Survival analysis is commonly conducted in medical and public health research to assess the association of an exposure or intervention with a hard end outcome such as mortality. The Cox (proportional hazards) regression model is probably the most popular statistical tool used in this context. However, when the exposure includes compositional covariables (that is, variables representing a relative makeup such as a nutritional or physical activity behaviour composition), some basic assumptions of the Cox regression model and associated significance tests are violated. Compositional variables involve an intrinsic interplay between one another which precludes results and conclusions based on considering them in isolation as is ordinarily done. In this work, we introduce a formulation of the Cox regression model in terms of log-ratio coordinates which suitably deals with the constraints of compositional covariates, facilitates the use of common statistical inference methods, and allows for scientifically meaningful interpretations. We illustrate its practical application to a public health problem: the estimation of the mortality hazard associated with the composition of daily activity behaviour (physical activity, sitting time and sleep) using data from the U.S. National Health and Nutrition Examination Survey (NHANES).

There is a long history of mathematical and computational modelling with the objective of understanding the mechanisms governing cartilage׳s remarkable mechanical performance. Nonetheless, despite sophisticated modelling development, simulations of cartilage have consistently lagged behind structural knowledge and thus the relationship between structure and function in cartilage is not fully understood. However, in the most recent generation of studies, there is an emerging confluence between our structural knowledge and the structure represented in cartilage modelling. This raises the prospect of further refinement in our understanding of cartilage function and also the initiation of an engineering-level understanding for how structural degradation and ageing relates to cartilage dysfunction and pathology, as well as informing the potential design of prospective interventions. Aimed at researchers entering the field of cartilage modelling, we thus review the basic principles of cartilage models, discussing the underlying physics and assumptions in relatively simple settings, whilst presenting the derivation of relatively parsimonious multiphase cartilage models consistent with our discussions. We proceed to consider modern developments that start aligning the structure captured in the models with observed complexities. This emphasises the challenges associated with constitutive relations, boundary conditions, parameter estimation and validation in cartilage modelling programmes. Consequently, we further detail how both experimental interrogations and modelling developments can be utilised to investigate and reduce such difficulties before summarising how cartilage modelling initiatives may improve our understanding of cartilage ageing, pathology and intervention.

Low levels of physical activity (PA) during adolescence contribute to obesity and poor health outcomes in adolescence, and these associations endure into adulthood. The aim of this study was to assess the associations between motives for PA and the level of PA among adolescent boys and girls. We obtained data regarding motives for PA and frequency of PA in 2010 via the Health Behavior in School-aged Children cross-sectional study in the Czech and Slovak Republics (n = 9018, mean age = 13.6, 49% boys). Respondents answered questions about their motives for PA and the frequency of their PA. Motives for PA were assessed using 13 items, which were structured in four groups. We explored the association between the motives for PA and sufficient PA using univariate and multivariate logistic regression models adjusted for age, and separately for boys and girls. "Good child" motives and Achievement motives were significantly associated with sufficient PA among both boys and girls. Health motives were associated with sufficient PA only among boys, and Social motives were associated with sufficient PA only among girls. Motives for PA were associated with the level of PA, and this association was partially gender dependent. These gender differences should be considered in interventions focusing on enhancement of PA.

The article presents the results of research carried out using a water model of a refining ladle for the Fe-Si ferroalloys treatment. These studies were aimed at improving the efficiency of refining and homogenization of liquid Fe-Si ferroalloy in the refining ladle by using a new method of blowing gas through a system of nozzles installed at the bottom of the ladle. The obtained results allowed to determine the proper location of the plug at the bottom of the refining ladle and the possibility of using combined blowing. The tests were carried out for a refining ladle with a capacity of 3 m3 using a physical model on a linear scale of 1:3. The gas flow rate used in the model corresponded proportionally to the value previously used in industrial practice and amounted to 26.8 l/min. Experiments were performed for both combined blowing applications and through a purging plug at the bottom of the ladle. In the case of combined blowing, the volume of the gas stream was divided into two blowing sources (lance and purging plug). As a result of laboratory tests, one of the variants was selected and tested in industrial conditions. These studies confirmed the improvement in the efficiency of refining treatment of the FeSi alloy in terms of reducing the carbon and aluminum content in the alloy.x.

How people use their time has been linked with their health. For example, spending more time being physically active is known to be beneficial for health, whereas long durations of sitting have been associated with unfavourable health outcomes. Accordingly, public health messages have advocated swapping strategies to promote the reallocation of time between parts of the time-use composition, such as "Move More, Sit Less", with the aim of achieving optimal distribution of time for health. However, the majority of research underpinning these public health messages has not considered daily time use as a composition, and has ignored the relative nature of time-use data. We present a way of applying compositional data analysis to estimate change in a health outcome when fixed durations of time are reallocated from one part of a particular time-use composition to another, while the remaining parts are kept constant, based on a multiple linear regression model on isometric log ratio coordinates. In an example, we examine the expected differences in Body Mass Index z-scores for reallocations of time between sleep, physical activity and sedentary behaviour.

BACKGROUND: Coronary heart disease (CHD) death rates have fallen across most of Europe in recent decades. However, substantial risk factor reductions have not been achieved across all Europe. Our aim was to quantify the potential impact of future policy scenarios on diet and lifestyle on CHD mortality in 9 European countries. METHODS: We updated the previously validated IMPACT CHD models in 9 European countries and extended them to 2010-11 (the baseline year) to predict reductions in CHD mortality to 2020(ages 25-74years). We compared three scenarios: conservative, intermediate and optimistic on smoking prevalence (absolute decreases of 5%, 10% and 15%); saturated fat intake (1%, 2% and 3% absolute decreases in % energy intake, replaced by unsaturated fats); salt (relative decreases of 10%, 20% and 30%), and physical inactivity (absolute decreases of 5%, 10% and 15%). Probabilistic sensitivity analyses were conducted. RESULTS: Under the conservative, intermediate and optimistic scenarios, we estimated 10.8% (95% CI: 7.3-14.0), 20.7% (95% CI: 15.6-25.2) and 29.1% (95% CI: 22.6-35.0) fewer CHD deaths in 2020. For the optimistic scenario, 15% absolute reductions in smoking could decrease CHD deaths by 8.9%-11.6%, Salt intake relative reductions of 30% by approximately 5.9-8.9%; 3% reductions in saturated fat intake by 6.3-7.5%, and 15% absolute increases in physical activity by 3.7-5.3%. CONCLUSIONS: Modest and feasible policy-based reductions in cardiovascular risk factors (already been achieved in some other countries) could translate into substantial reductions in future CHD deaths across Europe. However, this would require the European Union to more effectively implement powerful evidence-based prevention policies.

Concern for the environment and rational management of resources requires the development of recoverable methods of obtaining metallic materials. This also applies to the production of aluminium and its alloys. The quality requirements of the market drive aluminium producers to use effective refining methods, and one of the most commonly used is blowing an inert gas into liquid aluminium via a rotating impeller. The efficiency and cost of this treatment depends largely on the application of the correct ratios between the basic parameters of the process, which are the flow rate of the inert gas, the speed of the rotor and the duration of the process. Determining these ratios in production conditions is expensive and difficult. This article presents the results of research aimed at determining the optimal ratio of the inert gas flow rate to the rotary impeller speed, using physical modeling techniques for the rotor as used in industrial conditions. The tests were carried out for rotary impeller speeds from 150 to 550 rpm and gas flow rates of 12, 17 and 22 dm3/min. The research was carried out on a 1:1 scale physical model, and the results, in the form of visualization of the degree of gas-bubble dispersion, were assessed on the basis of the five typical dispersion patterns. The removal of oxygen from water was carried out analogously to the process of removing hydrogen from aluminium. The curves of the rate of oxygen removal from the model liquid were determined, showing the course of oxygen reduction during refining with the same inert gas flows and rotor speeds mentioned above.

Many school-based physical activity (PA) interventions have been developed, but only a few have assessed their long-term effects. A PA intervention taking place in the first four years of some Slovenian primary schools entails an enhanced physical education (PE) curriculum, including two extra lessons of PE per week, a wider selection of PE content, and additional outdoor education delivered by both a specialist PE teacher and a general teacher. The effects of the intervention on children's physical fitness (motor tasks and anthropometry) were evaluated within a quasi-experimental study. In total, 324 children from nine Slovenian primary schools either received the enhanced curriculum (intervention (n=160)) or standard PE (control (n=164)), and were followed for a four-year intervention period and seven years post intervention. Data from the SLOFIT database were used to compare differences in the physical fitness of children each year. Linear Mixed Models were used to test the influence of the PA intervention. Over an 11-year period, the PA intervention group significantly differed in all motor tasks, but not in anthropometric measures or body mass index, after controlling for year of measurement and sex. Differences between the control and intervention groups decreased with time. This study highlights the importance of tracking the long term effects of PA interventions. PA intervention in the first four years of Slovenian primary school offers the possibility of improving physical performance in children; initiatives aiming to increase their performance (physical fitness, physical activity) and health outcomes are warranted.

The magnetic signatures of ocean M 2 tides have been successfully detected by the low-orbit satellite missions CHAMP and Swarm. They have been also used to constrain the electrical conductivity in the uppermost regions of the Earth's mantle. Here, we concentrate on the problem of accurate numerical modelling of tidally induced magnetic field, using two different three-dimensional approaches: the contraction integral equation method and the spherical harmonic-finite element method. In particular, we discuss the effects of numerical resolution, self-induction, the galvanic and inductive coupling between the oceans and the underlying mantle. We also study the applicability of a simplified two-dimensional approximation, where the ocean is approximated by a single layer with vertically averaged conductivity and tidal forcing. We demonstrate that the two-dimensional approach is sufficient to predict the large-scale tidal signals observable on the satellite altitude. However, for accurate predictions of M 2 tidal signals in the areas with significant variations of bathymetry, and close to the coastlines, full three-dimensional calculations are required. The ocean-mantle electromagnetic coupling has to be treated in the full complexity, including the toroidal magnetic field generated by the vertical currents flowing from and into the mantle.
