BACKGROUND: During otosclerosis surgery, operative trauma can lead to decreased bone conduction. AIMS: The study aims to observe the bone conduction changes after otosclerosis operations and analyse possible factors affecting the postoperative decrease in bone conduction. MATERIAL AND METHODS: Authors retrospectively processed the data of 109 patients and evaluated pure tone audiometry before surgery and consequently 2 days, 1 month and 1 year after surgery. RESULTS: We noted a deterioration of bone conduction >5 dB on the second postoperative day in 28% (30/109) of patients, which persisted one year after the surgery in 9% (10/109) cases. Analysis of individual factors affecting bone conduction loss revealed a higher risk of permanent loss of bone conduction in patients with early postoperative loss in higher frequencies, in older patients and patients with a preoperative threshold of bone conduction >20 dB. Revision surgery was not a statistically significant factor. CONCLUSION AND SIGNIFICANCE: The bone conduction decrease after otosclerosis surgery is usually temporary. The recovery of bone conduction is influenced by the age of patients and the level of bone conduction before the surgery. The early postoperative decrease of bone conduction in higher frequencies is a negative predictive factor for permanent hearing loss.

Unilateral otosclerosis combined with avascular necrosis of stapes crura is a rare entity. It should be considered in a case of high grade otosclerosis. Symptoms are the same as in patients who suffer from common otosclerosis. Patients complain on progressive hearing loss and tinnitus. The diagnosis is made clinically by conventional audiologic evaluation and radiologically by x-ray mastoid Schuller's view and CT scan. HRCT scan makes visible all parts of ossicular chain and gives surgeon some information about ossicular chain damage. Surgery with stapedotomy and stapes prosthesis implantation in a case of otosclerosis with avascular necrosis of stapes crura can be success therapy to improve patient's hearing

The most frequently used technique of surgery of otosclerosis used during the past ten years in the Central Military Hospital was Schuknecht's technique of wire and fat. 575 thus operated patients account for 89.4% of the total number of 643 operations. A compensation of the bone reserve to 0-10 dB was achieved in the late results of 56.3% of the operated patients, in another 30% there were auditory gains of speech frequencies, on average above 10 dB. Favourable late results were thus achieved in 86.3% of the operated patients, deterioration of hearing was recorded in 4.8% (13 operated patients). Pistons of Schuknecht's type were used in 23 patients with the obliterating form of otosclerosis. As to late results, in 21 of the evaluated patients compensation of the bone reserve to 0-10 dB was recorded in 28.6%. The mean auditory gains as regards speech frequencies higher than 10 dB were achieved in 47.6%. Favourable late results were thus recorded in 76.2% of operated patients with the obliterating form of otosclerosis. Deterioration of hearing occurred in this group in one operated patient (4.8%).

At the Otolaryngological Clinic, Faculty Hospital Bratislava since 1971 to 1987 673 microsurgical operations on the stapes were made in patients with otosclerosis, incl. 73 using the method of stapedotomy and interposition of a teflon piston. The method was very successful. Hearing improved in 91.7% of the patients. In no instance marked deterioration of hearing or deafness developed. The surgical operation is associated with a smaller risk of damage to the inner ear. Patients tolerate stapedotomy well and the results are better than after stapedoplasties according to Schuknecht. The decibel gain is more even and at higher frequencies more marked than after stapedectomy. When the piston is used, greater precision and manual skill of the surgeon is essential, as compared with stapedectomy.
