Cíl: Tato studie se zaměřila na posouzení spolehlivosti a vzdělávací hodnoty videí o hysterektomii pomocí transluminální endoskopické operace s přirozeným vaginálním vstupem (vNOTES – vaginal natural orifice transluminal endoscopic surgery) na YouTube a jejich vhodnosti pro školení chirurgů. Materiál a metody: Dne 12. června 2024 byl YouTube prohledáván pomocí klíčových slov „vNOTES hysterektomie“, „TVNOTES hysterektomie“, „transvaginální transluminální endoskopická hysterektomie s přirozeným otvorem“, „vNOTES“ a „vaginální hysterektomie“. Kritéria pro zařazení splnilo celkem 73 videí. Byly zaznamenány metriky zapojení diváků, jako je doba od nahrání, počet zhlédnutí, hodnocení Líbí se mi, Nelíbí se mi, komentáře a délka videa. Byly vypočteny poměry, jako je poměr zobrazení, podobný poměr a index výkonu videa (VPI – video power index). Videa byla kategorizována pomocí modifikované globální škály kvality (GQS – global quality scale) a hodnocena na základě bodovacího systému odvozeného ze standardizovaného 10krokového postupu hysterektomie vNOTES se skóre v rozmezí 0–15. Výsledky: Ze 73 videí bylo 40 (53,8 %) označeno jako nekvalitní, 13 (17,8 %) jako středně kvalitní a 20 (27,4 %) jako dobré. Mezi skupinami nebyly nalezeny žádné významné rozdíly, pokud jde o dobu od nahrání, počet zhlédnutí, hodnocení Nelíbí se mi, komentářů nebo poměru Líbí se mi. Videa ve skupině dobré kvality však měla výrazně vyšší počet lajků a skóre VPI. Kritické prvky, jako je příprava a polohování pacientky, nastavení operačního sálu, obřízka děložního čípku a uzávěr klenby, byly ve videích nižší kvality nedostatečně řešeny. Videa s didaktickým hlasem měla výrazně více zhlédnutí, lajků a komentářů než videa s hudbou nebo bez zvuku. Mezi délkou videa a metrikami zapojení nebyly nalezeny žádné významné korelace. Závěr: Většina videí o hysterektomii vNOTES (53,8 %) na YouTube postrádá komplexní vzdělávací obsah, přičemž pouze malá část je považována za vhodnou pro chirurgický výcvik. Hodnocení zájmu diváků nemusí souviset s mírou užitečnosti videí. Chirurgové a organizace by se měli zaměřit na produkci vysoce kvalitních instruktážních recenzovaných videí, aby zlepšili vzdělávací hodnotu YouTube jako zdroje informací.
Objective: This study aimed to assess the reliability and educational value of vaginal natural orifice transluminal endoscopic surgery (vNOTES) hysterectomy videos on YouTube and their suitability for training surgeons. Materials and methods: On June 12, 2024, YouTube was searched using the keywords “vNOTES hysterectomy,” “TVNOTES hysterectomy,” “transvaginal natural orifice transluminal endoscopic hysterectomy,” “vNOTES,” and “vaginal notes hysterectomy.” A total of 73 videos met the inclusion criteria. Viewer engagement metrics, such as time since upload, number of views, likes, dislikes, comments, and video duration were recorded. Ratios such as a view ratio, a like ratio, and Video Power Index (VPI) were calculated. The videos were categorized by the modified Global Quality Scale (GQS) and evaluated based on a scoring system derived from a standardized 10-step vNOTES hysterectomy procedure, with scores ranging from 0 to 15. Results: Out of 73 videos, 40 (53.8%) were categorized as poor quality, 13 (17.8%) as moderate, and 20 (27.4%) as good. No significant differences were found between groups in terms of time since upload, views, dislikes, comments, or a like ratio. However, videos in the good-quality group had a significantly higher number of likes and VPI scores. Critical elements such as patient preparation and positioning, setup of the operation room, circumcision of the cervix, and vault closure were inadequately addressed in lower-quality videos. Videos with a didactic voice had significantly more views, likes, and comments than those with music or no sound. No significant correlations were found between video length and engagement metrics. Conclusion: The majority of vNOTES hysterectomy videos (53.8%) on YouTube lack comprehensive educational content, with only a small fraction deemed appropriate for surgical training. The interest rates of the viewers may not be correlated with the usefulness rates of the videos. Surgeons and organizations should focus on producing high-quality, peer-reviewed instructional videos to improve the educational value of YouTube as a resource.

BACKGROUND: Neuromuscular diseases (NMDs) are rare disorders characterized by progressive muscle fibre loss, leading to replacement by fibrotic and fatty tissue, muscle weakness and disability. Early diagnosis is critical for therapeutic decisions, care planning and genetic counselling. Muscle magnetic resonance imaging (MRI) has emerged as a valuable diagnostic tool by identifying characteristic patterns of muscle involvement. However, the increasing complexity of these patterns complicates their interpretation, limiting their clinical utility. Additionally, multi-study data aggregation introduces heterogeneity challenges. This study presents a novel multi-study harmonization pipeline for muscle MRI and an AI-driven diagnostic tool to assist clinicians in identifying disease-specific muscle involvement patterns. METHODS: We developed a preprocessing pipeline to standardize MRI fat content across datasets, minimizing source bias. An ensemble of XGBoost models was trained to classify patients based on intramuscular fat replacement, age at MRI and sex. The SHapley Additive exPlanations (SHAP) framework was adapted to analyse model predictions and identify disease-specific muscle involvement patterns. To address class imbalance, training and evaluation were conducted using class-balanced metrics. The model's performance was compared against four expert clinicians using 14 previously unseen MRI scans. RESULTS: Using our harmonization approach, we curated a dataset of 2961 MRI samples from genetically confirmed cases of 20 paediatric and adult NMDs. The model achieved a balanced accuracy of 64.8% ± 3.4%, with a weighted top-3 accuracy of 84.7% ± 1.8% and top-5 accuracy of 90.2% ± 2.4%. It also identified key features relevant for differential diagnosis, aiding clinical decision-making. Compared to four expert clinicians, the model obtained the highest top-3 accuracy (75.0% ± 4.8%). The diagnostic tool has been implemented as a free web platform, providing global access to the medical community. CONCLUSIONS: The application of AI in muscle MRI for NMD diagnosis remains underexplored due to data scarcity. This study introduces a framework for dataset harmonization, enabling advanced computational techniques. Our findings demonstrate the potential of AI-based approaches to enhance differential diagnosis by identifying disease-specific muscle involvement patterns. The developed tool surpasses expert performance in diagnostic ranking and is accessible to clinicians worldwide via the Myo-Guide online platform.

Mobilní aplikace umožnují dnes pacientům monitorovat aktuální pylovou situaci, zaznamenávat vlastní klinické obtíže a lépe dodržovat compliance k léčbě. Klinické projevy pylové alergie zahrnují nejčastěji alergickou rhinokonjunktivitidu. V příspěvku zmiňuji zásady symptomatické farmakoterapie alergické rýmy a kauzální léčbu - specifickou alergenovou imunoterapii.
Pollen season lasts longer because of its earlier start due to recent climate changes. Mobile apps enable pollen monitoring, help patients describe their clinical symptoms, and improve compliance with therapy. Allergic rhinoconjunctivitis is the most frequent clinical symptom. This paper provides info about symptomatic pharmacotherapy as well as about causal treatment- about specific allergen immunotherapy.

The evolving gambling environment, marked by increased accessibility and innovative promotions, has led to rising expenditures globally. Despite this, the relationship between social media use and problem gambling in the Czech Republic remains underexplored. To investigate the association between social media use and problem gambling severity among Czech adults using the Problem Gambling Severity Index (PGSI). Is there a difference in gambling severity based on demographic characteristics? (1); Is there a relationship between social media news consumption and gambling severity? (2); Is there a connection between negative motives for social media use and problematic usage? (3); Is there a link between problematic social media use and gambling severity? (4). Primary research, designed as a cross-sectional study, was conducted in the Czech Republic in November 2024 on a sample of 3093 respondents (48.4% male). All participants were aged 16 years or older. The sample was selected using quota sampling based on multiple identifying quota variables, with minimal differences in the proportions of quota indicators compared to the general population. Non-parametric tests and ordinal logistic regression analyzed relationships between demographics, social media use, and PGSI scores. Higher PGSI scores were found among males, younger participants, those with lower education, and the unemployed or students. Greater social media news consumption correlated with increased gambling severity. Disruptive social media behaviors, such as waking up to check notifications and interrupting activities, were significantly associated with higher PGSI scores. Problematic social media use involving deceit and family conflict substantially heightened the risk of severe gambling problems. These findings highlight the need for targeted prevention initiatives, regulatory policies, and enhanced digital literacy to safeguard vulnerable groups from the risks posed by digital gambling platforms. Addressing both gambling behaviors and problematic social media use is crucial in mitigating potential harms.

Sociální sítě se staly běžnou součástí našich životů. Jejich možný vliv na lidskou psychiku, obzvláště u dětí a dospívajících, se stává opakovaně předmětem vzrušených debat jak u laické, tak odborné veřejnosti. některé aspekty sociálních sítí mohou mít negativní dopady na psychiku. Často zmiňované je sociální srovnávání, zejména srovnávání se s těmi, kteří jsou vnímáni jako úspěšnější, což může vést k pocitům méněcennosti a frustrace. dalším negativním faktorem je strach z promeškání, tedy obava, že ostatní prožívají zajímavější a hodnotnější události než my sami. tento strach může vést k úzkosti a stresu. Za problematické je považováno i nadměrné užívání sociálních sítí, které se projevuje silným zaujetím, neschopností kontrolovat čas strávený na sítích a negativním dopadem na důležité oblasti života, jako jsou vztahy, práce nebo studium. sociální sítě však mohou mít i pozitivní vliv na psychiku. Budování sociálního kapitálu, tedy sítě vztahů a kontaktů, může vést k pocitům sounáležitosti a podpory, a tím přispívat k celkové psychické pohodě. Výzkumy zaměřené na vliv sociálních sítí na psychiku dospívajících přinášejí mnohdy rozporuplné výsledky. některé naznačují mírný negativní vliv, jako je nárůst deprese a úzkosti, zatímco jiné poukazují na mírný pozitivní vliv, jako je vyšší spokojenost se životem. Vliv sociálních sítí se liší v závislosti na individuálních, sociálních a vývojových faktorech. některé studie poukazují na existenci vývojově podmíněných tzv. období citlivosti, během nichž je vliv sociálních sítí na psychiku dospívajících významnější. další výzkumy prokázaly souvislost mezi užíváním sociálních sítí a rizikovým chováním, jako je sebepoškozování a poruchy příjmu potravy. Účinnost intervencí zaměřených na omezení používání sociálních sítí, jako je například „digitální detox“, je nejasná. dosavadní studie naznačují, že pozitivnější vliv na celkovou psychickou pohodu má zkrácení času stráveného na sociálních sítích, nikoliv jejich úplné opuštění.
Social networking sites have become a reality in our lives. their potential impact on the human psyche, especially for children and adolescents, has repeatedly become a subject of enthusiastic debate among both the general public and academics. some aspects of social networking can have negative effects on the psyche. social comparison is often cited, particularly comparison with those who are perceived to be more successful, which can lead to feelings of inferiority and frustration. another negative factor is the fear of missing out, i.e. the fear that others are experiencing more interesting and worthwhile events than oneself. this fear can lead to anxiety and stress. excessive use of social networking sites is also considered problematic, with strong preoccupations, an inability to control time spent on the sites and a negative impact on important areas of life such as relationships, work or study. However, social networking can also have a positive effect on the psyche. Building social capital, i.e. a network of relationships and contacts, can lead to feelings of belonging and support, thus contributing to overall psychological well-being. research on the impact of social networks on adolescents‘ psyche has often produced contradictory results. some suggest a slight negative effect, such as an increase in depression and anxiety, while others point to a slight positive effect, such as increased life satisfaction. the impact of social networks varies depending on individual, social and developmental factors. some studies point to the existence of ‚developmental windows of sensitivity‘ during which the influence of social networks on the adolescent psyche is more significant. other research has shown a link between social networking use and risky behaviors such as self-harm and eating disorders. the effectiveness of interventions aimed at reducing social networking use, such as ‚digital detox‘, is still unclear. studies to date suggest that reducing time spent on social networking sites has a more positive effect on overall psychological well-being than leaving them altogether.

As the green transformation sweeps across industries in the digital age, tourism stakeholders face a pressing need to utilize online platforms and digital influencers for sustainability messaging. Drawing on the Motivation-Opportunity-Ability framework, this study investigates the influence of green ownership psychology and green knowledge sharing on tourists' repatronage intentions. It focuses on the mediating role of cooperative green activity programs and the moderating impact of tourists' green trust in social media influencers. Results from a two-wave survey of 602 tourists in Vietnam show that green practices directly increase repatronage intentions. Cooperative green activity programs significantly mediate this relationship, enhancing the impact of green marketing practices on repatronage intentions. Furthermore, tourists' green trust in social media influencers is a crucial moderating factor. It increases the effectiveness of green ownership psychology and green knowledge sharing in influencing repatronage intentions through cooperative green activity programs. The study highlights how the emotional and cognitive aspects of green marketing contribute to tourists' decisions to revisit or continue using tourism services. Offering original insights into the interplay of these elements in green marketing within the tourism industry, this research enriches the application of the Motivation-Opportunity-Ability framework. It provides valuable implications for tourism companies aiming to foster sustainable practices and enhance customer loyalty, highlighting the role of digital influencer engagement and cooperative sustainability initiatives in green marketing strategies.

INTRODUCTION: Initial evidence suggests that engaging with accepting communities on social media such as Instagram may inform sexual minority youths' sense of stigma and well-being. However, as existing research has predominately drawn upon cross-sectional or qualitative designs, it is currently unclear whether the positive experiences identified in previous research accumulate, endure, or evolve over time. We also know relatively little about whether engagement with accepting online communities is primarily a compensatory or enhancing behavior. Thus, drawing upon minority stress theory and broaden-and-build theory, this study explores the longitudinal reciprocal relationships between perceived stigma, well-being, and engagement with accepting Instagram communities. METHOD: Three-wave panel data were collected from 460 sexual minority youth in the United States and Poland (M age = 18.58, SD = 1.64), and data were analyzed using a random intercept cross-lagged panel model. RESULTS: At the between-person level, engagement with accepting Instagram communities was positively associated with perceived stigma and negatively associated with well-being. No significant within-person associations emerged between perceived stigma and engagement with accepting Instagram networks. However, a positive reciprocal relationship was found between well-being and engagement with accepting Instagram communities. Cultural context had no moderating effect on the hypothesized model. CONCLUSIONS: Results suggest that whilst the interrelations between perceived stigma and engaging with accepting online networks may be short-lived, engaging with supportive Instagram communities may contribute to an upward spiral of positive emotions. Findings therefore extend the existing literature regarding the potential benefits of social media use amongst sexual minority youth.

The European Chemical Biology Database (ECBD, https://ecbd.eu) serves as the central repository for data generated by the EU-OPENSCREEN research infrastructure consortium. It is developed according to FAIR principles, which emphasize findability, accessibility, interoperability and reusability of data. This data is made available to the scientific community following open access principles. The ECBD stores both positive and negative results from the entire chemical biology project pipeline, including data from primary or counter-screening assays. The assays utilize a defined and diverse library of over 107 000 compounds, the annotations of which are continuously enriched by external user supported screening projects and by internal EU-OPENSCREEN bioprofiling efforts. These compounds were screened in 89 currently deposited datasets (assays), with 48 already being publicly accessible, while the remaining will be published after a publication embargo period of up to 3 years. Together these datasets encompass ∼4.3 million experimental data points. All public data within ECBD can be accessed through its user interface, API or by database dump under the CC-BY 4.0 license.
