedal or distal crural bypass surgery for limb salvage is a method with very good long-term results. For patients in whom a suitable autologous venous graft is not available, the use of a venous allograft is an alternative procedure. Case report: A 68 years old man with ischaemic disease of lower extremities and gangrene of the left foot was admitted to our Centre in August 2014. He underwent percutaneous transluminal angioplasty of crural arteries of his left lower extremity. This, however, failed to improve peripheral circulation. The patient was then indicated for pedal or distal crural vascular reconstruction. Since no suitable autologous vein was available, distal bypass surgery using a donor graft remained the only option for limb salvage. Amputation of the toes on the left foot due to gangrene was necessary. Subsequently, femoro-pedal bypass to the left common plantar artery was performed using a great saphenous vein allograft. The post-operative course was without complications, the pedal bypass was patent and toe amputation was with good healing. The patient remained in follow-up care. Conclusion: A good outcome of vascular reconstruction with an allograft depends on the availability of a suitable allograft and good patient compliance with post-operative care. In the case presented here, the pedal bypass grafting by means of an allograft helped to save the patient’s limb.

PURPOSE: Knowledge of accessory flexor carpi ulnaris (AFCU) is not only important for proper orientation in the surgical field but it can be used for tendon transfer as well. AFCU commonly occurs with concomitant variants, and its presence should rise caution in order to prevent iatrogenic injury. METHODS: During a routine dissection for research data collection at the Institute of Anatomy, a AFCU with concomitant variants was observed in a European cadaver fixed with Thiel's method. A thorough review of the literature concerning all the encountered variants was performed. RESULTS: AFCU was found in the right upper limb with its insertion on the flexor retinaculum. Palmaris longus muscle was absent in this limb, and an accessory branch of the anterior interosseous artery coursed over the pronator quadratus muscle to anastomose with the ulnar artery 5 cm proximally to the pisiform. On the left hand, a variable lumbrical of the second finger originating from the flexor retinaculum was found, which was not described in the literature before. CONCLUSIONS: AFCU commonly occurs together with concomitant variants, and special attention is needed when performing surgery on such forearm.

An unusual combination of median nerve's variations has been encountered in a male cadaver during routine educational dissection. In particular, the median nerve was formed by five roots; three roots originated from the lateral cord of the brachial plexus joined individually the median nerve's medial root. The latter (fourth) root was united with the lateral (fifth) root of the median nerve forming the median nerve distally in the upper arm and not the axilla as usually. In addition, the median nerve was situated medial to the brachial artery. We review comprehensively the relevant variants, their embryologic development and their potential clinical applications.

OBJECTIVE: Carotid-femoral pulse wave velocity (PWV) has been shown to be a powerful predictor of cardiovascular morbidity and mortality. Sympathetic neural mechanisms may have a stiffening influence on arterial mechanical properties. The relationship between direct measures of sympathetic traffic and PWV in healthy humans has not been previously studied. We, therefore, tested the hypothesis that PWV is independently linked to muscle sympathetic nerve activity (MSNA) in normal individuals. METHODS: We measured MSNA (microneurography), PWV (Complior device), heart rate and blood pressure in 25 healthy male participants (mean age 43 +/- 10 years). RESULTS: PWV correlated significantly with age (r = 0.63, P < 0.001), SBP (r = 0.43, P < 0.05) and MSNA (r = 0.43, P < 0.05) but not with BMI, waist circumference, waist-to-hip ratio, heart rate, pulse pressure or DBP. Robust multiple linear regression analysis revealed that only age and MSNA were linked independently to PWV (r2 = 0.62, P < 0.001), explaining 39 and 25% of its variance, respectively. After adjustment of PWV for age and SBP, we further divided individuals into 'excessive' PWV (i.e. higher than expected from age and SBP) and 'optimal' PWV (i.e. lower than expected). BMI and blood pressure were similar in both subgroups. Individuals with excessive PWV had significantly greater MSNA than individuals with optimal PWV (30 +/- 10 vs. 18 +/- 11 bursts/min, P = 0.01). CONCLUSION: This study provides the first evidence that PWV is linked to MSNA in normal humans. The relationship between MSNA and PWV is independent of age, BMI, waist circumference, waist-to-hip ratio, heart rate, pulse pressure or blood pressure.

BACKGROUND: Aim of the study was to compare metabolic response of leg skeletal muscle during functional electrical stimulation-driven unloaded cycling (FES) to that seen during volitional supine cycling. METHODS: Fourteen healthy volunteers were exposed in random order to supine cycling, either volitional (10-25-50 W, 10 min) or FES assisted (unloaded, 10 min) in a crossover design. Whole body and leg muscle metabolism were assessed by indirect calorimetry with concomitant repeated measurements of femoral venous-arterial differences of blood gases, glucose, lactate and amino acids. RESULTS: Unloaded FES cycling, but not volitional exercise, led to a significant increase in across-leg lactate production (from -1.1±2.1 to 5.5±7.4 mmol/min, p<0.001) and mild elevation of arterial lactate (from 1.8±0.7 to 2.5±0.8 mM). This occurred without widening of across-leg veno-arterial (VA) O2 and CO2 gaps. Femoral SvO2 difference was directly proportional to VA difference of lactate (R2 = 0.60, p = 0.002). Across-leg glucose uptake did not change with either type of exercise. Systemic oxygen consumption increased with FES cycling to similarly to 25W volitional exercise (138±29% resp. 124±23% of baseline). There was a net uptake of branched-chain amino acids and net release of Alanine from skeletal muscle, which were unaltered by either type of exercise. CONCLUSIONS: Unloaded FES cycling, but not volitional exercise causes significant lactate production without hypoxia in skeletal muscle. This phenomenon can be significant in vulnerable patients' groups.

The purpose of this study was to determine if there is flowmediated vasodilation of the femoral artery in response to progressive increases in flow within a physiological range observed in the in vivo experiments. Femoral artery blood flow was determined in conscious rabbits (n=5) using chronically implanted flowprobes. Resting blood flow was 8.3±0.6 ml/min and increased to 39.9±5.4 ml/min during high intensity exercise. Femoral arteries (n=12, 1705±43 µm outer diameter) harvested from a separate group of rabbits were mounted on cannulas and diameter was continuously monitored by video system. Functional integrity of the endothelium was tested with acetylcholine. The arteries were set at a transmural pressure of 100 mm Hg and preconstricted with phenylephrine to 73±3 % of initial diameter. Using a roller pump with pressure held constant, the arteries were perfused intraluminally with warmed, oxygenated Krebs' solution (pH=7.4) over a physiological range of flows up to 35 ml/min. As flow increased from 5 ml/min to 35 ml/min, diameter decreased significantly (p<0.05) from 1285±58 µm to 1100±49 µm. Thus, in vessels with a functional endothelium, increasing intraluminal flow over a physiological range of flows produced constriction, not dilation. Based on these results, it seems unlikely that flow-mediated vasodilation in the rabbit femoral artery contributes to exercise hyperemia.

While investigating the cause of entrapment syndrome of the peripheral nerves in the elbow region, we observed variability of the pronator teres muscle and the relationship of this muscle to the median nerve and the surrounding vessels. Attention was also paid to the occurrence of the supracondylar process of the humerus and Struthers' ligament with regard to their ontogenetic and phylogenetic development. For this purpose, a classical anatomical dissection of the upper limbs of 68 adults, three fetuses and a phylogenetic assessment of five mammalian species was performed. In terms of variability in the anatomical structures of the elbow region, we found the most serious clinical condition to be where the median nerve ran through the pronator canal together with the ulnar vessels (1.5%), or when it passed through the ulnar head of the pronator teres (5.9%). The pronator teres examined by us in fetuses showed the same arrangement as in adult individuals, including the created ulnar head. The occurrence of a supracondylar process and Struthers' ligament was not observed in our collection. The presence of these structures was not confirmed during the fetal period, either. The phylogenetic part of the study re-opened the question of the meaning and function of the entepicondylar foramen, because we noted differences in the occurrence of this structure in two related genera with a very similar way of life (Djungarian hamster and golden hamster).

Cíl: Hypoperfuze periferních tkání a splanchnických orgánů během operace srdce v mimotělním oběhu může vézt ke vzniku závažných komplikací. Cílem studie bylo monitorování metabolismu a krevního toku v intersticiu periferní tkáně, kosterním svalu, během operace v mimotělním oběhu (MO) a v časném pooperačním období pomocí mikrodialýzy. Metody: Instersticiální mikrodialýza je minimálně invazivní metoda k biochemickému monitorování metabolických změn a krevního toku v extracelulárním prostoru tkání. Substance v intersticiu přecházejí přes semipermeabilní membránu zavedené mikrodialyzační sondy a mohou být analyzovány. Mikrodialýza v této studii byla prováděna 2 mikrodialyzačními sondami CMA (CMA Microdialysis AB®, Sweden), zavedenými do deltového svalu operovaného nemocného. Sondy byly perfundovány Ringerovým roztokem rychlostí 0,3 ml/hod. Dialyzáty byly odebírány v intervalech: začátek operace, začátek MO, konec MO, konec operace, po 2 hodinách v pooperačním období. Standardními biochemickými metodami byly v dialyzátech vyhodnocovány glukóza, urea, flycerol a laktát. Krevní průtok v intersticiu byl monitorován pomocí dynamické mikrodialýzy gentamicinu jako markeru. Mikrodialýza byla prováděna u 40 pacientů s ischemickou chorobou srdeční, operovaných v mimotělním oběhu. U 20 nemocných byl MO veden v normotermii (NT), u 20 nemocných v hypotermii (HT). Výsledky: U obou skupin, NT vs. HT, byla zjištěna obdobná dynamika intersticiálních koncentrací sledovalých látek v závislosti na fázi operace a v časném pooperačním období. Nízké iniciální koncentrace byly následovány jejich postupným zvyšováním během mimotělního oběhu, dalším zvýšením hodnot po ukončení mimotělního oběhu i v následující fázi operace. Hodnoty koncentrací sledovaných analytů pak byly vyšší ve skupině operovaných v normotermii, což bylo dáno normální metabolickou aktivitou buněk při normotermii (vs. nižší hodnoty při hypotermii). Bezprostředně po operaci docházelo, opět v obou skupinách, k poklesu sledovaných hodnot, s jejich postupným zvyšováním v následném pooperačním období v obou skupinách. Trend dynamických změn sledovaných analytů, jako látek mapujících metabolickou aktivitu kosterního svalu, dokladuje nižší metabolickou aktivitu buněk během hypotermie a její výrazný nárůst (oproti NT) ve fázi následné normalizace teploty tkáně. Analýzou koncentrací laktátu, jako látky mapující anaerobní metabolismus kosterního svalu, byly zjištěny obdobné dynamické změny v obou skupinách (NT vs. HT). Nebyla zjištěny významné odlišnosti, vázané na fázi operace ani fázi bezprostředního pooperačního průběhu při porovnání obou skupin. Analýza koncentrací gentamicinu, jako flow-markeru, ukázala nižší hodnoty koncentrací gentamicinu v dialyzátu, během operace, MO i v časném pooperačním období, u skupiny operovaných v normotermii (vs. HT), což dokladuje vyšší tkáňový průtok v kosterním svalu oproti skupině nemocných operovaných v hypotermii. Zduěr.-Výsledky mikrodialyzační studie prokázaly dynamické změny v intersticiálních koncentracích sledováných látek (glukózy, urey, glycerolu a laktátu) v závislosti na fázi srdeční operace v mimotělním oběhu a V časném pooperačním období. Vyšší tkáňová perfuze kosterního svalu byla zaznamenána u nemocných operovaných v normotermii. Bylo prokázáno, že dynamika změn koncentrací sledovaných látek v intersticiu kosterního svaluje dána jak metabolickou aktivitou tkáně, tak průtokem krve intersticiem svalu.
Aim: Hypoperfusion of peripheral tissues and splanchnic organs during cardiac surgery in extracorporeal circulation may lead to the origin of serious complications. The aim of the study was to monitor metabolism and blood pressure in interstital peripheral tissue, skeletal muscle, during the operation on the patient with extracorporeal circulation (ECC) in an early post-operation period by means of microdialysis. Methods: The interstitial microdialysis is a minimally invasive method for the biochemical monitoring of metabolic changes and blood pressure in extracellular space of tissue. The substances in interstitium pass across a semipermeable membrane of the inserted microdialysis probe and may be analyzed. Microdialysis in this study was performed by means of two microdialysis probes CMA (CMA Microdialysis AB \ Sweden) inserted into the deltoid muscle of the surgically treated patient. The probes were perfused by the Ringer solution at the rate of 0.3 ml/hour. The dialysates were sampled in the following intervals: beginning of the operation, beginning of ECC, end of ECC, end of the operation, two hours during the post-operation period. Standard biochemical methods were to evaluate, in the dialysates, glucose, urea, glycerol and lactate. The blood flow in the interstitium was monitored by means of dynamic microdialysis of gentamycine as a marker. Microdialysis was performed in 40 patients with ischemic heart disease, operated on in the extracorporeal circulation. In 20 patients the ECC was performed in normothermia (NT), while in the other 20 patients it was made in hypothermia (HT). Results: In both groups, NT versus HT, a similar dynamism of interstitial concentration of the observed substances in relation to the operation phase and in early post-operation period. Low initial concentrations were gradually increasing during the extracorporeal circulation and increased further after the end of extracorporeal circulation and also in the subsequent phase of the operation. The concentration values of the analytes under observation were higher in the groups operated on under normothermia, apparently due to normal cellular activity during normothermia (versus values in hypothermia). Immediately after the operation the observe values decreased in the both groups and subsequently gradually increased in the post-operation period in the both groups. The trend of dynamic changes of the observed analytes, selected as compounds indicating metabolic activity of skeletal muscles during hypothermia documents a lower metabolic activity of the cells during hypothermia and its marked increase (against NT) in the phase of subsequent normalization of the tissue temperature. Analysis of the concentrations of lactate, as a compounds mapping anaerobic metabolism of skeletal muscle, revealed similar dynamic changes in the both groups (NT vs. HT). There were no significant differences. related to the phase of the operation or the phase of immediate post-operation course when the both groups were compared. The analysis of gentamycine concentrations as a flow marker revealed lower gentamycine concentrations in dialysate during the operation, ECC and the early post-operation course in the group operated on in normotheramia (vs. HT), indicating a higher tissue flow in skeletal muscle against the group of patients operated on under hypothermia. Conclusion: The results of the microdialysis study demonstrated dynamic changes in interstitial concentrations of the observed compounds (glucose, urea, glycerol and lactate) related to the phase of operation on the heart in extracorporeal circulation and in early post-operation period. A higher perfusion of skeletal muscle was documented in patients operated on under normothermia. It became obvious that the dynamism in the changes of the compounds observed in the interstitium of skeletal muscle was determined by metabolic activity of the tissue as well as by blood flow in the muscle interstitium.

The authors present an anatomical study and clinical experience with radial forearm flap (RFF) and pronator quadratus muscle (PQM) application in the reconstruction of various body areas. The aim was to describe the anatomical placement and proportions of the PQM, the anatomical location of the major arterial branch of the radial artery supplying the PQM, and the application of this knowledge in clinical practice. The anatomical study was based upon an analysis of 13 fresh adult cadaver upper extremities, of which nine were female and four male; both arms from the same donors were used in four cases. The study of the PQM was performed using a dye-containing intraarterial injection, standard macro- and micro-preparation techniques, and chemical digestion. The data on the PQM size in males and females, thickness of the radial artery branch (the principal artery nourishing the muscle), and its position were analysed. The radial artery branch nourishing the PQM was identified in all cadaveric specimens of the anatomical study. In addition, 12 patients underwent reconstructions of soft and bony tissue defects using a RFF + PQM (pedicled or free flap). The radial artery branch perfusing the PQM was identified in all cases. The flap was used for the management of defects of the head (seven cases), arm (three cases) and lower leg (two cases). The harvest site healed well in all cases and, with the exception of one case in which a partial necrosis of the flap was observed, all flaps remained viable, which demonstrated the safety of the method.
