Enterobacterales produkující karbapenemázu (CPE – Carbapenemase-Producing Enterobacterales) představují globální zdravotnický problém. V Evropě dochází zejména v jižních a východních státech ke zvyšování podílu enterobakterií rezistentních vůči karbapenemům. Od roku 2021 roste také v České republice výskyt kmenů s produkcí karbapenemázy, změnila se i struktura zastoupení jednotlivých enzymů, zejména ve prospěch metalobetalaktamázy typu NDM. Tato situace je zřejmě následkem epidemie covid-19 a zhoršenou epidemickou situací ve zdravotnických zařízeních.
Carbapenemase-producing Enterobacterales (CPE) are a global health problem. In Europe, the proportion of carbapenem resistant enterobacteria is increasing, particularly in the southern and eastern countries. Since 2021, there has also been a widespread increase in the occurrence of CPE in the Czech Republic, the structure of carbapenemases has also shifted towards higher proportion of the NDM type. This situation is apparently the result of the COVID-19 epidemic and the worsened epidemic situation in healthcare facilities.

The isolation of Planococcus glaciei (designed strain CNCTC 7660) from blood of a patient with appendicitis is reported. Species P. glaciei (type strain CGMCC 1.6846 T) was for the first time identified as an environmental bacterium acquired from a glacier in China in 2009. To reveal the identity of the isolate CNCTC 7660, the 16S rDNA sequencing and the whole genome sequencing (Illumina MiSeq, Oxford Nanopore) were performed. The level of 16S rDNA gene sequencing similarity between CNCTC 7660 and CGMCC 1.6846 T was 99.55%. Phylogenetic analysis and average nucleotide analysis (ANI) based on the whole genome sequencing confirmed that the isolate CNCTC 7660 and CGMCC1.6846 T had ANI value above the taxonomic threshold for belonging to the same species (95%). The G + C content of CNCTC 7660 DNA was 46.8% (mol/mol). Except for the growth temperature, strains CGMCC1.6846 T and CNCTC 7660 were distinguished also biochemically. Due to the lack of information about the pathogenicity of P. glaciei, the possibility that it exerts pathogenicity in persons is suggested. But for understanding the nature of this species, further cases are needed.

V letošním roce uplyne 75 let od založení České národní sbírky typových kultur (CNCTC), která působí v rámci Centra epidemiologie a mikrobiologie ve Státním zdravotním ústavu v Praze. CNCTC byla oficiálně ustanovena v r. 1947 jako centrální sbírka lokálních souborů kultur spravovaných jednotlivými národními referenčními laboratořemi, navázala na tradici sbírky kultur, založené v SZÚ na konci 20. let minulého století. Prvním kurátorem byl doc. Juraj Strauss, za jehož působení byly do sbírky zařazeny typové kmeny získané ze zahraničních sbírek, zavedena jednotná dokumentace a evidence kultur a byly uplatněny moderní metody konzervace kmenů (lyofilizace). Dalším kurátorem sbírky se stal dr. Jiří Šourek, který vedl sbírku přes 40 let a významně ji rozšířil až na 5000 kmenů. Pod jeho vedením byla sbírka už s akronymem CNCTC registrována ve Světové federaci sbírek kultur (WFCC) a v Evropské organizaci sbírek kultur – European Culture Collections Organization (ECCO). Od roku 1999 vedla sbírku doc. MUDr. Helena Žemličková, Ph. D., sbírka získala statut Národní referenční laboratoře (NRL), byla provedena rozsáhlá revize sbírky (biochemické metody). V roce 2014 byla sbírka předána RNDr. R. Šafránkové, Ph.D., pokračuje revize sbírky pomocí moderních metod (MALDI-TOF MS, sekvenační analýza 16S rRNA, WGS...). Souhrn činností/aktivit CNCTC: dlouhodobé uchovávání kultur včetně distribuce (prodej), příprava okruhu EHK – Bakteriologická diagnostika, lyofilizace kultur na zakázku, revizní a publikační činnost.
This year marks 75 years since the foundation of the Czech National Collection of Type Cultures (CNCTC), which operates within the Centre for Epidemiology and Microbiology at the National Institute of Public Health (NIPH) in Prague. The CNCTC was officially established in 1947 as a central collection of local collections of cultures managed by individual national reference laboratories, continuing the tradition of the collection of cultures established at NIPH at the end of the 1920s of the last century. The first curator was doc. Juraj Strauss, during whose tenure type strains obtained from foreign collections were included in the collection, uniform documentation and records of cultures were introduced, and modern methods of strain preservation (lyophilization) were applied. A next curator of the collection was Jiří Šourek, who led the collection for over 40 years and significantly expanded it to 5,000 strains. Under his leadership, the collection, already with the acronym CNCTC, was registered in the World Federation of Culture Collections (WFCC) and in the European Culture Collections Organization (ECCO). From 1999, the collection was led by doc. MUDr. Helena Žemličková, Ph.D. It received the status of the National Reference Laboratory (NRL) and its extensive revision (biochemical methods) was performed. In 2014, the collection was handed over to RNDr. R. Šafránková, Ph.D.; its revision continues to be carried out using modern methods (MALDI-TOF MS, 16S rRNA sequencing analysis, WGS...). Summary of CNCTC activities: long-term preservation of cultures including distribution (sale), preparation of EQA (External Quality Assessment) – Bacteriological diagnostics, lyophilization of cultures to order, revision and publication activities.

This study aimed to define the taxonomic position and structure of a novel, taxonomically unique group of 26 Acinetobacter strains, provisionally designated Taxon 24 (T24). The strains were recovered from soil and freshwater ecosystems (n = 21) or animals (n = 5) in Czechia, Scotland, Germany, the Netherlands and Turkey between 1993 and 2015. They were non-glucose-acidifying, nonhemolytic, nonproteolytic, growing at 32 °C and on acetate and ethanol as single carbon sources, but not on 4-hydroxybenzoate and mostly not at 37 °C. Their whole-genome sequences were 3.0-3.7 Mb in size, with GC contents of 39.8-41.3%. Based on core genome phylogenetic analysis, the 26 strains formed a distinct clade within the genus Acinetobacter, with strongly supported subclades termed T24A (n = 11), T24B (n = 8), T24C (n = 2), T24D (n = 3) and T24E (n = 2). The internal genomic ANIb values for these subclades were >94.8%, while the ANIb values between them were <92.5%. The results of MALDI-TOF MS-based analyses agreed with this classification. The five subclades differed from each other in the results of one to six carbon source assimilation tests. Given the genomic and phenotypic distinctness, internal coherence, numbers of available strains and geographically diverse origin of T24A and T24B, we propose the names Acinetobacter terrae sp. nov. and Acinetobacter terrestris sp. nov. for these two taxa, respectively. The type strains are ANC 4282v (= CCM 8986T = CCUG 73811T = CNCTC 8082T) and ANC 4471T (= CCM 8985T = CCUG 73812T = CNCTC 8093T), respectively. We conclude that these two species together with the other T24 strains represent a widely dispersed Acinetobacter clade primarily associated with terrestrial ecosystems.

Kazuistika suspektní septikemie s izolací raritního kmene Corynebacterium sp. Izolace tohoto kmene ani kmenů geneticky blízkých druhů z klinického materiálu nebyla zatím v dostupné literatuře popsána. V článku jsou uvedeny kromě stručného klinického popisu také morfologické a biochemické vlastnosti, metody identifikace a vyšetření k antibiotikům. Cílem práce bylo především upozornit na nový a vzácně se vyskytující kmen korynebakteria.
A case report is presented of a patient with suspected septicaemia from whose blood culture a new strain of Corynebacterium sp. was isolated. Until now, no report of this strain isolated from human clinical materials has been available in the literature. In addition to a brief clinical description of the case, the article also features morphological, biochemical properties as well as antibiogram of the bacterium. It describes also methods used for the identification of this isolate. The aim of the work was to highlight a novel and rare coryneform strain.
BACKGROUND: The resistance of Streptococcus pneumoniae to macrolides is becoming an increasingly important issue and thus it is important to understand the genetics related to adaptation of this species to the widespread use of antibiotics in Europe. The 58 isolates of S. pneumoniae belonging to sequence type (ST) 416 and serotype 19A and to several different phenotypes originated from Italy, Portugal and Czech Republic were thus sequenced on Illumina MiSeq. The aim of the study was to describe genetical origine of isolates, investigate their macrolide resistance and suggest reasons for spread of ST416 in the Czech Republic. RESULTS: Investigation of genes associated with serotype determined serotype switch between 15B and 19A serotypes and core genome multilocus sequence typing (cgMLST) confirmed the origine of concerned isolates in Netherlands15B-37 clone. Inspected genomes proved variability of genes associated with the macrolide resistance even within closely genetically relative isolates. CONCLUSIONS: Participation of 19A/ST416 on the spread of Netherlands15B-37 is accompanied by serotype switch between 19A and 15B serotypes and with acquisition of genes involved in macrolide resistance to the clone that was originally macrolide susceptible. There is evident tendency to interchanging and modifications of these and surrounding genes, that could lead to accelerate spreading of this sequence type in regions with high macrolide consumption.

Enterobacterales produkující karbapenemázu (CPE – Carbapenemase-Producing Enterobacterales) představují globální zdravotnický problém. V Evropě dochází zejména v jižních státech ke zvyšování podílu enterobakterií rezistentních vůči karbapenemům. Od roku 2016 dochází také v České republice k plošnému nárůstu počtu případů, v roce 2019 bylo zaznamenáno 270 případů kolonizace nebo infekce CPE. Tento trend je zčásti způsoben několika epidemickými epizodami, odráží ale i zhoršující se endemickou situaci ve zdravotnických zařízeních. Přesto lze Českou republiku stále hodnotit jako stát s nízkým výskytem CPE.
Carbapenemase-producing Enterobacterales (CPE) are a global health problem. In Europe, the proportion of carbapenem-resistant enterobacteria is increasing, particularly in the southern states. Since 2016, there has also been a widespread increase in the number of cases in the Czech Republic, with 270 cases of colonization or CPE infection reported in 2019. This trend is partly due to several outbreaks, but it also reflects the deteriorating endemic situation in healthcare facilities. Nevertheless, the Czech Republic still remains a low CPE incidence country.
