V roce 2021 bylo do surveillance databáze v České republice (ČR) zařazeno celkem 264 případů invazivního pneumokokového onemocnění (IPO). Přestože hlášení IPO případů zaznamenalo vzrůst v zimě 2021, je ovlivnění pandemií covid-19 zřejmé i v roce 2021. Celková nemocnost IPO v roce 2021 dosáhla hodnoty 2,5/100 000 obyvatel (v roce 2020 – 2,3/100 000 obyvatel). Věkově specifická nemocnost u dětí pod jeden rok věku byla 0,9/100000 obyvatel – tj. 1 případ IPO (v roce 2020 – 4,5/100000 obyvatel, tj. 5 případů IPO) a ve věkové skupině 1–4 roky se dostala na hodnotu 2,6/100000 obyvatel – tj. 12 případů IPO (v roce 2020 – 1,8/100000 obyvatel, tj. 8 případů IPO). Nejvíce případů IPO opět zůstává ve věkové skupině 65 let a starší s absolutním počtem 140 IPO a s nemocností 6,6/100 000 obyvatel (v roce 2020 – 119 IPO – 5,6/100000 obyvatel). Počet případů IPO u očkovaných pneumokokovými vakcínami byl 14, děti 0–4 roky: 6 případů IPO, 5–39 let: 1 případ IPO, 65 let věku a více: 7 případů IPO. Celková smrtnost IPO v roce 2021 byla 18,2% (v roce 2020 – 17%). Bylo hlášeno 48 úmrtí v souvislosti s průkazem pneumokoka v primárně sterilním klinickém materiálu. Nejvyšší smrtnost byla ve věkové skupině 65+ (32 úmrtí na IPO). Není evidováno žádné úmrtí ve věkových kategoriích 0–39 roků. Z 264 případů IPO bylo zasláno do Národní referenční laboratoře pro streptokokové nákazy (NRL/STR) k typizaci 259 izolátů Streptococcus pneumoniae (98 %). Sérotypy 3, 8, 19A a 6C byly v roce 2021 zastoupeny nejčastěji.
In 2021, altogether 264 cases of invasive pneumococcal disease (IPD) were entered in the surveillance database of the Czech Republic (CR). Despite the increase in IPD observed in winter 2021, the impact of COVID-19 continues to be evident also in 2021. The overall incidence of IPD was 2.5/100 000 population (compared to 2.3/100 000 in 2020). The age-specific incidence rates reached 0.9/100 000 in children under 1 year of age, i.e. 1 IPD case, compared to 4.5 /100 000 in 2020, i.e. five cases of IPD, and 2.6/100 000 in children aged 1 to 4 years, i.e. 12 cases, compared to 1.8/100 000 in 2020, i.e. eight cases of IPD. The most afflicted age group was 65 years and over, with an absolute number of 140 cases, i.e. 6.6/100 000, in comparison to 119 cases, i.e. 5.6/100 000, in 2020. Fourteen cases of IPD occurred in persons previously vaccinated with pneumococcal vaccines, six of whom were children aged 0–4 years, one was from the age group 5–39 years, and seven were 65 years and over. The overall case fatality rate was 18.2% in 2021 in comparison to 17% in 2020. Forty-eight reported deaths were associated with the detection of pneumococci in clinical specimens from normally sterile body sites. The highest case fatality rate was recorded in the age group 65 years and over (32 deaths due to IPD). No fatal case was recorded in the age groups 0–39 years. Two hundred and fifty-nine isolates of Streptococcus pneumoniae (98 %) from 264 cases of IPD were referred to the National Reference Laboratory for Streptococcal Infections for typing. The most commonly identified serotypes in 2021 were 3, 8, 19A, and 6C.

Cíl práce: Cílem předkládané studie bylo analyzovat epidemiologické a klinické charakteristiky invazivního pneumokokového onemocnění (IPO) u dospělých pacientů v období před a po zavedení očkování dětí konjugovanými vakcínami v České republice. Materiál a metody: Do retrospektivní observační sentinelové studie byli zařazeni dospělí pacienti s IPO (kultivační průkaz S. pneumoniae z primárně sterilního materiálu) hospitalizovaní v Nemocnici Na Bulovce v letech 2000–2019. Výsledky: Ve studijním období bylo diagnostikováno celkem 304 případů onemocnění s věkovým mediánem 58 let (IQR 43-73) a zastoupením mužů a žen 1,49 : 1. Nejčastější klinickou formu představovaly bakteriemické pneumonie (185 případů; 60,9 %) a purulentní meningitidy (90; 29,6 %). Intenzivní péči vyžadovalo 157/293 pacientů (53,6 %) a 77 nemocných onemocnění podlehlo (25,3 %). Sérotyp byl určen u 292 izolátů (96,0 %) a nejčastěji byly prokázány sérotypy 3 (38; 12,5 %), 4 (28; 9,2 %), 7F (24; 7,9 %), 8 (21; 6,9 %) a 1 (18; 5,9 %). Klinické i epidemiologické charakteristiky infekcí vyvolaných nejčastějšími sérotypy se odlišovaly, přičemž onemocnění vyvolaná sérotypem 3 postihovala starší rizikové pacienty, častěji vyžadovala poskytnutí intenzivní péče a měla vyšší smrtnost. Zastoupení sérotypů, které nejsou obsažené ve 13valentní konjugované vakcíně, vzrostlo z 28,8 % (19/66) v letech 2000–2004 na 54,8 % (40/73) v období 2015–2019 (p = 0,001). Závěry: Studie prokázala, že infekce vyvolané nejčastějšími sérotypy se odlišují nejen svými epidemiologickými charakteristikami, ale i klinickým průběhem a smrtností. Ve sledovaném období došlo k významnému nárůstu IPO vyvolaných sérotypy, které nejsou zařazeny v 13valentní konjugované vakcíně, což limituje dostupné možnosti prevence u pacientů dospělého věku.
Objective: The aim of this study was to analyse epidemiological and clinical characteristics of invasive pneumococcal disease (IPD) in adults before and after the introduction of the general childhood conjugate pneumococcal vaccination programme in the Czech Republic. Material and Methods: The retrospective observational sentinel study included adults with IPD admitted to the Na Bulovce Hospital in Prague from 1/2000 through 12/2019. A case of IPD was defined as isolation of Streptococcus pneumoniae from a primarily sterile site. Results: A total of 304 IPD cases were diagnosed during the study period, with a male to female ratio of 1.49:1 and age median of 58 years (IQR 43-73). The most prevalent clinical forms were bacteraemic pneumonia (185 cases; 60.9%) and purulent meningitis (90; 29.6%). A total of 157/293 patients (53.6%) required intensive care, and the case fatality rate was 25.3% (n = 77). The serotype was determined in 292 (96.0%) isolates, the most prevalent being serotypes 3 (38; 12.5%), 4 (28; 9.2%), 7F (24; 7.9%), 8 (21; 6.9%), and 1 (18; 5.9%). Both clinical and epidemiological characteristics of IPD caused by the most prevalent serotypes differed considerably. Patients diagnosed with serotype 3 were older, more frequently required intensive care, and showed higher mortality. The proportion of IPD caused by non-PCV13 serotypes increased from 28.8% (19/66) in 2000-2005 to 54.8% (40/70) in 2015-2019 (p = 0.001). Conclusion: The study demonstrated that invasive diseases caused by the most prevalent pneumococcal serotypes differ in their epidemiological and clinical characteristics and case fatality rate. During the study period, there was a significant increase in IPD caused by non-PCV 13 serotypes, limiting the effect of vaccination in adults.

BACKGROUND: The resistance of Streptococcus pneumoniae to macrolides is becoming an increasingly important issue and thus it is important to understand the genetics related to adaptation of this species to the widespread use of antibiotics in Europe. The 58 isolates of S. pneumoniae belonging to sequence type (ST) 416 and serotype 19A and to several different phenotypes originated from Italy, Portugal and Czech Republic were thus sequenced on Illumina MiSeq. The aim of the study was to describe genetical origine of isolates, investigate their macrolide resistance and suggest reasons for spread of ST416 in the Czech Republic. RESULTS: Investigation of genes associated with serotype determined serotype switch between 15B and 19A serotypes and core genome multilocus sequence typing (cgMLST) confirmed the origine of concerned isolates in Netherlands15B-37 clone. Inspected genomes proved variability of genes associated with the macrolide resistance even within closely genetically relative isolates. CONCLUSIONS: Participation of 19A/ST416 on the spread of Netherlands15B-37 is accompanied by serotype switch between 19A and 15B serotypes and with acquisition of genes involved in macrolide resistance to the clone that was originally macrolide susceptible. There is evident tendency to interchanging and modifications of these and surrounding genes, that could lead to accelerate spreading of this sequence type in regions with high macrolide consumption.

The present study aims to evaluate the diagnostic yield of bronchoalveolar lavage (BAL) fluid in patients with hematological malignancies and describe the most common pathogens detected in BAL fluid (BALF.) An analysis of 480 BALF samples was performed in patients with hematological malignancies over a period of 7 years. The results of culture methods, PCR, and immunoenzymatic sandwich microplate assays for Aspergillus galactomannan (GM) in BALF were analyzed. Further, the diagnostic thresholds for Aspergillus GM and Pneumocystis jiroveci were also calculated. Microbiological findings were present in 87% of BALF samples. Possible infectious pathogens were detected in 55% of cases; 32% were classified as colonizing. No significant difference in diagnostic yield or pathogen spectrum was found between non-neutropenic and neutropenic patients. There was one significant difference in BALF findings among intensive care units (ICU) versus non-ICU patients for Aspergillus spp. (22% versus 9%, p = 0.03). The most common pathogens were Aspergillus spp. (n = 86, 33% of BAL with causative pathogens) and Streptococcus pneumoniae (n = 46, 18%); polymicrobial etiology was documented in 20% of cases. A quantitative PCR value of > 1860 cp/mL for Pneumocystis jirovecii was set as a diagnostic threshold for pneumocystis pneumonia. The absorbance index of GM in BALF of 0.5 was set as a diagnostic threshold for aspergillosis. The examination of BAL fluid revealed the presence of pathogen in more than 50% of cases and is, therefore, highly useful in this regard when concerning pulmonary infiltrates.

Cíl práce: Zjištění klonálních charakteristik souboru kmenů Streptococcus pneumoniae (S. pneumoniae) působících invazivní pneu-mokokové onemocnění (IPO) v České republice (ČR) v roce 2017. Porovnání klonálního členění kmenů probíhalo v Národní referenční laboratoři pro streptokokové nákazy (NRL) použitím rutinně využívané metody multilokusové sekvenační typizace (Multilocus Sequence Typing, MLST) a nově zavedené metody multilokusové analýzy tandemových repetic (Multiple-Locus Variable number tandem repeat Analysis, MLVA). Materiál a metodika: Studován byl soubor 87 kmenů S. pneumoniae, vybraný z izolátů doručených v rámci programu surveillance IPO v roce 2017 do NRL z celé ČR. Výběr byl veden přes sérotypy S. pneumoniae tak, aby soubor zahrnoval izoláty sérotypů zastoupených v pneumokokové konjugované 13valentní vakcíně (sérotypy 1, 4, 9V) a izoláty sérotypů nevakcinačních, vyskytujících se však v ČR ve značném zastoupení (sérotypy 8, 9N, 22F). Pro studium byla použita metoda MLST, která je světově rozšířeným standardem klonální charakterizace pneumokokových izolátů a využívá sekvenování souboru genových oblastí a metoda MLVA, která charakterizuje izoláty podle počtu tandemově repetitivních úseků v intergenových oblastech. Výsledky: MLST analýza ukázala a potvrdila vysokou míru klonální homogenity izolátů S. pneumoniae sérotypů 1, 9N, 9V, 22F a značnou genetickou proměnlivost izolátů sérotypů 4 a 8. Mezi klonálním členěním, poskytnutým oběma metodami, byla rámcová korelace. V porovnání s metodou MLST poskytovala metoda MLVA detailnější klonální odlišení. U izolátů s nově zjištěnými MLVA profily je žádoucí přiřazení nového MLVA typu (MT). Tato webová podpora MLVA schématu S. pneumonie však ustupuje do pozadí a neposkytuje aktuálně všechny služby oproti webové podpoře pro MLST charakterizaci. Závěry: Metoda MLST je a nadále zůstává u izolátů S. pneumoniae standardem při klonální charakterizaci původců IPO při provádění surveillance na úrovni místní i mezinárodní. MLST charakteristiky izolátů jsou využity při sledování klonální proměnlivosti národními i nadnárodními orgány ochrany veřejného zdraví. Metoda MLVA rutinně používána není, lze ji však použít jako doplňkovou pro zrychlené zjištění klonální příbuznosti izolátů ze situací lokálního charakteru, například ohniskových výskytů infekce. Zde může snáze zachytit vznik a šíření virulentní klonální varianty.
Aim: To determine clonal characteristics of Streptococcus pneumoniae (S. pneumoniae) strains causing invasive pneumococcal disease (IPD) in the Czech Republic (CR) in 2017. Clonal assignment of strains was performed in the National Reference Laboratory for Streptococcal Infections (NRL) by the routinely used method, multilocus sequence typing (MLST), and a newly introduced method, multiple-locus variable number tandem repeat analysis (MLVA). Material and method: The study strains were 87 isolates of S. pneumoniae selected from those referred to the NRL within the IPD surveillance programme from all over the CR in 2017. The study set covers S. pneumoniae isolates of both pneumococcal 13-valent conjugate vaccine serotypes (1, 4, and 9V) and non-vaccine serotypes (8, 9N, and 22F) widely spread in the CR. The study methods were MLST, the standard method used worldwide for the characterisation of pneumococcal isolates based on sequencing of a set of gene regions, and MLVA, which allows to characterise isolates based on the number of tandem repeats in intergenic regions. Results: MLST revealed and confirmed a high level of clonal homogeneity of S. pneumoniae isolates of serotypes 1, 9N, 9V, and 22F and a considerable genetic variability of serotype 4 and 8 isolates. There was a general correlation between the MLST and MLVA clonal complex assignments. In comparison with MLST, MLVA has superior clonal discriminatory power. Isolates with the newly determined MLVA profiles should be assigned to new MLVA types (MT). Nevertheless, the new web support of the MLVA scheme for S. pneumoniae is less relevant as it does not provide services comparable to those available from the web support for MLST characterisation. Conclusions: MLST continues to be the standard method for clonal characterisation of S. pneumoniae isolates from IPD for the purposes of both national and international surveillance. MLST characteristics of isolates are helpful in the study of clonal variability conducted by both national and transnational public health protection authorities. MLVA is not routinely used but can serve as a complementary method for rapid identification of clonal relatedness between isolates, e.g. those from local outbreaks. It is more suitable for the detection of emergence and spread of a virulent clonal variant.
BACKGROUND: The aim of this study was to evaluate the presence of pilus islet 1 (PI-1) and to determine its clade type in pneumococcal isolates with reduced susceptibility to penicillin (penicillin non-susceptible pneumococci - PNSP) and/or resistant to macrolides isolated prior to and after the introduction of pneumococcal conjugate vaccines (PCVs) in the Czech Republic. METHODS: Clinical isolates of serotypes 9V (n = 68) and 19A (n = 89) were examined. Isolates were characterised by multilocus sequence typing (MLST). The presence of PI-1 was determined by screening for the sortase B, C, and D genes located within PI-1. In the presence of PI-1 pilus, clade types were classified by PCR. RESULTS: In the pre-PCV period (2000-2007), the prevalence of PNSP was 3.9% and 2.7% of isolates were resistant to erythromycin. During 2012-2015 (post-PCV period), the rates of PNSP remained stable (3.6%), but resistance to erythromycin increased to 8.3%. While in 2000-2007, resistance to antibiotics was associated mainly with serotype 9V, in 2012-2015, it was replaced by serotype 19A. PI-1 positive isolates were seen in both serotypes. All isolates (68) of serotype 9V belonged to the Spain9V-3 (CC156) clone and carried PI-1 of clade type I while 96.5% (56/58) of isolates of 19A serotype belonged to the Netherlands15B-37 (CC199) clone and carried PI-1 of clade type II. CONCLUSIONS: Both major antibiotic resistant clones carried PI-1, although they differ in the clade type. Thus the role of PI-1 should be evaluated in further studies and potentially considered in the spread of antibiotic resistant clones.
