INTRODUCTION: Currently, it is possible to preserve the auditory nerve in a large number of cases, but the preservation of the hearing itself is unpredictable. Apart from wait and scan strategy and stereoradiotherapy, hearing after vestibular schwannoma surgery is considered to remain stable even in long-term follow-up. MATERIALS AND METHODS: Twenty-eight patients had preserved hearing after retrosigmoid suboccipital microsurgery of the vestibular schwannoma between 2008 and 2014. A standard audiological protocol was performed together with an magnetic resonance imaging evaluation of the fluid content of the inner ear. RESULTS: The mean difference in pure-tone average between the direct and final postsurgical examination was 12.758 dB ( p = 2.5E - 06). The word recognition score deteriorated by 17.45% ( p = 0.03516). The mean American Academy of Otolaryngology-Head and Neck Surgery score on the second examination was 2.5, and that on the second examination was 3.111 ( p = 0.00483). There was no significant deterioration in the healthy ear.The signal intensity ratio in the basal turn of the cochlea increased by an average of 0.13 points ( p < 0.05).Patients with persistent tumor or nodular enhancement in the internal acoustic meatus deteriorated significantly in hearing according to the American Academy of Otolaryngology-Head and Neck Surgery scale compared with patients without any finding in the meatus ( p = 0.01299). CONCLUSIONS: There is a discrete but gradual deterioration of the hearing in the postoperative period. Hearing impairment is more pronounced in patients with a nodular process in the internal acoustic meatus, regardless of whether it is growth active. After surgery, the pathological content of the inner ear normalizes (evaluated on T2 magnetic resonance imaging sequences).

Cíl: Nedoslýchavost má významný dopad na mezilidskou komunikaci. Jedná se o příznak, který může být způsoben řadou rizikových faktorů. Jedním z nich je kouření. Cílem práce je zjistit, zdali existuje souvislost mezi kouřením a nedoslýchavostí a určit míru asociace mezi kouřením a nedoslýchavostí spolu s proměnnými, jako je pohlaví a věk. Metodika: Audiometricky byly srovnávány prahy sluchu na frekvencích 500, 1 000, 2 000, 4 000, 6 000 a 8 000 Hz dle věku. Výskyt nedoslýchavosti a sluchové prahy byly porovnávány podle věkových skupin a úrovně kouření dle Brinkmanova indexu (BI). Průměrné ztráty sluchu na jednotlivých frekvencích v souvislosti s kouřením byly porovnány mezi muži a ženami ve 4 věkových skupinách. Výsledky: Dle významných statistických rozdílů byl pozorován vliv kouření na práh sluchu častěji u žen než u mužů. Vliv kouření na prahy sluchu byl statisticky prokázán u mužů s BI ≥ 400 ve věku 20-44 let na frekvencích 500-2 000 Hz a ve věkové skupině 45-54 let na frekvencích 1000-6 000 Hz, u mužů s BI ≤ 400 ve věkové skupině 55-64 let a 65-74 let na frekvenci 500 Hz. U žen s BI ≥ 400 byl vliv kouření na prahy sluchu prokázán u skupiny 45-54 let na frekvencích 500, 2 000, 4 000, 6 000 a 8 000 Hz, u skupiny 55-64 let s BI ≤ 400 na frekvencích 500, 1 000, 4 000, 6 000 a 8 000 Hz a u skupiny 65-74 let s BI ≥ 400 na frekvencích 500-4 000 Hz. Závěr: Incidence nedoslýchavosti se zvyšuje a stává se aktuálním globálním problémem. Vzniká z multifaktoriálních příčin, a proto je nutné zaměřit se na ty, které jsou dobře preventabilní, jako je hluk a kouření.
Objective: Hearing loss has a significant impact on interpersonal communication. It is a symptom that can be caused by many risk factors, including smoking. This work has aimed to determine whether there is a connection between smoking and hearing impairment and to determine the degree of association, considering variables such as gender and age. Methodology: Hearing thresholds at frequencies of 500, 1,000, 2,000, 4,000, 6,000, and 8,000 Hz were compared using audiometry. Prevalences of hearing loss occurrence and hearing thresholds were compared according to age groups and smoking levels determined by the Brinkman index (BI). The average hearing thresholds at individual frequencies depending on smoking were compared between men and women divided in four different age groups. Results: According to significant statistical differences, the effect of smoking on hearing thresholds was observed more often in women than in men. The effect of smoking on hearing loss was statistically proven in men with BI ≥ 400 aged 20-44 years at frequencies of 500-2,000 Hz, in men aged 45-54 years at frequencies of 1,000-6,000 Hz, and in men with BI ≤ 400 aged 55-64 years and 65-74 years at 500 Hz. In women with BI ≥ 400, the effect of smoking on hearing thresholds was detected in the 45-54 age group at frequencies of 500, 2,000, 4,000, 6,000, and 8,000 Hz, in women with BI ≤ 400 aged 55-64 years at 500, 1,000, 4,000, 6,000, and 8,000 Hz, and in women with BI ≥ 400 aged 65-74 years at frequencies of 500-4,000 Hz. Conclusion: The incidence of hearing loss is increasing and it is a problem of global extent. As it can be caused by multifactorial causes, it is necessary to focus on those that are well preventable, such as noise and smoking.

Porucha sluchu může provázet nejen onemocnění zevního, středního a vnitřního ucha, ale též sluchových drah a center mozku. Diagnostika topiky léze se opírá o otorinolaryngologické vyšetření doplněné širokým spektrem audiologických metod. Možnosti medikamentózní a chirurgické intervence jsou omezené. Pokud jich nelze využít nebo jsou neúčinné, korigujeme sluch přístrojově. K dispozici je bohaté portfolio sluchadel, pomůcek pro kostní vedení a přímou stimulaci sluchového nervu (kochleární implantát). Preskripce sluchových pomůcek je ošetřena legislativně.
Hearing impairment occur not only external, middle and inner ear diseases, but also auditory pathways and brain centers. Diagnosis is based on otorinolaryngological examination supplemented by a wide range of audiological methods. The options for medication and surgical intervention are limited. If they can not be used or are ineffective, the hearing loss can be corrected by a rich portfolio of hearing aids, bone conduction aids and cochlear implants. Prescription of hearing aids is treated legislatively.

AIMS: Patients with the Pendred syndrome suffer very often from a hearing loss. They may be good candidates for a cochlear implantation, but unfortunately, due to the fluctuating character of the hearing loss, they may escape such indication. In the study, we compared speech production and speech acquisition in 2 groups of implanted patients: those with the Pendred syndrome, and standard non-syndromic patients. METHODS: Ten patients with Pendred syndrome were analyzed for speech perception and production. The control group consisted of 41 non-syndromic implanted patients. All implantees were scored according to speech perception, speech production, and the sum of both. The data were statistically analyzed. RESULTS: No statistical difference was found in language acquisition and production in implantees with Pendred syndrome when compared to non-syndromic patients with cochlear implants. Nor there was any difference in speech production and acquisition between the 2 compared groups regarding surgical age, time elapsed after surgery, or age during the testing. CONCLUSION: In this study evaluating language and speech production and acquisition, patients with Pendred syndrome who underwent cochlear implants show comparable results to their implanted peers with deafness of a different etiology.

Invertovaný papilom středouší je vzácným nálezem. Jedná se o benigní, avšak lokálně agresivně rostoucí tumor s tendencí k malignímu zvratu. Diagnóza je obvykle stanovena až pooperačně podle definitivního histologického vyšetření, protože před- ani peroperační nález není patognomický a imponuje jako zánětlivé změny při chronickém středoušním zánětu. Léčbou volby je chirurgická terapie, radioterapie jako léčebná modalita je vyhrazena pro inoperabilní tumory, popřípadě recidivy. V naší kazuistice uvádíme případ 62letého muže, který podstoupil sanační operaci před mnoha lety ve spádové nemocnici z indikace chronického středoušního zánětu. Na naší klinice byl vyšetřován pro pulzatilní tinitus a zhoršení sluchu. Během provedené tympanoplastiky z kombinovaného přístupu nebyl očekávaný tumor (pouze tuhé jizvy) nalezen, a proto byla provedena sanace středoušní dutiny. Až definitivní histologické vyšetření ukázalo invertovaný papilom. Po operaci indikované CT s kontrastní látkou prokázalo perzistenci tumoru. Byla indikována radikální mastoidektomie, která proběhla bez komplikací. Pacient je nyní 32 měsíců v kompletní remisi, bez chronických zánětlivých změn.
Inverted papilloma is a very rare finding in the middle ear. It is a benign, though locally aggressive tumour with a tendency towards malignant transformation. The diagnosis is usually established postoperatively, based on the histological examination, because the local finding is not specific and resembles chronically inflamed tissue in chronic otitis media. Surgery is the treatment of choice. Radiotherapy is reserved for inoperable tumours or a recurrent disease. In this case report, we present a case of a 62-year-old man, who underwent canal wall down tympanoplasty for chronic otitis media in a district hospital many years ago. A few years ago he was examined for pulsatile tinnitus and hearing impairment at our clinic. A combined-approach tympanoplasty did not reveal the expected tumour (only stiff scars) and a clearance of the middle ear cavity was performed. Histological examination revealed an inverted papilloma. A CT scan performed postoperatively proved a tumour persistence. Therefore, a radical mastoidectomy was indicated. The patient is disease-free with no signs of recurrence during 32-month follow-up.

INTRODUCTION: To systematically investigate all surgeries for cholesteatoma behind an intact tympanic membrane at our department. To identify predictive factors that can help the surgeon to plan surgery, surgical techniques, and follow-up treatment. MATERIAL AND METHODS: This retrospective study evaluates 21 child patients, who were operated in the period 2007-2017 on for cholesteatoma behind an intact tympanic membrane. RESULTS: A total of 202 primary operations were performed for cholesteatoma. In 21 cases (10,4%) there was a cholesteatoma behind an intact tympanic membrane and in 11 (5,45%) cases of it there was the congenital cholesteatoma. The most frequently affected area was the anterior-superior quadrant. The preoperative hearing loss increased significantly with disease severity (I-IV by Potsic). CONCLUSIONS: The classification system according to Potsic is sufficient and fully corresponds to the surgeon's needs. It has been clearly shown that a higher CC stage is associated with worse postoperative hearing results.

OBJECTIVES: The mucopolysaccharidoses (MPS) are inherited lysosomal storage disorders with multisystemic and highly variable clinical manifestation. ENT symptoms are common and early signs of MPS. The most common ENT diagnoses are chronic/recurrent rhinosinusitis, acute otitis media, otitis media with effusion, hearing loss and airway obstruction. METHODS: A single-centre retrospective chart review of 61 patients (36 M/25F) with different MPS subtypes (MPS I (n = 15), MPS II (n = 10), MPS III (n = 17), MPS IV (n = 15) and MPS VI (n = 4)) was conducted. The age of ENT presentation and frequency of ENT symptoms, surgeries and their distribution among MPS subtypes was studied. The relationship between ENT presentation, first ENT surgery and the age of diagnosis was also evaluated. RESULTS: Median age at the first ENT manifestation was 2.8 years, median age at MPS diagnosis 4.1 years. The great majority of patients (90%) manifested at least one ENT diagnosis; often before the diagnosis of MPS (75%). Chronic/recurrent rhinosinusitis was the most prevalent ENT diagnosis (77%), followed by upper airway obstruction (65%) and hearing loss (53%). Chronic/recurrent rhinosinusitis was the first ENT symptom to appear (median age 2.2 years), followed by otitis media with effusion (3.7 years) and hearing loss (4.5 years). At least one ENT surgery was performed in 57% of patients; in 69% before MPS diagnosis was established. Median age of the first ENT surgery was 4.1 years. ENT symptoms and surgical procedures were earliest present in MPS II. CONCLUSIONS: Our study documents high and early occurrence of various otolaryngologic symptoms in MPS and thus highlights the role of ENT specialist in prompt diagnosis of these rare diseases and their long-term management.
