Stručný autorský přehled výsledků stimulační terapie epilepsie v nc. dentatus, nc. caudatus a v epileptickém ložisku, je historickým dokladem využítí v léčbě technické možnosti stereotaxe a externalizovaných elektrod poprvé standardním způsobem. Byla to současně příležitost uvažovat o významu paměti v genezi epilepsie a o možnosti nových postupů v jejím léčení.
The concise overview of the results of dentate nucleus, caudate nucleus and direct epileptogenic focus stimulation provides historical evidence about the first – time use of stereotactic techniques and externalised electrodes in epilepsy patiens in standard fashion. At the same time the technique have provided the possibility to think of the role of memory in epileptogenesis and also of the new treatment approaches for epilepsy patients
Úvod: Technika susceptibilitně vážených MR obrazů (SWI) se stala vhodným nástrojem k detekci mikro krvácení (MK) a také sledování změn signálu V bazálních gangliích. Tyto změny signálu jsou přisuzovány zvýšené koncentraci železa, ale mohou být také způsobeny kalcifikacemi nebo starším krvácením. V této studii bylo SWI použito k detekci MK a signálových změn v nucleus caudatus (NC), putamen (PUT) a globus palidus (GP) u pacientů s Alzheimerovou chorobou (AD) a mild cognitive impairement (MCI). Material a metody: Celkem bylo zhodnoceno 47 subjektů, z toho bylo 19 pacientů s AD, 16 pacientů s MCI a 12 zdravých subjektů. Vyšetření byla realizována na MR systému Siemens Trio Tim 3T a měření SWI bylo provedeno s následujícími parametry sekvence: TR = 33 ms, TE = 20 ms, sklápěcí úhel = 15°, velikost voxelu 0,6 x 0,6 x 0,8 mm, čas měření 5:20 min. Byl hodnocen relativní signál v NC, PUT a GP a v celém zobrazeném objemu mozku zaznamenán počet jednotlivých MK. Změny signálu v BG a počet MK byly korelovány s údajem MMSE. Výsledky: Z vyšetřených subjektů se alespoň 1 leze MK nalezla u 29 subjektů (62 %), více než 4 leze u 8 subjektů (17 %). U pacientů s AD se MK vyskytovaly ve vyšší míře (v průměru 1,89 leze na pacienta) než u MCI (1,38 leze na pacienta) a kontrol (0,41 leze na pacienta). Tyto rozdíly však nebyly signifikantní. Nejvyšší pokles signálu ve strukturách BG s efektem susceptibility byl zaznamenán u některých subjektů v GP, menší pak v PUT, naopak změny V NC prakticky nebyly. I v tomto případě je častější výskyt poklesu signálu u pacientů, lze ho však najít i u řady zdravých kontrol. Závěr: Oba studované jevy pomocí SWI představují jiný aspekt možného příspěvku k progresu demence typu AD (vaskulární příspěvek a de¬ gradace tkáně v BG). Změny na SWI mohou přispět do celkového morfologického skóre pacienta a vysvětlit příčinu v některých případech.
Introduction: Recently, the new technique of susceptibility weighted imaging (SWI) has become suitable tool to detect micro-bleeding (MB) an d also to follow MR signal changes in basal ganglia (BG). These signal changes are supposed to be closely connected to iron depos ition increase but they also could be caused by calcification or older hemorrhage. In this study, SWI was used to detect MB and to measure sig nal in nucleus caudatus (NC), putamen (PUT) and globus palidus in patients with Alzheimer disease (AD) and mild cognitive impairment (MCI). Material and methods: We examined 47 subjects: 19 AD patients, 16 MCI patients and 12 healthy age matched volunteers. All examinations were done on Siemens Trio 3T MR system using SWI sequence with following parameters: TR = 33 ms, TE = 20 ms, flip angle = 15°, voxel size of 0,6 × 0,6 × 0,8 mm, measurement time 5:20 min. Relative signal in NC, PUT and GP was evaluated and number of MB in entire brain vol ume was calculated. Signal changes in BG were correlated with MMSE. Results: At least one MB lesion was found in 29 subjects (62 %) and more then 4 lesions in 8 subjects (17 %). In AD patients group, MB lesions were found more frequently (1.89 lesions per subject in average) then in MCI group (1.38 lesions per subject) and control group (0.41 lesions per subject). However, these differences were not significant. The most significant signal decrease was found in GP in some subjects but almost no change of signal intensity in NC was detect ed. Also in this case, more often the low signal in GP was found in AD group but we could see this effect also in more healthy subjects. Conclusions: Both effects studied using SWI represent different aspects of possible contribution to the progression of the dementia of the AD type (vascular contribution and tissue degradation in BG). Evaluated changes in SWI can provide valuable information into the e ntire charac- terization of the clinical state of AD patients and also explain some cases.

Creutzfeldt - Jakobova choroba (CJch) je rýchlo progredujúce neurodegeneratívne ochorenie s bezpodmienečne fatálnym koncom patriace do skupiny transmisívnych spongioformných encefalopatií, spôsobené zmnožením a intraneurálnou kumuláciou patologických prionových bielkovín PrPres v mozgu. sCJch je najrozšírenejšia forma CJch, má neznámu etiológiu, ročná incidencia choroby je približne 1 - 1,5 prípadov na milión ľudí. Klinická symptomatika je pestrá, napodobňujúca iné ochorenia. Postihuje jedincov vo veku 45 - 75 rokov, muži i ženy bývajú postihnutí rovnako často, k úmrtiu dochádza v priebehu 6 mesiacov. V retrospektívnej analýze 17 pacientov s podozrením na CJCh v rokoch 2002 až 2006 sme sa rozhodli aplikovať diagnostické kritériá Poserovej a kol., vrátane prítomnosti proteínu 14-3-3 a typických MRI zmien. Pokúsili sme sa zistiť, do akej miery sú tieto prídavné kritériá aplikované v našom súbore špecifické, ako diagnostický marker pre stanovenie diagnózy sCJch už ante mortem. Výsledky špecifickosti typických MR zmien – hyperintenzívny signál nc. caudatus, putamen a niektoré kortikálne oblasti mozgu a senzitivity proteínu 14-3-3 sú v súlade s doteraz v literatúre udávanou vysokou špecifickosťou a senzitivitou.
Creutzfeldt - Jacob disease (CJD) is fatal progressive neurodegenerative disease, belonging to the group of transmissible human spongiform encephalopathies. It is caused by multiplication and cumulation of pathologic prion proteins PrPres in human brain. Sporadic CJD is the most common form, its etiology is unknown, and incidence is approximately 1 to 1.5 cases in million people. Clinical symptoms are various, imitating other diseases. Patients between 45 and 75 years are affected; men and women are affected in the same ratio, leading to death in 6 months. In retrospective analysis of 17 patients with suspicion of CJD between 2002 and 2006 we decided to apply Poser,s diagnostic criteria, including protein 14-3-3 and typical MRI findings. We tried to investigate how these additional criteria are specific for our group of patients as a diagnostic marker of sCJD already during life. We found out high specificity of typical MRI findings – hyperintensive signal of nc. caudatus and putamen and also some cortical structures of brain and sensitivity of protein 14-3-3, in correlation with already published studies.
Prionové choroby, alebo prenosné spongioformné encefalopatie človeka sú progresívne, fatálne ochorenia, charakterizované neurologickými aj psychiatrickými príznakmi. Najčastejším prionovým ochorením človeka je Creutzfeldtova-Jakobova choroba (CJch). Abnormality MRI pri sporadickej forme CJch sa prejavujú hyperintenzitou signálu v oblasti putamen a caput nuclei caudati (v porovnaní ku talamu a mozgovej kôre) na T2-váženom obraze a protón denzitne váženom obraze sekvencií MR. Opísané zmeny signálu v putamen a v nc. caudatus sú obvykle symetrické. Najnovšie údaje upozorňujú na vysokú senzitivitu difúzne váženého obrazu. Charakteristické rozmiestnenie symetrickej hyperintenzity v pulvinar thalami na MRI sa ukazuje ako typické pre nový variant CJch (vCJch). Táto typická zmena sa nazýva pulvinarový príznak. Prospektívna analýza dokumentuje pulvinarový príznak u 90 % pacientov s diagnózou vCJch. Zmeny MRI nálezu boli zaznamenané u mnohých pacientov postihnutých CJch a charakteristická distribúcia týchto zmien umožňuje presnú rádiologickú diagnostiku aj s rozlíšením jednotlivých subtypov ochorenia. Zavedenie jednotného zobrazovacieho protokolu a zvýšená pozornosť zameraná na opísané charakteristické zmeny, môžu výrazne prispieť ku včasnému rozpoznaniu CJch.
The aim of our study was to subcategorize Autistic Spectrum Disorders (ASD) using a multidisciplinary approach. Sixty four autistic patients (mean age 9.4+/-5.6 years) were entered into a cluster analysis. The clustering analysis was based on MRI data. The clusters obtained did not differ significantly in the overall severity of autistic symptomatology as measured by the total score on the Childhood Autism Rating Scale (CARS). The clusters could be characterized as showing significant differences: Cluster 1: showed the largest sizes of the genu and splenium of the corpus callosum (CC), the lowest pregnancy order and the lowest frequency of facial dysmorphic features. Cluster 2: showed the largest sizes of the amygdala and hippocampus (HPC), the least abnormal visual response on the CARS, the lowest frequency of epilepsy and the least frequent abnormal psychomotor development during the first year of life. Cluster 3: showed the largest sizes of the caput of the nucleus caudatus (NC), the smallest sizes of the HPC and facial dysmorphic features were always present. Cluster 4: showed the smallest sizes of the genu and splenium of the CC, as well as the amygdala, and caput of the NC, the most abnormal visual response on the CARS, the highest frequency of epilepsy, the highest pregnancy order, abnormal psychomotor development during the first year of life was always present and facial dysmorphic features were always present. This multidisciplinary approach seems to be a promising method for subtyping autism.
